1.Determination of the Concentration of Midazolam in Plasma by RP-HPLC-UV Detection

OBJECTIVE:To build up a method of determining the concentration of midazolam in plasma by RPHPLC-UV detection.METHODS:The separation was carried out by a reversed-phase Hypersil ODS column(250mm?4.0mm,5?m) with a mobile phase consisting of methanol-acetonitrile-0.02mol/L potassium phosphate buffer(pH 7.4) (65∶25∶10,V/V).Mida_zolam was extracted from alkalinizing plasma and soluted in the mobile phase then detected at 221nm.RESULTS:The calibration curve had the fine linearity in the concentration range 50~1 600ng/ml(r=0.9 999).The detection limit was 2ng/ml.The absolute recovery was 90.8%~95.4%,the relative recovery was 99.3%~101.3%.The within-day and between-day precision(CV%) was 1.94%~5.16%,3.00%~6.39% respectively.CONCLUSION:The method is simple,stable and highly sensitive and could meet with the research of clinical pharmacokinetics.
2.Graphene and Anthraquinone-2-sulfonic Acid Sodium Based Electrochemical Aptasensor for Pb2+

An electrochemical aptasensor for Pb2+ was constructed based on the layer-by-layer assembly of graphene (GR) and anthraquinone -2-sulfonic acid sodium salt (AQMS) on the surface of Pb2+ aptamer. The mercapto-modified Pb2+ aptamer was first anchored on a gold electrode. Then the highly conductive material of GR was adsorbed on apt through the unique π-π stacking interaction, which was further used for the assembly and signal amplification of the electroactive AQMS. Upon interaction with Pb2+ ions, the apt on the aptasensor undergone conformational switch from a single-stranded form to the G-quadruplex structure, causing the GR assembled with AQMS releasing from the electrode surface into solution. As a result, the electrochemical signal of AQMS on the aptasensor was substantially reduced. Base on this concept, a useful platform for detection of Pb2+ ions was constructed. Under the optimal experimental conditions, the attenuation of peak currents (△Ipa ) showed linear relationship with the logarithm of Pb2+ concentrations (lgCPb2+) over the range from 5. 0×10-10 to 5. 0×10-8 mol/ L. The detection limit was estimated to be 6. 0×10-11 mol/ L.

4.Determination of the Concentration of Propofol in Plasma by RP-HPLC with Fluorescence Detection

OBJECTIVE:To build up a method for the determination of propofol in plasma by RP-HPLC with fluores?cence detection.METHODS:The separation was performed on a reversed-phase Zorbax Eclipse XDB-C 18 column(150mm?4.6mm,5?m)with a mobile phase consisting of methanol-acetonitrile-0.005mol/L sodium acetate buffer(pH4.0)(55∶20∶25,V/V).Propofol was extracted from plasma and dissolved in the mobile phase then detected at276/310nm.RESULTS:The calibration curve had the fine linearity in the concentration range of0.015625~8?g/ml(r=0.9998).The limit of detection(LOD)was1ng/ml(S/N ratio=3),the limit of quantification(LOQ)was10ng/ml.The absolute recovery was89.33%~93.37%,the relative recovery was97.75%~103.31%.The within-day and between-day precision(RSD%)was1.38%~5.02%and4.45%~9.056%respectively.CONCLUSION:The method is simple,stable and highly sensitive and can meet with the research of clinical pharmacokinetics.
5.Experimental study of sympathetic nerve on atrioventricular conduction modulation in acute inferior myocardial infarction

Objective To study atrioventricular conduction during right coronary artery infarction and its correlation with sympathetic nerve.Method AIMI model by ligating the fight coronary artery of denervated autonomic nervous cats was used.The A,H and V-wave were measured from the His electrode by template matching.The AA,AH,HV intervals with and without sympathetic nerve stimulation were measured respectivelv.Results The decreased percentage of AH intervals was (7±5)% vs.(14±5)% (without pacing) and (12±2)% vs.(23±7)% (with pacing) respectively in ischemia and normal after sympathetic nerve stimulation,and the decreasing amplitude were 50% and 48% respectively.Conclusion The regulating function of sympathetic nerve on atrioventricular conduction decreased after ischemia related to the right coronary artery.This decrease may be one of the mechanisms that lead to atrioventficular block after acute inferior myocardial infarction.
6.Non-traumatic rhabdomyolysis:clinical analysis of thirty-nine cases.

Objective To evaluate clinical features,predisposing factors,therapeutic regimen and prognosis of non-traumatic rhabdomyolysis.Methods Clinical picture,therapeutic regimen and prognosis were investigated in 39 cases with non-traumatic rhabdomyolysis by retrospective analysis.Results Non-traumatic rhabdomyolysis mostly presented fever,asthenia,myalgia and/or muscular tenderness,swelling of involved muscles,red urine and oliguria or anuria.The complications and comorbidity of rhabdomyolysis included acute renal failure(ARF),disorders of metabolites and electrolytes,compartmental syndrome,infection,and multiple organ dysfunction.Infection(33.3%)was the most common etiology of non-traumatic rhabdomyolysis,followed by drugs(25.6%),metabolite or electrolyte derangements(10.3%)and alcohol intoxication(7.7%)etc.Therapeutic regimen covered treatment of the underlying diseases,volume repletion,alkalization and dealing with the complications.For the patients with established renal failure,renal replacement therapy was essential.Overall mortality was 15.4%,while the mortality in the patients with ARF was 20.7%.If surviving ARF,the patients' renal function promised to be normalized consequently.Conclusion Non-traumatic rhabdomyolysis is a syndrome with a variety of causes,different clinical presentations and versatile combination of complications,which confounds the diagnosis.However,if treated properly and in time,the survivors in all probability will recover from ARF.

AIM To study the effects of homoharringtonine(HHT) on inhibition of proliferation and induction of apoptosis of nasopharyngeal carcinoma cells CNE 2Z. METHODS The inhibitory rates of the proliferation and IC 50 were detected by MTT method. The apoptosis was analyzed by flow cytometry (FCM), agarose gel electrophoresis and Hoechst 33258/PI fluorescence staining. RESULTS After cells were treated with HHT of different concentrations for 24, 48 and 72 h,respectively,the inhibitory rates of the proliferation rised with concentration and time. The IC 50 values of 24, 48 and 72 h were (0 629?0 039), (0 483?0 011) and (0 389?0 027) mg?L -1 , respectively. The difference among IC 50 values was obvious ( P
8.Clinical observation of concurrent radiotherapy combined with S-1 for elderly patients with locally advanced esophageal cancer

Objective To evaluate the clinical effects and adverse reactions of concurrent radiotherapy combined with S-1 for elderly patients with locally advanced esophageal cancer.Methods From January 2011 to April 2013,a total number of 50 elderly patients with locally advanced esophageal carcinoma were randomly divided into concurrent chemo-radiotherapy combined with S-1 group (treatment group) (25 cases) and radiotherapy alone group (control group) (25 cases).Intensity modulated radiation therapy (IMRT) was administered in all cases,with a total dose of (58-60) Gy/(29-30) fx,conventional fractionation.Patients in the treatment group received S-1 of 80 mg/m2 orally bid on day 1-14,with 21 days as a cycle.All the patients should take it until the end of the radiotherapy treatment.Results The efficacy of the 47 cases could be evaluated.In treatment group,5 cases achieved CR,14 cases PR,3 cases SD,1 case PD.The response rate (RR) was 82.6 % (19/23).In control group,3 cases achieved CR,12 cases PR,6 cases SD,3 cases PD.The RR was 62.5 % (15/24).There was a statistical significant difference between the two groups.The one-year local control rates of treatment group and control group were 63.6 % and 43.5 %,respectively,and the one-year overall survival rates were 68.2 % and 39.1%,respectively.The main toxicities included bone-marrow suppression,gastrointestinal reactions,radiation esophagitis and radiation pneumonitis.The incidences of bone-marrow suppression and gastrointestinal reactions were higher in treatment group than those in control group,but the toxic reaction was mild and tolerable.There were no significant differences in terms of bone-marrow suppression,gastrointestinal reactions,radiation esophagitis and radiation pneumonitis between both groups.Conclusion Concurrent chemotherapy combined with S-1 is more effective in the treatment of the elderly patients with locally advanced esophageal cancer and the toxicity is tolerable,which is worth studying furtherly.
9.Comparison of surgical outcomes between microsurgery lumbar discectomy and microendoscopic discectomy for lumbar disc herniation

Objective To compare the surgical outcomes between microsurgery lumbar discectomy and microendoscopic discectomy for lumbar disc herniation. Methods A prospective study was conducted on the surgical procedures for lumbar disc herniation.The target of our study was a group of 33 patients who underwent surgery by microsurgery lumbar discectomy(MSLD group)and 36 patients who underwent surgery by microendoscopic discectomy(MED group).The items investigated were the operation time,amount of bleeding,duration of hospitalization,pre-and postoperative scores based on judgment criteria for treatment of lumbar spine disorders established by the Japanese Orthopaedic Association score,visual analog scales (VAS,0 to 10) for lumbago and sciatica before surgery and at discharge,perioperative complications.Results The mean duration of follow-up was 2 years and 2 months (11 months to 4 years).There were no significant differences between the 2 surgical procedures in the frequency of the pre-and postoperative Japanese Orthopaedic Association scores or postoperative VAS for lumbar pain and sciatica,operation time and duration of hospitalization. Statistically significant differences were observed in amount of bleeding and operation time,but the differences were not large, and may not have been clinically significant.Conclusion Both microsurgery lumbar discectomy and microendoscopic discectomy are appropriate for lumbar disc herniation.
10.Pancreatic cell apoptosis and endocrine alterations in rats with acute obstructive pancreatitis

Objective To investigate the occurrence of apoptosis and its relationship with endocrine hormones altemtions in rats with acute obstructive pancreatitis(AOP). Methods The model of AOP wag establisbed by ligation of pancreaticobiliary duct.8,12 hrs after operation,the serum insulin,glucagons and amvlase were determined;pancreatic tissues were harvested and apoptotic rate wag evaluated by TUNEL and flow cytometry(Annexin V-FITC/PI assay).Results 8 and 12 hrs after AOP induction,serum amylase levels wefe(1198±687)U/L and(1698±1103)U/L respectively;serum insulin levels were(8.1±5.8)ng/ml and (12.7 ±6.9)ng/ml respectively;sertlm glucagon levels were(6.8±4.6)ng/ml and(7.3±2.9)ng/ml respectively;all these parameters were significantly high than(404±222)U/L,(5.6±2.7)ng/ml and(2.6±2.1)ng/ml in the sham operation group(P<0.05).AnnemnV FITC/PI assay confirmed apoptosis occurred both in exocrine acinus cells and endocrine panclreas islet;and the apoptotic rate wag(20.5±11.2)%and (15.5±8.9)%at 8 and 12 hrs after AOP induction,which wag significantly high than(4.2±1.6)%in the sham operation group(P<0.05).Conclusions Cell apoptosis occurred in both acinar and islet in the model of AOP,and this may be the pathophysiological basis of endocnne hormones alterations in the model of AOP.
