1.Roles of MMP-2 and MMP-9 in the central nervous system
Journal of Medical Postgraduates 2003;0(09):-
The matrix metalloproteinases(MMP) are important enzymes that regulate developmental processes, maintain normal physiology in adulthood and have reparative roles in specific stages after an insult to the nervous system.Conversely, the concordant presence and significant up-regulation of several MMP members in virtually all neurological conditions result in pathological lesions. Thus, the MMPs have double functions, capable of mediating repair and recovery on the one hand and being involved in producing injury on the other. This article highlights the beneficial roles of MMP-2 and MMP-9 in normal and reparative processes within the nervous system and discusses the detriments of both encountered in spinal cord injury and central nervous system cancers, and we also review the availability of MMP inhibitors for clinical use.

Objective To evaluate the effect of segmental pulmonary veins isolation for atrial fibrillation (AF) and investigate the possible factors that may affect its result. Methods In 120 (105 men; age 50.0?8.6 years) consecutive patients with paroxysmal (n=99) or persistent (n=21) AF, segmental PVs electrical isolation with Lasso mapping catheters was performed. The relationship of the outcome of the initial procedure and factors such as age, sex, type of AF, left atrial diameter, case history, left ventricular ejection fraction and presence of hypertension were analyzed by statistical methods. Results 52 (52.5%) out of the 99 patients with paroxysmal AF and 6 (28.5%) out of the 21 with persistent AF were free of AF after a single ablation procedure. Univariate analysis revealed that left atrial enlargement (P=0.001), persistent AF (P=0.046) and old age (P=0.047) were related to the recurrence of AF after the first procedure. Patients with paroxysmal AF apparently had a better outcome than those with persistent AF after repeat ablation but of no statistical significance (P=0.094). Logistic regression analysis revealed that left atrial enlargement was the only independent risk factor of recurrent AF after the first procedure. Conclusion About 50% of the patients with paroxysmal AF are free of AF after single ablation. Left atrial enlargement is the only independent risk factor of recurrent AF while old age and persistent AF influence the outcome of the first procedure.

Objective To investigate the preoperative and intraoperative assessed values of the color-coded digital subtraction angiography (DSA ) for intracranial arteriovenous malformation (AVM). Methods Fifteen patients with AVM performed preoperative routine whole brain DSA were analyzed retrospectively, and the iFlow software was used to perform color-coded DSA of image post-processing. A comprehensive analysis such as the range of lesions, vascular architecture and hemodynamics of AVM was conducted on the two-dimensional DSA images and color-coded DSA. Results Of the 15 patients with AVM,9 were small-sized (including 4 nidi showed diffuse type),3 were medium-sized,and 3 were large-sized;8 patients had single feeding artery, and 7 had multiple feeding arteries;11 had single draining vein, and 4 had multiple draining veins. When showing the size of AVM nidus, particularly diffuse type nidus, the color-coded DSA was clearer than the two-dimensional image. The color-coded DSA could visually display the traveling of the superficial and deep draining veins,at the same time, the primary and secondary draining veins could be identified according to the size of the area under the curve and the full width at half maximum, and intuitively reflected the complete cycle of cerebral blood flow. Conclusion The color-coded DSA can quickly and accurately depict the range of AVM,angioarchitecture features and intraoperative hemodynamic changes.

Objective:To compare the safety and efficacy of domestic Firebird2 TM rapamycin eluting stent and imported Endeavor Resolute stent in the treatment of coronary heart disease patients. Methods:The clinical data of 889 patients with coronary heart disease who underwent percutaneous coronary intervention (PCI) in Wuhan Asian heart hospital from 2010 to 2015 were analyzed retrospectively A case-control study was conducted. According to the type of stent used, 550 cases were divided into domestic Firebird 2TM group and 325 cases were divided into imported endeavor resolve group The incidence of major adverse cardiovascular events (MACE) was compared between the two groups. All patients was followed up for 1 year. Results:There was no significant difference in gender, age, body mass index, medical diseases, whether it was emergency PCI, previous PCI history, coronary artery bypass grafting history and left ventricular between the two groups (all P>0.05). The success rate of PCI in the two groups was 100%. The TIMI blood flow of target vessels in the two groups was grade 3. There was no stent unloading during the operation, and balloon pretreatment and post-treatment were performed. The results of one-year follow-up showed that there was no significant difference in the incidence of mace (3.1% (17/550) and 2.2%(7/325)) and the incidence of independent end points between domestic Firebird 2 TM group and endeavor group (all P>0.05); There was no significant difference in survival without mac between the two groups ( P>0.05). Conclusion:The domestic Firebird2 TM rapamycin eluting stent has the same clinical efficacy as the imported Endeavor Resolute drug eluting stent.

Journal of China Pharmaceutical University 2017;48(6):751-758
The transport packages are primarily developed for transportation and storage, whose capabilities directly affect the product qualities during the circulation.During pharmaceutical development, the domestic pharmacy enterprises tend to put emphasis on the inner packing, but neglect the potential hazard elements in the distribution cycles and performance tests for transport packages of pharmaceutical products, which ultimately result in the disregard of hidden quality peril. This paper analyzes the necessity and significance ofthe performance tests for pharmaceuticals, follow by its domestic and overseas status. And then several testing standards for transport packages, especially ASTM D4169 (Standard Practice for Performance Testing of Shipping Containers and Systems),are introduced in order to provide some suggestions of the performance testing for transport packages of pharmaceutical products.

Objective To develop a simple and effective method for monitoring cortical ischemia after temporary occlusion of the parent arteries during anterior circulation intracranial aneurysm surgery. Methods Fifty-two patients with anterior circulation aneurysm (58 aneurysms) received craniotomy from April to November 2008, and at the same time, cortical electroencephalograpby (EEG) and scalp EEG were monitored during the surgery.According to the international 10/20 electrode placement system, scalp electrodes were placed on O1, O2, P3, P4, T5, and T6 for monitoring the changes in the depth of anesthesia. A cortical strip electrode was placed on the cortical surface supplied by the artery that was possibly blocked during the operation, which was used to monitor the possible cortical ischemia. For patients who had cortical EEG suppression after the temporary occlusion of the parent arteries Were compared with the changes of scalp EEG. Whether there were ischemic events in the corresponding supply territory after vascular occlusion were observed after surgery. Results Of the 58 aneurysms, 40 aneurysms and 41 major arteries were occluded temporarily. After being occluded temporarily in 19 arteries of 18 patients, cortical EEG changed significantly, while scalp EEG did not change significantly. Only 9 patients had ischemic events in the corresponding supply territories after the occlusion in the cortical EEG significant change group. The changes in the depth of anesthesia had the consistent impact on cortical and scalp EEG. Conelusions Simultaneous monitoring of cortical and scalp EEG is a simple and effective method for monitoring cortical ischemia during anterior circulation intracranial aneurysm surgery, and may effectively identify the effect of anesthesia on EEG.
