1.The Secondary preventive effects and safety observation of warfarin for ischemic stroke

ObjectiveTo observe prophylactic effect and reliability of warfarin n preventing recurrant ischemic stroke.Methods168 patients identified as ischemic stroke admitted in Chaozhou Central Hospital from March 2001 to May 2004 were randomly divided into 2 groups,warfarin group and ticlopidine group (set as control).82 cases were in warfarin group,receiving warfarin 0.75~2.25mg/d orally.Prothrombin and international normalized radio (INR) were monitored,and INR was 1.5~2.0.86 cases were in ticlopidine group;they received 250mg/d ticlopidine orally.All patients were followed up every monty for 3 years,and recurrent ischemic stroke and drug side-effect were documented.ResultsThere were 8 cases (9.76%) of recurrence in warfarin group and 26 cases (30.02%) of recurrence in ticlopidine group.There was statistically significant difference between the two groups (P
2.A clinical study on ten-day sequential therapy for H.pylori eradication

Objective To observe the clinical curative effect of ten-day sequential therapy composed of omeprazole,amoxicillin,clarithromycin,levofloxacin in Helicobacter pylori (Hp) eradication.Methods 120 adult patients with positive rapid urease test of gastric mucosa were selected,they were randomly divided into the sequential group (60 cases) and control group (60 cases).The sequential group was treated with 10 d sequential therapy and control group was given 10 d standard therapy for eradication of Hp.In the two groups,patients with active ulcerative continued taking omeprazole for 4 weeks,the 14C urea breath test was taken after stopping drug,negative for Hp eradication.The clinical efficacy of two groups was observed.Results Hp eradication rate of sequential group was 94.8 %,which was significantly higher than 79.3 % of the control group (x2 =6.20,P < 0.05).The incidence rate of adverse reactions in the sequential group was 17.5%,which in control group was 15.0%,the difference was not significant (x2 =0.06,P > 0.05).Conclusion Ten-day sequential therapy composed of omeprazole,amoxicillin,clarithromycin,levofloxacin for eradication of Hp is safe,the cure rate is high,and the adverse reaction is similar with standard therapy,it is worth the clinical promotion.
3.Expression of stathmin and its influence to anti-microtube chemotheropy in non-small-cell lung cancer

Objective To investigate the expression of stathmin and evaluate its influence to anti-microtubule adjuvant chemotherapy in non-small-cell lung cancer(NSCLC).Methods The clinical data and survival status of 78 NSCLC patients were collected,and their paraffin-embedded tissue were detected immunohistochemically with a rabbit anti-human stathmin polyclonal antibody.The clinical significance of stathmin expression and its influence to overall survival rate were analyzed statistically between patients who received paclitaxel or vinblastine adjuvant chemotherapy.Results The positive expression of stathmin could only be observed in the cytoplasm of cancer cells.Among 78 patients,40 (51.3 % ) patients were stained stathmin-positive.The positive rate of stathmin was significantly higher in male than female,in central type than peripheral type,in pleura-involved than non-involved,and in dead patients than survival patients ( P < 0.05 ),but showed no significant differences in patients with different age,differentiated grade,pathological type,clinical stage,or lymph-node metastasis status.The expression of stathmin had a significant influence to overall survival rate(x2 =4.348,P <0.05 ),and those stathmin-negative patients showed a longer survival time.In stathmin-negative patients,those who received adjuvant chemotherapy with vinblastine exhibited a shorter survival time than those with paclitaxel,but the P =0.06.In stathmin-positive patients,the survival rate or time showed no difference between groups with paclitaxel and vinblastine.The differentiated grade,metastasis to lymph node and expression of stathmin were independent risk factors influencing survival rate.The positive expression of stathmin could only be observed in the cytoplasm of cancer cells.Among 78 patients,40 (51.3 % )patients were stained stathminpositive.The positive rate of stathmin was significantly higher in male than female,in central type than peripheral type,in pleura-involved than non-involved,and in dead patients than survival patients ( P < 0.05 ),but showed no significant differences in patients with different age,differentiated grade,pathological type,clinical stage,or lymph-node metastasis status.The expression of stathmin had a significant influence to overall survival rate(x2 =4.348,P < 0.05 ),and those stathmin-negative patients showed a longer survival time.In stathmin-negative patients,those who received adjuvant chemotherapy with vinblastine exhibited a shorter survival time than those with paclitaxel,but the P =0.06.In stathmin-positive patients,the survival rate or time showed no difference between groups with paclitaxel and vinblastine.The differentiated grade,metastasis to lymph node and expression of stathmin were independent risk factors influencing survival rate.Conclusion Our study suggested that the detection of stathmin in resected NSCLC tumor tissues may be helpful for prediction of prognosis,but helpless for making a choice between paclitaxel and vinblastine.NSCLC patients with stathmin-negative,no metastasis to lymph node or good-differentiated grade may have a better prognosis.
4.Study of curative effect and safety of low dose of urokinase combined with low molecular heparin calcim

ObjectiveTo study the curative effect and safety of low dose of urokinase (UK) combined with low molecular heparin calcium.MethodsSixty-four cases of sudden cardiac arrest patients were divided into treatment group and control group by random digits table with 32 cases each.Two groups were given cardiopulmonary resuscitation according to the 2005 international guide for cardiopulmonary resuscitation and emergency cardiovascular care.Early in the recovery,the patients in treatment group were pumped in vein with low dose of UK(200 000 U) and injected subcutaneous with low molecular heparin calcium (4100 U ) in 30 minutes.The rate of return of spontaneous circulation (ROSC),survival rate longer than 24 hours and 30 days in two groups and patients dying of bleeding or bleeding conditions in treatment group were observed.ResultsThere were 11 cases (34.4%) of ROSC in control group,and compared with 20 cases (62.5%) in treatment group,there was significant difference (P<0.05).There were 5 cases (15.6%) of survival longer than 24 hours in control group,and compared with 13 cases(40.6%) in treatment group,there was significant difference (P < 0.05 ).There were 2 cases (6.2%) of survival 30 days in control group,and compared with 8 cases (25.0%) in treatment group,there was significant difference (P< 0.01 ).Nobody had subcutaneous bleeding or other organ hemorrhage in control group.But there was I patient who had subcutaneous limited ecchymosis in the injection site in treatment group.The difference of fibrinogen before and after treatment in treatment group was statistically significant(P < 0.01 ),but there was no significant difference in prothrombin time and platelet count before and after treatment in treatment group (P > 0.05).ConclusionsIt is safe and effective in cardiopulmonary resuscitation with low dose of UK combined with low molecular heparin calcium.

Objective To assess the impact of miR-429 on lung adenocarcinoma cell SPC-A1 growth inhibition.Methods Pre-miRTM miR-429 precursor was synthesized and transfected to the SPC-A1 cells by liposome; qRT-PCR assay was used to quantify the miR-429 expression levels; The proliferation of SPC-A1 cells was evaluated by Cell Counting Kit-8 (CCK8).The cell apoptosis was evaluated by Annexin V Assay; The cell cycles of each group were assayed by flow cytometry;Western-blot was used to analyze the expression of cylines.Results The expression level of miR-429 was highly induced after transfection (P < 0.001) ;CCK-8 assay showed the cell proliferation activity of pre-miR-429 group was lower than that of blank and control group 48h and 72 h after transfection(P =0.0167,0.0383,P =0.0320,0.0465),whereas the apoptosis rate had no significant difference between pre-miR-429 and control 24h after transfection by Annexin V Assay(P > 0.05) ; The flow cytometry at 48h after transfection showed that miR-429 decreased the percentage of cells in G1 phase,but increased in S phase,indicating the cell cycle arrest at S phase(P =0.0010,0.0010 ; P =0.0068,0.0133) ; however,the expression level of Cyclin E in pre-miR-429 group had no difference compared with control.Conclusion miR-429 could inhibit cell proliferation and promote cell cycle arrest of lung adenocarcinoma cell SPC-A1.miR-429 may play a potential tumor suppressor role in lung adenocarcinoma cell SPC-A1.
6.Establishment of the Diagnostic Model in Solitary Pulmonary Nodule Appearing as Ground-glass Nodule

Purpose To explore the independent predictors of malignant solitary pulmonary nodule (SPN) manifesting as ground-glass nodule (GGN),and to establish a prediction model.Materials and Methods The clinical data and CT images of 362 patients (group A) with pathological-confirmed SPN appearing as GGN in Shanghai Chest Hospital Shanghai Jiaotong University from January 2014 to December 2015 were retrospectively analyzed.The independent predictors of malignant SPN were identified,and the clinical prediction model was established.Another 119 SPN patients in Affiliated Zhoushan Hospital of Wenzhou Medical University were selected as group B to verify the diagnostic efficiency of the prediction model.Results Using multivariate Logistic regression analysis,clear border (OR=6.274,P<0.01),smooth edge (OR=0.391,P<0.01),lobulation (OR=3.387,P<0.01),pleural retraction sign (OR=2.430,P<0.01),and vocule sign (OR=3.076,P<0.01)were identified as independent predictors of malignant SPN.The area of the model under the ROC curve was 0.859 with 95% CI (0.804-0.903).The diagnostic accuracy rate,sensitivity,specificity,positive predictive value and negative predictive value were 85.92%,91.03%,81.97%,92.03% and 73.53%,respectively.Conclusion In this study,the independent predictors of malignant SPN appearing as GGN were identified,and the prediction model was established.The model can accurately identify SPN and provide effective help for early diagnosis of SPN.
7.A randomized crossover study of elbow-forearm support and traditional shoulder sling in the treatment of shoulder subluxation after stroke

Objective:To compare the immediate effectiveness of elbow forearm support with that of a traditional shoulder sling in reducing glenohumeral subluxation (GHS) after a stroke.Methods:Eight stroke survivors with GHS were randomized to receive either 30 minutes of intervention of an elbow forearm support treatment or a traditional shoulder sling treatment twice within 24 hours. Their healthy and affected shoulders were X-rayed before and right after the treatment is ongoing as well as after the end of the 30 min of treatment. The vertical (VD) and horizontal (HD) distances from the lower edge of the acromion to the center of the humeral head were measured. The satisfaction of the patients and their relatives was surveyed.Results:The average VD and HD improved significantly more after wearing the elbow forearm support. Moreover, the patients and their relatives expressed greater satisfaction with the elbow forearm support.Conclusion:Either an elbow forearm support or a traditional shoulder sling will have an immediate effect in reducing shoulder subluxation, but the elbow forearm support is more effective and gives greater satisfaction.
8.In vitro toxicity of melamine against Tetrahymena pyriformis cells.

This study assessed the toxicity of melamine against the unicellular eukaryotic system of Tetrahymena (T.) pyriformis exposed to 0, 0.05, 0.25, 0.5, 2.5, and 5 mg/mL of melamine. Cell growth curves of different cultures, the half maximum inhibition concentration (IC50) value of melamine, and morphological changes in cells were obtained via optical and transmission electron microscopic observation. The effects of eleven melamine concentrations, 0.25, 0.5, 1, 1.5, 2, 2.5, 3, 3.5, 4, 4.5 and 5 mg/mL, on protein expression levels of T. pyriformis were examined using matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF-MS). The results showed an obvious inhibitory effect of melamine on the growth of eukaryotic cells. Cell growth dynamics indicated that the IC50 value of melamine on T. pyriformis was 0.82 mg/mL. The cellular morphology was also affected in a concentration-dependent manner, with characteristics of atrophy or cell damage developing in the presence of melamine. The relative contents of the top four main proteins corresponding to peak mass-to-charge ratios (m/z) of 4466, m/z 6455, m/z 6514, and m/z 7772 in the MALDI-TOF-MS spectra were all found to be closely correlated with the melamine concentrations. In conclusion, exposure of eukaryotic cells to melamine could inhibit cell growth, cause changes in cytomorphology and even disturb the expression of proteins in a concentration-dependent manner. The described method of examining four sensitive proteins affected by melamine was also proposed to be used in a preliminary study to identify protein biomarkers in T. pyriformis.

9.Study on relationship between blood supply from pulmonary artery and pathological characteristics of patients with primary bronchogenic carcinoma.

BACKGROUNDAt present, it has been known that the bronchogenic artery participates in the blood supply of primary bronchogenic carcinoma, but there is controversy about the blood supply from pulmonary artery in primary bronchogenic carcinoma. The aim of this study is to assess the relationship between the blood supply from pulmonary artery and pathological characteristis of patients with primary bronchogenic carcinoma.
METHODSThe pulmonary arteries in 43 surgical samples of bronchogenic carcinoma were marked, then the iopromide was used to selective pulmonary arteriography in digital subtraction angiography (DSA). The relationship between tumor with blood supply from pulmonary artery and the pathologic characteristics was observed.
RESULTSThere were 34 samples with blood supply from pulmonary artery ( 79.07%) , and 9 samples without blood supply from pulmonary artery (20.93%). The development rate of peripheral lung cancer (100.00%) was significantly higher than that of central lung cancer (64.00%) (P < 0.01) . The development rate of squamous cell carcinoma (91.30%) was remarkably higher than that of adenocarcinoma (61.11%) (P < 0.05). The development rate of poorly differentiated lung cancer (95.00%) was remarkably higher than that of well and moderately differentiated lung cancer (65.22%) (P < 0.05). There was a positive relationship between the tumor size and the development rate (P < 0.05).
CONCLUSIONSIn primary bronchogenic carcinoma, the pulmonary artery blood supply exists in most of tumors. There is relationship between the blood supply from pulmonary artery and general type, histopathology, cell differentiation and tumor size of lung cancer. The blood supply from pulmonary artery doesn't relate to tumor stage.
10.Analysis of the correlation between thyroid function and cognitive function ients with subcortical arteriosclerotic encephalopathy

Objective To explore the correlation between thyroid function and cognitive ability in patients with subcortical arteriosclerotic encephalopathy.Methods Montreal cognitive assessment scale (Montreal Cognitive Assessment,MoCA) was used to evaluate the cognitive function of SAE patients with normal thyroid function and hypothyroidism.And the difference in the risk factors of SAE cognitive ability in patients with different thyroid functions was analyzed.Results There were no significant differences in gender,age,risk factors of hypertension,cerebral infarction,diabetes and smoking between the two groups (P>0.05).The levels of FT3,FT4,TSH,MoCA and ADL in patients with hypothyroidism were (2.92±0.35) pmol/L,(15.61±2.76) pmol/L,(13.05± 1.64) mU/L,(12.73±5.75) points,(45.64±25.77) points,respectively.The levels of FT3,FT4 and TSH in normal thyroid function group and MoCA,ADL scores were (4.27±0.55)pmol/L,(16.74 ± 2.35) pmol/L,(2.73 ± 0.38) mU/L,(18.15 ± 5.35) points,(62.17 ± 26.72) points,respectively.The data of the hypothyroidism group was significantly lower than thst in the normal thyroid function group,and the difference was statistically significant (t =3.591,3.012,12.753,8.967,15.442,P<0.05).FT3 levels in SAE patients were positively correlated with MoCA score and ADL score (r=0.518,0.617,P=0.026,0.018),while there was no significant correlation between FT4 level and MoCA and ADL score (r =0.015,0.007,P =0.852,0.074).MoCA score and ADL score were negatively correlated with TSH level (r=-0.651,-0.582,P=0.016,0.005).Conclusion Thyroid function is significantly correlated with cognitive ability of SAE patients.Thyroid function may be a clinical index for patients with subcortical arteriosclerosis,and provide a basis for rational drug use in patients with subcortical arteriosclerosis.
