1.Apoptosis in cardiac ailograft and its relation with acute rejection in rats

Objectives To detemline whether apoptotic cell death is involved in rat cardiac allograft rejection and investigate the relevance of apoptosis with acute rejection and its implication.Methods Groups of Wistar rats underwent heterotopic heart transplantation from allogeneic SD or syngeneic Wistar rats.The cardiac grafts were harvested at 1,3,5,or 7 days after transplantation and underwent the detection of apoptotic cell death using in situ terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate nick end labeling(TUNEL).Histopathological rejeclion grade and apoptotic index(AI)were analyzed.Results The incidence of apoptotic cells was increased steadily over time in allografts,in contrast to syngeneic grafts.The apoptotic cells in allografts were mainly cardiac myocytes and few infiltrating lymphocytes.The AI of rejection grade 1,2,3 and 4 was significantly higher than that of rejection grade 0(P<0.01).Conclusions TUNEL can display apoptosis of single cell in situ.Apoptosis is an important mechanism of tissue injury in acute cardiac allograft rejection in rats.Myocyte apoptosis can be used as a valuable index to estimate the injury of grafts and monitor acute rejection.
2.Effect of Cyclosporin A to T Lymphocyte Subsets and Toxoplasmosis After Heart Allotransplantation in Rat

Objective To evaluate the effect of Toxoplasma gondii infection on immune function of the rat recipients and onset of Toxoplasmosis after heart transplantation and its correlation with the use of Cyclosporin A(CsA). Methods ELISA was used to detect recipient's specific circulating antigen (CAg) and antibodies (IgG, IgM) after the transplantation. T lymphocyte subsets in peripheral blood were examined by using immunofluorescence stain and flow cytometry (FCM) before and after heart allograft 5,10,15,20 days in rats. Results The use of CsA increased the risk of infection by T. gondii and accelerated the increase of CD8 + T lymphocyte after the transplantation. The incidence of donor acquired T.gondii infection was higher than that of reactivated silent infection in recipients before operation. The percentage of CD8 + T lymphocyte was evidently elevated due to the onset of toxoplasmosis and the ratio CD4 +/CD8 + was reduced or inverted in the meanwhile. Conclusion The immune suppression after use of CsA was the main reason leading to an activation of the silent infection of T.gondii . CD8 + was the main cytotoxic cell elevated during the infection.

Objective To study the threshold effect of government health expenditure on residents' health.Methods Using threshold panel data model to perform the empirical research.Results There is threshold effect of government health expenditure on residents' health.There are two threshold values.As the increase of the proportion of the government health expenditure to the total fiscal expenditure,it has two changes of the impact on the health of residents.Conclusion The government should increase the health expenditure,and increase the proportion of the government health expenditure to the total fiscal expenditure,but the government should not blindly pursue the high input of health expenditure,focus on the efficiency of health expenditure,and avoid of resource waste.
4.Relationship between Th17 cell number and Helicobacter pylori-infected chronic gastritis in children

Objective To explore the relationship between peripheral Th17 cell number and chronic gastritis in Helicobacter pylori(H.pylori)-infected children.Methods Children were diagnosed as chronic gastritis by endoscopy.The degree and activity of inflammation were graded by histopathology examinations.The patients with both 13C urea breath test and urease test positive were diagnosed as H.pylori infection.The peripheral Th17 cell number was measured by flow cytometry and expressed as a ratio to total T cell.Results The Th17 cell number in HP group (chronic gastritis with H.pylori infection,n =33),non-HP group (chronic gastritis without H.pylori infection,n =24) and normal controls (n =15) were (1.55 ±0.30)%,(1.06 ±0.33)%,and (1.04 ±0.35)%,respectively.HP group included a statistically higher Th17 cell number than the other groups (all P < 0.05),while no obvious difference was found between non-HP group and controls (P > 0.05).According to the degree of inflammation,the chronic gastritis with H.pylori infection was categorized into non-apparent (n =10),mild (n =8),moderate (n =9) and severe (n =6) subgroups.The Th17 cell number in each subgroup was (1.64 ± 0.21)% (non-apparent),(1.61 ± 0.23)%(mild),(1.25 ± 0.29) % (moderate) and (1.75 ± 0.20) % (severe),respectively.The moderate group had a lowest Th17 cell number among 4 groups (P < 0.05).And significant differences did not exit in the other 3 groups (P > 0.05).The HP group patients with different inflammatory activity had a Th17 cell number of (1.23 ±0.25)% in nonapparent (n=15),(1.53 ±0.15)% in mild (n=6),(1.55 ±0.32)% in moderate (n=6) and (1.71 ±0.35)% in severe (n =6) subgroup,respectively.However,there were no significant differences among 4 subgroups (P > 0.05).Conclusions In the progress of chronic gastritis with H.pylori infection,Th17 cells may play a role as a double-edged sword by protecting and fighting against H.pylori infection and immunopathologic insults.This would provide more insights into the treatment of H.pylori infection.
5.An observation on preprocedural white blood cell count and long-term patency of stent implanted in subclavian artery

Objective To study relationship between preprocedural white blood cell count(WBC) and long-term patency of stent implanted in the subelavian artery.Methods Data of 113 cases with stents implanted in the atherosclerotic SUbclavian arteries in Anzhen Hospital,Beijing during January 2005 to January 2008 were analyzed.Student t-test was used to compare the difierence in WBC count between those with patency and those with stenosis after the procedure and chi-square test was used to compare the difference in stent pateney between those with higher and lower WBC counts.Results There Was significant difference in WBC counts between 102 cases with patency and 11 cases with stenosis of the stents after implantation(t=-5.051.P<0.001).Percentage of patency ofthe stents in 36 cases witll higher WBC count(>7.195×109/L)was significantly higher than that in 77 cases with lower WBC count(≤7.195× 109/L)before the procedure(P=0.001).Among 113 cases with stents implanted,109 cases(96.5%) could keep their stents patent one year after implantation,and 60 cases(89.6%)could keep stents patent three yeats after implantation.Conclusions In addition to those traditional factors associated with long-term patency of stents after their implantation,inereased WBC count alSO associated with stenosis or occlusion of the stents after the procedure.
6.The effect of high dose albumin on permeability of blood-brain barrier in brain of rats after ischemic-reperfusion

Objective To determine the effect of high dose albumin on permeability of blood brain barrier (BBB) in brain of rats after ischemic-reperfusion (IR) in order to explore its possible mechanism.Methods Establishment of brain ischemic reperfusion rat model by using middle cerebral artery occlusion (MCAO).Medicine treatment was given by caudal vein injection after 2 hours of MCAO.Thirty-six healthy male SD rats were then randomly (random number) divided into 6 groups (n =6 in each):6 h and 24 h sham-operation groups (Group Sham:operation without ischemia),6 h and 24 h normal saline groups (Group NS:NS injection 5 ml/kg) and 6 h and 24 h albumin group (Group Alb:25 % Alb injection 1.25 g/kg).Six hours and 24 hours after the end of reperfusion,rats were measured by Zea-Longa score (neural function deficit) separately.Serum concentration of S100B was examined by the ELISA kit and Evans blue in brain tissue was detected by spectrophotometer.The level of AQP4 was examined by Western blot and immunohistochemistry.All data were analyzed by one-way analysis of variance (ANOVA),The intergroup comparisons were analyzed by the least-significant-difference (LSD) test by using SPSS version 17.0 software.Differences were considered statistically significant if P < 0.05.Results Zea-Longa score significantly increased in both group NS and group Alb at 6 h and 24 h (P =0.000).However,there was no significant difference in ZEA-LONGA score of 6 h and 24 h between group Alb and group NS (P =1.000).The serum concentration of S100B in group NS 6 h was significantly lower than that in group Alb at 6h (196.67±20.11 vs 160.04±14.00,P=0.000),and at24h (2.45±0.07 vs.2.23±0.07,P=0.000).Furthermore,concentration of Evans blue in brain tissue in group Alb was significantly higher than that in group NS at both 6 h (0.97 ± 0.08 vs.0.74 ± 0.06,P =0.000) and 24 h (2.45 ± 0.07 vs.2.23 ± 0.07,P =0.000).The expression of AQP4 in brain tissue was higher in group Alb than that in group NS at both 6 h (0.72 ±.0.11 vs.0.57 ± 0.06,P < 0.01) and 24 h (0.80 ± 0.03 vs 0.61 ± 0.02,P <0.01).Conclusions High dose albumin contribute slightly in improvement of neural deficit in rats after IR.On the contrary,it can also aggravate the IR injury,which increases brain edema then increase the permeability of BBB.The mechanism may be associated with over-expression of AQP4 in brain tissue.
7.Internal fixation for unstable scapular fracture.

OBJECTIVETo investigate surgical methods and effects of unstable scapular fracture.
METHODSFrom October 2008 to August 2011, 14 patients with unstable scapular fracture were treated. There were 12 males and 2 females with an average age of 38.7 years (ranging, 21 to 55 years).The time from injury to hospitalization was 1 hour to 10 days (mean 3 days). Among patients,3 patients had scapular body fracture,4 patients had scapular neck and body fracture,4 patients had scapular neck and acromion fracture,2 patients had glenoid cavity and coracoid fracture, 1 patient with scapular spine and coracoid fracture. All patients were treated by operation, Hardegger classification system was applied to evaluate the function.
RESULTSAll patients were followed up for 2 to 36 months (mean 12.4 months). The healing time ranged from 6 to 8 weeks. No infection and internal fixation failure occurred. The effect result of Hardegger scoring system was excellent in 9 cases, good in 3 cases, fair in 2 cases.
CONCLUSIONSurgical treatment for unstable scapular fracture can reconstruct stability of shoulder joint, promote early rehabilitation of joint function, and maximally restore shoulder function.

8.Isolation and purification of natural tissue inhibitor of metalloproteinase-3 from human placenta

BACKGROUND: Matrix metalloproteinase (MMP) is a kind of protease family, its activity can be inhibited by tissue inhibitor of metalloproteinase (TIMP), especially by TIMP-3.OBJECTIVE: To fully isolate and purify natural TIMP-3, and to create enzyme-linked immunoassay of TIMP-3.DESIGN: Single-sample observation SETTING: Central Laboratory of Shenyang Medical College MATERIALS: Human placenta from Department of Obstetrics & Gynecology, Fengtian Hospital Affiliated to Shenyang Medical College (Informed consent was obtained from the relatives of patients) and MMP-1 from Research Institute. Fuji Chemical Industries, Ltd.METHODS: This experiment was conducted in the Central Laboratory of Shenyang Medical College between March 2001 and May 2002. Firstly,4 mol/L urea Tris-buffer solution (pH8.0) was used to prepare homogenate solution of placenta. Secondly, homogenate solution was performed chromatography through CM52 positive ion-exchange resin and Sephacryl S200 gel filtration. Thirdly, relative molecular weight and purity were detected by SDS- polyacrylamidedel gel electrophoresis (SDS-PAGE).Fourthly, Western blotting was used to identify the characters of purified protein. Fifthly, the inhibitory rate of TIMP-3 to MMP-1 was measured with immumofluorescence method.MAIN OUTCOME MEASURES: ① The relative molecular mass of protein showed by PAGE. ② Results of Western blot. ③ The inhibitory rate of TIMP-3 for MMP-1. ④ The recovery rate of TIMP-3 following purification.RESULTS: ①The isolated and purified TIMP-3 from human placenta consisted of non-glycosylated and glycosylated protein, with relative molecular mass of 24 000 and 27 000, respectively. ② The inhibitory concentration of non-glycosylated and glycosylated TIMP-3 was 1.1×1010 mol/L and 1.2×1010 mol/L respectively to MMP-1. The inhibitory concentration of placenta-derived TIMP-3 was significantly higher than that of recombinant TIMP-3 for MMP-1. ③ The recovery rate of TIMP-3 was 23.4% following two-step chromatography.CONCLUSION: Extracellular matrix of human placenta-derived TIMP-3 consists of non- glycosylated protein and glycosylated protein; Two kinds of purified TIMPs-3 have remarkable inhibitory concentration for MMP-1, and significantly higher in comparison with recombinant metalloproteinase inhibitory factor-3.
9.Value of noneinvasive vascular examinations in diagnosing deep venous thrombosis

Objective To evaluate the different value of color Doppler ultrnsonography and Doppler vascular examinations in diagnosis for deep venous thrombosis(DVT)in the lower extremities.Methotis Imaging of color Doppler ultrasound scanning was employed as diagnostic criteria for DVT on 178 lower extremities of 146 suspected patients,as compared to the result by Doppler vascular examinations.Results Color Doppler ultrasonograph showed hish accuracy in diagnosis for DVT,as compared to that by Doppler vascular examination with 97.9 percent(142/145)positive for the femoral and popliteal veins and relatively lower positive diagnostic vallie for thrombosis in the inferior vena cava,iliac vein,anterior tibial vein,posterior tibial vein and calf veins.Conclusions Color Doppler ultrasonography is superior to Doppler vascular examination in determining DVT of the lower extremities and can be used as a main diagnostic method for it.Doppler vasculiar examination can be used as an initial screening method for DVT and deep venous angiography should not be used as a routine diagnostic measure for it.
10.Comparison of color Doppler ultrasonogarphy and Doppler ultrasound for the diagnosis of lower extremity deep venous thrombosis

Objective To evaluate the diagnostic role of color Doppler ultrasonogarphy(CDU),Doppler ultrasound(DU)in the diagnosis of lower extremity deep venous thrombosis(DVT). Methods In this study,84 patients(92 lower extremities)of lower extremity DVT were underwent CDU and DU and lower extremity deep venous angiography respectively. Results Total consistent rate,sensitivity,specificity,omission diagnostic rate,mistake diagnostic rate,Youden index,Odd product.positive predictive value,negative predictive value and Kappa of CDU in diagnosing lower extremity DVT was respectively 96.7%,95.7%,97.8%,4.3%,2.2%,0.935,990.0,97.8%.95.7%and 0.935(P=0.037).As Kappa of CDU(0.935)>0.75 and its P(0.037)<0.05.CDU can theoreticallv substitute for deep venous angiography;Above-mentioned indexes of DU were respectively 89.1%,87.2%,91.1%,12.8%,8.9%,0.783,70.0,91.1%,87.2%and 0.783(P=0.065). Conclusion CDU iS becoming preferred and more reliable noninvasive method in diagnosing lower extremitv DVT.
