Chinese Journal of Radiology 1999;0(10):-
Objective To evaluate the clin ical value of Multi-slice helical CT three dimensional angiography (3D-MSCTA) as first method for diagnosing intracr anial aneurysms. Methods We studied patients with clinical suspected intracranial aneurysms (13 patients with subarachnoid hemorrhage among cases). All these patients under went 3D-MSCTA and Digital Subtraction Angiography (DSA), 16 patients of them accepte d operation treatment. Row data was acquired by Multi-slice helical CT-AQUILION (Toshiba): scan speed 0.5 s/rot, image slice thickness 1.0 mm, helical pitch 3 .0/5.0. Contrast media (Angiografin) was injected intravenously (1.0-2.0 ml/kg) at spee d of 2.5-3.0 ml/s, delay time was 15-23 sec, reconstruction interval 0.5 mm, recons truction slice thickness 1.0 mm. Source images were processed using a workstation SGI-O2 , images post-processing software was ALATOVIEW,Version 1.21. The reconstructed images were then processed into shaded volume rendering (SVR) and maximal intensity projection (M IP) and Fly-through images. Entire brain DSA was performed obtaining anterioposteri or, lateral, and oblique images. Images of 3D-MSCTA and DSA were analysed by 3 radiologists and 2 neurosurgeons. Results 25 aneurysms were d etected by 3D-MSCTA. Aneurysms′s body, neck, source vessel and the relationship between the aneurysm and surrounding structures was clearly and surely displayed. 22 of 25 aneurysms were detected by DSA,another 3 were (1 anterior communicating artery aneurysm and 2 left middle cerebral artery aneurysm) was not detected. Sixteen of patients un derwent operation treatment, and the results of 3D-MSCTA corresponded very well to those of operation. Maximal diameter of aneurysms body was 14.0 mm and minimal diameter w as 1.7 mm. Conclusion 3D-MSCTA is a high sensitivity and rapid and no ninvasive method for detecting intracranial aneurysms. We suggest that 3D-MSCTA may be the first cho ice for diagnosing intracranial aneurysms.

Journal of Acupuncture and Tuina Science 2011;09(2):79-83
Objective: To observe the effect of preoperative effect on peri-operative pain in patients following a thoracotomy. Methods: 120 cases following lung-cancer thoracotomy were randomly allocated into four groups, 30 in each group. Cases in group A and B were treated with acupuncture analgesia 3 d before operation; cases in group A and C were treated with acupuncture analgesia after operation; and cases in group D were treated with general anesthesia. The pain management indexes in four groups were all controlled below 3. After that, analgesia-related β-endorphin and stress-related cortisol were observed before and after operation. In addition, the specific doses of postoperative analgesic-Fentanyl in four groups were compared. Results: The comparison of β-endorphin between group A, C and D showed P<0.05 one day before operation, so did group B, C and D 1 day before operation. The intra-group comparison of cortisol between the day of admission and 1 day after extubation and between 1 day before operation and one day after extubation in group A, B and D showed P<0.05, so did group C between the day of admission and 1 day after extubation. In addition, the contents of Fentanyl in postoperative analgesic pump in four groups showed P<0.05 through one-factor analysis of variance, showing a significant difference. Conclusion: Preemptive analgesia could increase the β-endorphin in patients following a thoracotomy and showed remarkable advantage when compared with the conventional postoperative analgesia. It did not cause significant difference regarding stress index cortisol. Acupuncture has no remarkable advantage when compared with operation and extubation for the major immediate stress. Additionally, postoperative acupuncture could be a substitute for the dose of pain killers and the match can be reduced by 20%.

Objective To investigate the effects of castration treatment by luteinizing hormonereleasing hormone(LHRH)analog(goserelin)on bone loss and bone resorption in men with prostate cancer. Methods Serum sex hormones, bone mineral density(BMD),serum and urine concentrations of bone turnover markers were determined in men with prostate cancer(n=36) and in age-matched controls(n=13).BMD in the lumbar spine(L2-4),femoral neck, trochanter,Ward's triangle and total femoral was determined by dual energy X-ray absorptiometry(DEXA).Results After 12 months of LHTH analog therapy, the BMD of the femoral neck, total femoral and Ward's triangle decreased significantly in men with prostate cancer compared with the controls, all P<0.05. No significant bone loss was observed in the control subjects. The concentrations of the serum markers of bone formation,i.e.bone alkaline phosphatase(BALP),and urine markers of bone resorption,i. e.DPD,Cros,Ca/Cr were significantly increased in patients treated with LHRH analog compared with control subjects, all P<0.05,respectively.Conclusions These findings demonstrate that a significant increase of bone turnover and loss of bone mass in men with prostate cancer after receiving LHRH analog therapy were correlated with the decreased levels of serum androgen and estrogen, and suggest that measuring BMD by DEXA and assesing the bone turnover markers can early detect the bone loss and osteoporosit induced by goserelin castration treatment.

Postoperative pancreatic leakage remains a persistent problem after pancreaticoduodenectomy. For patients with a soft and nonfibrotic pancreas, double binding continuous hemstitch suture is an optimal method for anastomosis. From January 2011 to June 2012,92 cases of periampullary carcinoma with a soft pancreas underwent pancreaticoduodenectomy, and then a modified technique of pancreaticogastrostomy was performed with 2 continuous hemstitch sutures placed in the mucosal and seromuscular layers of the posterior gastric wall, respectively. The median time for pancreaticogastrostomy was 12 minutes, and only 15 patients had postoperative complications. Two patients developed pancreatic leakage(1 grade A and 1 grade B)postoperatively.Pancreaticogastrostomy with double binding continuous hemstitch sutures is a simple and safe reconstruction procedure for patients with a soft and fragile pancreas who received pancreaticoduodenectomy.
