Objective:To evaluate the efficacy of paroxetine on functional low fever. Methods:Using a randomized and placebo controlled trail, a total of 54 with functional low fever were randomized into paroxetine group and placebo group. In paroxetine group, 28 patients received oral paroxetine 1 tablet (20mg/tablet), one times a day for 8 weeks. There were 26 patients in the placebo. The criteria of assessing the therapeutic efficacy on functional low fever and Hamilton Depression Scale (HAMD; 17 items) were used to assess the therapeutic efficacy at the 4th and 8th weekend respectively.Results:After 8 weeks of treatment, the efficacy rate on functional low fever and antidepression were 78.6%, 82.1% in paroxetine group and 26.9%, 23.1% in the placebo group respectively. OBJECTIVE: To investigate the clinical feature,diagnostic and therapeutic methods of ultimobranchial fistula and cyst of thyroid. METHOD: Four cases of ultimobranchial fistula and cyst of thyroid in our hospital were reported and the relevant literatures were reviewed. RESULT: The branchial fistula and cyst of four cases were adhered to thyroid,with unclear anatomical landmarks, the function of recurrent laryngeal nerve were bad before the operation in two cases. The nerve of four cases were retained, the function of thyriod and parathyroid gland were normal and no recurrence were found. CONCLUSION Ultimobranchial fistula and cyst of thyroid is rare and is easier to be misdiagnosed and wrong treated. Surgical excision is effective for it and recurrent laryngeal nerve should be protected when performing the operation.

Objective To investigate circulating betatrophin levels in non-alcoholic fatty liver disease patients (NAFLD). To explore its association with various metabolic parameters in Chinese subjects. Methods From August 2014 to April 2015 , 58 patients with NAFLD and 41 non-NAFLD control subjects with age and sex matched were enrolled in the Department of Endocrinology of Zhujiang Hospital. Results In NAFLD group, serum betatrophin concentrations significantly increased (781.96 vs. 431.89 pg/mL, P < 0.05) and associated only with HDL-cholesterol level. In normal group, betetrophin levels are associated with multiple metabolic parameters, such as fasting glucose, insulin, C peptide, HOMA-IR, QUICK index and HDL-C levels. Conclusions Serum betatrophin concentration significantly elevated in non-alcoholic fatty liver disease patients and was negatively correlated with HDL-C level (r = -0.479, P < 0.001).

Objective To investigate the association between chronotropic incompetence (CI)and motor ability in type 2 diabetic mellitus (T2DM) patients complicated with metabolic syndrome (MS).Methods One hundred and twenty patients with T2DM were enrolled in the study,including 66 cases with MS (MS group) and 54 without MS (non-MS group).The electrocardiographic exercise test was performed,the CI indexes including ratio of heart rate (rHR),heart rate reserve (HRR)were measured and the max metabolic equivalents(maxMETs) were calculated.Result The rHR(0.77 ±0.04 vs.0.81 ±0.06,t =3.41,P ＜ 0.05),HRR (0.62 ± 0.05 vs.0.70 ± 0.07,t =5.96,P ＜ 0.05) and maxMETs (8.8 ± 1.1 vs.9.7 ± 1.2,t =0.16,P ＜ 0.05) in patients of MS group all lower than those in non-MS group.The maxMETs were positively correlated with rHR (r =0.81,P ＜ 0.05) and HRR (r =0.78,P ＜ 0.05).Conclusion The results indicate that CI as well as maxMETs and rating of perceived exertion can be used as monitoring index in type 2 diabetic patients complicated with metabolic syndrome.

Objective To study the effect of combined transplantation of pancreatic islets and hepatocytes on the treatment of diabetic rats and the possibility on hepatocytes inducing islet allograft tolerance. Methods Forty diabetic SD rats were randomly divided into 4 groups: normal control group,diabetic control group, pancreatic islet grafted group,and hepatocyte and pancreatic islet transplanted group. The pancreatic islets of fetal SD rats were implanted into the gastric subserosa of recipients. The alteration of diabetes and immune status were observed. Results There were 58.8% (20/34) diabetic rats cured after pancreatic islets transplantation. A new method based on adherence of cellulolytic bacteria to insoluble cellulose for isolation and purification of thermophilic cellulolytic anaerobes was reported, in which Hungate anaerobic operating techniques were used to roll tubes with insoluble cellulose powder as substrate.

Objective To develop the characteristic ESI-MS spectra of five Bulbus Fritillariae species and to be used for identification of Bulbus Fritillariae species.Methods The Bulbus Fritillariae medicinal materials were dipped in 18% ammonia solution,then the alkaloids were extracted by ultrasonic using the mixed solvent of ether-chloroform-ethanol(25∶8∶2.5).The extracts were analyzed directly by ESI-MS in positive ion mode.Results Bulbus alkaloids easily formed molecular ions(~+) in MS and dimerized easily in alcohol solution,which did not dimerize in acidity solution.Collision induced dissociation(S-CID) made these alkaloids easily lose a molecule of H_2O.The MS spectra of the extracts of five Bulbus Fritillariae species indicated that the major molecular ions were similar to each other,but different varieties showed very different relative abundances of these molecular ions.Conclusion The total alkaloid extracts of five Bulbus Fritillariae species have strong characteristic ESI-MS spectra and these spectra are of good reproducibility and precision,respectively.These characteristic spectra are useful for the identification of Bulbus Fritillariae species.

The relation between patent applications and published papers in hospitals of Guangdong Province since 2000 on China Intellectual Property Network was analyzed , which showed that the quality and quantity of patents were lower in hospitals of Guangdong Province than in those of Shanghai and Beijing , the quality of patents was re-lated with the medical service level in hospitals, the patent applications were centralized in large class A hospitals of Guangzhou , and inventive patents were those of medical preparations .Suggestions were thus put forward for hos-pitals of Guangdong Province to improve their awareness of patent application, the quality of patents, transformation of patents, cooperation with patent service organizations, research of patent application, and balance of patent dis-tribution in different areas.

Objective To investigate the efficacy and safety of Solifenacin for the treatment of overactive bladder in children. Methods A retrospective analysis was done in a children admitted to our hospital in January 2010,with lower urinary tract obstruction lead to overactive bladder.The boy was 4 years old,suffering from difficult urinating,urinary frequent,urgency and incontinence for 4 years.Imaging studies (B ultrasound,intravenous urography,CT,bladder voiding urethrography) found bilateral hydronephrosis and kidney collecting system separation with left 3.3 cm and right 2.3 cm in diameter; ureteral dilation with left 1.3 cm,right 1.6 cm in diameter.A transverse septum crossing the urethra at a right angle distal to the verumontanum was detected.According to history,physical examination and imaging examination,a diagnosis for posterior urethral valves was established.Under general anesthesia,urethroscopy found that the valve was attached below urethra wall,divided into two forks towards urethral lateral wall.The valve was excised.Three months later,although good voiding,the hydronephrosis and hydroureter still did not improve.After cystostomy and 6 months follow-up,hydronephrosis and hydroureter still not improved. B ultrasound showed the renal pelvis dilation with left 3.2 cm,right 2.0 cm in diameter,and bilateral ureteral dilation with left 1.3 cm,right 1.4 cm in diameter.Solifenacin (5 mg/tablet),1/3 tablet,about 1.7 mg per day was added. Results After oral Solifenacin one month,hydronephrosis and hydroureter began to improve.After 3 months,hydroureter disappeared; hydronephrosis improved significantly.. B-ultrasound showed the renal collecting system mild dilation with left 1.1 cm,right 1.0 cm in diameter.After 13 months follow-up,no side effects was detected. Conclusions Combined with the surgical treatment,Solifenacin can alleviate hydronephrosis secondary to OAB,and can also protect the renal function.

High flow nasal cannula oxygen therapy (HFNC) is a new type of oxygen therapy. Because of its unique physiological effects, the clinical application is becoming more and more popular. The article summarized HFNC has obvious advantages in oxygenation and comfort. In the clinical, HFNC is widely used in adult patients with acute hypoxemic respiratory failure, respiratory failure after the operation or extubation and respiratory tract invasive operation. But there is no literature reports in improving ventilation to reduce carbon dioxide. If patients′ respiratory, cycle and oxygenation show no significant improvement after application of HFNC for a short time, it should be replaced as soon as possible for the noninvasive and invasive mechanical ventilation. In this article, the characteristic and application of HFNC will be reviewed.
