1.The analysis of application about MRI and CT examination of lacunar cerebral infarction

Objective:To observe the diagnosis effect about the MRI and CT examination for the patients with lacunar cerebral infarction(LAC).Methods: 82 cases early LAC patients during April 2014- June 2016 were diagnosed by two methods, MRI and CT, respectively. And the application effect of the two methods were compared based on image result.Results: In 82 cases of LAC 742 lesions confirmed by MRI and only 145 lesions confirmed by CT, and there was statistical significant between the two methods; especially in front lobe and thalamic, lesions detection rate using MRI was higher than CT, and there was statistical significant between the two methods(x2=6.59,x2=5.64,x2=6.42;P<0.05); the difference of detection rate in capsula internal also was statistical significant(x2=7.43,P<0.05); the number of lesion diameter less than 5mm was 256 using MRI and it was 3 using CT, the difference also was statistical significant(x2=6.39,P<0.05).However, in parietal lobe, basal ganglia and brainstem, all of the difference were not statistical significant (x2=0.18,x2=1.25,x2=0.81;P>0.05);Conclusion: Both of CT and MRI can be used in early diagnosis of LAC, and MRI examination is more accurate for early or micro lesions and lesions happened in the frontal lobe, thalamus, capsula internal than CT. Therefore, MRI can be used as first choice eximination method in early diagnosis of LAC.
2.The study of proper depth of muscle relaxant in posterior fossa surgery under total intravenous anesthesia

Objective To investigate the proper depth of muscle relaxant without affecting elec-trophysiological monitoring in patients undergoing posterior fossa surgery under total intravenous an-esthesia (TIVA).Methods Forty patients selected for posterior fossa surgery were randomly divided into two groups A and B.Group A:no muscle relaxants were administered after anesthesia induction until the EMG was completed.Group B:muscle relaxants maintained in range of TOF 2%-25% dur-ing electrophysiological monitor.TIVA were used to all patients and BIS was maintained in range of 40-60.Two electrophysiological data were applied during the electrophysiological monitoring:sponta-neity EMG and evoked EMG.EMG amplitude was recorded at different TOF value.Operation time and the dosages of propofol,sufentanil,rocuronium bromide were recorded.Also,the SBP,DBP,HR at perioperational period and facial nerve function before surgery,one week and six months after sur-gery were recorded.Results The electrophysiological monitoring completed successfully in all pa-tients.The total amount of propofol in group A significantly increased than those in group B(P <0.05).Three patients in group A had body movement during the operation.Four cases in group B couldn??t perform electrophysiological monitor when the TOF had three signals.There were no differ-ence between the two groups in SBP,DBP,HR and facial nerve function.Conclusion Maintaining TOF value in range of 2%-25% under non-depolarizing muscle relaxant during CPA surgery doesn??t affect intraoperative nerve electrophysiological monitor and prevent body movement,total dose of sed-ative anesthetics is also decresed.
3.Practice and Experience of Government Hospital Transforming the Mode of Economic Growth

Transforming economic growth mode is one of the most important guiding ideologies of the National twelfth Five-year development program. Since governmental hospitals are non-profit institutional organizations, it is a new topic for hospital managers to transform economic growth. Current situation of governmental hospitals is analyzed and the three main misunderstandings are pointed out. Take the Taian central hospital as the main object and analyzed the practice process of “putting hospital development scale control and revenue growth as the base and premise, shortening the average hospital stay, reducing the medicine proportion and consumable items proportion as the main means, and usd charging level, patients personal commitment to the cost proportion as important evaluation indicators of transformation economic growth mode” . By analyzing the practice process, the experience of transformation of economic growth mode is concluded, which provides some references for transforming economic growth mode in governmental hospitals.
4.Anti-inflammatory Effect of Apigenin on Microglia after Oxygen Glucose Deprivation and Reperfusion

Objective To study the neuroprotective effect of apigenin on microglia which was exposed to oxygen glucose deprivation and reperfusion (OGD/R), which was characterized by its influence on IL-1? and TNF-? expression. Methods Primary microglial cultures were prepared from newborn rat brain. The purity of isolated cells were identified by GSA-IB4. The cells were randomized into 5 groups:normal group, DMSO group and apigenin-treated groups (10, 25, 50 ?mol/L). The cells of DMSO group and apigenin-treated group were exposed to 8 h of OGD and 24 h of reperfusion in the presence or absence of apigenin at a range of concentrations. 5.Study of anti-lipid A antibodies of bacterial endotoxin from phage antibody library

Objective To study the treatment of sepsis caused by G - bacteria, anti-lipid A antibodies of bacterial endotoxin were screened from phage antibody library. Methods The mRNA was extracted from human B-lymphocytes against lipid A of bacterial endotoxin, reversely transcripted and amplified by polymerase chain reaction using general primers scanning Fd and light chain of IgG. The amplified fragments were inserted into pCOMB3 vector and electrotransfected competent E.coli XL 1-blue cells. Furthermore, the recombinant phage was lysed by coculture with helper VCSM13. Results Fab displayed on the surface as fusion protein with the N terminal of coat protein Ⅲ, and 4.8?10 6 clone library was established. Antibodies against lipid A of bacterial endotoxin were screened. Specific antibodies against lipid A of bacterial endotoxin were enriched by 100 times after three rounds of panning with lipid A.Conclusions Three clones exhibited specific binding to lipid A is identified by direct and competitive ELISA methods. The succcess of isolating anti-lipid A proves the usefulness of phage display system in human McAb preparation. The result shows that we have got the recombinant phage antibody.

7.Regional distribution of propofol at different doses in the brain during propofol anesthesia in neonatal rats

Objective To investigate the proper dose of propofol injected intraperitoneally ( i.p.) in neonatal rats and to study the regional distribution of propofol at different doses in the neonatal brain.Methods Part I:Sixty postnatal 7-day-old rats were randomly divided into 5 groups, which received different doses of propofol injected i.p.The behavior, anesthetic intervals and arterial blood gas were recorded.Part II: Twenty neonatal rats were randomly divided into 2 groups:anesthesia group ( group A) and sedation group ( group S) , and were injected propofol i.p.at the proper dose ac-cording to the results of Part I.Rats were decapitated when they reached the ideal anesthesia depth.The regional concen-tration of propofol in different regions of the brain was examined by high performance liquid chromatography.Results 25 mg/kg propofol i.p.was the sedate dose for neonatal rats, while 75 mg/kg i.p.was the anesthetic dose.In the group S, the concentration of propofol in the thalamus was significantly higher than in other regions (P<0.05), while in the group A, the concentrations of propofol in the frontal and parietal cortex were obviously lower, and the concentrations of propofol in the hippocampus and cingulate gyrus were obviously higher than that in other regions (P<0.05).Conclusions Propo-fol is a suitable anesthetic for neonatal rats and its distribution in the brain is quite different when given at different doses.
8.Effect of age, body figure and risk information in making treatment decision for female patients with breast cancer

Objective To determine the effect of age,body figure and risk framing in making treatment decision for female patients with breast cancer.Methods Two groups of healthy female:one group of 60 female ages 18-24 from undergraduate student and the other group of 60 female ages 35-60 from the university community.Healthy women imagined that they had been diagnosed with breast cancer and received information regarding lumpectomy versus mastectomy and recurrence rates.Participants indicated whether they would choose lumpectomy or mastectomy and reasons.Results The different concern levels of body appearance between younger and older women influences treatment selection of lumpectomy versus mastectomy.Facing the risk information,older women were unlikely to select the breast conserving operation.Conclusions The factors of age and the attention to body figure would effect the choice of breast conserving operation or mastectomy.Risk information on the selection of treatment would not affected directly,but the effects of risk information on younger and older female is different.Nurses should provide the age-appropriate information who recently have been diagnosed with breast cancer regarding treatment alternatives to ensure their active participation in the decision-making process,female who have different levels of investment in body figure also may have different concerns about treatment,and should be alert on empathetic of such concerns.
9.Improving orthopedics clinical teaching quality by combining anatomy and image knowledge

Students generally felt difficult in studying during clinical practice and the orthopedics teaching quality was unsatisfactory due to the discrepancies existed among teaching of anatomy,imagie and orthopedics clinical practice concerning the time,content,requirement and method.Students' understandings of anatomy and image knowledge were improved through launching theoretical study by means of lectures and case discussion.Students' understandings of orthopedics diseases were promoted through preoperative diagnosis and intraoperative conformation using anatomy and image knowledge for the patients they were responsible for.In all,orthopedics clinical teaching quality was promoted through applying these measures.
10.The status and related factors of daily activities in patients with chronic obstructive pulmonary disease

Objective To determine relationships between daily activities measured as modified version of Pulmonary Functional status and Dyspnea Questionaire (PFSDQ-M) and functional capacity and symptoms experienced in patients with chronic obstructive pulmonary disease (COPD). Methods Convenience sample of 94 COPD patients with stable condition were assessed by interview with PFSDQ-M Chinese version and modified Medical Research council dyspnea scale (MMRC) respectively. Pulmonary function test (PFT) and 6-minute walking test (6MWT) were undergone on the same day or within one week as interview. PFSDQ-M has three subscales,i.e.,change experienced by patient with activities (CA),dyspnea with activities (DA) and fatigue with activities (FA). Results Scores of CA,DA and FA correlated to 6-minute walking distancer= (-0.37)- (-0.42),FEV_1 r=(-0.27)-(-0.32),FEV_1/FVC r= (-0.27)- (-0.32),dyspnea rated by MMRC (r=0.55-0.60) and BODE index (r=0.35-0.40),respectively (all P<0.01). dyspnea explained 26% of the variance in changes of activities. Conclusions Physical activities moderately changed in patients with stable COPD;Dyspnea is the best predictor of limitation of daily activities.
