1.Correlation between the expression of hypoxia-inducible factor-1α in the tissue of gastric cancer and the recurrence of the gastric cancer after surgery

Objective To explore the correlation between the expression of hypoxia-inducihle factor-1α (HIF-1α) in the tissue of gastric cancer and the recurrence of gastric cancer after surgery.Methods A total of sixty patients with gastric cancer who received curative gastrectomy with complete follow-up data and recurrence after the surgery were taken as recurrence group.At same period,48 patients with gastric cancer treated and followed up for five years without recurrence were set as control group.The expressions of HIF-1α,CD34 and vascular endothelial growth factor (VEGF) in the gastric tissue section of recurrence group and control group were detected by immunohistochemistry streptavidin-peroxidase (S-P) staining.Chi-square test and Spearman correlation analysis were performed for the correlation analysis between the positive rates of HIF-1α and clinical pathological features.Kaplan-Meier method and Log-rank test were used for the analysis of survival condition in recurrence group.The independent prognostic factors of gastric cancer were analyzed by univariate analysis and Cox regression model.Results In recurrence group,the positive rate of HIF-1α expression was higher in cases with higher degree of gastric cancer differentiation,deeper invasion,lymph node metastasis,ascites,vascular invasion,lower TNM-stage and positive expression of VEGF.The differences were statistically significant (x2 =4.781,6.591,6.567,5.138,5.320,5.881 and 7.365,all P<0.05).The positive rate of HIF-1α expression of recurrence group was correlated with the positive expression of VEGF and microvascular density (MVD) (r=0.350 and 0.317,both P<0.05).The positive rate of HIF-1α expression of recurrence group (78.3%,47/60) was higher than that of control group (58.3 %,28/48) and the difference was statistically significant (x2 =5.027,P<0.05).The overall survival period of recurrence was nine months and one-year survival rate was 30.0%.Of which,the overall survival period and one-year survival rate of cases with negative HIF-1α expression were 18 months and 53.8% and with positive expression were eight months and 21.3%.HIF-1α expression was independent prognostic factor of gastric cancer (OR =2.166,95%CI:1.183 to 3.965).Conclusions The expression of HIF-1α plays an important role in the angiogenesis of gastric cancer and is closely related with gastric cancer recurrence.Its expression can be considered as an indicator of gastric cancer recurrence and prognosis.
2.Clinical, laboratory, and neuroimaging characteristics of neuroacanthocytosis

Objective To investigate the clinical,laboratory,and neuroimaging characteristics of neuroacanthocytosis.Methods Eight patients with neuroacanthocytosis were retrospectively analysed.Acanthocytes were tested by peripheral blood smear,wet preparation with saline dilution,and scanning electron microscope.Results Two male and 6 female patients were included.The age at onset was between 10 and 35 years,with a mean age at onset of 22 years.Four patients firstly presented with oral-facial-lingual dystonia,3 patients firstly presented with involuntary movements of the distal limbs and experienced the oral facial dystonia during the course of disease,and 1 patient primary presented with a parkinsonian syndrome.Four patients had generalized tonic-clonic seizures were reported in 4 patients,and 4 patients had cognitive impairment.Hypotonia and hyporeflexia were reported in 6 patients.The peripheral blood smear revealed the presence of acanthocytes in 7 patients,in addition,wet preparation with saline dilution and scanning electron microscope revealed the presence of acanthocytes in the remaining one.All patients showed slightly elevated serum creatine kinase.Brain magnetic resonance imaging (MRI) showed variable atrophy of the bilateral caudate nuclei and putamen,with or without a rim of increased T2-intensity in 6 patients,but the films of 2 patients were read as normal.Electromyography and nerve conduction velocity were examined in 4 patients.The results indicated axonal damage in 2 patients,and were normal in the other 2 patients.Acanthocytosis was confirmed by peripheral blood smear in 7 cases,by wet preparation with saline dilution in 8 cases and by scanning electron microscope in 2 cases.Conclusions Neuroacanthocytosis is a progress neurodegenerative disorder mainly affected the basal ganglia. The clinical characteristics include oral facial dystonia,limbs chorea,cognitive impairment,and seizures. Brain MRI showed variable atrophy of the bilateral caudate nuclei and putamen.The peripheral blood smear,wet preparation with saline dilution,and scanning electron microscope methods of peripheral blood examination are critical in the diagnosis of neuroacanthocytosis.
3.Synthesis, biological activity and molecular docking research of N-{(4-oxo-thiochroman-3-yl)phenyl-methyl}acetamide derivatives as α-glucosidase inhibitors.

In order to develop potent antidiabetic agents that have inhibitory effect to a-glucosidase, twelve β-acetamido ketone derivatives such as N-{[(substituted-4-oxo-thiochroman-3-yl)phenyl]-methyl}acetamide are designed and synthesized through one-pot Dakin-West reaction. Their chemical structures are confirmed by 1H NMR, 13C NMR, IR and HR-MS. In vitro α-glucosidase inhibition assays of compounds 4a-41 were carried out using glucose oxidase method. The result indicated that most of them possess inhibitory activity in vitro. Compound 4k showed the most potent inhibitory activity with 87.3% inhibition of α-glucosidase at the concentration of 5.39 mmol x L(-1). The structure-activity relationship of these β-acetamido ketone derivatives was discussed preliminarily. Moreover, the molecular docking method was used to study the interaction mode of compound 4k and α-glucosidase. Our results will be helpful for designing of α-glucosidase inhibitors in the future.
4.Correlation between the types of the constitution in TCM and the sleep status in PLA Navy divers

Objective To investigate the correlation between the types of constitution in TCM and the sleep status in the PLA Navy divers. Methods Eighty-nine PLA Navy divers who performed 10m diving professional training were selected. Constitution in TCM was classified and determined by the standardized standard Constitution in Chinese Medicine Questionnaire, sleep status was evaluated by the Pittsburgh Sleep Quality Index. Results 62.9% of Navy divers were the mild constitution in TCM. The eight kinds of the biased constitution in TCM are ranked with yang deficiency, phlegm-dampness, dampness-heat, qi deficiency, yin deficiency, blood stasis, qi stagnation and special intrinsic quality. Among 78 Navy divers with good sleep quality, there were 49 divers (72.1%) with the mild constitution in TCM and 19 (27.9%) with the biased constitution in TCM. Among 21 Navy divers with poor sleep quality, there were 7 divers (33.3%) with the mild constitution in TCM (accounting for) and 14 (66.7%) with the biased constitution in TCM. For Navy divers with poor sleep quality, the sleep quality scores were positive correlated with the blood stasis constitution in TCM (r=0.481,P<0.05). Conclusion Sleep status is correlated with the types of the constitution in TCM, and regulating constitution in TCM can improve sleep quality in PLA Navy divers.
5.Value of anal sphincter electromyography, orthostatic hypotension and dizziness in diagnosing multiple system atrophy.

OBJECTIVETo explore the value of anal sphincter electromyography (ASEMG), orthostatic hypotension (OH), and dizziness in diagnosing multiple system atrophy (MSA).
METHODThe characteristics of ASEMG and OH were compared among patients with dizziness (MSA and non-MSA), patients without OH (MSA and non-MSA), and patients with probable MSA (OH and non-OH).
RESULTSTotally 476 patients underwent ASEMG examinations. Dizziness was the onset symptom in 69 patients. Between the MSA group and non-MSA group, the mean duration of dizziness [(14.6 +/- 2.1) vs. (12.8 +/- 2.0) ms, P < 0.01] and satellite potential occurrence rate [(22.7 +/- 11.8)% vs. (12.2 +/- 8.9)% , P < 0.01] were significantly different, while the OH rate (84.6% vs. 55.2% ) and the difference of the blood pressure between standing and supine positions were not significantly different. In 162 patients with symptom of dizziness, the mean duration of dizziness [(15.3 +/- 2.7) vs. (12.8 +/- 1.9) ms, P < 0.001], satellite potential occurrence rate [(25.4 +/- 12.8)% vs. (13.5 +/- 10.4)%, P < 0.001] , and difference of the diastolic blood pressure [(18.5 +/- 17.0) vs. (11.7 +/- 12.7) mmHg, P < 0.05] were significantly different between the MSA group and non-MSA group, while the normal rate of blood pressure at standing position (60% vs. 41.9%) and the difference of systolic blood pressure were not significantly different. In 146 patients with abnormal blood pressure at standing and supine positions, the mean duration of dizziness [(15.0 +/- 2.4) vs. (12.8 +/- 1.7) ms, P < 0.001] and satellite potential occurrence rate [(22.0 +/- 12.2)% vs. (10.6 +/- 8.5)%, P < 0.001] were significantly different between the MSA group (n = 61) and non-MSA group (n = 85). In 125 patients with probable MSA, the mean duration of dizziness [(15.5 +/- 2.4) vs. (15.9 +/- 2.2) ms, P > 0.05] and satellite potential occurrence rate [(24.3 +/- 12.6)% vs. (22.7 +/- 12.4)%, P > 0.05] were not significantly different between those with OH and those without OH. The rates of dizziness and the percentage of dizziness as the onset symptom were 93.2% and 52.3% in OH group and 44.4% and 8.3% in non-OH group.
CONCLUSIONSASEMG is better than OH in diagnosing patients with dizziness suspected as MSA. Neurogenic lesion can be found by ASEMG in patients without OH, which is helpful in the early diagnosis of MSA.

6.Dose dense chemotherapy in the postoperative adjuvant treatment for breast cancer.

OBJECTIVETo investigate the safety and feasibility of dose dense chemotherapy in the postoperative adjuvant treatment for breast cancer.
METHODSThe data of 114 breast cancer patients treated with adjuvant dose dense chemotherapy was retrospectively analyzed.
RESULTS114 breast cancer patients received postoperative adjuvant dose dense chemotherapy from Jan. 2004 to March 2005. The patients were treated with different regimens according to their recurrence risk: 43 with paclitaxel + epirubicin (PE), 8 with taxotere + epirubicin (TE), 36 with cyclophosphamide + epirubicin + 5-Fu ( CEF) and 27 with epirubicin +5-Fu (EC). Totally, 111 patients except 3 completed the dose dense chemotherapy smoothly. Major toxicities severer than grade III included: neutropenia in 6.3% of the patients, nausea and vomit in 11.7%, peripheral neuropathy in 3.6%.
CONCLUSIONPostoperative adjuvant dose dense chemotherapy for breast cancer patient is feasible and safe, but the long-term outcome still needs further observation.

7.CT manifestation for excavated-type of rhomboid fossa of the clavicle.

OBJECTIVETo explore the manifestation of CT for excavated-type of rhomboid fossa of the clavicle.
METHODSNine cases with rhomboid fossaes of the clavicle of 8 patients accepted CT and 1 case added MRI together;all 8 patients were male who aged from 17 to 70 years old with mean age of 42.5 years old;three dimensional reconstruction of all CTs were made, the distance between focus and inside end of clavicle and the size of all focus were measured respectively, then the position, shape, margin of focus were analyzed.
RESULTSAll focuses located near the inside end of clavicle and the distances between focus and inside end of clavicle were lower than 2 cm and the mean value was 1.3 cm, the size of all focuses was from 1.05 to 3.45 cm and the mean value was 2.18 cm. All 9 focuses of 8 patients located in the posterior and nether edge of inside end of clavicle, 5 cases located in right and 4 cases located in left side(both right and left side occurred in 1 patient. Seven focuses showed "fishhook sign" and the rest 2 focuses were small and without fishhook shape;the cortex of clavicle of all 9 cases showed local minus and nearly marrow showed integrated sclerotic margin. Regular soft tissue as strip can be seen in 7 focuses and the rest small focuses without the symptom.
CONCLUSIONSCT could show certain characteristics for excavated-type of rhomboid fossa of the clavicle and certain value for its diagnosis and identification.

9.TAB182 promotes DNA homologous recombination repair by stabilizing RPA2 mRNA

Objective:To investigate the regulating molecules and acting mechanism of TAB182 in HR pathway.Methods:TAB182 in human breast cancer MCF-7 cells was knocked down by shRNA strategy, the TAB182 knockdown MCF-7 as the TAB182 knockdown group, and the MCF-7 cell using the shRNA negative control as the TAB182 negative control group. RNA sequencing and qRT-PCR were performed to screen and verify the differentially expressed genes of HR pathway related to TAB182 depression. Western blot was used to detect protein expression. Immunofluorescence staining of nuclear RAD51 and BrdU was used to check the 3′ ssDNA formation by the end resection. The cell cycle arrest and apoptosis were measured by flow cytometry. Cloning formation assay was used to evaluate the sensitivity TAB182-knockdown cells to radiation.Results:Both quantitative RNA sequencing and qRT-PCR assays showed that TAB182-knockdown significantly decreased the mRNA expression of RPA2( t=17.97, P<0.05). Compared with the TAB182 negative control group, the protein level of RPA2, the number of RAD51 foci, and the 3′ ssDNA-binding nuclear protein marker BrdU in TAB182-knockdown cells were significantly reduced. At 4, 8, and 12 h after actinomycin D treatment, the attenuation of RPA2 mRNA in the TAB182-knockdown cells was accelerated ( t=5.37, 3.79, 3.69, P<0.05). Compared with the TAB182 negative control group, the radiosensitivity and radiation-induced apoptosis in the TAB182-knockdown group were increased ( t=3.48, 11.05, P<0.05), and at 24 h after irradiation, the cell cycle block time was prolonged ( t=8.40, P<0.01). Conclusions:TAB182 plays a role in maintaining RPA2 mRNA stability, thereby promoting HR repair. TAB182 knockdown cells are highly sensitive to ionizing radiation.
10.Cluster analysis on TCM patterns in patients with thyroid cancer after surgery

Objective To investigate the distribution features of TCM patterns and differentiation method of thyroid cancer through cluster analysis on data of TCM four examinations after surgery.Methods The data of TCM four examinations were collected from 304 patients by applying cross-section epidemiological survey,and then given quantitative classification and a database was established.The quantitative scores of each patient were given cluster analysis by using sample clustering analysis method (Q clustering) for getting the proportion of each category of four examination information.The preliminary criterion for patterns of thyroid cancer was made according to the proportion of four examinations information and expert survey of Delphi method at earlier stage,which was combined with clinical practice to determine the patterns.Results All 304 patients were clustered into 9 categories indicated by from A1 to A9.The frequency of A1 was 77 cases;A2,7;A3,1;A4,7;A5,105;A6,7;A7,87;A8,11 and A9,2.Based on clinical practice and experts' experiences,A1 and A7,A5 and A6,and A3 and A4 were merged and evaluated respectively as pattern of dual deficiency of qi and yin (164 cases,53.9%),pattern of liver depression and qi stagnation (112 cases,36.8%) and spleen-kidney yang deficiency pattern (8 cases,2.6%).A2 was evaluated as pattern of yin damaged by stasis heat (7 cases,2.3%).The proportions of four examination information of A8 and A9 were disperse and evaluated as other patterns (13 cases,4.4%).Conclusion The distribution of TCM patterns in the patients with thyroid cancer after surgery has its regularity.It is feasible to evaluate TCM patterns with cluster analysis combining with experts' experiences.
