1.Experimental study of homogenous bladder transplantation in rats

Objective To establish a model of homogenous bladder transplantation without blood vessel anastomosis in rat,and to investigate the outcome of the transplanted grafts.Methods 20 newborn SD rats were used as donors,and a total of 20 Wistar rats of 5-week-old were used as the reeipiemts for transplantation of homogenous bladder.The grafts were harvested from the SD rats,and they were wrapped with the greater omentum of the recipients without blood vessel anastomosis.Immunosuppressive agent was given postoperatively.10 rats were sacrificed on the 7th day and the remainings rats were sacrificed on the 14th day after bladder transplantation.The transplanted bladder grafts were observed grossly and microscupicully.Results Of 20 rats undergone homogenous bladder transplantation,19 rats were alive and the grafts were in good condition microscopically.There was no obvious rejection reaction in the transplanted bladders.The bladder mucosa,blood vessels and muscle layers of the grafts appeared to be normal microscopically.Conclusions The homogenous bladder transplantation into the omentum without blood vessel anastomosis in rats is considered to be a kind of stable animal model,with a higher survival rate of the tramsplanted bladder.
2.Long-term complications of continent ileocolic urinary reservoir with in situ appendix conduit through umbilicus

Objective To evaluate the long-term complications of continent ileocolic reservoir with in situ appendix conduit through umbilicus and their management.Methods We reviewed 37 patients with bladder diseases who underwent the surgery of continent ileocolic diversion with in situ appendix conduit through umbilicus and followed up for 85 to 136 months(mean 109 months).Long-term postoperative complications were documented.Results Complications occurred in 15 cases(40.5%),including stone formation in urinary pouch in 2 cases,efferent limb stricture and difficulty in catheterization in 3 cases,ureter stricture and hydronephrosis in 7 cases,foreign matter in urinary pouch in 2 cases,efferent limb mucosa exstrophy in 1 case.These complications were correctly treated and good results were achieved.Conclusion Long-term complications of continent ileocolic diversion with in situ appendix conduit through umbilicus were related to nursing in daily life after operation and unsuited design in operation.Nursing in daily life is considered very important in preventing postoperative complications.
3.Ovarian function in uremic patients receiving dialysis

In order to evaluate the pathogenisis, definite the position and mechanism of ovarian dysfunction. Methods Pi-tuitarygonadal hormones ,-.P and prolactin (PRL) were measured in 25 premenopausal hemodialysis women using enzyme im-munoassay (E3A). Results Patients' serum PRL levels significantly elevated, FSFKLH levels also elevated, whereas P levels were obviously lower compared with normal women. Conclusion (1) Hypothalamic-pituitary-ovarian axis was impaired in uremia women as indicated by the prevalence of acydicity and the failure of LH,E2 levels and marked low P levels. (2)In uremia women clomiphenum stimulation test was positive suggested that the pituitary-ovarian axis were normal, whereas hypothalamic was impaired. (3) Hyperprolactinemia, owing to increased pituitary prolactin secretion. Suppression with bromocriptine was unstable. (4) During dialysis, symptom treating is suitable rather than trigger the ovalation. Successful renal transplantation is the best treatment.
4.THE EXPRESSION IN SITU OF TELOMERASE RNA AND THE PROGNOSIS OF BLADDER TRANSITIONAL CELL CARCINOMA

To study the expression of human telomerase RNA(hTR) in different grades of bladder transitional cell carcinoma and its relation to its prognosis. With in situ hybridization, the expression of hTR was observed in 67 cases of bladder transitional cell carcinoma specimens, to be compared with specimens of 10 normal bladder tissue and 10 benign lesions. The RNA probe used in hybridization was telomerase reverse transcript (TERT), and the marker was digoxin. There was a significent correlation between the expression of hTR and grading and prognosis. hTR is considered as an important and independent prognostic factor in this carcinoma.
6.Contrast study of turn technique and chop technique in phacoemulsification

Objective To explore the clinical superiority of two kinds of chopping nucleus methods in phacoemulsification——etch and turn technique, stop and chop technique.Methods Three hundred and sixty-five eyes were performed with phacoemulsification, among which 85 eyes were performed with turn technique and 280 eyes were performed with chop technique.The results of the two kinds of chopping nucleus were analyzed.Results (1)Time of chopping nucleus:time of turn technique averaged 81.26 seconds while chop technique averaged 40.71 seconds; (2)Visual acuity in the first week after operation:turn technique:vision of 76 of 85 eyes were 0.5 of better, 7 eyes were 0.2~0.5(8.2%) and 2 eyes were 0.2 or worse; chop technique: vision of 253 of 280 eyes were 0.5 or better (90.4%), 21 eyes were 0.2~0.5(7.5%) and 6 eyes were 0.2 or worse (2.1%);(3)Reaction of corneal endothelial cells after the operation: central endothelium rugosity appeared in 4 eyes (4.7%) with turn techinque and in 7 eyes (2.5%) with chop technique. Conclusion The operation of the former is less difficult and the chopping nucleus time is longer. The time of the operation on the latter is shorter and it is appriate for hard nucleus and should be applied and spreaded. There are no marked differences as for the vision after the operation and the microcorneal injury between them.
7.Prolonging the Shelf Life of RBCs Stored at Non-4℃

Objective To prolong the shelf life of RBCs stored at non-4℃, and improve the ability of blood supplying during wartime. Methods 12 bags of packed RBC were collected from 12 volunteers presenting blood, respectively, and each was averagely divided into 5 parts(groups): combination group, new solution group, ammonia group, control group and 4℃ group. The fist 4 groups were stored at room temperature, and were tested for ATP concentration daily. 4℃ group was stored at 4℃, and RBC ATP concentration was measured at the end of its shelf life, which served as a critical ATP concentration. When RBC ATP concentration of the fist 4 groups decreased to the level of critical ATP, the time of preservation was their shelf life. 6 bags of packed RBC were added the same solution as the combination group, and each was averagely divided into 4 parts(groups)after stored at room temperature for 12 days, which were washed for 0, 5,10 and 15min before centrifugation, respectively. The residual ammonia and the recovery ratio of RBC were measured after washing. Results The shelf life of combination group,new solution group,ammonia group and control group was 12,6,6 and 5 days, respectively. The residual ammonia could be effectively cleaned up after either washing 4 times with 5 min balance time or 3 times with 10 or 15 min balance time. The recovery ratio of RBC was 88.5% and 83.1% after washing 3 and 4 times, respectively. Conclusion The combination of new solution and ammonia was a good additive solution for RBC preservation at non-4℃, and the residual ammonia can be cleaned up using normal saline.
8.Effects of Pharmaceutical Care on Blood Glucose Control and Medication Compliance in Patients with Dia-betes Mellitus

OBJECTIVE:To explore the effects of pharmaceutical care on blood glucose control and medication compliance in patients with diabetes mellitus. METHODS:Nighty-two discharged patients with diabetes mellitus in our hospital from Apr. to Sept. 2015 were divided into intervention group and control group by random number table method,with 46 cases in each group. Both groups were given Chlorpropamide tablets+Metformin glibenclamide tablets(Ⅰ)for 3 months;intervention group additionally re-ceived pharmaceutical care as medication education,telephone follow-up,etc. The levels of glycosylated hemoglobin and blood glu-cose,medication compliance were compared between 2 groups before and after treatment. RESULTS:Before treatment,there was no statistical significance in the levels of glycosylated hemoglobin and blood glucose,or medication compliance between 2 groups (P>0.05). After treatment,the levels of glycosylated hemoglobin and blood glucose in 2 groups were significantly decreased,and the intervention group was significantly lower than control group;the effects of blood glucose control in patients younger than 60 years old were better than in patients older than 60 years old,with statistical significance(P<0.05). After one month of treatment, the proportion of good medication compliance were increased significantly in 2 groups,and the intervention group was significantly higher than control group. After 3 months of treatment,the proportion of good medication compliance in 2 groups were significant-ly higher than before treatment,and 1 month after treatment,while the intervention group was significantly higher than control group. The improvement of medication compliance in patients younger than 60 years old was better than in patients older than 60 years old,with statistical significance (P<0.05). CONCLUSIONS:Pharmaceutical care contributes to blood glucose control and improves the medication compliance,especially for those younger than 60 years old.
9.Clinical Study on Treatment of Senile Constipation with Combination of Acupuncture and Chinese Medicine

This study was aimed to explore the clinical effect of San-Jiao (SJ) acupuncture combined with Chinese medicine and Cisapride tablets in the treatment of senile constipation. A total of 118 patients were selected according to the inclusion criteria. According to the visiting sequence, patients were randomly divided into the acupuncture with Chinese medicine group of 60 cases and drug group of 58 cases. In the acupuncture with Chinese medicine group, SJ acupuncture was combined with Huang-Di (HD) powder in the treatment. In the drug group, Cisapride tablets were applied. Both groups were treated for 20 times. The scale was used in the comprehensive evaluation of integral effect evaluation of main symptoms, psychological evaluation before and after the treatment. The results showed that there was significant difference on the integral before and after treatment in both groups (P< 0.01). The posttreatment integral showed that the acupuncture with Chinese medicine group was better than the drug group (P<0.01). There was no significant difference on total effective rate between two groups. However, comparison on good improvement rate showed that the acupuncture with Chinese medicine group was better than the drug group (χ2 =28.25, P < 0.01). It was concluded that the acupuncture with Chinese medicine group had better effect on the im-provement of clinical symptom integral, psychological integral, and comprehensive effect evaluation. SJ acupuncture combined with HD powder was a safe treatment method with good effect.
10.Clinical study of Alpha lipoic acid injection combined mecobalamin and prostaglandin E on treating patients with diabetic neuropathy

Objective To investigate the clinical effect and safety of alpha lipoic acid injection combined mecobalamin and prostaglandin E on type 2 diabetic patients complicated with diabetic peripheral neuropathy(DPN).Methods One hundred and sixty type 2 diabetic patients complicated with DPN in the General Hospital of Benxi Iron and Steel Group Corporation from Jan.2011 to Dec.2012 were randomly divided into the treatment group(n =80) and the control group (n =80).Three cases of the treatment group and 5 cases of the control group discharged early from the study because of their own reasons.There were 77 cases in the treatment group and 75 cases in the control group.On the basis of controlling blood glucose,patients in the two groups were given 500 μg mecobalamin combined with intramuscular injection once two days,as well as prostaglandinE 10 μg injection once a day.Patients in treatment group were added with 600 mg alpha lipoic acid for intravenous injection once a day for 10-14 days.Total symptom score (TSS),nerve conduction velocity,satisfaction and adverse reactions were evaluated before and after treatment.Results TSS score,tingling score,burning sensation score and hypoesthesia score,numb score in treatment group were (3.5 ± 2.5),(1.1 ± 0.4),(0.9 ± 0.7),(1.3 ± 0.4),(1.3 ± 0.9),significantly lower than those in control group (4.3 ± 2.1,t =2.11,P <0.05;1.5 ±0.5,t =1.86,P<0.05;1.3 ±0.5,t =1.83,P <0.05;1.7 ±0.5,t =1.87,P <0.05; 1.9 ± 0.4,t =1.91,P < 0.05).The median nerve conduction velocity,peroneal nerve motor conduction velocity,median nerve sensory conduction velocity,common peroneal nerve sensory conduction velocity of patients in treatment group were (53.6 ± 1.4) m/s,(49.6 ± 1.1) m/s,(47.3 ± 1.1) m/s,(48.2 ± 1.9) m/s,lower than those in control group((48.5 ±2.7) m/s,t =-4.94,P <0.05;(43.9 ±2.1) m/s,t =-5.36,P <0.05; (41.6 ± 1.8) m/s,t =-5.09,P <0.05;(43.2 ±2.5) m/s,t =-4.27,P < 0.05,P <0.05).In the treatment group,97.4% (75/77) physicians and 92.2% (71/77) patients were satisfied with treatment effect,while in the control group,84.0% (63/75) physicians and 78.7% (59/75) patients were satisfied with treatment effect.During the study periods,there were 3 cases with facial flushing and 1 cases of dizziness in the treatment group,and 2 cases of facial flushing and 1 cases of dizziness in the control group.All adverse reactions were spontaneous remission without any special treatment.Conclusion Alpha lipoic acid intravenous drip is effective in term of treating type 2 diabetic peripheral neuropathy,and with high safety.
