1.Clinical observation of Tiaomai-Yinon the treatment of tachyarrhythmia with the TCM patterns of deficiency of Qi and Yin, and heat of blood stasis

Objective To study the effect ofTiaomai-Yin with the therapy method of clearing heat and cooling blood to treat the tachyarrhythmia with the TCM patterns of deficiency of Qi and Yin, and heat of blood stasis.MethodsA total of 60 patients with tachyarrhythmia were randomly recruited into a control group and a treatment group, 30 patients in each. The treatment group was treated withTiaomai-Yin, while the control group was treated with Metoprolol. After both groups were treated for one month, the changes of TCM symptoms, ECG and 24 h DCG were observed.Results The total effective rate of the treatment groups was significantly better than the control group(86.67%vs. 56.67%;χ2=7.957,P<0.05).Tiaomai-Yin significantly improve the symptoms of shortness of breath (1.40 ± 2.11vs. 2.10 ± 1.87;t=-3.341,P<0.01), tiredness (1.30 ± 2.33vs. 2.10 ± 1.84;t=-3.262,P<0.01), dry mouth (0.09 ± 1.34vs. 2.50 ± 2.41;t=-3.194,P<0.01), and tongue (1.90 ± 1.65vs. 3.90 ± 1.81;t=-3.217,P<0.01) and pulse (2.60 ± 1.71vs. 5.10 ± 2.55;t=-3.283,P<0.01).ConclusionTiaomai-Yin had effects in treating tachyarrhythmia with the TCM patterns of deficiency of Qi and Yin, and heat of blood stasis with improvement of symptoms and fewer side effects.
2.Clinical results of femoral head replacements for femoral intertrochanteric fractures in the elderly

Objective To compare the clinical results of femoral head replacement for femoral in- tertrochanteric fractures and femoral neck fractures in aged patients.Methods Forty cases of femoral in- tertrochanteric fractures and 45 cases of femoral neck fractures (aged over 80 years) underwent femoral head re- placement from August 1996 to April 2002 in our department.The clinical results of the two groups were retro- spectively compared and analyzed statistically.Their follow-up periods,ranging from 3 to 7 years,averaged 4.6 years in the intertrochanteric fracture group and 5.5 years in the neck fracture group.Results The differences in time of hospitalization,perioperative blood loss,postoperative drainage,ambulation trine,early compiications be- tween the two groups were of no statistical significance (P>0.05).The differences in operation time and inequality in leg length after operation between the two groups were of statistical significance (P<0.05).The operation time was longer and more limb-length inequality occurred in the intertrochanteric fracture group than in the neck fracture group.There was no significant difference in Harris score at the last follow-up between the two groups.The X-ray at the final follow-up showed that there was insignificant difference in postoperative hip joint space,hip pain,or stem loosening between the two groups (P>0.05).The stem revision incidences were of no statistical significance be- tween the two groups (P>0.05).The two groups reported no acetabular protrusion.Conclusion With standard cemented prostheses,femoral head replacements can achieve as similar functional outcomes for patients with in- tertrochanteric fractures as for those with femoral neck fractures.
3.Investigation of the API distribution homogeneity in lyophilized product-pemetrexed disodium for injection by confocal micro-Raman spectroscopy mapping

The method of homogeneity evaluation for active pharmaceutical ingredient (API) spatial distribution in lyophilized product was investigated for the first time with confocal micro-Raman spectroscopy mapping, using pemetrexed disodium for injection as a model drug. Certain areas of the lyophilized product were scanned to obtain Raman spectra. The classical method ("peak clipping" method) was employed for mapping with characteristic Raman peaks of the API and the excipient. Due to the API being finely dispersed in the excipient in lyophilized products, the classical method cannot discriminate between the two ingredients making the distribution homogeneity difficult to evaluate. The "ratio of characteristic peak intensities" method was then utilized. Using this method, the relative intensity of the characteristic Raman peaks of the API to the excipient was applied for mapping and the relative content of API to excipient was calculated for a homogeneity evaluation of the drug distribution. 5.Current situation of full-time healthcare-associated infection management staff in Xianyang City of Shaanxi Province

Objective To understand current situation of full-time healthcare-associated infection(HAI)manage-ment staff in hospitals in Xianyang,explore the causes of instability of HAI management team.Methods Stratified random sampling method was adopted,hospitals were divided into secondary and tertiary levels,then numbered ac-cording to different levels,surveyed hospitals were selected randomly,general condition,professionals,and causes of instability of 52 selected full-time HAI management staff members in 29 secondary and above hospitals in Xian-yang were surveyed.Results Full-time HAI management staff were mainly women,accounting for 89.13%;54.35% of staff were aged 30-50 years;47.83% had bachelor degree and above;41.30% had intermediate profes-sional titles;65.22% were nurses;91.30% engaged in HAI management for <9 years;HAI management staff were statistically different in age and education level(both P<0.05).63.04% of full-time HAI management staff engaged in nursing before engaging in HAI management work;63.04% engaged in HAI management work accord-ing to administrative arrangement,the top three causes for instability of HAI management team were heavy work-load(91.30%),lacking enough attention of leaders(78.26%),and low pay(63.04%).Conclusion Structure of HAI management full-time staff in hospitals in Xianyang is irrational,the main causes for unstable management team is heavy workload,insufficient attention paid by leaders,and low pay.
6.Expression of the keratin 10 related genes in buccal mucous membrane exfoliated cells and changes of liver function in coal-burning arsenism

Objective To observe expression of the keratin 10 (K10), c-myc and cox2 in buccal mucous membrane exfoliated cells, and changes of liver function of coal-burning arsenism patients, in order to explorate coal-burning arsenic poison circumference biology symbol. Methods The buccal mucous membrane exfoliated cells were collected from both arsenism patients and normal controls. Real-time PCR was employed to detect the changes of K10, c-myc, cox2 genes expression in these cells. Simultaneously, circumference venous blood was extracted and examinated liver function. Results K10 was found obviously overexpressed in arsenism patients(3.60±0.94) compared to control(1.82±0.68), the difference had statistics significance(t=2.15, P<0.05), c-mye and cox2 did not found obviously changed(c-myc: 3.50±2.77,3.39±2.07; cox2:5.90±1.40,4.73±1.91; t=1.26,1.65, P> 0.05). The serum ALT of patients(25.83±2A5) obviously increased than control(36.86±1735, t=2.55, P<0.05). Conclusions Expression of K10 gene in buccal mucous membrane cells may be regarded as sensitive molecular markers for skin pathologic changes in arsenic patients. The liver is a sensitive target organ of inorganic arsenic.
7.Expression of cardiotrophin-1 and effects of irbesartan in adriamycin induced cardiomyopathy in rats.

OBJECTIVE:
To investigate cardiotrophin-1(CT-1) expression in the ventricle and the effects of angiotensin II type I receptor antagonist (AT(1)RA) irbesartan on the ventricular remodeling in adriamycin myocardiopathy.
METHODS:
Thirty male SD rats were randomized into 2 groups: a control group (n=10) and a model group (n=20). The model group was administered adriamycin and 18 rats survived. And theses rats were randomized again into 2 groups. One was treated with irbesartan [50 mg/(kg x d), with stomach-tube], and the other received equal saline, so did the control group. After 12 weeks, the protein level of CT-1 was detected by immunohistochemistry.
RESULTS:
Ventricular CT-1 in the model control group and the treatment group was higher than that in the control group and the correlation analysis showed that ventricular CT-1 of the model control group was positively correlated with the left ventricular weight index, and CT-1 of the treatment group was lower than that of the model control group.
CONCLUSION
CT-1 was assumed to take part in the ventricular remodeling. The mechanism of irbesartan on the ventricular remodeling may be related to the downregulation of CT-1 expression.

8.Effect of simvastatin on the reserve of heart function and endothelial function in X-syndrome.

OBJECTIVE:
To determine the therapeutic effect of simvastatin combined with traditional medicine on patients with X-syndrome, and on the reserve of heart function and endothelial function.
METHODS:
Forty patients with X-syndrome were recruited from September 2006 to September 2007 and randomly divided into 2 groups (a simvastatin group and a control group). The control group received routine treatment including beta receptor blocker, calcium-channel blocker (CCB) and long active nitrate. The simvastatin group received simvastatin and the routine treatment. The clinical condition and exercise test (TET) were performed before and after the treatment.The levels of triglyeride (TG), total cholesterol (TC), low density lipoprotein (LDL-C), high density lipoprotein (HDL-C), endothelin-1 (ET-1) and nitric oxide (NO) were measured.
RESULTS:
The frequencies of chest pain in the simvastatin group were lower than those in the control group. The levels of ET-1, ET-1/NO, TG, TC, and LDL-C were significantly decreased in the simvastatin group as compared with the control group after the treatment. The levels of HDL-C and NO were significantly increased in the simvastatin group as compared with the control group after the treatment. The time in TET was significantly increased in the simvastatin group as compared with the control group. The frequencies of chest pain were positively related to the level of ET-1/NO and negatively related to the time in TET.
CONCLUSION
Simvastatin is effective for patients with X-syndrome and may improve the endothelial function and the reserve of heart function.

9.Characteristics of 3.0T dynamic contrast-enhanced magnetic resonance imaging of lung cancer and its correlation with microvessel density.

OBJECTIVETo explore the characteristics of 3.0T dynamic contrast-enhanced magnetic resonance (DCE-MR) imaging of lung cancer and its correlation with microvessel density (MVD).
METHODSThirty-seven patients with pathologically proven lung cancer underwent DCE-MR with liver acquisition with volume acceleration sequence. DCE-MR images were acquired intermittently for a total of 4 minutes on a 3.0T MR scanner. The relative enhancing percentage (SI%) at each time point was measured. The shapes of T-SI% curves were defined as A (rapidly ascending followed by a descending branch) and B (rapidly ascending branch followed by a plateau). The early peak enhancement (SIEP%), early peak time (TEP), maximum enhancement (SIpeak%), and peak time (Tpeak) were recorded and compared according to different dimensions, locations, histological types, and differentiation grades of lung cancer. Tumour specimens were immunostained for CD31 and CD34 in ten patients who had undergone surgical resections. The enhancement values were correlated with MVD. Results The SIEP% and SIpeak% of tumors with smaller dimensions (< or = 5 cm) were significantly higher than those with larger dimensions (> 5 cm) (P = 0.014, P = 0.024). The SIEP% and SIpeak% were positively correlated with the tumor MVD. Conclusion The SIEP% and SIpeak% of lung cancer correlate with tumor dimension and can reflect MVD in tumor.
