Objective: To study the influence on the displacement of abutment with various retainer designs for bilateral distal-extension removable partial dentures(RPDs). Methods: Combined clasp, extension clasp and telescopic crown were used to retain RPDs. When food chip was masticated between the upper and lower dentition on one side, the displacement of abutment on both sides was measured by a high sensitive laser traneferring-detector and the data were statistically analyzed. 2.Influence of inhaled budesonide and salbutamol on oxidative stress and serum inflammatory cytokines in children with bronchial asthma

Objective To observe the clinical effect of budesonide combined with salbutamol in the treatment of children with bronchial asthma,and its influence on oxidative stress and serum interleukin-2 (IL-2),soluble IL-2 receptor (sIL-2r) levels.Methods 70 children with bronchial asthma were divided into observation group and control group according to the different treatment,35 cases in each group.The observation group was inhaled budesonide combined with salbutamol,the control group was given salbutamol inhalation therapy.The clinical efficacy of the two groups was observed,and the changes of serum oxidative stress factor,IL-2 and sIL-2r levels before and after treatment in the two groups were compared.Results In the observation group,20 cases were cured,7 cases were markedly effective,effective in 5 cases,the total effective rate was 94.3%.In the control group,17 cases were cured,6 cases were markedly effective,effective in 4 cases,the total effective rate was 77.1%.The total effective rate between the two groups had statistically significant difference (x2 =26.543,P < 0.05).After treatment,the malondialdehyde (MDA),end products (AOPP) of the observation group were (16.26 ± 1.76)U/L,(42.123 ± 5.745) μmol/L,respectively,which were lower than those of the control group [(22.16 ± 2.84) U/L,(63.63 ± 6.22) μ mol/L,t =23.354,25.348,all P < 0.05].The glutathione peroxidase (GSH-PX),superoxide dismutase (SOD) of the observation group were (236.46 ± 32.25) ng/L,(19.643 ± 3.587) ng/L,respectively,which were higher than those of the control group [(197.43 ± 18.42) ng/L,(18.244 ± 2.446) ng/L,t =18.943,15.123,all P <O.05].The IL-2,sIL-2r levels of the observation group were (5.28 ± 2.65) μg/L,(30.376 ± 2.147) mg/L,respectively,which were significantly lower than those of the control group [(9.84 ± 3.22) μg/L,(37.564 ± 1.323)mg/L],the differences were statistically significant (t =21.378,24.643,all P < O.05).Conclusion Inhaled budesonide combined with salbutamol in the treatment of children with bronchial asthma is helpful to control the symptoms of asthma,and can inhibit oxidative stress,reduce serum sIL-2r and IL-2 levels,it is worthy of promotion.

Statins are widely used as important lipid-lowering drugs,and studies have proven the safety of statins in patients with liver diseases.In recent years,there have been more and more studies on the effects of statins in the prevention and treatment of hepatitis C,fatty liver disease,liver cirrhosis,and hepatocellular carcinoma.Studies have shown that besides the lipid-lowering effect,statins also have various other effects,including anti-inflammatory effect,immune regulation,anti-proliferation,and antioxidant effect,and they can also improve vascular endothelial function and inhibit platelet aggregation.Their roles in the prevention and treatment of liver diseases await further research.

5.Effect of Minocycline on Hippocampal Neuroapoptosis and Cognitive Dysfunction in Aged Rats

Objective To investigate the effect of minocycline on isoflurane-induced hippocampal neuroapoptosis and cognitive dysfunction in aged rats. Methods Forty-five male SD rats were randomly assigned into 3 groups ( n=15): blank control group ( group C) , 1.5% isoflurane group ( group I) and 50 mg??kg-1 minocycline+1.5% isoflurane group ( group M+I) . Minocycline was injected intraperitoneally 12 h before the start of anesthesia for group M+I.Group I and group M+I were exposed to 1.5% isoflurane for 4 h, while group C were exposed to 30% O2-70% N2.At the end of anesthesia, five rats in each group were randomized to analyse arterial blood gas. The other rats in each group were sent back to their home cage until they were fully awake.Fourteen days after anesthesia, Morris water maze was used to assess the cognitive function, and then hippocampi of rats were dissected for detection of the expression of cleaved caspase3, Bax and Bcl-2. Results No difference was found in arterial gas analysis among the 3 groups (P>0.05).Compared with group C, the rats in the group I spent more time locating the platform on the third and fourth training days and the time percentage that the rats in group I spent in the target quadrant was much less (P<0.05).However, these changes were reversed in group M+I (P<0.05).The isoflurane-induced increased level of Bax and cleaved caspase3 and decline of anti-apoptotic factor Bcl-2 were restored by minocycline pretreatment ( P<0.05) . Conclusion Minocycline could attenuate cognitive dysfunction induced by isofluranein aged rats.The mechanism is associated with inhibition of hippocampal neuroapoptosis which is increased by isoflurane.
6.Efficacy analysis on simplified intensity-modulated radiotherapy with high or conventional dose and concurrent chemotherapy for patients with N1 esophageal carcinoma

Objective To investigate the feasibility of simplified intensity-modulated radiotherapy (sIMRT) and concurrent chemotherapy against neck and upper thoracic esophageal carcinoma with lymph node metastasis.Methods sIMRT plans were designed for 44 patients of neck and upper thoracic esophageal carcinoma with lymph node metastasis, 20 of which underwent high dose sIMRT (hsIMRT group) and 24 underwent conventional dose sIM RT (csIMRT group).Three target volumes were defined:PGTVnd, target volume of lymph node lesion, irradiated to 68.1 Gy ( 2.27 Gy × 30 fractions ) for the hsIMRT group, and 60 Gy (2.0 Gy ×30 fractions) the csIMRT group; PTV1, the target volume of primary lesion, to be irradiate to 63.9 Gy (2.13 Gy × 30 fractions) for the hsIMRT group and 60 Gy (2.0 Gy × 30fractions) for the csIMRT group; PTV2 , the prophylacticly irradiated volume, to be irradiated to 54 Gy (1.8 Gy ×30) for both groups.The sIMRT plan included 5 equiangular coplanar beams.All patients received DDP + 5-FU regimen concurrently with radiotherapy at 1 -5 d and 29- 33 d, respectively.Chemotherapy was repeated for two cycles 28 days after the radiotherapy was finished.Results The treatment was completed for all patients within 6 weeks.During the treatment only one patient with grade 3 acute bronchitis was observed in the hsIMRT group.The complete response (CR) rate for the lymph node lesion of the hsIMRT group was 75% ( 15/20 ), significantly higher than that of the csIMRT group [45.8% ( 11/24), x2 = 3.84, P < 0.05].The 1-, 2-, and 3-year progression-free survival rates of the hsIMRT group were 60%, 40%, and 25% , respectively,all significantly higher than those of the csIMRT group (41.7%, 25%, and 8.3% respectively, x2 = 4.11,P < 0.05).However, there were not significant differences in the total survival rate, and the CR and PR of the esophageal lesion between these 2 groups.The major toxicity observed was grade Ⅰ -Ⅱ leukoctyopenia.Conclusions sIMRT generates desirable dose distribution for neck and upper thoracic esophageal carcinoma.hsIMRT has a better short-term efficacy than csIMRT.High dose radiotherapy toward metastatic lymph nodes helps increase progression-free survival.

Hyperlipidemia pancreatitis in children is mostly genetic metabolic disease.The incidence of acute pancreatitis in children is only (3-13)/100 000, and pancreatitis caused by hyperlipidemia accounts for 9% of acute pancreatitis.A child suffering from V-type hyperlipidemia pancreatitis was admitted to the Children′s Hospital Affiliated to Nanjing Medical University in July 2019.The missense mutation at position c. 2770G>A of CFTR gene (nucleotide 2770 in coding region changed from guanine to adenine) in children was detected by gene sequencing, thus resulting in amino acid change p. D924N.It is extremely rare to report that CFTR gene mutation causes hereditary pancreatitis, and there is no literature report on c. 2770G>A site.This case is reported as follows, hoping to provide reference and inspiration for pediatricians.
8.Expression of interleukin-6 and hepcidin in diffuse large B-cell lymphoma and their relationship with anemia

Objective To study the expression of interleukin-6 (IL-6) and hepcidin in patients with diffuse large B-cell lymphoma (DLBCL) and their significance in anemia. Methods 45 DLBCL patients with or without anemia were analyzed. Peripheral blood samples were collected during diagnosis, and the concentrations of IL-6, hepcidin, serum ferritin and hemoglobin (Hb) were measured. 24 healthy volunteers were collected as controls. Results The levels of plasma hepcidin and IL-6 in patients with DLBCL were (347±171)μg/L and 0.27 ng/L (0-9.61 ng/L), respectively, and compared with those [(175 ± 92)μg/L] and 0 ng/L in healthy controls, the differences were statistically significant (both P<0.001). Plasma hepcidin levels in patients with high lactate dehydrogenase (LDH) (P=0.003), B symptoms (P=0.040) or age-adjusted international prognostic index (IPI)>1 (P=0.010) were increased. The levels of IL-6 in patients of male (P=0.003), stage Ⅲ-Ⅳ (P=0.008) or IPI>1 (P=0.004) were significantly higher. The level of hepcidin was highly correlated with serum ferritin (r=0.77, P<0.001), weakly correlated with IL-6 (r=0.31, P=0.030), and not correlated with Hb (r=-0.12, P=0.3). There was a negative correlation between IL-6 expression and Hb (r=-0.35, P=0.009). Multivariate analysis showed that IL-6 could predict anemia (P=0.03), whereas hepcidin could not (P=0.89). Conclusion The elevated hepcidin level is frequent in DLBCL, and the elevated IL-6 plays the major role in the development of anemia.
9.Extraction of the salt-tolerant cDNA in mangrove Avicennia marina by mRNA differential display.

RNAs of the leaves in Avicennia marina, which cultured in 50@1000 and 0@1000 salinity condition respectively, were isolated for mRNA differential display analysis. Screened by OligodT12 GC and eight 10-oligonucleotide arbitrary primers, differential cDNA fragments-csrg1(600 bp), csrg2(550 bp), csrg3(480 bp), only appeared in the leaves of Avicennia marina cultured in 50@1000 salinity condition. After detected by RNA dot hybridization, only csrg1 appeared the difference between the RNAs of the leaves in Avicennia marina cultured in 50@1000 and 0@1000 salinity condition respectively, and csrg1 was confirmed as the salt-tolerant cDNA. csrg1 was cloned and sequenced. After searching in Genbank, there were no similar sequences reported.

10.Clinical characterization and treatment of acute spinal cord decompression sickness after repeated dives.

OBJECTIVETo analyze and summarize the characteristics and treatment of acute spinal cord decompression sickness. To explore the factors that influence the treatment effect.
METHODS77 cases of acute spinal cord decompression sickness patients should be divided into 4 groups according to the pressurized treatment and drug treatment options. They were group I, group II, group III and group IV. At the same time they were get hyperbaric oxygen therapy and other treatment. The evaluation index, were Frankel function classification and paraplegia index. There were 17 factors that affected the treatment effect.
RESULTSThe rate of early cure was 57.14% (44/77). The rate of late cure was 74.03% (57/77). Their difference was statistically significant (P<0.05). In 3 months and 1 year the cure rate of group IV and group B were the highest. But there was no difference between them(P>0.05). They were higher than group ii and group I (P<0.05). The Frankel function classification in 3months and 1 year in each group was higher than before treatment (P<0.05). The paraplegia index in 3 months and 1 year in each group was lower than before treatment (P<0.05). In 3 months and 1 year the Frankel function classification was increased gradually and the paraplegia index was decreased gradually in group I , group II, group III (P<0.05). In group IV and group III the Frankel function and the paraplegia index had not significant difference (P>0.05). Among the 17 factors that affect the treatment effect there are 9 factors that affect the proportion of the large.
CONCLUSIONSThe first choice of treatment method for the patients with acute spinal cord decompression sickness would be group III. Drug therapy was also imporpant. At the same time the hyperbaric oxygen therapy and other treatments were taken. Although the cure rate was not high in this article. But most of the cure is within 3 months. Within 1 year.the cure rate still could be improved. 9 factors that affect the efficacy of acute spinal cord decompression sickness was more noteworthy.
