The 109th Annual Meeting of the American Urological Association was held in Orlando, Florida, USA in May 2014, which received more than 1,000 abstracts on prostate cancer (PCa), covering new epidemiological data about PCa, new theories of early screening, novel molecular markers, new surgical methods, new ideas of diagnosis and treatment of castration-resistant PCa, and progress in basic researches. This paper focuses on the new theories of early screening and novel molecular markers of PCa, including the risk factors of PCa, a revolutionary understanding of the relationship between testosterone and PCa, and new application of PSA, new imaging techniques and molecular markers in the early diagnosis of PCa.

2.Applications of global left ventricular myocardial strain in detection of anthracycline-induced cardiotoxicity in breast cancer survivors with postoperative chemotherapy

Objective To assess the clinical significance of three-dimensional speckle tracking imaging (3D-STI) in the evaluation of left ventricular myocardial function in breast cancer survivors with postoperative anthracycline-based chemotherapy.Methods A total of 51 breast cancer survivors with postoperative anthracycline-based chemotherapy were recruited and 31 female healthy volunteers as the controls.Conventional echocardiography and 3D-STI derived parameters [including global longitudinal strain (GLS) and global area strain (GAS)] were measured before and at 3 and 6 anthracycline-based chemotherapeutic cycles.The receiver operating characteristics (ROC) curve was constructed to determine optimal sensitivity and specificity of 3ID-STI derived parameters for the prediction of future cardiotoxicity.Results In comparison with the controls and baseline cases,GLS and GAS deteriorated significantly (P < 0.05 for both).No statistical difference in GCS and conventional parameters showed before and after the initiation of chemotherapy (P <0.05,respectively).The ROC curves showed that GAS as the best 3D-STI predictor of patients who develop cardiotoxicity during the follow-up.The area under ROC(AUC) of GAS was 0.894,and its optimal cut-off value was-28.4%,with a specificity of 88.0% and a sensitivity of 82.9%.AUC of GLS was 0.802,and its optimal cut-off value was-14.3%,with a specificity of 62.0% and a sensitivity of 85.4%.Conclusions Early decreases in GAS and GLS based on 3D-STI may allow the prediction of subsequent cardiotoxic development accurately in breast cancer survivors with postoperative anthracycline-based chemotherapy.
3.Relationship between insulin resistance and erythrocyte insulin receptors in patients with cerebral infarction

BACKGROUND: Epidemiologic studies have shown an association between higher insulin levels and coronary artery disease, and metabolic studies have associated insulin resistance and compensatory hyperinsulinemia with non-insulin-dependent diabetes mellitus, hypertension, obesity,and lipid disorders.OBJECTIVE: To investigate the relationship between insulin resistance (IR) and erythrocyte insulin receptors (EIRs) in the patients with cerebral infarction (CI).DESIGN: Case-control trial.SETTING: Department of Neurology, China-Japan Union Hospital of Jilin University.PARTICIPANTS: From January 2004 to October 2004, 40 patients with CI, who were in-patients in China-Japan Union Hospital of Jilin University,were selected for the study. Meanwhile, 30 healthy doctors or nurses were recruited as normal controls.METHODS: The levels of blood glucose and serum insulin under fasting and 2-hour after oral glucose tolerance test (OGTT) were detected in the 40 patients with CI and 30 healthy doctors or nurses. Fasting blood glucose multiplied by fasting serum insulin was insulin resistance index (IRI). The number of insulin receptors and their binding affinity on every erythrocyte were determined using modified Gambhir's method. The correlation between the number of EIRs and IRI was analyzed.MAIN OUTCOME MEASURES: Comparison of CI group with controlRESULTS: Data of 40 patients with CI and 30 controls were analyzed, and under fasting and 2-hour after OGTT: The level of serum insulin under fasting and blood glucose, serum insulin at 2-hour after OGTT in CI group were higher than those in normal group [(13.30±5.15), (9.85±4.36) mU/L,(8.27±1.65), (6.32±1.37) mmol/L, (75.21±21.12), (28.26±6.31)mU/L,P < 0.01,EIRs: The number of insulin receptors with high and low affinity and maximum specific binding rate in the patients with CI were significantly less than those in normal group [20.30±4.50, 23.80±4.10; 2 223.80±509.30,2 610.10±435.10; (10.62±3.55)%, (13.21±2.94)%, P < 0.01]. Multiple linear regression analysis and correlation analysis showed the numbers of two types of EIRs in the patients with CI were negatively correlated with IRI (r=-0.458, -0.439, P < 0.01, both).CONCLUSION: Insulin resistance can lead to cerebral infarction. Decrease in insulin receptors may play an importance role in cerebral infarction induced by insulin resistance.
4.CT-guided radiofrequency ablation of isolated pulmonary metastases from colorectal carcinoma

Objective To evaluate the safety,local efficacy and survival rates of isolated pulmonary metastases from colorectal carcinoma treated with CT-guided radiofrequency ablation.Methods A total of 28 patients with isolated pulmonary metastases from colorectal carcinoma were performed with CT-guided radiofrequency ablation (radiofrequency ablation group).The size of the lesion was 1.2-5.0 (2.9 ± 1.0) cm.During the same time,9 patients,who did not take any treatment for isolated pulmonary metastases from colorectal carcinoma,were collected as control group.The size of the lesion was 2.0-5.0 (3.6 ± 1.0) cm.The local progression-free rates and overall survival rates were compared.Results All the cases were able to complete the radiofrequency ablation in radiofrequency ablation group.During and after the procedure,no major complications occurred.The 1-,2-,and 3-year local progression-free rates and overall survival rates in radiofrequency ablation group were significantly higher than those in control group.The local progression-free rates were 89.3% vs.76.2%,78.6% vs.33.9%,70.7% vs.16.9%.The overall survival rates were 96.4% vs.77.8%,85.7% vs.44.4%,56.7% vs.22.2%.The differences between two groups had statistical significance (P < 0.01).Conclusion CT-guided radiofrequency ablation for isolated pulmonary metastasis from colorectal carcinoma is a safe,effective,minimally invasive treatment,and can significantly improve the local progression-free rates and overall survival rates.
5.Effects of Components of Medium for Photo-synthetic Bacteria on its Synthesizing Carotenoid

The medium of photo-synthetic bacteria synthesizing carotenoid was optimized with response surface methodology. The regression equation expressing the relationship between carotenoid yield and main components of culture medium was established by stepwise analysis. The R-squared in the model of regression equation was 0. 958, which meant the established equation could predict the carotenoid yield well at the range of factors in this design. It was indicated that the optimum medium components were: 0. 81% citric acid, 0. 35% NH4Cl and 0. 18% corn syrup. On those conditions, it was predicted that the highest production of carotenoid was 13. 34 mg/L, which was 2. 04 times higher than initial carotenoid yield before optimization.
6.SNP Detection Relative to HBV Infection by DNA Piezoelectric Biosensor

OBJECTIVE To establish a SNP detection method by DNA piezoelectric biosensor and detect a SNP relative to HBV infection. METHODS To establish a model experiment with synthesis DNA sequences as target and find the lowest sensitivity. After extraction of genome DNA from inpatient blood sample, the SNP sites located in ESR1 gene region in samples were detected by SNP detecting method established. RESULTS The frequency shift of target-A was 416.0?21.5Hz, the frequency shift of target-G was 9.4?5.0Hz. And it could be detected that the lowest sensitivity of target-A was 2?10-11 mol/L. The three genotypes of blood samples, TT, TC and CC, had different frequency shifts, 109.4?13.4Hz, 52.0?11.4Hz and 7.2?4.5Hz, respectively. CONCLUSIONS SNP in blood sample could be detected specifically and sensitively by DNA piezoelectric biosensor.
7.Identification of Clostridium perfringens Causing Human Disease by Multiplex PCR

OBJECTIVE To search a method for identifying Clostridium perfringens and genotyping their toxin for gene diagnosis by multiplex PCR.METHODS The mutiplex PCR was developed with three sets of primers(designed) based on the sequences of three C.perfringens toxin genes(CP?,CP? and CPE) published in GenBank to identify C.perfringens and genotype their three toxin genes.RESULTS Three expected(sequences) were (obtained) successfully by multiplex PCR and identified by electrophoresis.CONCLUSIONS The(specific) sequences of C.perfringens could be amplified and their three genes of toxins could be identified by this multiplex PCR(system).Such method should be helpful for developing gene diagnosis well.
8.Immobilization Method on Piezoelectric Quartz Crystal DNA Sensors Based on Monolayer SAM

OBJECTIVE To discuss a highly effective method to immobilize probe on the surfaces of piezoelectric DNA sensors.METHODS Pseudomonas aeruginosa probe was immobilized on the gold surface of gene sensor(array) with routine self-assembly method(SAM)(non-reduction method) and SAM with deoxidized probe((reduction) method),respectively.The changes in frequency and time-cost were compared in reactions with(different) concentrations of probe.RESULTS Reduction method had the advantage of more probe immobilization;less time consumed in testing and higher changes in frequency during the reaction than non-reduction method.CONCLUSIONS Reduction method has a better ability to immobilize probe on the surfaces of piezoelectric DNA sensors.
9.Evaluation on safety and efficacy of tirofiban in patients with acute ST-elevation myocardial infarction undergoing emergency percutaneous coronary intervention

ObjectiveTo evaluate the safety and efficacy of tirofiba in the treatment of patients with acute ST-elevation myocardial infarction (STEMI) undergoing emergency percutaneous coronary intervention (PCI). MethodsA total of 158 patients with acute STEMI were randomly divided into tirofiban group 1 (59 cases, received tirofiban before PCI), tirofiban group 2 (56 cases, received tirofiban when PCI) and control group(43 cases, only received PCI). The coronary reperfusion flow(TIMI grade) of infarct related artery (IRA) after PCI, the resolution of the sum of ST segment elevation(sum STR) at 90 min after the procedure, the changes of myocardial enzyme at 6 h and 12 h afterwards, the left ventricular ejection fraction (LVEF) 1 week later, the major adverse cardiac events(MACE) within 30 d, bleeding and thrombocytopenia complications were analyzed and compared among the three groups. ResultsTIMI reperfusion grades in tirofiban group 1[98.3%(58/59 )]and tirofiban group 2[92.9%(52/56)]were higher than those in control group[60.5%(26/43)](P <0.05). The resolution of sum STR at 90 min after PCI in tirofiban group 1 [(89.3 ± 6.9)%]and tirofiban group 2[(82.4 + 7.3)%]was higher than that in control group[(65.6 +8.1 )%](P< 0.01 ),and there was significant difference between tirofiban group I and tirofiban group 2 (P<0.05 ). The occurrence of MACE within 30 d was lower in tirofiban group 1 and tirofiban group 2 than that in control group (P< 0.05). The level of CK-MB at 6 h and 12 h afterwards was lower in tirofiban group 1 than that in tirofiban group 2,and tirofiban group 2 was lower than control group (P< 0.05). LVEF 1 week later in tirofiban group 1[(56.2 + 6.4)%]was higher than that in tirofiban group 2[(51.1 + 4.9)%]and control group[(49.8 + 5.7)%](P < 0.05),but there was no significant difference between tirofiban group 2 and control group (P > 0.05). Although bleeding incidence in tirofiban group 1 and tirofiban group 2 was higher than that in control group, no severe bleeding and thrombocytopenia was observed. Conclusion Tirofiban can safely and effectively reduce the incidence of the ischemic events in the patients with acute STEM1 during preoperative of emergency PCI.
10.Comparison and analysis of cognitive assessment between Loewenstein occupational therapy cognitive assessment and mini-mental state examination in stroke patients

Objective To compare the assessment results of Loewenstein occupational therapy cognitive assessment (LOTCA) and mini-mental state examination(MMSE) in stroke patients,and to analyze the correlation between the two assessments and to explore the applicable values of LOTCA in assessing vascular cognitive function impairment (VCI).Methods According to the diagnostic standard of cognitive impairment with MMSE,thirty stroke patients with cognitive impairment were selected as cognitive impairment group,thirty stroke patients without markedly cognitive impairment as stroke control group,and thirty normal subjects served as normal control group.All the subjects' age,gender and level of education were matched in the three groups. All the subjects were assessed with LOTCA and MMSE.ResultsThe total scores of LOTCA had high positive correlation with the total scores of MMSE in cognitive impairment group ( r =0.934,P < 0.01 ),the correlations were also found between every sub-items of LOTCA and those of MMSE (P < 0.01 ).The total scores and the scores of sub-items of LOTCA in cognitive impairment group were significantly lower than those in stroke control group and were those in normal control group(P < 0.01 ),particularly lower scores in LOTCA sub-items of thinking operations,orientation,visuomotor organization.The scores of sub-items of LOTCA,including thinking operations,visuo-motor organization,attention,orientation,spatial perception were all significantly lower in stroke control group than those in normal control group(P <0.01 ),especially in thinking operations and visuo-motor organization sub-items.Conclusions Correlations were found between LOTCA and MMSE.Compared to MMSE,LOTCA could detect vascular cognitive function impairment earlier and is much more comprehensive,so it is suitable to assess vascular cognitive impairment patients with no dementia.
