1.Incidence and risk factors of retinopathy of prematurity among the multiple and single gestation births

Objective To analyze the incidence and the risk factors of retinopathy of prematurity (ROP) among the multiple and single gestations births.Methods A prospective control study was applied to the research.The preterm infants were selected that from September 2013 to December 2015 in neonatology department of our hospital.They were divided into multiple gestations group (52 cases) and single birth infants group (600 cases).RetCam Ⅲl retina camera and binocular indirect ophthalmoscope video were used to screening the ocular fundus.The cure rate of the two groups was compared with chi-square test.Results The incidence of ROP in multiple gestations group (32.69%) was higher than single birth infants group (20.83%) (P =0.047).There was significant difference about the multiple and single gestations births in ≤ 30 weeks (P =0.040),and there was no statistically significant difference in > 30 weeks (all P >0.05).With the increase of birth weight,the ROP detection rate of two groupa was reduced.There was no statistically significant difference about different birth weight among the two groups (all P > 0.05).The oxygen usage of > 5 days ROP detection rate was higher than 5 days or less in the two groups (P =0.025,0.001).There was no significant difference in ROP detection rate between two groups with ≤5 d oxygen usage.There was significant difference about the multiple and single gestations births in oxygen usage of > 5 days (P =0.020).Multivariate analysis found that multiple gestations and small gestational age were risk factors for ROP.Conclusion The smaller gestational age,the lower birth weight,and the longer oxygen therapy with the preterm infants,the higher the incidence of ROP.The number of different gestational is also the influences factors of ROP.Multiple gestations overall incidence of ROP is higher than the single birth infants.The smaller the gestational age and the longer oxygen usage,multiple gestations incidence of ROP is higher than single gestation in different births.However,there is no relationship in different birth weight among the multiple and single gestations.

Based on the current situation and the application situation of Intranet and Internet technologies of Gansu Provincial Maternity and Child-care Hospital,the paper takes advantage of interconnection and interworking technologies of Intranet and Internet,constructs the day parting appointment registration and examination system in a variety of ways,introduces the overall design,deployment and implementation of the system,and summarizes the application experience.This system saves greatly the time for the queuing of patients,relieves the pressure of waiting for treatment,and improves the visiting experience of patients.
3.CXCR4 expression of bone marrow CD34+cells in myelodysplastic syndromes and its correlation with cell migration

Objective:To evaluate the expression of CXCR4 and the migration rate of bone marrow stromal CD34+cells in differ-ent risk groups with myelodysplastic syndromes (MDS) using correlation analysis. Methods: Forty MDS patients were divided into low-and high-risk groups based on the International Prognosis Scoring System (IPSS). The former was composed of 20 patients with IPSS<1.5, whereas the latter was composed of 20 patients with IPSS≥1.5. Bone marrow (BM) samples of these patients and 10 nor-mal controls were collected. CD34+cells were separated and purified. The expression of CXCR4 was determined by flow cytometry. The migration rate of CD34+cells on the chemotactic effect of SDF-1αand on the effect of bone marrow stromal cells were measured. Results:The expression rate of CXCR4 was higher in the high-risk MDS group than in the low-risk and control groups (P<0.000 1). No significant differences existed between the low-risk and the control groups (P>0.05). The migration rate of CD34+cells on the ef-fects of SDF-1αand marrow stromal cells were significantly increased in the high-risk MDS group compared with those in the low-risk and control groups (P<0.000 1). Migration rate of CD34+cells on the effect of marrow stromal cells was positively correlated with CX-CR4 expression (P=0.000 1). Conclusion:The CXCR4 expression and migration rates of CD34+cells on the effect of marrow stromal cells are significantly higher in the high-risk MDS group than in the low-risk group. Migration rate has a positive correlation with the CXCR4 expression, which further indicates that MDS is a heterogeneous group of hematopoietic stem cell malignancies. The expres-sion and function of SDF-1 and its receptor CXCR4 differ within each group with various risks. SDF-1 and CXCR4 may be involved in MDS pathogenesis.
4.Review of systematic reviews of acupuncture for diabetic peripheral neuropathy

Objective: To review the systematic reviews of acupuncture for diabetic peripheral neuropathy (DPN) and to provide evidence for clinical decisions. Methods: Published systematic reviews targeting acupuncture treatment of DPN were searched using computer through both Chinese and English databases till July 1, 2019. Two researchers screened the papers based on inclusion and exclusion criteria and conducted report quality evaluation, methodological quality assessment and evidence quality grading using the preferred reporting items for systematic reviews and meta-analyses (PRISMA), assessment of multiple systematic review 2 (AMSTAR 2) and grading of recommendations assessment, development and evaluation (GRADE). Results: Ten systematic reviews were included, involving 11 outcome measures. According to PRISMA, 6 items were sufficiently reported while 1 item was not; AMSTAR 2 appraised that all the included systematic reviews were of low quality in the methodological evaluation; according to GRADE, of the 30 clinical evidences, only 5 were graded moderate while the remained were graded low or extremely low. Descriptive analysis showed that acupuncture can significantly improve DPN symptoms, accelerate the conduction velocities of sensory and motor nerves, and up-regulate the content of plasma nitric oxide (NO), while the adverse reaction rate was low. Conclusion: Acupuncture can produce satisfactory clinical efficacy in treating DPN, but the existing problems, such as low-quality evidence, unitary outcome measures, poor methodological quality of systematic reviews and nonstandard reporting, need to be treated cautiously; meanwhile, more high-quality clinical trials are required to elevate the level of evidence.
5.Apoptosis-inducing effect of carbazole alkaloid (HY-1) in human erythroleukemia K562 cells.

OBJECTIVETo investigate apoptosis-inducing effect and its mechanisms of HY-1, a carbazole alkaloid, on human erythroleukemia K562 cells.
METHODSCell proliferation was detected by sulforhodamine B (SRB) assay after treated with HY-1 at indicated doses. Cell cycle analysis was performed by flow cytometry, mitochondria membrane voltage change was assessed by rhodamine 123 staining, annexin V-PI apoptosis detecting kit and DNA agarose gel electrophoresis were used to identify apoptosis-inducing effect of HY-1. The alterations of apoptosis-relating proteins were detected by Western blot.
RESULTSThe IC(50) of HY-1 in K562 cells was (29.05 +/- 0.90) micromol/L by SRB assay. HY-1 had significant apoptotic inducing effect on K562 cells in a dose- and time-dependent manner as verified by appearance of Sub-G(1) peak on histogram of flow cytometry analysis, reduction of mitochondria membrane voltage, appearance of double positive cell group in Annexin V-PI apoptosis detecting test, and remarkable DNA ladder. The expression of cytosolic cytochrome c was apparently increased. Pro-caspase-9, pro-caspase-3 and PARP were all cleaved to active segments. There was no change in the expression of caspase-8.
CONCLUSIONHY-1 exerts its anticancer activity through triggering apoptosis of K562 cells by mitochondria-activating pathways.

6.Clinical effects of nasal alar sailboat resection combined with catgut embedding therapy for severe hypertrophy of nasal alar

Objective To study the clinical effects of sailboat resection of nasal alar flap combined with catgut embedding in the treatment of severe hypertrophy of the nasal alar.Methods Patients with severe alar hypertrophy were photographed before operation,and the length of alar,the height of nasal ala,the length of nostrils and the width of nostrils were measured.After one month and half a year follow-up,the length of alar,nasal alar height,nostril length,nostril width and nasal width were measured,and the local scar condition was scored according to Vancouver scar scale (VSS).Results Through the combination of two kinds of surgical treatment of severe nasoalar hypertrophy,the surgical effect of satisfaction showed that,with the comparison before and after surgery,nose length and height,length and width of the nostril and the nasal base width were significantly reduced (P<0.05) after half a year and one month after operation.The data showed no significant changes (P>0.05),and so the postoperative effect could be sustained;local scar scale,after half a year and one month after operation,were significantly decreased (P<0.05),and the postoperative long-term scar was not obvious.Conclusions The effect of alar sailboat-resection combined with catgut embedding therapy is satisfactory for the treatment of severe alar hypertrophy.It is one of the ways to treat severe alar hypertrophy.
7.Prognosis of patients with huge hepatocellular carcinoma after R0 resection

Objective To study the prognosis of patients with huge hepatocellular carcinoma (HCC) after R0 resection.Methods 517 patients with primary HCC who underwent R0 resection from January 1997 to December 2008 at the Affiliated Hospital of Medical College Qingdao University were analyzed retrospectively.Results The 5-and 10-years overall survivals (OS) in patients with huge HCC (≥10 cm; n=69) and in patients with HCC <10 cm (n=448) were 24%,18% and 49%,30%,respectively.The median OS was 23.0 and 58.0 month (P<0.001,log rank test) ; and the median disease-free survivals (DFS) were 15.3 and 34.8 month (P<0.001),respectively.The recurrence rate within the first year and the extrahepatic recurrence after resection in patients with huge HCC was significantly higher than in patients with HCC <10 cm (44.9% vs.24.3%,P=0.022;32.7% vs.16.0%,P=0.004).Independent poor prognostic factors of OS and DFS for patients with huge HCC after R0 resection were portal hypertension and vascular invasion.Preoperative transcatheter arterial chemoembolization (TACE) was an independent prognostic factor for better DFS.Conclusions Surgical resection for huge HCC is safe and feasible.For huge HCC after resection,portal hypertension and vascular invasion were poor prognostic factors.Preoperative TACE improved DFS after resection.
8.Galactose base level of N-sugar chain and clinical significance in immunoglobulin of schizophrenia

Objective To investigate the correlation among serum galactose immunoglobulin of quantitative change, total IgG and CIC(circulating immune complexes)levels in patients with schizophrenia.Methods Test the number changed of PVL affinity type Ig in 80 patients with schizophrenia and 80 controls′serum with the PVL affinity ELISA method.The specimens without central nervous system organic disease,heart,liver,kidney diseases, and autoimmune diseases are collected from March to June 2015 in Hospital 261 of PLA′s hospitalized patients.The total of cases is 80.The cases of male are 52,and the cases of female are 28.The age is from 19 to 60,and the average of age is(27.2 ±6.2).Control group is selected from health checkers from the same petiod.The cases of male are 52,and the cases of female are 28.The age is from 19 to 61,and the average of age is(27.8 ±5.4).The relationships among serum PVL-Ig,total IgG and CIC levels in patient group and control group were investigated by rectilinear correlation analysis.Results The serum PVL-IgG level(3.128 ±0.531)(A value)in schizophrenia was significantly higher than control group(2.672 ±0.492)(A value),(t=3.846,P<0.01).PVL-IgM(0.351 ±0.082)(A value)and PVL-IgA(1.287 ±0.205)(A value)compared with the control group(0.362 ±0.073,1.203 ± 0.216)(A value), there was no difference(t=0.241,0.6013;P>0.05).There was positive correlation between serum PVL-IgG and CIC in patients(r=0.423 1,P<0.01).There was no significant correlation with IgG,IgM and IgA.Conclusions People with schizophrenia have an excess of PVL-IgG expression,which maybe participated in the pathological process of immunological injury.PVL-IgG may be one of the molecular mechanisms of patients′CIC stranded or cleared of obstacles.
9.Preliminary experiences with the da Vinci S surgical system in thoracic surgery

Objective The goal of this report was to evaluate the applicability of the da Vinci S surgical system in thoracic surgery.Methods The da Vinci S surgical system consists of a console, a patient cart, and a vision system.The patient cart loading with 3 robot arms and 1 central endoscope arm connects the console which commands the movements of the arm tips.The robot arm tips are introduced via small chest wall ports and attached to the arms of the robot.The surgeon, sitting at the console, manipulates highly sensitive sensors that transfer the surgeon's movements to the arm tips.The so called EndoWrist'technology offers up , down, left, right, and twist, seven degrees of movements, thus exceeding the capacity of a surgeon's hand in open surgery.17 intrathoracic lesion cases, including 12 Myasthenia Gravis, 1 diaphragm hernia, 2 esophageal cancer, 1 pulmonary cancer , 1 pneumothorax, were evaluated for clinical application of the da Vinci S surgical system.Results Out of 17 surgical procedures, 14 procedures were done using the robot from beginning to the end, including 12 thymectomies, 1 diaphragm hernia repair, 1 pulmonary bleb dissection.Only gastric mobilizations, the abdominal part procedures were done using the da Vinci S system, and the thoracic part procedures were done through small incision thoracotomy in 2 cases with esophageal cancer.One resection of left upper lobectomy had to be converted due to surgical problem.The postoperative courses were uneventful.Conclusion The da Vinci operating robot can do nearly all kinds of thoracic operations.Advanced general thoracic procedures can be performed safely and effectively with the da Vinci S robot allowing precise dissection.This benefit becomes evident most elegantly in thymectomies.The robot operation procedures can be done by the doctors with open and assisted thoracopic surgery experiences and other personnels getting trained in a short period of time, but case selection and preparing emergency thoracotomy at any time is needed to ensure the patient safety.
10.Study on a pedigree with Leydig cell hypoplasia caused by novel mutation of luteinizing hormone receptor

Objective To investigate a Chinese pedigree suffering from Leydig cell hypoplasia ( LCH) based on clinical data and genetic diagnosis. Methods The patient was diagnosed by means of clinical data, hormone profiles, and human chorionic gonadotropin ( hCC) test. The luteinizing hormone/chorionic gonadotropin receptor(LHCGR) gene of the patient and family members was amplified and sequenced. Results The patient presented with male pseudohermaphroditism, low level of testosterone, which did not respond to hCG. Genetic analysis of the LHCGR revealed two novel mutations: a missense mutation located in exon 5, resulting in Ile replaced by Thr in the extracellular domain; and a splice site mutation in the 3' terminal of intron 6( IVS6-3 C→A). Proband's sister (46, XX) who lacked clinical manifestations showed the identical genotype with the patient. Conclusions A mutation in the consensus sequence of 3' splice site, in addition to a missense mutation (Ile 152Thr)in the extracellular ligand-binding domain is the cause of inactivation of the LHCGR gene in patient with Leydig cell hypoplasia.
