1.The Significance of Bone Scan in Trauma in Orthopaedic Field

Clinical experience with 99mTc-methylenediphosphonate was presented in 115 patients with trauma, who were treated at Seoul University Hospital from Jan. 1980 to Dec. 1983. The results were summarized as followings: 1. Bone scan exhibited increased uptake in all cases examined within 1 year of trauma and the frequency of scan positivity was diminished as the interval between trauma and the scan increased. 2. The possible factors associated with positive scans in cases more than 2 years after trauma were malunion and post-traumatic arthritis, but the age, general condition of the patient were not appeared to be significantly related to persistent positive scan. 3. Bone scan was thought to be sensitive enough to detect early stress fracture ot accompanied by radiographic abnormali ty. 4. It seemed to be reasonable to judge that the fracture was old when the bone scan of vertebral compression fracture in the young age group was negative except very early stage of trauina. 5. Bone scan was very sensitive to find early post-traumatic avascuhr necrosis in femoral neck fracture when the roentgenographic finding was equivocal.

2.Case-Control Study on Some Risk Factors of Cerebrovascular Diseases in Rural Community: Matched Analysis with One Control per Case.

The main objectives of this study were to know the prevalence of cerebrovascular disease, to find the important risk factors of cerebrovascular diseases. This study is a part of Eumseong Community Health Project supportes by GTZ(West German Government). 116 perceived cases of cerebrovascular disease were first screened by health interview and examinations and 80 cases were diagnosed as a cerebrovascular group. For comparison, 80 cases were matched with their neighbor controls of the same sex and the similar age. 1. The prevalence rate for cerebrovascular diseases was 476.3 per 100,000 population which is considered to be very high compared with that of other countries. Age adjusted rate for cerebrovascular diseases was 261.6 per 100,000. 2. Among the risk factors examined for the association with cerebrovascular diseases by case-control study, and analyzed by paired marginal test(McNemar's chi-square-test) and odds ratio, only hypertension showed high significant statistical association.

3.The Role of CTGF in Osteosarcoma Progression.

PURPOSE: To examine the expression of Connective Tissue Growth Factor (CTGF) in osteosarcoma and to evaluate its role in osteosarcoma invasion and proliferation. MATERIALS AND METHODS: The mRNA expression of CTGF from 23 patient-derived osteosarcoma cell lines was examined, and the role of CTGF in cell invasion and proliferation was examined using siRNA transfection. RESULTS: The over-expression of CTGF mRNA was observed in 17 cell lines (74%). CTGF-specific siRNA transfection into SaOS-2 and MG63 cell lines resulted in efficient knockdown of CTGF expression on Western blot analysis. siRNA transfected cells showed decreased migration on Matrigel invasion assay and decreased cell proliferation on WST-1 assay. CONCLUSION: These results indicated that the CTGF expression may play an important role in osteosarcoma progression, and may be a therapeutic target of osteosarcoma.

5.Results of Limb Salvage Surgery in Primary Malignant Bone Tumors

Limb salvage operation in primary malignant bone tumors is increasing recently, according to the improvement of diagnostic methods, surgical technique and adjuvant chemotherapy. The purpose of this study is to obtain the interim results of survival rate and the functional grade of the limb in primary malignant bone tumors treated by limb salvage operation. Between 1985 to 1993, 63 primary malignant bone tumors treated by limb salvage operations. Tumor prosthesis arthroplasty was performed in 49 patients, resection arthrodesis in 9 patients, and IM nailing with cement molding in 5 patients. Autoclave autograft was combined in 2 patients. Adjuvant chemotherapy and postoperative radiotherapy were performed in selected patients. The average follow-up period was 50 months(ranged 12 months to 116 months). In the cases of osteosarcoma, estimated survival rate was 61.9% based on Kaplan-Meier survival plot. In the parosteal sarcoma, the survival rate was 87.5% and 80% in chondrosarcoma patients. According to functional grading by Enneking, 66% was excellent, 20% was good, 11% was fair and one patients was poor. Complications occurred in 16 patients:wound infection was developed in 6 patients, local recurrence in 4, peroneal nerve palsy in 2 patients and femoral stem loosening in 2 patients. Fracture in resection arthrodesis and pulmonary metastasis were also occurred. Reoperation was performed in 10 patients at average 31 months after initial operation. Revision tumor persthesis arthroplasty was performed in one patient due to local recurrence and in 2 patients due to femoral stem loosening. Three amputations were done due to recurrence of tumor. IM nailing with cement molding was performed in one patient due to deep infection and repeated arthrodesis was done in a fracture patients. Scheduled custom-made tumor prosthesis arthroplasty was performed within a year in 2 patients treated with IM nailing with cement molding. In conclusion, with the careful preoperative assessment, adjuvant chemotherapy and skillful surgical technique, limb salvage operation would provide the primary malignant bone tumor patients for longer survival and better quality of life.

6.Anterlor Fusion for the Tuberenlosis of the Spine

The anterior fusion became the popular measure in the treatment of the tuberculosis of the spine. We analysed the 208 cases of the tuberculosis of the spine (174 anterior fusion) who were treated at our department during the period of 1966 to 1973 and compared these results with that obtained during the period of 1957 to 1966. Following has been observed from this comparison. 1. Spine was the most common site in bone and joint tuberculosis (51.7%) but with decreasing tendency. And it was followed by hip, knee, ankle and sacroiliac joints of order. 2. Common age incidence was below 30 years (72.7%) and most common in below 5 years (20.7%). 3. Average duration of disease was 2 years and 2 months. This value was 6 months lower than in previous study. 4. The most common site was changed from lumbar to thoracic vertebrae. 5. The incidence of sinus, cold abscess formation decreased but paraplegia increased in incedence due to high prevalence in thoracic vertebrae. 6. Anterior fusion was performed in 86.1% of the cases. 7. Postoperative complications was found in 17.2%. Adequate rnanagement of chest tube in thoracic areas and blood vessels in lumbosacral areas may be necessary.

10.The Relationship between Change of Lymphocyte Inositol Monophosphatase mRNA Level by Lithium and Clinical Course in Bipolar Affective Disorder.

OBJECTIVE: Lithium inhibits the action of inositol monophosphatase(IMPase) in phosphoinositide(PI) signal transduction system at therapeutically relevant concentration. The depletion of inositol by lithium itself cannt explain the lithium's therapeutic effect. However, attention has focused on the abnormality of PI signal transduction system as the pathophysiology of bipolar affective disorder(BPD). We investigated whether IMPase mRNA levels of lymphocytes would be different between BPD patients(n=16) and age, sex-matched normal controls(n=16). We also investigated the change of IMPase mRNA level by lithium during 4 weeks to probe the possibility that IMPase mRNA levels could predict the therapeutic response to lithium and clinical course. METHOD: Relative IMPase mRNA levels in lymphocyte were quantified by reverse transcriptase(RT)-PCR in sixteen drug-free BPD patients and sex, age-matched normal controls. The psychopathology of patients were measured using YMRS(Young Mania Rating Scale) and CGI(Clinical Global Impression). RESULTS: There was no significant difference in IMPase mRNA levels between BPD patients and normal controls. And the IMPase mRNA levels were not significantly changed by 4 week treatment with lithium. However, the basal IMPase mRNA levels were negatively correlated with the changes of CGI after 4 weeks. Furthermore, the patients with relatively high basal IMPase mRNA levels showed much more improvement during 4 weeks. CONCLUSIONS: BPD patients and normal controls were not distinguished by lymphocyte IMPase mRNA level. Although we do not support the hypothesis that lymphocyte IMPase activity would be related with the pathogenesis of BPD and the action of lithium, these data raise the possibility that lymphocyte IMPase mRNA levels could function as a predictor of therapeutic response and clinical course of BPD.
