1.Importance of proper window setting in visual assessment of dopamine transporter imaging: A case of early-onset Parkinsonism related to Park2 gene mutation

In the early stages or atypical manifestation of parkinsonism, dopamine transporter imaging can assist
the early diagnosis. We describe a 19 year-old man presenting with progressive gait disturbance,
cervical dystonia and head tremor. 18F-FP-CIT PET (FluoroPropyl-Carbomethoxylodopropyl-nor-BTropane
positron emission tomography) was done and interpreted as normal at other hospital, and his
diagnosis remained baffling. He visited our hospital several months later, and the FP-CIT PET image
was reviewed by the nuclear medicine physician in our hospital, who also interpreted it as normal.
However, we reviewed his FP CIT-PET image because his clinical picture was strongly suggestive of
juvenile parkinsonism. After adjusting the window setting of the PET image, we could appreciate the
decreased uptake in the bilateral basal ganglia. Thus he was finally diagnosed as juvenile parkinsonism
and gene test confirmed Park2 gene mutation. In conclusion, proper window setting is important during
visual assessment of dopamine transporter imaging.

3.Assessment of Autonomic Function in Alzheimer's Disease and Mild Cognitive Impairment Using Heart Rate Variability.

OBJECTIVES: Alzheimer's disease(AD) and mild cognitive impairment(MCI) affect several nervous structures involved with the autonomic nervous system. Association between neuropsychiatric deficits and heart rate variability has been observed. But cardiac autonomic function in AD has been scarcely studied and the results reported are conflicting. We investigated autonomic function in normal control, MCI, AD using heart rate variabil-ity(HRV) technique. METHODS: Time and frequency-domain variability of 5-min R-R interval series was comparatively evaluated in 26 normal control subjects, 22 MCI subjects and 34 AD subjects. Analysis of variance(ANOVA) was used to compare the differences across groups. Correlations between MMSE-KC and HRV components were performed using Pearson's correlation coefficient. RESULTS: No significant difference was observed among the groups in time, frequency-domain analysis of HRV (p>0.05). HRV were not found to be significantly correlated with the degree of cognitive impairment. CONCLUSIONS: There were no differences in HRV with MCI, AD subjects when compared with normal controls. Further investigation is required to use HRV technique as noninvasive parameters of MCI and AD.

6.Attitudes of General University Students and Medical College Students Toward Conducting an Autopsy.

The attitude toward the use of autopsies affects the incidence of performance of the procedure. In foreign countries, many studies on this subjects have been conducted. However, in Korea, no study has been reported. We hypothesized that the general university students may be negative, whereas the medical college students may have positive attitude toward the use of autopsies. In contrary to our hypothesis, there existed only a little difference for the attitude toward conducting an autopsy between the general university students and the medical students. Both groups had positive attitudes toward autopsy. When the medical students become medical doctors, the main reason for not requesting patients to be conducted an autopsy is that their family members may feel offended. These results show that the low incidence of autopsy cases in Korea is caused not by the attitude of the general population, but by the attitude of medical doctors. Thus, we should make every effort to increase the incidence of autopsy by educational programs and discussions about autopsy and death.

8.Laparoscopy-assisted Billroth I Gastrectomy Compared with Hand-assisted Laparoscopic Surgery for Early Gastric Cancer -A Prospective Study-.

PURPOSE: To compare standard laparoscopy-assisted Billroth I gastrectomies including standard lymph node dissection (LABIG) with hand-assisted laparoscopic surgery with the HandPort system (HALS) for the removal of early gastric cancers (EGC). METHODS: A prospective study was performed on 26 patients of EGC at Ewha Womans University Mok-Dong Hospital from July 1999 to August 2001. Seventeen patients (Group L) received LABIG using conventional laparoscopy-assisted methods and 9 patients received LABIG using HALS (Group H). We used staplers for the anastomosis, and a standard D2 lymph node dissection was done with ultrasonic shears or electrocautery. RESULTS: In group L, pathologic reports revealed 14 EGC (stage IA 14 cases), and 3 pm cancers (stage IB 1 case, II 2 cases). In group H, there were 9 early gastric cancers (stage IA 8 cases, IB 1 case). Significant differences (P<0.05) were present between group L and H in regards to the number of harvested lymph nodes (30.8 vs 18.9), estimated blood loss (462.1 vs 286.7 ml) and postoperative transfusion amounts (0.59 vs 0 unit). There were no differences in the mean operating time, distance from the lesion to the resection margin, postoperative leukocyte count, frequencies for pain control, wound size, time to diet, weight loss, serum protein, and postoperative hospital stay. Complications were present in 1 case in group L (enterocutaneous fistula) and 1 case in group H (gastric atony). There was one conversion to open surgery in group H. CONCLUSION: LABIG including standard lymph node dissections with both standard laparoscopic surgery and HALS were performed with equal outcome. The choice of surgical method depends on the characteristics of the lesion and the patient's physical factors.

9.Episodic astasia-abasia associated with hyper perfusion in the subthalamic region and dorsal brainstem

Astasia-abasia refers to the inability to stand or walk despite possessing good motor strength and
conserved voluntary coordination. Although it is usually regarded as a psychogenic disorder, organic
causes have been reported. Herein we describe a patient who presented with alcohol-induced episodic
astasia-abasia. Interestingly, SPECT performed during an episode showed hyperperfusion in the dorsal
brainstem and subthalamic region. These areas roughly coincide with the mesencephalic locomotor
region and subthalamic locomotor region, respectively, and it is conceivable that abnormal neural
activity in these areas is related to the symptoms in our patient.
