1.Comparative observation of small splint and locking proximal humerus plate for the treatment of closed fracture of surgical neck of humerus

Objective To investigate the clincial effect of small splint and locking proximal humerus plate(LPHP) for the treatment of closed fracture of surgical neck of humerus.Methods The clinical data of 130 cases of closed fracture of surgical neck of humerus using small splint and LPHP from October 2006 to January 2011 was retrospectively analyzed.Patients were divided into two groups with 65 cases each by random number table,splint group was fixed proximal humerus with small splitn,LPHP group was fixed with LPHP.Comparing the Constant-Murley shoulder score and radiographic evaluation between two groups.Results According to Constant-Murley shoulder score:the excellent rate was 30.8%(20/65 )of splint group,significantly lower than that of LPHP group [ 61.5%(40/65 ) ],the difference was statistically significant (P <0.05 ).According to radiographic evaluation:the excellent rate was 33.8%(22/65 )of splint group,significantly lower than that of LPHP group [ 72.3% (47/65) ],the difference was statistically significant (P < 0.05).Conclusion LPHP internal fixation for proximal humeral surgical neck fracture patients can get a good reset,fixed reliability,fewer complications,which is an effective method of treatment.
2.Surgical treatment of occult breast cancer

Objective To investigate the surgical treatment of occult breast cancer.Methods The clinical data of 11 cases of occult breast cancer were analyzed retrospectively.Results In 11 female cases aged between 44and 70 years old,2 cases were accompanied with supraclavicular node metastasis.Axillary node enlargement was presented as the first sign,which was located in left side in 4 cases and right side in 7 cases.3 cases underwent axillary lymph lode desection,2 cases accepted axillary node excision and other 2 cases with supraclavicular node enlargement only underwent excisional biopsy.All patients received radiochemotherapy and endocrine therapy.All cases were followed-up from 8 months to 9 years and 6 months,during which 3 cases died,with the 5 years survival rate of 54.5%(6/11).Conclusion Combined treatment is suitable to occult breast cancer.Modified radical mastectomy or breast conserving surgery should be conducted,but axillary lymph node desection may be conducted when the diagnosis is established.
3.The role of fiberopfic ductoscopy in diagnosing and treating nipple discharge

Objective To explore the value of fiberoptic ductoseopy in diagnosis and treatment of nipple discharge diseases.Methods From March 2007 to March 2008,91 patients with nipple discharge were admitted in our hospital.These patients were diagnosed and treated using flberoptic ductoscope FVY-780,and their documents were retrospectively analyzed.Results All the 91 cases were successfully diagnosed and treated with fiberoptic ductoscopy.54 cases(59.3%) were diagnosed as protrusion lesions,19 of whom experinced multiple protrusion,and 5 cases(5.5%) of early breast carcinoma were confirmed by postoperative pathology.37 patients(40.7%) diagnosed as non-protrusion lesions were treated by irrigation therapy,some of whom improved or cured.Conclusions Fiberoptic ductoseopy is the first choice of examination for nipple discharge,and it has important clinical values in choosing procedures for it can show precisely the cause and lesion site.Surgery can thereby be avoided for non-protrusion lesions.Locating the site of lesions in mammary ducts guided by fiberoptic duetoscopy may make operations more accurate and minimally invasive.
4.Professor Wang Zhen’s Experience of Herb Pairs in Treating Cough Caused by Wind

Objective]To summarize Professor Wang’s clinical thoughts and experience of herb pairs in treating“cough caused by wind”. [Methods]To collect and analyse the theory of Wang’s experience of herb pairs in treating“cough caused by wind”by studying from him for a long time. [Results] Professor considered that“cough caused by wind”is a kind of disease characterized by nature of “wind”, which is mainly cough, with wind syndrome unique performance. It is similar to airway hyperresponsiveness diseases,including modern medicine on the infection cough, cough variant asthma, gastroesophageal reflux cough, upper airway cough syndrome, eosinophilic bronchitis, allergic cough, etc, which should be treated by the way of “dispelling wind ventilating lung and relieving cough ”. According to different wind cough sydromes, choose different herb couples.“Feixingcao-Zicao-Xiancao”,“Jiegeng-Shegan”,“Zhipipaye -Zhiziwan -Zhidonghua”,“Zhimahuang -Suye”,“Xinreng -Zhebei”,“Shengbaishao -Shenggancao”are the most commonly used herb pairs in practice.[Conclusion] Professor’s experience of herb pairs in treating“cough caused by wind”is well worth learning for it always gets outstanding effect.
5.Susceptibility phenotype of Staphylococcus aureus and methicillin-resistant Staphylococcus aureus SCCmec genotyping

Objective To explore the characteristics of drug resistance of Staphylococcus aureus and methicillin-resistant Staphylococcus aureus SCCmec genotyping.Methods The disc agar diffusion method was taken to test 500 Staphylococcus aureus susceptibility and methicillin-resistant Staphylococcus aureus by multiplex PCR SCCmec genotyping assay.Results The rate of methicillin-resistant Staphylococcus aureus was 40.0% (200/500),and had a high resistance rate to clindamycin,sulfamethoxazole and tetracycline,which celebrate neomycin and quinolones resist ance rates,while a completely resistant to clindamycin and β-lactam drug,and other performance of multi-drug resist ance,resistance to vancomycin had not uncovered any prime strains; through SCCmec genotyping of MRSA,which mainly SCCmec Ⅲ and SCCmec Ⅳ well SCCmec Ⅴ type,another seven unclassified.Conclusion The separation of Staphylococcus aureus,methicillin-resistant Staphylococcus aureus performance of multiple drug resistance performance of its SCCmec genotyping is SCCmec Ⅲ type and SCCmec Ⅳ and SCCmec Ⅴ type.

7.Expression of glucose-regulated protein in rat retina with acute high intraocular pressure and its significance

BackgroundThe progressive death of retinal ganglion cells (RGCs) is the primary pathological characteristics of visual defects in glaucomatous eye.Research showed that endoplasmic reticulum stress (ERS) is involved in this progression.Glucose-regulated protein 78 ( GRP78 ) is a special marker of ERS.To understand the change in expression of GRP78 in the retina under the pressure induced is very important for the protection of visual function.Objective The present study was to observe the expression of GRP78 in rat retina with acute high intraocular pressure (IOP) and investigate the possible effect of ERS in acute glaucoma damage. MethodsFiftysix Wistar rats were randomly assigned to normal control group,anterior chamber punctured group and 12 hours,1 day,3,7,14 days groups following acute IOP rising.The acute high IOP models were established in the right eyes of 4.0 Wistar rats by paracentesis of the anterior chamber and infusion of normal saline solution into the anterior chamber.The histopathology changes of the retina were examined by hemotoxylin and eosin staining.The expression of GRP78 protein and mRNA in the retina were detected by immunohistochemistry and real-time fluorescence quantitative PCR.ResultsThe retinal layers and cells were clear with normal alignement in the rats of the normal control group and the anterior chamber punctured group.The edema and thickening of retinas appeared in 12 hours after molding and peaked in 1 day after molding.Then the retina decreased in thickness and atrophied from 3 days through 14 days.The expression levels of GRP78 protein (A value) were 0.195±0.006 in the nounal rats and gradually increased from 12 hours through 3 days (0.268±0.017) following molding with the peak value at 1 day (0.499±0.039 ),showing significant differences in comparison with the normal control group (t =0.098,0.304,0.073,P<0.05),and the reduction in expression at 7 and 14 days were not significantly different from the normal control group ( t =0.002,0.001,P>0.05 ).The relative value of GRP78 mRNA ( 2△△C1 ) in the retina was 1.011 ±0.013 in the normal control group and gradually up-regulated from 12 hours through 3 days with the peak value at Iday (2.141±0.171 ) and then down-regulated from the third day onwards.A significant difference was seen in 2-△△C1value between the normal control group and 12 hours group,1 day group or 3 group ( t =0.525,1.130,0.409,P<0.05 ).However,the 2-△△C1 values of GRP78 mRNA at 7 and 14 days was similar to that of the normal control group (t=0.020,0.004,P>0.05).ConclusionsGRP78 participates in the process of RGCs damage following acute high IOP.The results suggest that interfering with ERS may be helpful for protecting the retina and optical nerve from pressure-induced damage.
8. Effect of apigenin on microglia in penumbra after acute transient focal cerebral ischemia-reperfusion injury in rats

Objective: To study the effect of apigenin on the expression of microglia in penumbra and cerebral water content after acute transient focal cerebral ischemia-reperfution injury in rats. Methods: The acute transient focal cerebral ischemia-reperfusion models in rats were established with the modified suture method. Male Sprague-Dawley rats were randomly divided into Sham group, model group, and apigenin groups (there were 6, 24, 48, 72 h, and 7 d reperfusion treatments in the model and apigenin groups, n = 12). All of them are 11 groups. The neurological behavior scores were valued. By FITC labeled isolectin B4 (FITC-ILB4) histochemistry staining, the infiltration of monocytes and the changes of cell morphology and number of brain-derived microglia in penumbra of six rats in every group were observed under laser scanning confocal microscope (LSCM). Water content was measured in isolated brain tissue of other six rats. Results: The positive cells of ILB4 (ILB4+) including microglia cells (Rhod 6G-) and infiltration of monocytes (Rhod 6G+) were found in cerebral ischemic area around penumbra of rats after 6 h ischemia-reperfusion in model group; The morphology changed to Amoeba-like; Microglia increased significantly after 48 h and reached to peak in 72 h, which mainly belonged to the proliferation of brain-derived microglia in Amoeba-like morphology. Microglia cell decreased in 7 d, and microglia in apigenin group obviously decreased more than that in model group (P<.05, 0.01) with the similar morphological change in corresponding time points. In 48 and 72 h of cerebral ischimia, the water content in brain tissue of rats in apigenin group was markedly lower than that in model (P<0.01). There was negative line correlation between the neurological behavior score and the number of ILB4 + cells in penumbra of model group (r=-0.415, P<.05). Apigenin could reduce the degree of neurological deficiency in model group and mitigate the brain injury effectively. Conclusion: A part of microglia cells inpenumbra are associated with brain injury; Apigenin shows the protection on acute transient focal cerebral ischemia-reperfution injury in rats, which maybe relates with down-regulating the microglia cell number and inhibiting the excessive activation of microglia cell.
9. Prenatal assessment of normal fetal pulmonary grey-scale and lung volume by three-dimensional ultrasonography

Objective: To quantitatively analyze the fetal lung echo and right lung volume in the third trimester by real-time three-dimensional ultrasound (3-D US) and evaluate the feasibility of fetal lung maturity. Methods: A total of 732 women with normal singleton pregnancies between 28 and 42 weeks of gestation underwent ultrasound examination. The 3-D US equipment with a 3.5-5 MHz transabdominal transducer was used for the fetal biometric measurement. The echogenicity ratio between fetal lung and liver was compared. The fetal lung volume was calculated by the rotational multiplanar technique for volume measurement (VOCAL). Results: The right fetal lung volume increased with the increase of gestational age with a linear positive correlation (r = 0.884, P<0.01). After 34 weeks, the echogenicity ratio of fetal lung to liver was more than 1.1. Conclusion: The echogenicity of lung/liver and fetal lung volume could be used as normal fetal predictable indicators for fetal lung maturity.
