1.Research process of mTOR pathway in proliferative - relatively diseases in ophthalmology

The mammalian TOR ( mTOR ) pathway is a key regulator of cell growth and proliferation which is also a fundamentally important player in a large variety of human diseases. The pathway regulates many key cellular processes and is implicated in a large number of pathological conditions, including cancer, obesity, diabetes, and cardiovascular disease, age - related diseases and other proliferative disease. It integrates signals from nutrients, energy status and growth factors to regulate many processes, including autophagy, ribosome biogenesis and metabolism. Our article reviews the advancement of laboratory results on mTOR pathway in proliferative diseases in ophthalmology.
2.The evolution of criteria for the diagnosis of Alzheimer′s disease

Alzheimer′s disease ( AD) is the most common disease leading to dementia .With aging of the population , the morbidity of AD is increasing significantly , which brings serious burden personally and socially .So the early diagnosis of AD has be-come the hotspot in the current research field .In order to highlight new ideas for the early diagnosis of AD , the current review will ana-lyze from the first international diagnostic criteria for AD dementia to the latest conceptual framework for research on subjective cogni -tive decline in preclinical Alzheimer′s disease.
3.Study of the binding intensity of bone-cement interface increased by bone cement oscillator in fatigue experiment

[Objective]To evaluate the effect of bone cement oscillator on increasing bone cement-bone interface binding intensity.[Method]Sixty fresh femoral bones of adult pig were randomly divided into experiment group and control group under the 30%,50%,70% ultimate shear strengh of the cement/bone.There were 10 samples in each subgroup.Mechanics test was performed by MTS-810 material testing machine,and the binding intensity of bone cement-bone interface in oscillating cement-bone interface binding intensity in experiment group was compared with control group.[Result]Mechanics test showed bone cement group were significantly stronger than control group under the 30%,50% ultimate shear strengh of the cement-bone(P 0.05).[Conclusion]Bone cement oscillator can increase binding intensity of bone-cement interface,and effectively reduce long-term aseptic loosening of artificial prostheses.
4.Influence of preoperative transcatheter arterial chemoembolization on liver resection in patients with resectable hepatocellular carcinoma

ObjectiveTo study the influence of preoperative transcatheter arterial chemoembolization (TACE) on liver resection in patients with resectable hepatocellular carcinoma (HCC).MethodsTACE was performed before liver resection in 62 out of 126 patients, and perioperative risk factors were compared with that of the 64 patients without TACE including liver perioperative function alterations, average blood loss during operation, the average time of clamping porta hepatis and operation, abdominal drainage at 1-, 2-, and 3-days post-op, mortality and morbidity. ResultsThere was not significant difference in liver function alteration in the two groups, there was no mortality in the two groups. Liver cirrhosis in TACE group was more serious than that in no TACE group, the operation time was longer in TACE group. Blood loss, and abdominal drainage were much more in TACE group than in no TACE group. However postoperative complications did not differ.Conclusion Preoperative TACE for resectable HCC increases surgical difficulty and risk. Preoperative TACE for resectable HCC needs to be used on baseis of strict selection.
5.Optimal antiplatelet therapy after coronary stenting,a single-center randomized controlled pilot study

Objective To explore the feasibility and efficacy of an optimized antiplatelet therapy according to laboratory test after coronary stenting.Methods Between June 2006 and February 2007,a total of 305 patients who underwent coronary stenting in General Hospital of Shenyang Command were enrolled.Patients were randomly assigned to receive optimized(optimal group,n=154)or standard antiplatelet therapy(standard group,n=151).Clopidogrel resistance(CR)was defined as a less than 10% reduction of platelet aggregation at 24h after clopidogrel treatment.The antiplatelet regimen for standard group was dual antiplatelet therapy with aspirin and clopidogrel.In optimal group,CR patients received cilostazol for 6 months in addition to dual anitplatelet therapy,whereas non-CR patients received standard dual antiplatelet therapy.The primary endpoint of present study was the composite analysis of death,myocardial infarction(MI)or stroke.Secondary endpoint was the composite analysis of death,MI,stroke,revascularization or peripheral artery occlusion.Results There were 41 and 33 CR patients in optimal and standard group,respectively.Cilostazol in addition to dual antiplatelet therapy decreased significantly the PAR of CR patients in optimal group(77.5%?12.1% vs 64.5%?12.1%,P
6.Short-term outcomes of high loading dose clopidogrel pretreatment before coronary stenting in patients with acute coronary syndromes

Objective To compare the short-term efficiency and safety of high loading dose (600 mg) clopidogreal pretreatment with that of routine loading dose (300 mg) before coronary stenting in patients with acute coronary syndromes (ACS). Methods Prospective registry method was used in this study. Between February 2003 to July 2004, a total number of 316 hospitalized patients with ACS received 600 mg clopidogrel pretreatment, before coronary stenting. 309 patients with the same disease conditions who received 300 mg clopidogrel pretreatment between October 2001 to January 2003 were included as the control. The primary endpoints were the presence of subacute in-stent thrombosis. 30 days after the procedure. The composite endpoints were death of all causes, myocardial infraction and revascularization of the target ressel. The secondary endpoint was hemorrhagic events at 30 days after the stenting procedure. Results The baseline clinical and angiographic characteristics and the result of stenting between the two groups had no significants difference. 7.A multifunction glasses in the recovery term of cycloplegia and its primary application

Objective:To design functionality glasses in order to solve or alleviate various visual disorder during cycloplegia.Methods:Based on the principle of the polarizing lens and pinhole lens, the glasses frame was combine with them so as to be suitable for the recovery of cycloplegia and for the usage in near and distant vision situation.Results: This glasses has a series of advantages including small size, light weight, convenience and beauty. Through the combination of different lenses, the symptom of visual unclear during cycloplegia wasimproved, and the symptom of glare and squint in distant vision also was improved. Besides, the symptom that adjustment of near vision couldn't be activated was improved, and near vision blurry leaded by peripheral aberration coming from mydriasis also was improved. On the other hand, this device was easy and simple on combination and was convenient on usage.Conclusion: This design contributes to correct refractive error, and enhance the definition of near or distant vision. And it is suitable for various situation of usage including near, distant, indoor and outdoor.
8.Effect of early dirigation on the recovery of foot function after the operation of the fracture of calcaneus: the blind evaluation based on 6-month follow-up

BACKGROUND: Most of the displaced fractures of calcaneus reed operation treatment. Because early postoperative dirigation is sometimes ignored,most patients receive only good anatomic reduction but less pleased functional reduction.OBJECTIVE: To compare the effect of different starting time of the postoperative dirigation on the functional recovery of the joint of the foot after the operation of the fracture of calcaneus with joint surface displacement.DESIGN: Non-randomized and concurrently controlled retrospective analysis with blind evaluation.SETTING: Orthopedic Department of Renji Hospital of Shanghai Second Medical University.PARTICIPANTS: Cases with the fracture of the calcaneus with joint surface displacement, in the Orthopedic Department of Renji Hospital of Shanghai Second Medical University were chosen from October 2000 to June 2003. There were totally 64 cases(72 sides), male 40 sides and female 32sides, whose ages varied from 22 to 68 years old. According to the starting time of the postoperative dirigation, the cases were divided into two groups,the 1-week dirigation group with 34 cases(38 sides), male 20 sides and female 18 sides, and the 6-week dirigation group with 30 cases(34 sides), male 20 sides and female 14 sides.METHODS: Totally 64 cases were involved in the result analysis. In the two groups, the same operative therapeutic methods and postoperative dirigation were taken except the starting time of the dirigation. The dirigation was started respectively 1 or 6 weeks after the operation and lasted for 6weeks. Six months later, follow-ups were done according to the Maryland Grade(excellent 90-100, good 75 -89, moderate 50 -74, poor < 50) to evaluate the activity of the talocrural joint and the stability of the anterior half of the foot and the gait as well.MAIN OUTCOME MEASURES: Primary results: the postoperative function of the injured foot of the two groups. Secondary results: the adverse events and side effect.Maryland Grade: In the 34 cases(38 sides) of the 1-week dirigation group,the average mark was 87. 8, and in the 30 cases(34 sides) of the 6-week dirigation group, the average mark was 73.2, and the difference had signifigroup, 3 cases had poor healing of the incision, and achieved the healing after 2-week medication. And 2 cases had injury of the lateral dorsal cutaneous nerve of the foot. In the 6-week dirigation group 2 cases had poor healing of the incision and achieved the healing after 2-week medication, and 1 case had injury of the lateral dorsal cutaneous nerve of the foot. There was no significant difference between the two groups.CONCLUSION: After the operation of the fracture of calcaneus with joint surface displacement, early application of dirigation is beneficial to the functional recovery of the foot and talocrural joint without unsafety.
