1.The antidotal effects of high-dosage gamma-aminobutyric acid on acute tetramine poisoning as compared with sodium dimercaptopropane sulfonate.

To investigate the therapeutic effect of high-dosage gamma-aminobutyric acid (GABA) on acute tetramine (TET) poisoning, 50 Kunming mice were divided into 5 groups at random and the antidotal effects of GABA or sodium dimercaptopropane sulfonate (Na-DMPS) on poisoned mice in different groups were observed in order to compare the therapeutic effects of high-dosage GABA with those of Na-DMPS. Slices of brain tissue of the poisoned mice were made to examine pathological changes of cells. The survival analysis was employed. Our results showed that both high-dosage GABA and Na-DMPS could obviously prolong the survival time, delay onset of convulsion and muscular twitch, and ameliorate the symptoms after acute tetramine poisoning in the mice. Better effects could be achieved with earlier use of high dosage GABA or Na-DMPS. There was no significant difference in prolonging the survival time between high-dose GABA and Na-DMPS used immediately after poisioning. It is concluded that high-dosage GABA can effectively antagonize acute toxicity of teramine in mice. And it is suggested that high-dosage GABA may be used as an excellent antidote for acute TET poisoning in clinical practice. The indications and correct dosage for clinical use awaits to be further studied.

2.Analysis of surveillance results of iodine deficiency disorders in Xunhua county Qinghai province in 2008

Objective To understand the iodine deficiency disorders in Xunhua county Qinghai province, and to provide reference values for setting up prevention and control strategy. Methods According to historical data of Xunhua county, 1 township center elementary school and 1 village elementary school were sampled in Qingshui, Baizhuang, Hongqi, Jiezi and Galeng townships. A total of 9 elementary schools were investigated (Caleng has only 1 elementary school). Fourty children aged 8-10 years were checked thyroid size by palpation in every school. Twenty children were chosen to assay their urinary iodine and edible salt iodine in every school. Thirty 5-grade students were surveyed by questionnaire on health education in every school. Ten housewives were inquired prevention knowledge on iodine deficiency disorders and salt iodine level was tested in every village. Results Goiter rate was 7.9%(30/378) in children aged 8-10 years, with Baizhuang and Galeng the highest (11.9%,5/42), and Qingshui the lowest (2.4%, 1/42). The median was 196.5 μg/L in children aged 8-10 years, and urinary iodine level lower than 50 μg/L was accounted for 5.3% (11/208), but that value was 25.0% (5/20)in Galeng. The average qualified rate of using edible salt was 91.4%(159/174), and that rates were 79.0%(15/19), 83.3%(15/18) and 89.5%(17/19) in Chajia, Tuanjie and Minzhu villages, respectively. The average score were 2.8, 2.1 in health education survey of the 5-grade students and housewives. Conclusions After years effort in prevention and control of iodine deficiency disorders, remarkable progress has been made in Xunhua County. The monitoring of iodized salt and the knowledge spreading are still need to be strengthened.
3.Stabilometer test for the quantified evaluation of equilibrium function in stroke patients with hemiplegia

BACKGROUND: The patients with hemiplegia of stroke commonly appear balance disturbance and the mean of assessment on it is applied in quality commonly. But the implementation of assessment on rehabilitation schedule and therapeutic results requires much more quantitative standards on balance function.OBJECTIVE: To probe into interrelationship between every parameter of stabilometer test and variables in other function evaluations in the patients with hemiplegia of stroke as well as the value of evaluation on functional recovery in the patients with hemiplegia of stroke.DESIGN: Quantitative assessment based on the qualitative assessment.SETTING: Department of Otolaryngology and Department of Rehabilitation Medicine in General Hospital of Chinese PLA.PARTICIPANTS: Nineteen first-attack cases of hemiplegia of stroke received rehabilitation treatment in Department of Rehabilitation Medicine of General Hospital of Chinese PLA from September 2002 to April 2003, diagnosed as disorder of cerebral hemisphere on lateral internal carotid artery. Of those, 14 cases were males, 5 cases females; aged varied from 33 to 55 years and the course of sickness was at the range from 0.5 to 4 months.METHODS: Nineteen patients with hemiplegia of stroke received functional independence measures(FIM), timed up-go test(TUGT), Berg balance scale (BBS) and evaluation on balance measure with stabilometer. Of those, stabilometer is a kind of quantitative analysis method, including the parameters of total length of locus, speed, peripheral area, average central changing site of X wavering direction, average central changing site of Y wavering direction, etc.MAIN OUTCOME MEASURES: Pearson correlation analysis was used to determine the interrelationship between every parameter measured with stabilometer and other measured functional variables.RESULTS: By intention-to-treat management analysis, 19 cases of hemiplegia of stroke all entered analysis on results. Among the parameters of stabilometer test, the moderate negative correlation presented in the comparison of total length of locus, speed and peripheral area with BBS during eyes opened and closed ( r = - 0. 475 to - 0. 705, P < 0.05), of which, the correlation between peripheral area and BBS score during eyes closed was better(r= -0.475, P < 0. 05) . Correlation did not present in the comparison of average central changing site of X wavering direction and average central changing site of Y wavering direction with BBS during eyes opened and closed( r = - 0. 346 to 0. 234, P > 0.05) . Moderate positive correlation presented in the comparison of TUGT result with total length of locus, speed and peripheral area during eyes closed( r = 0. 508 to 0. 583, P < 0.05). The correlation did not present in the comparison of the parameters determined with stabilometer and FIM ( r = - 0. 048 to - 0. 296, P > 0.05).CONCLUSION: Stabilometer test can determine balance function in quantity in patients with hemiplegia of stroke, and reflect the severity of balance disturbance, of which, total length of locus speed and peripheral area during eyes opened and closed can judge balance function of patients sensibly and reliably.
4.Clinical observation of the tension-free vaginal tape-obturator treatment for 130 cases of female stress urinary incontinence

Objective To observe the clinical short-and long-term curative effect of the surgical treatment of female stress urinary incontinence (SUI) with tension-free vaginal tape-obturator (TVT-O) treatment,and discuss the safety of the operation and postoperative quality of life.Methods The data were collected from 130 patients with SUI who were underwent TVT-O treatment.The patients'perioperative period,follow-up of postoperative complications,and comparison of the quality of life before and after surgery were analyzed retrospectively.Results A total of 130 patients was successfully completed their surgeries with a mean operative time (47.01 ± 18.82)min,average blood loss (64.38 ±99.62)ml,mean catheterization (2.67 ±0.90)d,and the average length of stay (4.73 ±2.14)d.A total of 101 cases was completed the postoperative follow-up with (a) preoperative quality of life in patients with ⅡQ-7 score 6 to 21 points and an average of (15.74 ± 3.87) min,(b) symptoms of lower urinary tract UDI-6 score of 3 to 22 minutes and the average (10.51 ± 3.70) min,(c) postoperative quality of life improved significantly ⅡQ-7 score from 0 to 21 points and an average of (1.59 ±4.37)points,and (e) UDI-6 score 0 to 14 points and an average of (1.63 ± 2.66)points.A total of 73 patients (72.3%) had the postoperative urinary incontinence,which subjective symptom were completely cured,significant improvement was 11 cases (10.9%),ineffectiveness was 6 cases (5.9%),pad using was 13 patients (12.9%),vaginal mesh exposure and erosion was 2 patients(2.0%),the sexual life postoperatively was affected in 8 patients (7.9%).Conclusions TVT-O treatment of SUI is not only easy to operate but also has clinically high security,few complications,low recurrence rate,and significantly improved patients' living quantities.However,patch compli cations can not be ignored and need further discussion.
5.Partial internal sphincterectomy for the treatment of internal anal sphincter achalasia in childhood

Objective To evaluate the long-term results of partial internal sphincterectomy for the treatment of internal anal sphincter achalasia in childhood. Methods The clinical,radiographic,manometrical and histochemical data of 6 cases were reviewed retrospectively. All patients had received partial internal sphincterectomy and were followed-up for 2 to 8 years. Results All patients presented with severe constipation with or without soiling. No stenosis zone of intestine could be noted in 3 patients by barium enema examination. The rectoanal inhibition reflex on rectal balloon inflation was absent in all patients. The normal acetylcholinesterase activity on rectal biopsies was demonstrated by histochemical staining. Ganglion cells within internal anal sphincter was noted in all cases. On follow-up,all patients regained regular bowel habits and are not on any laxatives. Conclusion The long term results of partial internal sphincterectomy for the treatment of internal anal sphincter achalasia in childhood are satisfactory.

7.Nanometer silver dressing plus recombinant bovine basic fibroblast growth factor gel for residual burn wounds

OBJECTIVE: To investigate the curative effects of nanometer silver dressing and recombinant bovine basic fibroblast growth factor gel on burn residual wounds.METHODS: Forty burn patients with residual wounds because of deep second degree burn and full-thickness burn, were randomly divided into control group and management group. There were 20 patients in both groups. The patients of management group were treated by nanometer silver dressing and recombinant bovine basic fibroblast growth factor gel. The patients of control group were treated by saline and paraffin absorbent gauze. Healing time, wound healing rates at different time points,cases of infected wound and results of bacterial culture before and 7 days following treatment, and drug adverse reaction were recorded.RESULTS: The healing time of management group was significantly shorter than the control group (P < 0.01). The wound healing rates of management group was significantly higher than the control group at different time points (P< 0.01). The cases of infected wound was significantly fewer than the control group after treating (P < 0.01). The pathogenic bacteria detection rate was significantly lower than the control group after 7 days (P < 0.01).CONCLUSION: There was better antibacterial activity, decurtating the healing time when the management of nanometer silver dressing and recombinant bovine basic fibroblast growth factor gel on burn residual wounds were put into practice.

Chemotherapy is the preferred therapeutic approach for advanced ovarian cancer, but a successful long-term treatment is prevented by the development of drug resistance. Recent works have underlined the involvement of non-coding RNAs, microRNAs (miRNAs) in cancer development, with several conjectures regarding their possible involvement in the evolution of drug resistance. This study is to investigate the promoting effects and mechanism of miR-125b involved in the development of chemoresistance in ovarian cancer. The different expression of miR-125b in cisplatin-sensitive ovarian cancer cell line (OV2008) and its resistant variant (C13*) was identified by real-time PCR. An in vitro cytotoxicity assay and apoptosis assay using CCK-8 assay and flow cytometry, were carried out to detect the effect of miR-125b and Bak1 on cisplatin resistance of cells. Real-time PCR, Western blotting and luciferase reporter assay were used to detect whether Bak1 is a target of miR-125b. As compared with OV2008 cells, the expression levels of miR-125b in C13* cells were increased. It was found that the up-regulation of microRNA-125b caused a marked inhibition of cisplatin-induced cytotoxicity and apoptosis and a subsequent increase in the resistance to cisplatin in OV2008 and C13* cells. Moreover, Bak1 was a direct target of miR-125b, and down-regulation of Bak1 suppressed cisplatin-induced apoptosis and led to an increased resistance to cisplatin. Our study indicates that miR-125b has a significantly promoting effect on chemoresistance of C13* cells and up-regulation of miR-125b expression contributes to cisplatin resistance through suppression of Bak1 expression. This finding has important implications in the development of targeted therapeutics for overcoming cisplatin resistance in ovarian cancer.
9.Diagnosis and treatment of thrombosis of right hepatic artery after surgery for severe pancreatitis

The treatment for severe pancreatitis includes surgical and non-surgical methods,and the key points of treatment include surgical timing,surgical method selection and the management of postoperative complications.Hepatic artery thrombosis after surgery for severe pancreatitis is rarely seen,and few experiences in the diagnosis and treatment for this disease have been summarized.A patient with the course of severe pancreatitis of 10 years and suffered from 3 different kinds of diseases including thrombosis of right hepatic artery was cured by open surgery for 2 times and intervention therapy in the Affiliated Hospital of Hainan Medical College in October 2011.The treatment experience was summarized based on the clinical data of this patient.
10.Expression of heat shock protein 70 in gastric mucosa of rats during the early stage after severe scald injury and the effect of insulin

Objective To investigate the changes of the expressions of heat shock proteins 70 (HSP70) and insulin and HSP70 protect effect on the gastric mucosa of rats with scald injury, and explore the relationship between insulin and HSP70 . Methods With a model of 30 total body surface area (TBSA) full-thickness burned rats,the expression and distribution of HSP70 in the gastric mucosa was detected with immunohistochemical method and analyzed by a micro-image analysis system, and at the same time the pathological changes of the gastric mucosa tissue of each group were analyzed by Western blot and immunohistochemistry at the 3rd,6th,12th,24th and 48th hour postburn. Results The expression of HSP70 obviously decreased at the 48th hour post scald injury. The expression of HSP70 in the treatment group was significantly higher than that in the scalded group at most time points except the 12th(9.40±1.52,P=0.065) hour and at equal parallel time[(6.80±1.10,8.60±0.55,10.80±1.64,11.40±1.34),P<0.05]. The gastric mucosal injury index in the scalded group was significantly higher than that in the treatment group and at equal parallel time [(4.05±0.36,11.97±1.15,20.98±2.83,13.92±0.94,1.60±0.55),P<0.05]. In pathological observation, the control group manifested the intact gastric mucosal tissue formation, the scalded group showed obvious gastric mucosal tissue injury in the early phase of scald injury, while the treatment group showed less severe injury than the scald group. A positive correlationwas found in the gastric mucosal injury index and HSP70(r=0.904,P<0.01) and also between the serum glucose and HSP70(r=0.961,P<0.01).Conclusions Insulin increased the expression of HSP70 and decreased the gastric mucosal injury index in the gastric mucosal tissue of SD rats in the phase of scald injury. It may be one of the vital mechanisms of insulin protecting the gastric mucosal tissue.
