In recent years,gut microecology has attracted increasing attention because of its close relationship with the development and pro-gression of various liver diseases and associated complications.The impairment of liver function can disturb gut microecology,whereas the imbalance in gut microecology in turn promotes the aggravation of liver diseases.This paper reviews the relationship between gut microecolo-gy and various liver diseases,and discusses the role of modulating gut microecology in the treatment of liver diseases.A new idea is provided for the prevention and treatment of liver diseases based on the modulation of gut microecology.

Objective To investigate the effects of siRNA silencing of inhibitor of apoptosis protein gene on the proliferation of airway smooth muscle in bronchial asthma mice .Methods Tweenty female BALB/c mice were randomly divided into experimen‐tal group and control group .Mice in the experimental group were constructed asthma models .The trachea and bronchi were collect‐ed for cell culture .Respectively ,the cultured airway smooth muscle cells in the experimental group and the control group were grouped:PBS control group (added to PBS solution) ,missense chain control group (transfected with missense chain) and siRNA transfection group (transfected with siRNA Survivin) .The expressions of Survivin mRNA ,Survivin protein and caspase‐9 protein in airway smooth muscle cells were detected .The levels of IL‐6 and CCL5 in cell supernatant were detected .Results RT‐PCR re‐sults showed that ,the relative expression levels of Survivin RNA of airway smooth muscle cells in the siRNA transfection group of the experimental group were lower than the PBS control group and missense chain group ,the differences were statistically signifi‐cant (P<0 .05) .Western blot results showed that ,the expressions of Survivin proteins in the siRNA transfection group of the ex‐perimental group were lower than the PBS control group and missense chain control group ,while the caspase‐9 protein were higher , In the experimental group ,the synthesis and secretion levels of IL‐6 and CCL5 of airway smooth muscle cells in the siRNA transfec‐tion group were lower than the PBS control group and missense chain control group ,the cell proliferations at 2-4 d in the siRNA transfection group were lower than the PBS control group and missense chain control group ,the proportion of G0/G1 phase in the siRNA transfection group were higher than the PBS control group and missense chain control group ,but the proportion of S/G2 phase were lower than the PBS control group and missense chain control group ,cells apoptosis rates of the siRNA transfection group were lower than the PBS control group and missense chain control group ,the differences were statistically significant (P<0 .05) .Conclusion siRNA specific silencing Survivin gene could accelerate the airway smooth muscle cell apoptosis ,and inhibit cell abnormal proliferation and secretion .

To evaluate CT in the diagnosis of tuber culosis of the brain． CT findings in 95 cases with tuberculosis of t he bran confirmed clinically were analyzed retrospectively． Results: The CT detectability fate was 80%． The direct CT si ngs included exudation in basal system, meningitis, enhancement and intracranial tuberculosi s． The indirect signs had hydrocephalus infarction, atrophy calcification, periventricular hypo densith and subdural fluid accumulation etc． The most usual appearance was hydrocephalus． Conclusions: (1) It was helpful to diagnose when very two kinds of direct sings or a kind of dir ect sings and more than two kinds of indirect sings appeared． (2)The plain and contrast canning s hould be routinely used in tuberculosis of the brain．

Objective To investigate the predictive value of red cell volume distribution width RDW),RDW to platelet ratio (RPR),aspartate aminotransferase (AST) to platelet ratio index APRI),fibrosis index based on the 4 factor (FIB-4) and aspartate-alanine aminotransferase ratio (AAR) in assessing esophageal varices (EV) in patients with hepatitis B virus (HBV)-related cirrhosis.Methods Between December 2013 and July 2015,a total of 190 patients with HBV-related cirrhosis and no previous history of endoscopic therapy for EV hospitalized at the Department of Hepatology,Tianjin Third Central Hospital were enrolled.Upper gastrointestinal endoscopy were conducted in all patients to diagnose EV.According to the morphology and degree of EV as well as bleeding risk,patients were categorized into mild,moderate and severe stages.RDW,platelet counts,alanine aminotransferase,aspartate aminotransferase were tested,and RPR,APRI,FIB-4,AAR were calculated.Spearman correlation was used to evaluate the association between EV and these indexes.Receive operating characteristic (ROC) curves were generated and the areas under the curves (AUC) were calculated to assess the performance of these indexes in predicting esophageal varices bleeding (EVB).Results RDW ([16.78±2.27]％),RPR (0.41±0.18),FIB4 (8.99±5.91),APRI (2.09[1.35,2.90]) in patients with EVB were all significantly higher than those ([14.37±1.86]％,t=-7.449;0.19±0.09 [t=-8.132];5.72±3.92 [t=-3.658];1.29 [0.70,2.39;z=-2.996]) without EVB (all P＜0.05).However,AAR had no statistical significance between two groups (t =1.216,P=0.226).Both of EV and EVB had positive correlation with RDW,RPR,FIB-4 and APRI (all P＜0.05) and no correlation withAAR(P＞0.05).RDW (F=9.604),RPR (F=47.530),FIB-4 (F=18.071) and APRI (H=12.320) showed statistically significant differences among patients with different stages of EV (all P＜0.05),whereas AAR showed no difference (F=1.177,P=0.320).The AUC of RPR (0.896) for EVB was highest,followed by RDW (0.824,P＜0.05).AUC of FIB-4 and APRI was 0.690 and 0.642,respectively,with no statistical difference (P＞0.05).The optimal cut-off levels of RDW,RPR,FIB-4 and APRI were 14.450 (sensitivity:88.24％,specificity:64.03％),0.209 (sensitivity:96.10％,specificity:69.10％),6.912 (sensitivity:66.67％,specificity:73.38％) and 1.338 (sensitivity:76.47％,specificity:51.08％),respectively.Conclusions RPR and RDW are closely correlated with EV in HBV-related cirrhotic patients,which are of clinical importance in predicting EVB.Both of them can be used as clinical screening methods and RPR may be superior to RDW.Although FIB-4 and APRI are correlated with EV,their predictive value of EBV are low.

BACKGROUND: Hydroxyapatite is a kind of ideal orthopedic material, but its Iow strength and brittleness need to improve. The research suggested that the improvement of the toughness and strength of bioceramics which composite hydroxyapatite (HAP) and zirconia (3% mol yttria-stabilized cubic zirconia) and fabricated ceramic scalpel possesses of the wound recover faster with smaller side. So it is significant that research on the incision concrescence of nano-sized hydroxyapatite-zirconia bioceramical scalpel.OBJECTIVE: To observe the effects of incision using nano-sized HAP-TZP bioceramics as scalpel.DESIGN: A single sample study.SETTING: Second Department of Orthopaedics, the Second Affiliated Hospital of Harbin Medical University and Harbin Institute of Technology.MATERIALS: The experiment was carried out in the Animal Laboratory of the Second Affiliated Hospital of Harbin Medical University (Provincial Laboratory) from March to May 2006. A total of 54 SD rats, of 4 months old, weighing 120-180 g, of both genders, were selected in this study. The experiment materials included nano-sized hydroxyapatite Quartz Clock Company), ammonia (Suihua Chemical Reagent Company), and anhydrous ethanol (Tianjin Chemical Reagent Company).METHODS: The nano-sized Hydroxyapatite/zirconia composite powder was synthesis by rubbing according to 40% hydroxyapatite + 60% zirconia powders (mass ratio) were mixed and milled. The knife-edge biomaterials, which fabricated by hot pressed sintering, was grinding and used in incision experiment after autoclaving. The back of the rats was shaved and removing a patch of skin under ether anesthesia made a length of 2 cm open excision wound and stitched up the tear by 1# string. The each 18 rats were sacrificed on 3, 7 and 14 days post-wounding and used for analyses respectively. A full-thickness specimen containing the wound was dissected out from each of the surgical sites. The hematoxylin-eosin (HE) staining was applied in the pathological observation.MAIN OUTCOME MEASURES: The tissue sections of back and the results of pathological observation. RESULTS: All 54 rats were involved in the results analysis without any loss. There was long gap with minimal cellular reaction in the scalpel wounds that has not healed on day 3. There was inflamed cells infiltration at the wounded margin edge. By week 1, the tiny detritus which some of them appear the repaired response of epidermis rend were still obviously found in the area of the scalpel wounds where the detritus was found to be significantly less, the wounds changed smooth and started to heal. The optical microscopy observed indicated that there was incomplete epithelialization and hyalinization of dermal collagen at the wounded area. There were obvious inflamed cells to infiltrate in the margin subcutaneous tissue of the wounds where the newborn loose collagenous fibers filled in scalpel wounds was the tissue repairing response phase. There was a great deal of the infiltrated white blood cells in the subcutaneous tissue. The tiny detritus of epidermis by the optical microscopy observed rend could not be found in the area of the scalpel wounds which were healed by week 2. The newborn capillary nets appeared in the newborn repaired collagen fibers tissue. The inflamed cells to be infiltrated were reduced. The compact and strong collagenous fibers filled in scalpel wounds were the normal wound repairing and tissue. The white blood cells infiltrated obvious reduction in the subcutaneous tissue. The normal blood cells in the newborn capillaries could be seen. CONCLUSION: The experiments prove that a HAP-ZTP bioceramic scalpel, which possesses excellent bioactive, is nontoxic, nonallergenic and noncarcinogenic for incisions.

Objective: To study the possible mechanism of malnutrition in peritoneal dialysis patients. Methods: Multicenter prospective cohort study was designed . Forty-four peritoneal dialysis patients who survived two years after a cross-sectional study were included in the present study. We divided our patients into two groups according to the dietary protein intake(DPI) during follow-up period, DPI lower than 0.78 g/( kg?d) or DPI higher than 0.78 g/( kg?d). Patients' nutritional status was evaluated during the cross-sectional study and at the end of follow-up. 22 patients were divided into two groups according to the nutritional status improved or worsened in subjective global assessment (SGA) during the follow-up period. Besides nutritional status, patient's residual renal function, volume status and the incidence of cardiovascular event during the follow-up period were also evaluated. Results: Based on SGA , the incidence of malnutrition in low-DPI group was 60.9% in the cross-sectioned study and 45.5% after the follow-up. In high-DPI group, it decreased from 57.1% to 28.6%. The residual renal function declined significantly in worse nutritional status group, the incidences of volume overload and cardiovascular events during follow-up study were significantly higher in worse nutritional status group than that in improved nutritional status group. Conclusion: Malnutrition in peritoneal dialysis patients may not be simply due to lower protein intake. The changes in residual renal function, fluid overload and cardiovascular disease, accompanied with low dietary protein intake may be the possible cause of malnutrition in peritoneal dialysis patients.

Transforming economic growth mode is one of the most important guiding ideologies of the National twelfth Five-year development program. Since governmental hospitals are non-profit institutional organizations, it is a new topic for hospital managers to transform economic growth. Current situation of governmental hospitals is analyzed and the three main misunderstandings are pointed out. Take the Taian central hospital as the main object and analyzed the practice process of “putting hospital development scale control and revenue growth as the base and premise, shortening the average hospital stay, reducing the medicine proportion and consumable items proportion as the main means, and usd charging level, patients personal commitment to the cost proportion as important evaluation indicators of transformation economic growth mode” . By analyzing the practice process, the experience of transformation of economic growth mode is concluded, which provides some references for transforming economic growth mode in governmental hospitals.

Objective: To investigate the potential of gradient vector flow(GVF) Snake model as a method of image segment in radiographic absorptiometry method (RA) which is used to qualify bone mineral density. Methods: The Gradient Vector Flow model and the Region Growing method were applied in the segmentation of the middle phalanges image and aluminum wedge image separately in this paper. Then, the results can be compared. Results: The experiments shows that GVF Snake model is not only robust and practicable, but also segmentation results are in line with the actual border. Conclusions: GVF Snake model is very useful, and can be widely used in qualifying bone mineral density.

BACKGROUND:There is a certain cel subset in esophageal cancer tissues, with certain invasive and metastatic properties, which is closely related to the clinical therapeutic effect on tumors.
OBJECTIVE:To isolate tumor stem cel spheres in human esophageal carcinoma cel lines KYSE-150 and TE-1 and to analyze their proliferation and invasion ability. METHODS:KYSE-150 and TE-1 cels were cultured in serum-free medium to observe the formation of cel spheres. Cel proliferation and invasion were detected using MTT and Transwel chamber culture. Surface markers of cels were detected using flow cytometry. RESULTS AND CONCLUSION: Cel spheres that were stably subcultured were obtained from KYSE-150 and TE-1 cels cultured in serum-free medium. The proliferation and invasion abilities of cel spheres were significantly stronger than those of parent cels (P < 0.05). The number of CD44+, CD271+ and CD44+CD271+ cels in TE-1 and KYSE-150 cel spheres was significantly higher than that in the TE-1 and KYSE-150 parent cels (P < 0.05). These experimental results show that cel spheres isolated from human esophageal carcinoma cel lines TE-1 and KYSE-150 have tumor stem cel properties as wel as strong proliferation and invasion abilities. And moreover, CD44 and CD271 can be used as important surface markers of esophageal carcinoma stem cels.
