1.Differentiation Related Gene (Drg-1) as a Molecular Marker during the Treatment of in vitro Intermittent Androgen Deprivation in prostate Cancer.

PURPOSE: recent studies have reported that the expression of Drg-1 is up-regulated by androgen. It has been suggested that Drg-1 gene be used as a molecular marker for prostate cancer therapies like PSA. To de termine the role of Drg-1 gene as a molecular marker during intermittent androgen deprivation(IAD) therapy, we investigated the expression of Drg-1 and compared it with PSA expression in human prostate cancer cell lines treated with dihydrotestosterone (DHT) continuously or intermittently. MATERIALS AND METHODS: Two prostate cancer cells having different status of androgen receptor [LNCaP (androgen dependent) and PC-3 (androgen independent)] were used in this study. To know the change in PSA and Drg-1 expression after DHT treatment the cells were cultured in steroid-free RPMI media for 24 hours. 10(-7) and 10(-8)M of DHT and 10(-7)M bicalutimide was added into the cells and then cultured for 72 hours. And we established in vitro IAD model using LNCaP cells. Northern analyses were performed to determine the expression level of both PSA and Drg-1genes. Also, western analyses were performed to determine the protein level of proliferating cellular nuclear antigen and androgen receptor. RESULTS: Transcripts of Drg-1 were detected in both LNCaP and PC-3 cells but PSA was not expressed in PC-3 cells. The expression of Drg-1gene in LNCaP cells was up-regulated by 10(-8)M of DHT like PSA gene and down-regulated by 10(-7)M bicalutamide. In the treatment of intermittent androgen deprivation, the expression pattern of Drg-1was similar to that of PSA. However, up-regulation of PSA was detected earlier than of Drg-1. CONCLUSIONS: Based on observation, Drg-1 was up-regulated by androgen and down-regulated by anti-androgen. This suggests that Drg-1gene is useful for determining the androgen independency of prostate cancer during IAD.

2.The Clinical Value of Serum and Urinary CEA in Bladder Cancer.

Carcinoembryonic antigen(CEA) was measured in the serum and urine of 46 patients with bladder cancer and 15 patients with non-cancer disease as control group at the Paik Hospital. Pusan during the period from January, 1986 to June, 1988. The following results were obtained. 1. The upper limits of normal serum and urinary CEA levels in non tumor control group were 3.7 ng/ml and 2.3 ng/ml. Mean and positive rate of serum and urinary CEA levels in bladder cancer group were 4.40 ng/ml(37.0%) and 21.52ng/ml(65.2%). 2. Mean urinary CEA level was correlated significantly with the stage, grade and size of bladder cancer but no significance was seen in serum CEA. In the advanced cases(Stage C, D), serum CEA levels were elevated. 3. The diagnostic rate was not elevated in spite of urinary CEA determination combined with urinary cytology. After transurethral resection of cancer mass and radiation therapy, urinary CEA level decreased. As a result, serum and urinary CEA determinations in bladder cancer have no diagnostic value, but it will be useful in the study of the extent of cancer, effect of therapy, recurrence and prognosis.

4.Doxytycline in Treatment of Acute Gonococcal Uriethritis.

This series represents 128 cases of acute gonococcal urethritis treated with doxycycline at the Seoul Red Cross Hospital from the 1st. May to.5th, Oct., 1969. Results of this treatment were divided in three groups: Group I ; Single dose of doxycycline, 300 mg. was administered for this group. 36 cases fall into this category. Cure rate for this group was 36%. Group II; Doxycycline, 200mg. was given on the 1st treatment day and daily dose of 100 mg. for 4 ensuing days. 42 cases were included to this group. Cure rate was 72%. Group III Daily dose of 200 mg. was administered for the remainders, 50 cases for 5 consecutive days. Cure rate was 91%. It was concluded that 200 mg. of doxycycline daily for 5 days dose regime can be recommended for the treatment of acute gonococcal urethritis, especially in cases allergic to penicillin or in which penicillin treatment fails. No anaphylactic shock or some other undesirable side reaction in administering this drug was experienced.

5.A Case of Epidermolytic Hyperkeratosis.

Epidermolytic hyperkeratosis, previously called bullous congenital ichthyosiform erythroderma is a dominantly inherited disorcler and usually manifested by blisters at or shortly after birth. We report a case of epidermolytic hyperkeratosis in 9 year old girl vrho developed generalized erythema &: easily eroded vesicles at birth. Later the skin lesions progresed to characteristic thick, grayish-dark brown verruform cais, affecting predominantly flexural and intertriginous areas. Fiistologic findings show typical granular degeneration.

6.IgE and In Vivo Delayed Hypersensitivity to the Recall Antigens in Atopic Dermatitis and Their Relationship.

We measured serum 1gE by using IgE 'Eiken' radioirnrnunoassay Kit and delayed cutaneous hypersensitivity responsiveness by the Multitest CMI Kit (Institut Merieux) that simultaneously administrates 7 standardized recall antigens(tetanus, diphtheria, streptococcus, tuberculin, candida, trichophyton, proteus) and 70% W/V glycerin diluent control by multiple prick puncture. We studied 40 cases comprising 20 patients with atopic dermatitis (AD) and 20 healthy children as control. The results were obtained as follows; 1) The mean serum level of IgE in AD(63.3+/-395.2 1U/ml) was higher than in control(155.6+/-67.1IU/ml) (p<0.0l), but the intensity of reaction (IR) & rate of reaction (RR) in AD (l.34+/-1.06, 0.38+/-0.23) was not significantly decreased than in control (1.70+/-0.87,0.44+/-0.16). 2) 1n both AD & control group, tuberculin showed highest IR R RR. 3) Anergic state was found in only one patient with disseminated cutaneous lesions Sc 4) serum IgE, but rate of hypoergy was significantly increased in AD(65%) than in control (30%) (p<0.05). 4) Serum IgE was not statistically correlated with IR and RR, but IR was statistically correlated with RR (p<0.001, r=0.9504). 5) IgE level in severe AD (generalized skin lesion) was significantly increased than in both mild AD 8c control group (p<0. 001). but IR & RR were not statistically correlated with disease severity.

7.A Clinical Study on Pathologic Fractures

Pathologic fracture occurs in a bone with preexisting stuctual weakness. The author reviewed 30 cases of pathologic fractures in which admission record and X-ray finding was well preserved among 53 cases admitted patients in the department of Orthopaedic Surgery, Seoul National Universty Hospital for 10 years from 1967. The results were as follows: 1.Age & Sex incidence showed even distribution. 2. The most frequent involved site was femur (40%) 3. The underlying etiology was as follows: 2 chronic osteomyelitis 1 homophilia 7 benign tumor 20 malignant tumor 4. metastatic tumor was the most frequent cause in 30 cases. 5. Treatment was as follows: Conservative treatment 4 cases Surgery 21 cases.

9.Application of micro-ELISA in serodiagnosis of human paragonimiasis.

To observed whether the specific IgG antibody test using ELISA was useful in diagnosis of presently ill patients of paragonimiasis, a total of 95 sera were tested. The sera were collected from 21 egg positive cases, 8 from positive reactors of intradermal test, 7 from Clonorchis infected, 9 from other parasitic diseases and 50 from apparently non-infected cases. By the result, the sensitivity of the test was 86% and the specificity was 100%. There were no cross reactions between Paragonimus antigen and other parastic infections. Specific IgG antibody test by micro-ELISA was concluded to be useful for mass screening of the presently ill paragonimiasis in the field.

Upper gastrointestinal tract angiodysplasia is rare, but it is an important cause of upper gastrointestinal bleeding. We experienced a case of hemorrhagic angiodysplasia of duodenal bulb in a patient under chronic hemodialysis due to chronic renal failure. Repeated upper GI endoscopy revealed an angiodysplasia at the posterior wall of the duodenal bulb. It was successfully treated by bipolar electrocoagulation. We report a case of angiodysplasia in chronic renal failure patient with the review of the literatures.
