Paragonimus westermani mainly infests the lung parenchyma, and it is also causes ectopic parasitism of various sites, such as brain, liver and subcutaneous tissue but there have been no reports in literature about paragonimiasis infesting lumbar epidural space. A 58-year-old housewife complained of lumbago and radiating pain to both lower extremities for 20 years. Calcified extradural masses were found by preoperative plain X-ray, myelogram, and CT scan. Intraoperatively, cystic mass in the epidural space at L2-3 was found and confirmed to be paragonimiasis by histopathological study. A case of ectopic paragonimiasis in the lumbar epidural space at L2-3 level is presented.

Carcinoembryonic antigen(CEA) was measured in the serum and urine of 46 patients with bladder cancer and 15 patients with non-cancer disease as control group at the Paik Hospital. Pusan during the period from January, 1986 to June, 1988. The following results were obtained. 1. The upper limits of normal serum and urinary CEA levels in non tumor control group were 3.7 ng/ml and 2.3 ng/ml. Mean and positive rate of serum and urinary CEA levels in bladder cancer group were 4.40 ng/ml(37.0%) and 21.52ng/ml(65.2%). 2. Mean urinary CEA level was correlated significantly with the stage, grade and size of bladder cancer but no significance was seen in serum CEA. In the advanced cases(Stage C, D), serum CEA levels were elevated. 3. The diagnostic rate was not elevated in spite of urinary CEA determination combined with urinary cytology. After transurethral resection of cancer mass and radiation therapy, urinary CEA level decreased. As a result, serum and urinary CEA determinations in bladder cancer have no diagnostic value, but it will be useful in the study of the extent of cancer, effect of therapy, recurrence and prognosis.

Effects of several cytokines(IL -1beta, TNFalpha, and IFNgamma) have been examined on fetal rat osteoblast-like cells. To investigate whether cytokines play direct causal roles in production of lysosomal enzyme, fetal rat osteoblast-like cells were treated with IL-1beta, TNFalpha, and IFNgamma, respectively or combined. And acid phosphatase was determined by biochemical method. Alkaline phosphatase was assayed to determine the effects of IL-1beta, TNFalpha, and IFNgamma on the expression of this enzyme. And also experiment of calcified nodule formation was performed to assess the effects of cytokines on the bone-forming activity of osteoblast-like cells in vitro. Acid phosphatase activity was significantly increased by the addition of IL-1beta and TNFalpha, whereas decreased by IFNgamma, However, no significant changes in alkaline phosphatase activity was observed when the osteoblast-like cells were treated with IL-1beta and TNFalpha. Interestingly, IFNalpha showed stimulatory effect on alkaline phosphatase activity. The number of calcified nodules was decreased by treatment of cultures with 1ng/ml IL -lbeta, 20ng/ml TNFalpha, and 500 u/ml IFNgamma continuously for 21 days, while considerable number of calcified nodules were formed in control group of osteoblast-like cell in culture for 21 days. These results seem to suggest that cytokines may play crucial roles in bone remodeling through the direct action on the osteoblast-like cell.

The present study was intended to examine the spectrum of radiographic patterns of the stomach associated withacute and chronic pancreatitis and their complications. Subjects served for the study consisted of 70 cases ofpancreatitis (36 cases in acute stage and 34 cases in chronic stage). The principal cause of avulsion fracture of anterior superior iliac spine is sudden powerful contraction of sartorius muscle in strenuous sporting activities. The condition usually occurs in young people in whom the apophysis has still not firmly united to the ilium. We presented the three cases of avulsion fracture of anterior superior iliac spine which occurred during sprinting in Physical Fitness Test.

We reviewed 90 cases of popliteal cyst in 88 patients, managed at Department of Orthopedic Surgery, Severance Hospital from January 1976 to June 1985 and the following results were obtained. 1. The average age of the 88 patients was 42 years. There were 23 males and 65 females. Of the all patients, 47 were occured in right side and 39 the left. Two patients were both sides. 2. Of the all cysts, 62 were found to be bursa between the semimembranosus and the medial head of the gastrocnemius muscles(68.8%). 3. Associated adnormalities of the knee joint were apparent in 29(33%) and the most common abnormalities were osteoarthritis. 3. Communication of the cyst with the joint was apparent in 44 cases(49%). 5. Twelve patients were recurred and associated abnormality of the knee joint was osteoarthritis.
