1.A study on type 1 allergy to house dust mite in patients with atopic dermatitis.

The authors investigated the radioallergosorbent test (RAST) for the specific IgE and the prick test with hose dust mite in 129 patients with atopic dermatitis(AD) wit4 four or more family numbers. RAST and prick test results were described according to family background of respiratory atopy(RA) and severity of AD. The results are summarized as follows : l. A comparison between the prick test results and RAST reactions gave a concordance of 88%. The prick test was more sensitive than the RAST. 2. The level of specific IgE in patients with personal history(PH) or family history(FH) of RA was significantly increased compared to those in patients with only AD. 3. In patients without PH and FH of RA, the level of specific IgE in patients with FH of AD was significantly increased compared to those in patients without FH of AD. 4. The level of specific IgE did not reflect the severity of AD. From the above results, it could be concluded that type I allergy to house dust mite is not directly related to AD and it occurs predominantly in those patients with AD wha have a predisposition to RA.

2.The Percieved Health Status of Female Workers and It's Determinants in the Small and Medium Scaled Industries.

458 female workers working in the small and medium scaled industries in Inchon area were surveyed to study their general characteristics, health behavior, working and environmental conditions, current health status etc. Self-administered questionnaire was developed by authors and distributed to the female workers with the explanation by trained interviewer on the purpose of survey and the method how to answer. The results were as follows: t. General characteristics showed that most(75.3%) of workers were living in their own home, and their age distribution was relatively even from teenagers to the forties. The level of education was high school graduate(including drop-out) in 69.4%, suggesting that the education level of female workers was increasing gradually. The marrital status was 69% in unmarried, and 31% in married state. 2. Survey on their health status showed that more than 20% of female workers complained that they have some kind of disease and most of them complained that they have chronic symptoms more than three kinds. Also, they complained at least two or three symptoms in physical, mental or sensoryneural symptoms respectively. But their medical care utility rate was only 22%. 3. There were no significantly correlated variables between health status and general characteristics(by x2-test). Among the variables on the health behavior, only alcohol drinking was significant to health status. Perceived health status related to the working environment was significantly different to the degree of work satisfaction and awareness for the exposure to harzadous materials. 4. Multivariable analysis showed that work satisfaction, awareness for the exposure to harzadous materials, alcohol drinking, type of employment were significantly related to the physical or mental health index, and also marrital status was to the physical health index. Work satisfaction, awareness for the exposure to harzadous materials, marrital status were related to the sensoryneural health index. Alcohol drinking, work satisfaction, awareness for the exposure to harzadous materials, marrital status were significantly related to the number of complained chronic symptoms. In summary, this study showed that inspite of wide and various range of the general characteristics of female industrial workers in Korea comparing to the past, there was high prevalence of disease due to the poor working conditions and low work satisfaction etc. Therefore, more detailed disease preventive policies or improvement of the work environment, and systematic management to raise the work satisfaction of female workers are urgently needed. And also, occupational health policies increasing the access to health care to provide the adequate health service for the workers who have perceived need are necessary.

3.A Case of Successful Hepatic Resection after Local Radiotherapy with Combined Transarterial Chemoinfusion in Hepatoblastoma .

It has been widely accepted that complete surgical resection of hepatoblastoma is essential for long-term survival. But unfortunately less that 50% of hepatic tumors in children can be totally removed at the time of diagnosis. This report is to present the experience of successful resection of hepatoblastoma after concurrent radiotherapy with transarterial chemoinfusion in a child. We believe this modality of treatment enables complete resection of unresectable hepatoblastoma, which is resistant to the systemic chemotherapy.

4.Chondrosarcoma in Pelvis

Advances in multimodal treatment using radiation and chemotherapy, more sophicated diagnostic, radiologic methods, and newer reconstructive surgical techniques have led to aggressive surgical treatment of chondrosarcoma of the pelvis. Recently, good results have been reported in even high grade chondrosarcoma of the pelvis with surgery with/without adjuvant therapy. From January 1984 to June 1992, ten patients with chondrosarcoma of the pelvis were treated. Their average age was 40(18 to 55). Location of the lesion were ilium(5), periacetabular lesion(1), ischiopubic region(1) and sacrum(3). Nine cases were conventional type and one was dedifferentiated type. Surgical resection was done for nine patients, with which radical margin in one, wide margin in four, marginal margin in one and intralesional margin in three. Postoperative radiotherapy(6000-7000cGy during 6-7week) was done for five patients of having wide(1), marginal(1) or intralesional(3) surgical margin. One patient took radiotherapy only after biopsy. The average follow-up period was 2 years and 9 months(1 year to 6 years 1 month). Six patients wer continuously disease free(CDF) or disease free(DF). One patient is alive with disease(AWD). Three patients died of disease(DOD). With Kaplan-Meier's method, 5-year actuarial survival rate for total 10 cases was 60%, for operated group(9 cases) 66.7% and for operated conventional group(8 cases) 80%. With wider resection margin, better survival could be achieved.

6.IgG4-Related Sclerosing Sialadenitis: Report of Three Cases.

Chronic sclerosing sialadenitis, Mikulicz disease or Kuttner tumor has been recently recognized as a spectrum of IgG4-related sclerosing disease. IgG4-related disease is characterized by a high serum IgG4 level and tissue infiltration of IgG4-positive plasmacytes. We report three cases of chronic sclerosing sialadenitis with variably associated systemic involvement. All patients presented with a submandibular mass or swelling, and all the resected submandibular glands showed diffuse lymphocytic infiltration, lymphoid follicles, and septal fibrosis. Two of the specimens revealed numerous IgG-positive plasma cells, most of which were IgG4-positive on immunohistochemical staining. One of them was associated with dacryoadenitis and hypophysitis. The other patient had ureterorenal lesions. Immunohistochemical study was unavailable in remaining one case, but the histologic features along with elevated IgG level and associated pancreatitis supported the diagnosis. All patients received steroid therapy postoperatively and are doing well. Salivary gland involvement in IgG4-related fibrosclerosis should be recognized in systemic medical pathology.

7.The Role of Terminal Ureter in Vesicoureteral Reflux and Its Histological Analysis.

The normal function of the ureterovesical junction was fundamental to prevent vesicouretera1 reflux but the role of ureter in preventing vesicoureteral reflux has not been defined. To clarify the role of ureter in vesicoureteral reflux, a total 34 reimplanted juxta-vesical ureters from 22 patients were examined by light microscope, polarized microscope and computerized image analyzer. As the degree of reflux was increasing, the amount of ureteral muscle did not change significantly but that of ureteral collagen increased significantly In 6 patients with bilateral vesicoureteral reflux, which were different in grade each other, the amount of ureteral muscle and collagen were not different significantly between the lower grade and the higher grade. The amount of ureteral muscle tended to increase with grade of reflux in the dilated terminal ureters in excretory urography but not in the non-dilated. Inflammatory cell infiltration to ureteral wall gave no effect to the contents of ureteral muscle and collagen. It was suggested that the content of ureteral muscle decreased and the content of ureteral collagen increased with degree of reflux and the dilation of terminal ureter with muscle hypertrophy was a compensatory response to the increased workload.

9.Effects of cis - Platin on the Type I Collagen of the Osteoid in the Metaphysis of Rat Tibia.

The present study has been undertaken to pursue the cytotoxic effects of cis-Platin on the osteoid formation in metaphysis of rat tibia. By using the immunohistological staining method for type I collagen in rat tibial osteoid, the author detected the deposition of type I collagen, which is the collagenous constituent of endochondral osteoid, after administration of cis-Platin in experimental animals. For the immunological reactions of type I collagen, we used the rabbit anti-rat collagen type I polyclonal antibody as primary antibody and biotinylated goat anti-rabbit IgG as secondary antibody. The distributions of immunohistological reactions in the each of metaphyseal osteoids were analyzed with an image analyzer, and we studied the variances of type I collagens by statistical probabilities. In 12 hours after cis-Platin injection, immunoreactive area in the osteoid of metaphysis was distinctly decreased. Immunoreactive area of type I collagen in osteoids of 1 day and 3 days group metaphysis was increased more than that of 12 hours group and the type I collagen in the metaphysis showed weak immunoreactions of type I collagens with an image analyzer. In the osteoids of 7 days group after cis-Platin injection, the immunoreactive area was similar to that of control group. It is consequently suggested that cis-Platin would induce the decrease of type I collagen in the osteoid. But the type I collagen in tibial osteoid shows the increase from a few days after cis-Platin injection.
