1.Responses of Neck Muscles to Horizontal Semicircular Canal Excitation in Cats .

The vestibular system is a sensory motor system concerned with the sensation of linear or angular acceleration, eliciting several vestibular postural reflexes. This study was designed to figure out the functional relationship in vestibulocollic reflex. The electromyographic responses of the cervical muscles were observed during stimulation of the horizontal semicircular canal nerves. The sternomastoid muscle was inhibited by stimulation of the ipsilateral horizontal canal nerve and excited by stimulation of the contralateral horizontal canal nerve. The splenius capitis muscle was inhibited by stimulation of the ipsilateral horizontal canal nerve and excited by stimulation of the contralateral canal nerve. The semispinalis capitis revealed same pattern responses as splenius capitis or sternomastoid muscles to stimulation of the horizontal canal nerve. These experimental results indicate that the cervical extensor or flexor muscles respond in a uniform fashion to the stimulated horizontal canal nerve. And taken these results and other morphological study results together, the excitatory neural pathways from the vestibular nucleus in brain stem to cervical motoneurons are crossed to the opposite side but the inhibitory pathways from the nucleus to the cervical motoneurons are uncrossed.

2.Radiologic Findings of Takayasu's Arteritis: An Aortographic Analysis of 75 Cases.

Takayasu's arteritis is an arteritis of undetermined etiology, which affects the aorta, the proximal portions of its major branches, and the pulmonary arteries, and causes coarctation, occlusion, or aneurysmal dilatation of the affected vessels. Authors has reported 9 cases in 1973, and another 24 cases in 1977. Thereafter, during next 4 years, authors experienced another 42 cases and obtained some additional results. The results are as follows: 1. Among 75 cases, 10 are male and 65 female patients with sex ratio of 1:6.5, and about 2/3 of total patients are under 30 years of age. 2. Headache, dizziness, absent or weak radial pulse and hypertension are the most common symptoms and signs. 3. Conventional chest roentgenography may be helpful but not specific in diagnosis of Takayasu's arteritis. 4. The aortographic findings are characteristic and pathognomonic in diagnosis of Takayasu's arteritis. In our series, the most common findings are stenosis or occlusion of subclavian artery, diffuse narrowing and aneurysmal dilatation of abdominal aorta and its branches, narrowing with irregular contour of descending thoracic aorta, and renal artery involvement. 5. Involvement of the aorta was classified as extensive type in 38 cases, descending thoracic and abdominal type in 22 cases and arch type in 15 cases. 6. As total aortography in cluding abdominal aorta uncovers evidence of unsuspected involvement of aorta and its branches, it is of paramount importance in the diagnosis of Takayasu's arteritis.

3.Detection of Numerical Chromosomal Aberration in Squamous Cell Carcinoma of the Lung by In Situ Hybridization Using #17 Centromeric Probes.

This study was carried out to understand the relationship between specific chromosome changes and their phenotypic consequences at the tissue level of human lung cancers. Then paraffin-embedded human lung squamous cell carcinoma samples were investigated for in evidence of genetic alterations, using chromosome 7 and 17-specific repetitive alpha-satellite DNA probes. In situ hybridization procedure with chromosome-specific DNA probes was optimized for use on formalin-fixed paraffin-embedded lung tissue sections. The chromosome index ranged from 1.10 to 1.88(median, 1.49) for chromosome 7 and 1.20 to 1.98(median, 1.69) for chromosome 17. Normal lymphocytes and stromal cells showed one or two chromosome signals per cell in most cases. All tumors showed three or more chromosome signals per cell with range of 16.0% to 80.6% of cancer cells(median, 50.9%) for chromosome 7 and 32.7% to 84.7%(median, 69.9%) for chromosome 17. The chromosome index did not correlate with the DNA content in most cases. Chromosomes 7 and 17 were either overrepresented or underrepresented when they were compared with corresponding DNA index determined by FCM. An increase in copy number, particularly of chromosome 7 was associated with a less favorable phenotype, including high nuclear grade. In addition, chromosome alterations were differentially expressed in the different areas of the same tissue section, correlating with histologic heterogeneity. These results suggest that chromosome polysomy can be reliably detected in tissue sections using in situ hybridization. There is a strong correlation between genotypic abnormalities and tumor phenotype in human lung cancer. This capability will prove to be an important tool for determining the underlying genetic basis for tumor development, tissue phenotype heterogeneity and progression by allowing genetic determination to be made on paraffin-embedded tissue sections where tumor histologic architecture is preserved.

4.A Case of Pityriasis Lichenoides et Varioliformis Acuta Treated with Photochemotherapy.

A lg year-old female patient with pityriasis lichenoides et varioliforrnis acuta (PLEVA) was been treated with PUVA, At first, she was treated with tetracycline for 8 weeks with improvement somewhat. But 2 months later, her skin lesions aggravated slowly. So, we tried to treated her with PUVA and then she was cleared completely. The initial dose of UVA was 5 J/cm The UVA dosage increased by 10g of the previous exposure and increased to the maximum dose, 15 J/cm The patient exposed to UVA R times per week. 12 times after the PUVA therapy, the skin lesions begin to improve and completely improved at 21 times of Pl.JVA therapy. She was followed up every week and there was no side effect except hyperpigrnentation till 24 months. The total dose during treatment is 210 J/cm.

5.Percutaneous transhepatic biliary drainage

Percutaneous transhepatic biliary drainage was successfully made 20 times on 17 patients of obstructivejaundice for recent 1 year since June 1981 at department of radiology in Seoul National University Hospital. Thecauses of obstructive jaundice was CBD Ca in 13 cases, metastasis in 2 cases, pancreatic cancer in 1 case and CBDstone in 1 case. Percutaneous transhepatic biliary drainage is a relatively easy, safe and effective method whichcan be done after PTC by radiologist. It is expected that percutaneous transhepatic biliary drainage should bedone as an essential procedure for transient or permanent palliation of obstructive jaundice.

7.Clinical Evaluation on the Consecutive Esotropia after Exotropia Surgery.

We reviewed the clinical characteristics and surgical results of patients who had consecutive esotropia after exotropia surgery with at least 6 months of follow-up. The number of transient esotropic cases with more than 10 prism diopter postoperatively was 105[9%] in total 1128 cases and persistent esotropic cases at least for six months after surgery was 24[2%]. Significant correlations were found between the development of consecutive esotropia and the presence of amblyopia[p=0.04], poor stereopsis[p=0.02], suppression of the Worth 4-dot test[p<.01], and recession-resction surgery[p=0.02]. The corrective surgery for consecutive esotropia was performed on 12 cases[1.1%] with unilateral lateral rectus advancement[7 cases], bilateral lateral rectus advancement[3 cases], unilateral medial rectus recession[1 case] and bilateral medial rectus recession[1 case]. All of them showed good results.

9.Influence of Stomach Cancer Risk Factors on the Development of Gastric Dysplasia.

BACKGROUND: Both atrophic gastritis and intestinal metaplasia may progress to gastric dysplasia. This study aimed to analyze the factors influencing progression of atrophic gastritis and intestinal metaplasia to dysplasia. METHODS: People diagnosed with atrophic gastritis and intestinal metaplasia for the first time received a follow-up endoscopy and were investigated for the cumulative incidence rate of gastric dysplasia by age, gender, smoking habit, alcohol intake, rice consumption and family history of stomach cancer. RESULTS: The cumulative incidence rate increased with age, consuming > or =3 bowls of rice per day and family history of stomach cancer. Multivariate analysis showed that the cumulative incidence rate of gastric dysplasia increased in subjects >61 years (RR=2.54, P=0.014), in those consuming > or =3 bowls of rice per day (RR=1.46, P=0.021) and in those with a family history of stomach cancer (RR=1.31, P=0.037). CONCLUSIONS: More active management, such as intensive endoscopic follow-up examinations, lifestyle change and education regarding gastric dysplasia, are required in those older than 61 years, having a higher intake of grain or with a family history of stomach cancer.
