1.Liver Cirrhosis: Etiological diagnosis and morphological characteristics of 369 biopsy-proven cases.

To pursue a desirable format for the pathological diagnosis of liver cirrhosis, the authors attempted to classify 369 biopsy-proven cirrhosis on the basis of etiology and made effort to find out the morphological characteristics of each category. About 735 of total cases were HBsAg seropositive postnecrotic cirrhosis. Alcholic cirrhosis ws the second most frequent type, although accounted only 6.8%. In about 15%, the etiology was not known. Excluding the congenital biliary atresia, chronic biliary obstruction appeared to be a rare cause of cirrhosis among these biopsied cases. Of the HBsAg positive postnecrotic cirrhosis, the eAg seropositive cases tended to be micronodular and to show a higher necroinflammatory activity, in contrast to eAg seronegative cases and those complicated by hepatocellular carcinoma (HCC), suggesting that the loss of eAg is followed by a decrease of the destructive activity, active regeneration of hepatocytes and finally the development of HCC. alcoholic cirrhosis was micronodular in 64% and revealed histologic evidences of alcoholic liver disease in most cases. The results indicate that etiological diagnosis can be made in most cases of cirrhosis by the morphological characteristics and the precise clinical informations, including those on the NANB virus and the inborn error of metabolism, and that the pathological diagnosis should be more comprehensive, implicating the etiology, the nodular size and the necroinflammatory activity.

Recently there has been remarkable improvement in the treatment of chondrosarcoma, accompanied with the development of diagnostic tools, operative technique, replacement materials and designs, anticancer-chemotherapy and radiotherapy. In well-deferentiated chondrosarcom, wide excision is the only treatment required. Nevertheless, in dedifferentiated chondrosarcoma, anticancerchemotherapy or radiotherapy is usually recommended after the wide excision. From 1984 to 1994, 22 patients diagnosed as chondrosarcoma had been treated at Seoul National University Hospital. We analysed anatomic distributions, size, methods of treatment and their results. The avereage age was 38 years and 10 months(15 years 73 years) and the average follow-up period was 3 years and 10 months(1 year and 6 months

4.Echocardiographic Evaluation of Left Ventricular Performance in Normal Subjects.

Parameters of the left ventricular performance were evaluated by echocardiographic examination in 66 normal subjects. Effects of pulse rate, systemic arterial pressure, peripheral resistance and left ventricular end-diastolic dimension on the parameters were also observed. Normal value(mean+/-SD) of fractional shortening was 34.0+/-5.2%, ejection fraction 0.71+/-0.07, mean rate of circumferential fiber shortening 1.15+/-0.19 circ/sec, mean posterior wall velocity 4.05+/-0.70cm/sec, maximal posterior wall velocity 6.31+/-1.26cm/sec, mean normalized posterior wall velocity 0.82+/-0.14/sec, and maximal normalized posterior wall velocity 1.28+/-0.24/sec. There were negative correlations of peripheral resistance to fractional shortening, ejection fraction and mean rate of circumferential fiber shortening. Pulse rate and left ventricular end-diastolic dimension did not have significant effects on these parameters. Mean and maximal posterior wall velocity had negative correlations to peripheral resistance and positive correlations to left ventricular end-diastolic dimension. But mean and maximal normalized posterior wall velocity had no correlations to any of them.

7.The bipolar Hemiarthroplasty Using the Porous - coated Femoral stem in Avascular Necrosis of the Femoral Head.

The authors reviewed both the clinical and radiographic results of 36 bipolar hemiarthroplasty using the porous-coated Harris-Galante uncemented femoral stem performed for avascular necrosis in 30 patients from January 1985 to June 1992. The average age was about 45.6 years and the average follow-up was fifty-four months. Clinical results by the Harris evaluation method were excellent in 33 cases, good in 2 cases, and fair in 1 case. The radiographic fixation of the femoral stem at final follow-up was bony ingrowth fixation in 31 cases, fibrous ingrowth fixation in 4 cases, and unstable fixation in 1 case. There was neither medial nor superior migration of the bipolar cup of more than two millimeters.

10.The inhibition of wound contraction and biologic attachment of acellular dermal matrix on full-thickness skin defect in rat.

Although wound healing is a natural biologic process by which the homeostasis is preserved, the resultant hypertrophic scar or scar contracture may produce undesirable morphologic and functional sequelae. To overcome these problems, a number of attempts to produce skin substitute have been made in recent years, but none have proved to be entirely satisfactory. We compared the wound healing of full-thickness dorsal skin defects between ADM(acelluar dermal matrix)-applied wound and ADM non-applied wound group in 20 adult female Sprague-Dawley rats. The wound area was serially evaluated at 1,2,3 weeks and the difference in the mean area ratios between the two groups was then statistically analyzed using Wilcoxon rank sums test. We also performed histological evaluation of the wounds at 1,2,3 weeks. Results were as follows: 1. Experimentally produced ADM was easy to handle. Histologic findings showed no epithelium, no appendages, no cells dermal and subcutaneous layer and pure bundles of collagen. 2. The wound area ratio of ADM applied wound group was statistically larger than that of ADM non-applied(control) wound group. That is, ADM was able to inhibit the wound contraction compared with the ADM non-applied wound group. 3. In the ADM-applied group, we observed low grade exudates, no infection, fine take-up of ADM on full-thickness skin defect, gross and histologic findings of neovascularization, and progressive epithelization on ADM. These findings of biologic attachment implies the function of the ADM as a dermal substitute. These can be basis of the study on the development of artificial skin.
