Objective: To summarize the surgical results of valve replacement in children. Methods: From Jan.1990 to Dec.2002, 45 children ranging from 3 to 14 years (average 10.8 years) underwent cardiac valve replacement. There were 26 males and 19 females. 15 cases were younger than 10 years and 30 cases aged 10 to 14 years. Surgical indications for valve replacement included congenital valve disease (n=32), rheumatic heart disease (n=6), bacterial endocarditis (n=3), mitral insufficiency after atrioventricular canal defect repairing (n=3) and aortic insufficiency after VSD repairing (n=1). Mitral replacement was performed in 23 cases, aortic and tricuspid replacement in 9 cases, and combined mitral and aortic replacement in 4 cases. In forty cases, mechanical valves were used bioprosthetic valves or homograft valves in 5 cases. Results: Operation mortality was 4 4%(2/45 cases). The follow up periods were from 8 months to 12 years (average 4 9 years). Late mortality was 9 3% (4/45 cases). There was no anticoagulation related complications occurring. Conclusion: According to this study, valve replacement with warfarin anticoagulation in children was a safe and feasible technique. A suitable size prosthesis implantation could result in a healthy development.

Objective To investigate the therapeutic effect and mechanism of ursolic acid on carrageenan-induced chronic nonbacterial prostatitis (CNP) of SD rats. Methods Forty-two SD rats were randomly divided into five groups:normal control group, model control group, Pushitai group, ursolic acid high-dose group, and low-dose group.Except for normal control group, chronic nonbacterial prostatitis model was established using carrageenan. The rats of Pushitai group were treated with the solution of Pushitai at the dose of 148 mg??kg-1??d-1 for twenty days. The rats of ursolic acid high-dose and low-dose groups were administered with ursolic acid at the doses of 100 and 50 mg??kg-1??d-1 for twenty days, respectively. The rats of normal and model control groups were treated with isometric 5% CMC-Na for twenty days. The prostate index (PI) was calculated from the ratio of isolated rat prostate weight and body weight of rat.The expressions of IL-1β, TNF-α, and IL-10 were detected by ELISA. The histology and morphology of the prostate was observed with the aid of hematoxylin-eosin staining. Results Compared with the model control group, PI and the levels of IL-1β, TNF-α, and IL-10 in serum were significantly decreased in ursolic acid high-dose,and low dose groups ( P<0. 05 ) . Moreover, the pathological changes of prostate were improved significantly. Conclusion Ursolic acid has certain therapy effects on chronic nonbacterial prostatitis, which may be related to its regulating the expressions of IL-1β, TNF-α, and IL-10.

Objective To investigate the relationship between early colonic cancer and the pit patterns of abnormal colonic mucosa. Methods We examined the pit patterns of 124 polypi in 139 patients by magnifying colonoscopy and stereomicroscopy, and compared the relations between the pit patterns and pathological diagnosis of polypi. Results 5 cases of LST(laterally spreading tumor)and 9 cases of advanced cancer were found out of 124 polypi in 139 patients. 1 among these 5 cases of LST showed Ⅲ L pit pattern, and 4 showed Ⅳ pit pattern with both magnifying colonoscopy and stereomicroscopy. Magnifying colonoscopy showed high coincidence rate with stereomicroscopy. Conclusion Pit patterns are found to be very important in disguishing tumorous lesions from non-tumorous lesions, and to discover early colonic cancer. Ⅴ pit pattern indicates that the polyp is carcinomatous.

Objectives In order to further investigate the biological characteristics and the developmental mechanisms of laterally spreading tumor, a cell line from the primary culture of human colon LST tissue was isolated. After 70 passages, we identified the cells by morphologic, genetic and immunohistochemistry analysis. Results (1) The origin of LST cells is epithelial, and the majority of which were multiform epithelial cells. It displayed an approximately similar growth characteristics of tumor cells. The nest corpus doubling time was 36 hours. (2) The number of chromosome is 42-66. Eighty-five percent of chromosomes were triploid, showing abnormal karyotypes. (3) Immunohistochemistry showed that ESA and CK20 were expressed. (4) The ultrastructure of LST was almost the same as that of a tumor cell. Conclusion A novel LST cell line has been established and named as LST-R1. It facilitates further study of the biological characteristics of LST cells and the development of colon cancer.

Objective To observe the effects of salmeterol/fluticasone on the bone metabolism and bone density of the patients with moderate COPD and whether the effects is relate with inhaled corticosteroid course. Methods Sixty pa-tients with COPD were divided into 2 groups by random digit table. Patients in experimental group (n=30) were given conven-tional therapy with salmeterol/fluticasone one inhale(bid) for continuously 12 months while patients in control group (N=30) were given conventional therapy without any glucocorticoid. The bone mineral density (BMD) of the femoral triangle in all patients were measured by DXA before therapy, 3 months and 12 months after therapy. At the same time, biochemical indi-cators that are correlated with bone metabolism include bone gla protein (BGP) and alkaline phosphatase (ALP) were mea-sured by radioimmunoassay. Results The BMD, BGP and ALP of patients in experimental group and in control group be-fore therapy, 3 month and 12 month after therapy were compared. There is no significant difference (P>0.05) between these two groups before therapy and after 3 month but significant difference was shown after 9 months (P<0.05). Conclusion Short period inhaling salmeterol/fluticasone in patients with COPD is safe. But long-term inhaling salmeterol/fluticason may lead to decline of the bone density in patients with COPD.
