2.Arthroscopic Adhesiolysis of Partial Knee Ankylosis

Partial knee ankylosis is a recognized complication of following open operative procedure, trauma and disease processes about the knee. Intraarticular trauma or surgical procedures such as TKR, synovectomy or patellar fracture were main source of intraarticular or extraarticular soft tissue adhesion and contracture were resulted from distal femoral and proximal tibial fractures. The purpose of this study is to report the value of arthroscopic adhesiolysis of partial knee ankylosis and to evaluate the clinical results of its application in various conditions. From October 1982 to December 1992, 56 cases in 54 patients who had severe limitation of motion in the knee joint following open operative procedure or trauma about the knee were treated by fibroarthrolysis under the arthroscopic control. For release of thick fibrous tissue, we used our designed blunt metal bar. The following results were obtained. There were 30 males and 24 females and the age at the time of release ranged from 20 to 69 years old(average 38.7 years old). The interval between last open operative procedure or trauma and arthroscopic adhesiolysis ranged from 3 months to 6 years(average 16 months). The interval between arthroscopic adhesionlyssis and the last follow up evaluation ranged from 1 year to 9 years(average 49 months), The average preoperative range of knee motion was 42.2 and the postoperative range of motion under the anesthesia was 115.5 and the average final knee range of motion at follow up was 101.3. So the average loss of motion between postoperative and final motion was 14.2. Complications were one case of patellar and tibial condylar fracture respectively. In the author's experience, arthroscopic adhesiolysis seems to be applied to the various condition of the partial knee ankylosis and the results are better than the other surgical procedures when it was performed early enough16).

3.Epidermal Grafting after Removal of Recipient Epidermis by CO2 Laser Ablation in Vitiligo.

BACKGROUND: Epidermal grafting using cryotherapy for recipient sites is in widespread use. However the peripheral hypopigmented haloes that occur around the recipient sites require prolon gation of the treatment period. OBJECTIVE: We used a CO2 laser to remove the epidermis of the recipient sites for betteri results. METHODS: We treated lie localized vitiligo patients with CO2 laser to remove t.he epidermis and grafted suction blister rooves. We observed repigmentation and complications 1 month later. RESULTS & CONCLUSIONS : The superiority of this method is demonstrated by the fact 1) all prodedures can be completed on the day of operation 2) the incidence of hypertrophic scar and peripheral hypopigmented halos can be observed.The problems of this method are 1) uneven repigmentation of recipient. sites 2) hyperpigrnentation of recipient sites

The transoral approach to anteriorly placed lesions at the upper cervical region is not new, but is still infrequently used by neurosurgeons for lesions in this region. The indications and surgical methods vary widely, and there is much room for discussion of the technical details. We are reporting our experience with this technique in two patients; one is a traumatic odontoid fracture and another is a cervical epidural abscess.

6.Two Cases of Congenital Chylothorax Diagnosed by Prenatal Ultrasonography.

Congenital chylothorax is a rare disorder and can be diagnosed by prenatal ultrasonography recently. Most cases of congenital chylothorax were characterized by different clinical courses of respiratory distress. We describe two female cases with congenital chylothorax observed by ultra- sonography prenatally. In the first case, left-sided pleural effusion was noted by prenatal ultrasonography taken at 34 weeks of gestation, and then pleural fluid was extracted by intrauterine thoracentesis under sonography guidance. After birth, this patient was managed by TPN(total parentral nutrition) and intermittent thoracentesis without surgical treatment. But, pleural fluid was accumulated recurrently and respiratory distress was aggravated. At 15th hospital day, shock state was developed and patient died. In the second case, bilateral pleural effusion and ascites were noted by prenatal ultrasonography, and then patient was delivered immediately without intrauterine thoracentesis. After birth, the second case received conservative therapy including mechanical ventilation, TPN, intermittent thoracentesis and paracentesis. The patient was discharged with complete regression of chylothorax. We report the two cases with brief review of related literatures.

7.A Case of Vestibular and Facial Nerve Root Entry Zone Infarction in AICA Territory Presenting with Vertigo Which Mimics Labyrinthine Lesion.

Vertigo mimicking labyrinthine lesions may have resulted from ischemic insult to the inner ear or the vestibular nerve and nucleus in the AICA infarction syndrome. A 56-year-old female was admitted to the emergency room with vertigo and hearing loss in right ear. On neurological examination, she had left beating jerky torsional and horizontal nystagmus with falling and past pointing to right side. Brain magnetic resonance images showed high signal intensity in anterolateral portion of inferior pons on T2- weighted images. Severe right facial palsy of peripheral type developed 24 hours after admission. Audiometry and electronystagmography documented absent auditory and vestibular function on the affected side. We argue that vertigo of the acute infarction in AICA territory can be involved the eight and seventh nerve root entry zoon and mimic labyrinthine lesions

8.2 Cases of Nevus of Ota Associated with Nevus of Ito.

The nevus of Ota is a distinctive, mesodermal pigmentation of the facial skin commonly combined with pigmentation of the eye, usually congenital but capable of benign progression. It is usually unilateral, but in the 1st of these 2 cases, pigmentation of the facial skin and the eye was bilateral and associated with pigmentation of the left shoulder and the anterior cervical area(nevus of Ita). In the 2nd of these 2 cases, pigmentation of the eye and the shoulder was bilateral. Pigmentation of the eye accompanies that of sclera in every case, that of conjunctiva, iris and choroid in some case. It accompanies that of optic disc rarely, but in the 1st of these 2 cases, the pigmentation of optic disc was found in both eyes, in the 2nd, in the right eye. We have experienced the above-mentioned 2 cases of nevus of Ota associated with nevus of Ito which accompanied the pigmentation of optic disc, that are rare. We briefly reviewed the literature relating to nevus of Ota and Ito.

9.Spontaneous Intracerebellar Hematoma.

Twenty-five patients with spontaneous intracerebellar hematoma were analyzed, who had been admitted to Jeonju Jesus Hospital from 1982 to 1985. All cases were diagnosed by CT scan and the amount of the hematoma was assessed by the largest diameter and the height as large, moderated, and small. Three clinical types were defined to evaluate the method of treatment and the outcome. All twelve acute cases were operated and seven were dead-five from brainstem failure, one from rebleeding, and one from chest infection. All chronic cases were treated conservatively to yield good results. Of seven subacute cases, five were operated and two died. Overall mortality was 36% and surgical mortality was 53%. To avoid considerable loss of potential recovery, it is suggested that prompt confirmation of the intracerebellar hematoma by CT scan, emergency surgical management, proper pre-and post-operative care results in better prognosis in cases of this not-uncommon-not-incurable disease.

10.Ventriculoperitoneal Shunt Malfunction during Pregnancy.

A 24 year-old woman with a ventriculoperitoneal shunt is presented, who showed repeated episodes of shunt malfunction before and during pregnancy. Daily pumping of the flushing device relieved the symptoms at first, but reattack during the third trimester of pregnancy was managed with revision of the shunt to ventriculoatrial system. She delivered a healthy female baby successfully at term.
