1.A Clinical Study of Buerger's Disease

Buerger's disease was first identified in 1908 by Leo Buerger as a distinct clinicopathological entity, which causes severe pain and necrosis on distal parts of the extremities and occurs especially in young male adults. In spite of many treatment modalities, it is not completely cured but results in amputation in the long run. So, we studied a total of 120 cases of Buerger's disease, who were admitted at Yonsei University, Severance Hospital from Jan., 1975 to June, 1985 and we obtained the following results. 1. All the patients were male and majority of them were under 40 years of age (69.2%). 2. 118 cases were smokers and 2 cases were non-smokers. 3. The initial symptoms were resting pain, necrosis, and color change in descending order of frequency. 4. Diagnoses were made mainly by angiogram(75.8%) and most common site of obstruction was popliteal artery (49.4%). 5. After sympathectomy and angioplasty, there was temporary relief of symptoms, but none showed good results after long term follow up. As a result, amputations were performed after sympathectomy in 47 cases(39.2%), of which 36 cases(76.6%) received that surgery within 2 yeas after sympathectomy. 6. The pressure index not only helps diagnose the disease but also is a very useful method to objectively assess the severity of the initial symptoms and postoperative results.

2.A Study of the Effect of Changes in Ano-Rectal Function after Hysterectomy.

OBJECTIVE: It has been suggested that hysterectomy has a disturbing influence on bowel function, mainly constipation. We performed a prospective study to assess the changes of ano-rectal physiology after hysterectomy. METHODS: Fourteen consecutive patients were assessed before and two months after hysterectomy. A detail questionnaire was devised to allow assessment of bowel function and ano-rectal pressure test and balloon expulsion test were performed before and after hysterectomy. The parameters measured in ano-rectal pressure test included the minimal sensible volume, ano-rectal resting pressure, maximal squeezing pressure, recto-anal inhibitory reflex and balloon expulsion test. Data analysis was carried out by paired t-test. Statistical significance was inferred when the p value was<0.05. RESULTS: Among the fourteen patients, the straining in defecation was found in three patients after operation, other defication habits were not significantly changed after hysterectomy(P>0.05). There were no significant changes in ano-rectal pressure test after hysterectomy. The disturbance of balloon expulsion capacity was increased in four patients after hysterectomy(29%). CONCLUSION: Our results demonstrate that hysterectomy does not cause a decrease in ano-rectal pressure and rectal sensitivity, but has an adverse effect on rectal expulsion capacity in a some of patients.

3.A Study on the Drug Susceptibility Test of Multi-Drug Resistant Tuberculosis Patients.

Multi-drug resistant tuberculosis is an emerging threat to humans. Despite steady efforts of the national tuberculosis control program, current prevalence of multi-drug resistant tuberculosis rate is increasing in Korea. In Korea, it is effective to both improve the medical transfer system on tuberculosis, and also to make a new tuberculosis patient control system with integrated public-private sector. Improvement focused on the new medical transfer system is a suitable model considering current situation of the Korean medical system. This model can be achieved by replacing the traditional drug susceptibility test method, which requires a long turnaround time, with rapid molecular biological method, and improving the overall process of specimen transport system, report system, and guidelines for tuberculosis, as well. Using such model, doctors can discover multi-drug resistant tuberculosis patients at an earlier stage, prescribe appropriate drugs at the right time, and effectively support directly observed treatment short course strategy. Therefore, this new model for improvement of multi-drug resistant tuberculosis control program, medical transfer system-focused public-private integrated system, can present an effective tool for enhancing and modifying functions of the current national tuberculosis programme in Korea.

4.Losartan Prevents Maladaptive Auditory-Somatosensory Plasticity After Hearing Loss via Transforming Growth Factor-β Signaling Suppression

OBJECTIVES: Hearing loss disrupts the balance of auditory-somatosensory inputs in the cochlear nucleus (CN) of the brainstem, which has been suggested to be a mechanism of tinnitus. This disruption results from maladaptive auditory-somatosensory plasticity, which is a form of axonal sprouting. Axonal sprouting is promoted by transforming growth factor (TGF)-β signaling, which can be inhibited by losartan. We investigated whether losartan prevents maladaptive auditory-somatosensory plasticity after hearing loss. METHODS: The study consisted of two stages: determining the time course of auditory-somatosensory plasticity following hearing loss and preventing auditory-somatosensory plasticity using losartan. In the first stage, rats were randomly divided into two groups: a control group that underwent a sham operation and a deaf group that underwent cochlea ablation on the left side. CNs were harvested 1 and 2 weeks after surgery. In the second stage, rats were randomly divided into either a saline group that underwent cochlear ablation on the left side and received normal saline or a losartan group that underwent cochlear ablation on the left side and received losartan. CNs were harvested 2 weeks after surgery. Hearing was estimated with auditory brainstem responses (ABRs). Western blotting was performed for vesicular glutamate transporter 1 (VGLUT1), reflecting auditory input; vesicular glutamate transporter 2 (VGLUT2), reflecting somatosensory input; growth-associated protein 43 (GAP-43), reflecting axonal sprouting; and p-Smad2/3. RESULTS: Baseline ABR thresholds before surgery ranged from 20 to 35 dB sound pressure level. After cochlear ablation, ABR thresholds were higher than 80 dB. In the first experiment, VGLUT2/VGLUT1 ratios did not differ significantly between the control and deaf groups 1 week after surgery. At 2 weeks after surgery, the deaf group had a significantly higher VGLUT2/VGLUT1 ratio compared to the control group. In the second experiment, the losartan group had a significantly lower VGLUT2/VGLUT1 ratio along with significantly lower p-Smad3 and GAP-43 levels compared to the saline group. CONCLUSION: Losartan might prevent axonal sprouting after hearing loss by blocking TGF-β signaling thereby preventing maladaptive auditory-somatosensory plasticity.

5.Effect of Allopurinol on Methylmercuric Chloride in Cultured O1igodendrocytes.

In order to examine the neurotoxic mechanism of oxygen radicals on cultured bovine oligodendrocytes, cytoxic effect of oxygen radicals was examined when cultures were treated with various concentrations of xanthine oxidase (XO) and hypoxanthine (HX) in culture medium. In addition, the neuroprotective effect of iron-chelators against the neurotoxicity induced by oxygen radicals was evaluated by MTT assay. Cell viability was remarkably decreased in a time-dependent manner after exposure of cultured bovine oligodendrocytes to 20mU/ml XO and 0.1mM HX for 4 hours. In the neuroprotective effect of iron-chelators on oxidant-induced neurotoxicity, tetrakis (2-pyridylmethyl)ethylenediamine (TPEN) blocked the neurotoxicity induced by oxygen radicals, while DFX was not effective in blocking oxidant-induced neurotoxicity in these cultures. These results suggest that oxygen radicals are toxic in cultured bovine oligodendrocytes, and also selective iron-chelators such as TPEN are effective in blocking the neurotoxicity induced by oxygen radicals.

6.Meningeal Layers Around Anterior Clinoid Process as a Delicate Area in Extradural Anterior Clinoidectomy : Anatomical and Clinical Study.

OBJECTIVE: Removal of the anterior clinoid process (ACP) is an essential process in the surgery of giant or complex aneurysms located near the proximal internal carotid artery or the distal basilar artery. An extradural clinoidectomy must be performed within the limits of the meningeal layers surrounding the ACP to prevent morbid complications. To identify the safest method of extradural exposure of the ACP, anatomical studies were done on cadaver heads. METHODS: Anatomical dissections for extradural exposure of the ACP were performed on both sides of seven cadavers. Before dividing the frontotemporal dural fold (FTDF), we measured its length from the superomedial apex attached to the periorbita to the posterolateral apex which connects to the anterosuperior end of the cavernous sinus. RESULTS: The average length of the FTDF on cadaver dissections was 7 mm on the right side and 7.14 mm on the left side. Cranial nerves were usually exposed when cutting FTDF more than 7 mm of the FTDF. CONCLUSION: The most delicate area in an extradural anterior clinoidectomy is the junction of the FTDF and the anterior triangular apex of the cavernous sinus. The FTDF must be cut from the anterior side of the triangle at the periorbital side rather than from the dural side. The length of the FTDF incision must not exceed 7 mm to avoid cranial nerve injury.

7.Will Hypomagnesemia Induce Benign Eyelid Myokymia?

Background:
Myokymia consists of involuntary, fine, continuous contraction that spread across the affected striated muscle. Eyelid myokymia, unlike other dystonic facial contracture such as blepharospasm and hemifacial spasm, is assumed to be a benign, self-limited disorder. Eyelid myokymia is associated with stress, fatigue, excercise, and exccessive caffeine use. The pathophysiology is not well understood, but some drugs or brain structural lesions are the cause of the eyelid myokymia. Especially in Korea hypomagnesemia is generally known the main cause of eyelid myokymia, however there are no evidences so far. This study investigated the relation between eyelid myokymia and serum magnesium level.
Methods:
We performed a cross sectional study on 72 patients with myokymia and 197 controls. We investigated fatigue, sleep quality, alcohol, smoking, caffeine use, and exercise datas by interview. We analyzed laboratory datas including magnesium, calcium, phosphate, thyroid hormone in serum.
Results:
Demographic characteristics between the patients with myokymia and controls showed no significant differences in age, gender, smoking, and alcohol history. Fatigue and poor sleep quality were significantly higher in the myokymia group than control group. However, any laboratory results including magnesium showed no significant differences between two groups.
Conclusions
These data suggested that the eyelid myokymia is not related to the serum magnesium level as well as calcium and phosphate. Only fatigue and sleep quality were shown the relationship with eyelid myokymia.
8.Facial Cutaneous Rosai-Dorfman Disease: Dermoscopic Findings with Successful Surgical Treatment

Rosai-Dorfman disease (RDD) is a rare non-Langerhans cell histiocytosis characterized by an accumulation of activated histiocytes within the affected tissues. It is a heterogeneous disease that includes the classical (nodal) and extra-nodal variants. The cutaneous form of the disease without the characteristic lymphadenopathy is rare and is often misdiagnosed as other dermatologic diseases. Misdiagnosis as lymphoproliferative and infectious diseases such as lymphoma and tuberculosis have been reported in the literature. Herein, we report a case of facial cutaneous RDD with successful surgical treatment. In addition, we provide dermoscopic findings and literature review as dermoscopy can be a useful adjuvant tool in the diagnosis of cutaneous RDD.
9.Combined Chemoradiation of Advanced Pancreatic Cancer.

PURPOSE: This study was designed to evaluate the survival rate and prognostic factor of patients with advanced pancreatic cancer who received chemoirradiation. MATERIAL AND METHODS: From March 1993 to November 1995, twenty patients with unresectable pancreatic cancer were treated at the Department of Therapeutic Radiology, St Mary's Hospital, Catholic University Medical College. There were 11 men and 9 women. Age at diagnosis ranged from 34 to 75 years. All patient were treated according to a protocol consisting of 40 Gy external radiation by split course concomitant with intravenous 5-fluorouracil (5-FU) 500 mg/m2 given in a bolus injection 4 hours before radiatian on each of the first 3 days of each treatment course. Among them, 5 patients received incomplete radiotherapy. The follow-up period ranged from 1.3 to 29 months. RESULTS: In all the patients, median survival is 5.0 months and one and two-year overall survival rate was 34.3% and 25.8%, respectively. Median survival was 9.0 months and one-year survival rate was 33.3% in 15 patients with complete radiotherapy. The significant prognostic factors were stage, tumor location, and completion of chemoradio- therapy(p < 0.05). CONCLUSION: A combination of radiotherapy and chemotherapy resulted in improved median survival. However, the significant prognostic factars affecting survival rate in this analysis need to be verified further through randomized trial.

10.Prevalence of Parkinsonism in Ansan-city.

BACKGROUND: The prevalence of neurodegenerative disorders are not well documented in Korea. We assessed the prevalence of Parkinsonism in an elderly population in Ansan-city, which is a newly industrialized community with rural origins. METHODS: Randomly selected individuals were selected from a larger Ansan Cohort Study for this study. The sample in the Ansan cohort represented approximately 1.3% (4, 700) of 362, 625 adults (age >18) in the city registery in 1998. Among this group, 4, 218 subjects (1, 086 subjects, age> 60) agreed to be interviewed and receive physical examination and neuropsychological tests by a neurologist and neuropsychologist. All participants including those who either had bradykinesia and at least one another possible cardinal sign of Parkinsonism at the neurologic screening, those who reported that they had PD, or those who were taking antiparkinsonian drugs were examined. RESULTS: In our study, 16 subjects showed parkinsonism. The prevalence of Parkinsonism in this population was 0.37%. Prevalence increased with age, and the prevalence figures were 1.47% for those aged 60 years and older. Postural instability and gait disturbance type was more common in older age group. The results of the neuropsychological tests were as followings: (1) only 2 subjects showed low scores (< 20) in Korea-version Mini-mental status examination (K-MMSE), (2) 8 subjects showed 0.5 & 2 of clinical dementia rating (CDR) and (3) other 8 subjects showed 0 of CDR. CONCLUSIONS: Age is a risk factor for Parkinsonism. The result of our prevalence study is similar to those studies conducted in western countries.
