1.The Prognostic Significance of Tumor Microvessel Density in Colorectal Carcinoma.

PURPOSE:This study was carried out to evaluate the relationship with other clinicopath ologic factors and prognostic significance of tumor microvessel density in colorectal car cinoma. MATERIAL AND METHODS: 71cases of primary colorectal carcinoma (Modified Astler-Coller stage B and C) were analysed retrospectively who underwent curative resection at Wonkwang university hospital from September, 1991 to September, 1993. Male was 39cases. Age under 60 years was 31cases. 5 year survival rates were 80% (50 cases) in stage B and 52.4% (21 cases) in stage C. Tumor microvessels were stained by immuno histochemical method using anti-CD31 on paraffine embedded tissues, and were counted within 10x objective field (about 0.74 mm2) in the area of the most intense neovascu larization. RESULTS: Mean microvessel Density (MVD) was 56.3+/-18.0 (range 19~128). MVD was 55.2 in 11 cases of nonrecurrent group and 73.5 in 10 cases of recurrent group in stage C (p=0.012). There was no significant association between MVD and other parameters such as age, sex, tumor location and size, CEA, lymph node metastasis, and survival. The 5 year survival rates of 33 cases of MVD< or =56 and 38 cases of MVD>56 were 84.9 and 60.5% respectively(p<0.05). 5 year survival rates of MVD< or =56 groups adjusted for age, sex, tumor location, differentiation, and recurrence were higher than those of >56 groups. 5 year survival rates of MVD < or =56 and >56 groups in stage C were 100% (5 cases) and 37.5% (16 cases) (p<0.05). CONCLUSION:Tumor microvessl density may have somewhat prognostic significance in colorectal carcinoma.

2.Association of Dopamine D4 Receptor(DRD4) Gene Polymorphism with Korean Schizophrenic Patients.

BACKGROUND: No association between schizophrenia and dopamine D4 receptor polymorphisms have been reported. Despite these results, it is premature to exclude the association. It has been suggested that the susceptibility to develop schizophrenia could result from variation at a number loci which may interact or co-act with each other. Therefore, we investigated a possible assoication of combinations of exon III 48bp polymorphism [D4E3] and exon I 12bp polymorphism of the DRD4 gene [D4E1] with schizophrenia. METHOD: 207 unrelated Korean schizophrenic patients and 191 healthy controls wee recruited. DRD4 genotype was established using the polymerase chain reaction. Statistical analysis consisted of chi2 tests for Hardy-Weinberg proportions and genotypic and allelic frequencies in the patients and control groups. RESULT: There were no statistically significant differences in the each polymorphisms between schizophrenics and controls. And all genotype frequencies were within Hardy-Weinberg expectations. When the combinations of the polymorphism in schizophrenia and controls were compared, however, there were significant differences at A1A2*2/4 in the distributions of the combinations of D4E1 and D4E3(p<0.01). CONCLUSION: These findings suggest that the certain combination of D4E1 and D4E3 (A1A2*2/4) has the protective role to a susceptibility for schizophrenia.

6.The Relationship of Post-traumatic Stress, Job Stress and Turnover Intention in Emergency Department Nurses.

PURPOSE: This study was done to identify relationships among post-traumatic stress (PTS), job stress and turnover intention in emergency department (ED) nurses. METHODS: Participants were 250 ED nurses who had worked for one month or longer in the ED one of three university hospitals or seven general hospitals in Daegu City or Gyeong Buk Province in Korea. Structured questionnaires were used to measure PTS, job stress, and turnover intention of participants. Data were analyzed using descriptive statistics, one-way ANOVA, path analysis, and stepwise multiple regression with the SPSS program. RESULTS: There were significant relationships between PTS, job stress and turnover intention. PTS influenced turnover intention directly and was indirectly mediated by job stress. The experience of traumatic events influenced PTS, job stress, and turnover intention. Indirect experience of traumatic events in the ED was an important predictor, explaining 20.1% of PTS in high-risk post-traumatic participants. CONCLUSION: PTS can be an important factor for job stress and turnover intention. The direct and indirect experience of traumatic events can influence PTS, job stress, and turnover intention in ED nurses. Based on these results, strategies for managing PTS and relating job stress are recommended to reduce turnover intention in ED nurses.

7.Treatment of the Scaphoid Fracture

Twenty-three cases of the scaphoid fracture which were treated at the Department of the Orthopaedic Surgery, Catholic University Medical College during the period of december 1978 to december 1987 were analysed. The main purpose of this study was to clarify the significant factors affecting the union rate of the scaphoid fracture. The results obtained were as follows ; 1. The great majority of 23 cases was male with mean age 31. 2. Ulnar deviated stress oblique view with msking a fist was the most reliable means for diagnosing the fracture, location of fracture and stability of wrist after scaphoid fracture. 3. Union rate was obtained from 88.8% in undisplaced fresh fracture, 85.7% in displaced fresh fracture and 71.4% in old neglected fracture. 4. The union rate and associated factors affecting the prognosis after scaphoid frscture seems to be depended on the location of fracture, the severity of initial displacement of fracture fragments, time elapsed of treatment after fracture and accompanied injuries around the ipsilateral wrist.
