3.A radiological study on normal variations of abdominal aorta and its major branches

The study was to evaluate the normal variations of abdominal aorta and its major branches in Koreans. Using atotal of 363 abdominal angiograms and selective organ angiograms, normal variation of aorta and its major branchesincluding level of aortic bifurcation and of origin of major branches, multiplicity of renal arteries andvariations of celiac trunk were observed and analyzed. The results are as follows; 1. The celiac trunk usuallyarises from aorta at the level between middle 1/3 of 12th thoracic vertebra and upper 1/3 of 1st lumbar vertebra,most frequently at the level of the lower 1/3 of 12th thoracic vertebra. 2. The superior mesenteric artery usuallyarises from aorta at the level between disc space of 12th thoracic vertebra and middle 1/3 of 1st of lumbarvertebra, most frequently at the level of uppe 1/3 of 1st lumbar vertebra. 3. The both renal arteries usuallyarise form aorta at the level between middle 1/3 of 1st lumber vertebra and disc space of 1st lumbar vertebra,most frequently at the level of lower 1/3 of 1st lumbar vertebra. The frequency of the multiple renal artery was19% and right renal artery was higher than left side in 56%. 4. The level of aortic bifurcation is usually at thelevel between middle 1/3 of 4th lumbar vertebra and and disc space of 4th lumbar vertebra, most frequently at thelevel of lower 1/3 of 4th lumbar vertebra. 5.The classical normal pattern of celiac and hepatic arteries was shownin 74.6%. Anomalous origin of common hepatic artery was seen in 1.9%, anomalous origin of right hepatic artery in5.8% and accessory right hepatic artery in 2.9%. Anomalous origin of splenic artery was in 1.3% and originatedfrom superior mesenteric artery.

5.A Case of Xanthoma Tuberosum in Type II Hyperlipoproteinemia.

A 22-year-old male has had multiple, grouped, asymptomatic, yellowish flat papules, nodules, and plaques on the knees, elbows, butteeks, hands, and feet. Yellowish nodules first appeared on the heels after birth, and gradually increased in size and number and spread to the knees, elbows, buttocks, and hands. Physical examination revealed normaI except for the skin lesions. Gross finding of the serum was clear and paper electrophoresis revealed marked increase in betalipo-protein and mild increase in prebetalipoprotein, suggesting type II hyperlipoproteinemia Serum cholesterol was 510 mg%, and serum triglyceride was 190mg%. Histopathology showed many aggregates of foam cells on H-E stain and many lipid droplets in the dermis on oil red O stain.

6.Clinical Study of Scabies for 15 Years.

Human sparganosis is not uncommon and over 60 cases are reported in Korea but the case infested with multiple spargana is rare. The case we are reporting here is a 33 year-old Korean male having had an abscess and several bean-sized nodules on the left thigh. The patient had the history of eating frogs as a tonic food 3 years ago. Five spargana were removed from the lesions by surgical excision. Two of them were alive and other three were degenerated or calcified.

8.THE ROLE OF REGULATED PROTEINS RELATED TO CELL CYCIE IN CARCINOGENESIS.

The cell cycle is composed of G1, S, G2 and M phase. The transitions between different phases are regulated at checkpoint such as Start(restriction), S phase and mitotic checkpoint. These checkpoints are regulated by specific cyclins and Cdks(cyclin-dependent kinases). Especially, Start checkpoint in late G1 is though to be very important in control of cell cycle. In this study, it was shown various CDKN2(p16ink4A) alteration, including deletions, mutations, down regulations, and performed differential expression of p53, Cdk4, PCNA and pRb in stomach cancer tissues. 1. The frequency of CDKN2 mutations was not observed in the 19 primary stomach cancer tissues. In contrast to the mutations of CDKN2, mRNA levels was showed by Northern blot analysis that expression of CDKN2 was absent or decreased in 10 of the 19(53%) primary stomach adenocarcinoma. Western blot analysis was performed to determine the differential expression of p53, Cdk4, PCNA related to Start checkpoint. Overexpression of p53 was shown 38%, Cdk4 was expressed in all each specimens, and expression of PCNA was not shown. 2. As the other method to determine the differential expression of p53, Cdk4, PCNA and pRb, immunohistochemical analyses were performed on each 14 formalin-fixed and paraffin embedded tumor tissues of stomach adenocarcinoma. p53 overexpression was showed to clear nuclear staining only in tumor cells not in nonneoplastic cells. In staining for cdk4, the tumor was considered to be cdk4 positive if there was nuclear staining in tumor cells, regardless of cytoplasmic staining. PCNA staining for carcinoma tissues showed more intense nuclear staining in tumor cells than in nonneoplastic cells. pRb overexpression was show in tumor cells. Significant differences were observed in the expression of the proteins among the cancers from different anatomic site. Overexpression of adenocarcinomas had high rate of p53(57.1%) and pRb(71.4% ), and low late of cdk4(7.1% ) and PCNA(14.3% ), As these results, deletion of CDKN2 gene in human stomach cancer was not observed but mRNA expression was down regulated in restriction checkpoint, G1 phase. Inactivation of the CDKN2 gene due to hypermethylation may play an important role in development of cancer. And one of the abnormalities in p53, Cdk4, PCNA or pRb function occurs very common in various cancers, especially oral adenocarcinoma, osteosarcoma and squemous cell carcinoma, suggest that components in restriction checkpoint also play an critical role in the carcinogenesis and progression of cancers.

9.Two Cases of Chronic Otitis Media caused by Aspergillus terreus.

Fungi have been recognized as a significant cause of external otitis and it may be the primary pathogen or be part of a mixed infection. In the immunocompromised host, fungus is capable of producing infection in inner ear or middle ear. Otomycoses are most frequently caused by Aspergillus spp. and Candida sap. There are few reports that Aspergillus species other than A. fumigatus, A. niger and f. flavus have caused chronic otitis media. We report two cases of chronic otitis media caused by Aspergillus ferrous in Korea. One case is a 7-year-old girl who had recurrent serous otorrhea and otalgia for 4 years, was reattended otolaryngology clinics with otorrhea of 3 days durations and another is a 6-year-old girl who had serous otorrhea for 2 months and 3 day fever, was attended otolaryngology clinics with them. Microscopic appearance and colony morphology from ear discharge cultures revealed A. ferrous. The infection responded well to topical ketoconazole therapy. This report should help to raise medical personnel's awareness of such human opportunistic fungal ear infections.

10.Two Cases of Linear Lichen Simplex Chronicus.

We present two cases of linear lichen simplex chronicus. One is a 44-year-old male patient who had a linear licbenified patch on the left upper extremity, and the other is a 45-year-old female patient who had linear pruriginous nodules on the right lower extremity. The one is coincidental with localized lichen simplex chronicus, the other is coincidental with prurigo nodularis histopathologically.
