1.Arthroscopic Excision of Intra-articular Osteochondroma of the Elbow: A Case Report.

Osteochondromas are one of the most common benign bone tumors usually involving extraarticular metaphysis of long bone. Solitary intra-articular osteochondroma arising from the elbow joint has rarely been reported. We present a case of 23-year-old female who had pain and limited motion of the left elbow as a result of intraarticular osteochondroma of the distal humerus. Arthroscopic excision of the osteochondroma yielded complete relief of symptoms. Absence of recurrence was confirmed radiographically at two years after surgery. To the best of our knowledge, this is the first report of osteochondroma of the elbow successfully treated arthroscopically.

2.Comparison of Two Arthroscopic Coracoplasty Approaches in Subscapularis Tears.

BACKGROUND: Few studies have reported the results of arthroscopic coracoplasty concomitantly conducted with subscapularis tear. Therefore, this study was conducted to examine and compare the outcomes of arthroscopic subscapularis repair after arthroscopic coracoplasty using either the subacromial approach or rotator interval approach. METHODS: We retrospectively reviewed 51 patients who underwent coracoplasty with subscapularis repair. The patients were grouped according to whether the subacromial approach group (24 patients) or rotator interval approach group (27 patients) was used during coracoplasty. Preoperative and postoperative visual analogue scale scores, American shoulder and elbow surgeons scores, Korean shoulder scores, and range of motion (ROM) were assessed. Assessment of repaired rotator cuff tendon integrity was performed at 1 year after surgery using either magnetic resonance imaging or ultrasonography. RESULTS: At final follow-up, overall functional scores and ROM improved significantly in both groups when compared with preoperative values (p>0.05). The re-tear rates were not significantly different between groups; however, the rotator interval approach group showed a significant increase in ROM compared with that in the subacromial approach group (p<0.05). CONCLUSIONS: Arthroscopic coracoplasty conducted concomitantly with subscapularis repair can provide a satisfactory outcome. There were no significant differences between the two approach groups regarding final functional scores and re-tear rates. However, the rotator interval approach group showed a greater increase in ROM at final follow-up, especially in external rotation.

3.A Case of Monilethrix.

Monilethrix is a rare familiar disorder of unknown cause, occurring in either sex, and ueually confined to the scalp. In most cases it is manifested in early childhood. lt is characterized by localized or widespread dystrophy of hair and follicles, the shafts showing regular variations in diameter producing alternate "nodes" and "internodes" which confer a striking beaded or moniliform appearance on the hair. We experienced a case of monilethrix, which was associated with keratosis pilaris and occurred in three generations. The patient's one sibling, mother, 4 uncles and 2 aunts of mother side and grandmother with her sister were all affected with a similar diaorder of hair.

4.A Case of Bronchogenic Carcinoma with Skin Metastasis following a Pleural Biopsy.

A case of metastatic tumor of the skin is described with literature review. Cutaneous metastases of malignant tumor is rarely seen but lung cancer in male and breast cancer in female are not infrequently reported. This patient, 37 year old male, had lung cancer, had several chest tappings for pleural effusion and a pleural biopsy. 8 months later, skin manifestation of diffuse erythematous sclerotic patches and confluent nodules was noted on pleural biopsy site, right lateral chest wall. Histopathological finding was compatible with metastatic carcinoma. This metastatic skin tumor is considered to be arisen from the lung probably due to pleural biopsy.

6.Localization of Nerves Innervating Sublingual and Submandibular Gland in the CNS Using Cholera Toxin B Subnit and Pseudorabies Virus.

In the rat brain stem, neurons innervating the sublingual and submandibular gland were investigated by means of cholera toxin B subunit (CTB) and pseudorabies virus (PRV). Injection of CTB into the sublingual gland and PRV into the submandibular gland, neural tracer labeled neurons showed similar positions in central nervous system with PRV into the sublingual gland and CTB into the submandibular gland. CTB labeled-neurons were observed in superior salivatory nucleus, PRV labeled-neurons in superior salivatory nucleus and reticular nucleus. CTB was more fine tracer than PRV for observation of superior salivatory nucleus. The size of CTB labeled-neurons is larger in submandibular gland than in sublingual gland. The size of PRV labeled-neurons were nearly the same after injection to submandibular or sublingual gland. No neurons were labeled together with CTB and PRV. Neurons innervating sublingual and submandibular gland were localized independently in superior salivatory nucleus. These results provided a neuroanatomical data of the neurons innervating the sublingual and submandibular gland in the superior salivatory nucleus.

7.4-Hydroxybenzaldehyde Restricts the Intracellular Growth of Toxoplasma gondii by Inducing SIRT1-Mediated Autophagy in Macrophages

Toxoplasma gondii is an intracellular protozoan parasite that infects approximately one third of the human popu- lation worldwide. Considering the toxicity and side effects of anti-toxoplasma medications, it is important to develop effec- tive drug alternatives with fewer and less severe off-target effects. In this study, we found that 4-hydroxybenzaldehyde (4- HBA) induced autophagy and the expression of NAD-dependent protein deacetylase sirtuin-1 (SIRT1) in primary murine bone marrow-derived macrophages (BMDMs). Interestingly, treatment of BMDMs with 4-HBA significantly reduced the number of macrophages infected with T. gondii and the proliferation of T. gondii in infected cells. This effect was impaired by pretreating the macrophages with 3-methyladenine or wortmannin (selective autophagy inhibitors) or with sirtinol or EX527 (SIRT1 inhibitors). Moreover, we found that pharmacological inhibition of SIRT1 prevented 4-HBA-mediated expres- sion of LC3-phosphatidylethanolamine conjugate (LC3-II) and the colocalization of T. gondii parasitophorous vacuoles with autophagosomes in BMDMs. These data suggest that 4-HBA promotes antiparasitic host responses by activating SIRT1- mediated autophagy, and 4-HBA might be a promising therapeutic alternative for the treatment of toxoplasmosis.
8.A Study on Motivation for Pregnancy of Married Women in Korea.

PURPOSE: This study is to define the structural patterns of pregnancy motivation of married women. METHOD: Q-methodology was used on 110 statements collected from interviews with women and a literature review. 38 Q-samples were selected. The Q-samples were administerd to 30 randomized P-sample. RESULT: Data analysis of Q-type obtained by QUANL program shows three types of subjectivity in the perception of pregnancy motivation. Type 1, "Family-connection strengthening", Pregnancy motivation is characterized by the tendency to make home, to tie family member, to connect couple firmly. Type 2,"Self-satisfaction", Women of this type take pleasure and happiness from child rearing in terms of being a mother and having maternal affection. Type 3, "Self-centered", This type of pregnancy motivation puts on high emphasis on self achievement and they make a family planning suitable for the maternal role. CONCLUSION: The stronger was women's pregnancy motivation, the more positive were women's pregnancy experience and maternal role attainment. Therefore, When nurses take care of pregnancy women, they should understand the women's psycho-social aspects and then apply a program of nursing intervention to the pregnant women, It is necessary to continue to study on social and psychological problems related to pregnancy.

9.Molecular Epidemiology of Methicillin-Resistant Staphylococcus aureus Isolates with Toxic Shock Syndrome Toxin and Staphylococcal Enterotoxin C genes.

BACKGROUND: Many methicillin-resistant Staphylococcus aureus (MRSA) isolates in Korea possess a specific profile of staphylococcal enterotoxins in that the toxic shock syndrome toxin gene (tst) coexists with the staphylococcal enterotoxin C gene (sec). Because the analysis of staphylococcal cassette chromosome mec (SCCmec), a mobile genetic element mecA gene encoding methicillin resistance, showed that majority of these are SCCmec type II, these MRSA isolates with tst and sec may be genetically related with each other. This study was performed to investigate the genetic relatedness of tstand sec-harboring MRSA strains isolated in Korea by using pulsed-field gel electrophoresis (PFGE). METHODS: A total of 59 strains of MRSA isolates of SCCmec type II possessing tst and sec were selected for PFGE and phylogenetic analyses. These isolates were collected from 13 health care facilities during nationwide surveillance of antimicrobial resistance in 2002. RESULTS: The 59 MRSA isolates were clustered into 11 PFGE types, including one major group of 26 strains (44.1%) isolated from 7 healthcare facilities. Seven PFGE types contained 2 or more isolates each, comprising 55 isolates in total. CONCLUSIONS: Most of SCCmec type II MRSA isolates containing tst and sec showed closely related PFGE patterns. Moreover, MRSA isolates collected from different healthcare facilities showed identical PFGE patterns. These findings suggested a clonal spread of MRSA strains possessing tst and sec in Korean hospitals.

10.Antimicrobial Resistance Profiles of Staphylococcus aureus Isolated in 13 Korean Hospitals.

BACKGROUND: There has been a nationwide increase in infections caused by Staphylococcus aureus resistant to multiple antimicrobial agents in Korea. Although nationwide antimicrobial resistance pro-files have been reported recently by analysing routine antimicrobial test results, a more extensive resistance profile survey including those agents that are not used routinely is required for the inferring the mechanism of antimicrobial resistance. We assessed the resistance profiles of a variety of antimicrobial agents on Korean nationwide collection of S. aureus strains and analyzed the profiles according to methicillin resistance. METHODS: We collected a total of 253 clinical isolates of S. aureus from 13 clinical laboratories over the country during a month in 2002. Antimicrobial susceptibility testings were performed with 21 antimicrobial agents by disk diffusion method. Methicillin-resistant S. aureus (MRSA) isolates were also confirmed by the PCR detection of mecA gene. RESULTS: More than 90% of MRSA strains were resistant to the following antimicrobial agents tested: -lactams (92.0-98.3%), gentamicin (92.5%), tobramycin (94.9%), erythromycin (96.6%), and cip-rofloxacin (94.3%). But only 0-29% of methicillin-susceptible S. aureus (MSSA) strains were resistant to those agents. MRSA and MSSA strains were respectively resistant to chloramphenicol in 4.0% and 2.5%, rifampin in 12.1% and 1.3%, trimethoprim-sulfamethoxazole in 22.4% and 1.3%, and amikacin in 51.7% and 3.8%. No isolates were resistant to vancomycin and teicoplanin. Falsely susceptible results against beta-lactams in MRSA ranged from 1.7-7.5%. No significant differences in susceptibility were noted against the agents within the same classes such as macrolide and fluoroquinolone. The PCR results of mecA gene correlated 100% with the results of oxacillin disk diffusion test. CONCLUSIONS: Methicillin resistance in S. aureus was an indication of resistance against beta-lactams, gentamicin, tobramycin, macrolide, and fluoroquinolone in Korea. However, MRSA still retained the susceptibility to chloramphenicol, rifampin, and trimethoprim-sulfamethoxazole. This study may provide antimicrobial resistance profiles necessary for the understanding of antimicrobial resistance mechanisms of S. aureus.
