1.Cutaneous Metastasis from Prostatic Carcinoma.

Prostatic carcinoma is one of the most common malignancies in males of western countries but it is a rare malignancy in Korea. If it occurs, it commonly metastasizes to other organs. In-spite of its high incidence of distant metastasis, the cutaneous metastasis of the prostatic carcinoma is very rare and seems to have a poor prognosis. A 67-year-old man presented multiple nontender nodules on the skin of the right thigh and had episodes of multiple metastases from prostatic carcinoma to the lungs and bones. Histopathologic findings of the skin showed atypical cells infiltration at the dermis. Immunohistochemical staining for prostate specific antigen showed a positive reaction, therefore, we reached the diagnosis of cutaneous metastasis from prostatic carcinoma.

2.Effects of Age and Left Ventricular Hypertrophy on Left Ventricular Diastolic Function in Normotensives and Hypertensives.

Transmitral left ventricular diastolic inflow velocities determined by pulsed Doppler in 47 normotensives, 80 hypertensives and 44 age-matched hypertensives were measured to evaluate the effects of age and left ventricular hypertrophy on left ventricular diastolic fuction in normotensives and hypertensives. The results were as follows: 1) The peak E velocity in normotensives(77.82+/-13.53cm/sec) was significantly different from those of hyeprtensives(58.13+/-11.80cm, p<0.01) and age-matched hypertensives(59.31+/-10.94cm/sec, p<0.01). The peak E/A in normotensives(1.31+/-0.45) was significantly different from those of hypertensives(0.83+/-0.21,p<0.01) and age-matched hypertensives(0.83+/-0.20, p<0.01). 2)In normotensives, simple linear regression analysis revealed strong inverse correlations of peak E with age(r=-0.758, p<0.01), peak E/A with age(r=-0.748, p<0.01), but no significant correlation of peak E and peak E/A with LVMI(p>0.05). 4.Carney's Complex with Familial Atrial Myxoma.

The Carneys complex is a multisystemic tumorous disorder that features myxoma(heart, skin, and breast), spotty skin pigmentation(cutaneous and mucocutaneous), endocrine tumors(adrenal, testicular, and pituitary) and peripheral nerve tumors. The criteria far diagnosis of the complex is the presence of two or more of the above conditions. The condition is transmitted as an autosomal dominant trait. The most serious components of the syndrome are cardiac myxoma and psammomatous melanotic schwannoma. The cutaneous manifestations are a major clue in the disorder. We report a case of Carneys complex in a 19-year-old woman. She had spotty pigmentation on her face, left atrial myxoma, and myxoma on her right nipple. Her mother and sister had left atrial myxoma.

6.A Case of Porokeratotic Eccrine Ostial and Dermal Duct Nevus of Childhood Onset.

Porokeratotic eccrine ostial and dermal duct nevus is considered to be a rare congenital hamartoma of eccrine duct origin. Histopathologic studies showed multiple cornoid lamella-like parakeratotic columns, which were associated with hyperplastic eccrine ostia and distal sweat ducts. I describe the case of a 20-year-old man with childhood-onset porokeratotic eccrine ostial and dermal duct nevus. He had a 10-year history of multiple keratotic papules and comedo-like pits on the thenar area and thumb of the right hand. But in my case, immunohistochemical studies for carcinoembryonic antigen showed no association with eccrine ostium and adjacent cornoid lamella-like structures.

7.Bizarre Parosteal Osteochondromatous Proliferation: A Report of One Case

In 1983, Nora et. al. reperted 35 cases of bizarre parosteal osteochodromatous proliferations of the hands and feet. All lesions occured on proximal phalanges, metatarsals, or metacarpals. The gross appearance was typically that of a small osteochondroma. Radiologically, the proliferations lacked both central continuity of the tumor with the underlying osseus medulla and flaring of the adjacent cortices. Histologically, the lesion exhibited marked proliferative activity, irregular bohy cartilaginous interface, and enlarged, bizarre, and binucleate chondrocyte. The authors experienced a case of bizarre parosteal osteochondromatous proliferation of the right foot in a 61 year-old female which arised from the proximal phalanges of second toe, hostologic and radiologic findings were consistent with the bizarre parosteal osteochondromatous proliferation. The mass was treated by excision, and neither recurrence, nor malignant change was observed up to one year follow-up period.

8.Overgrowth of the Tibial Shft following Fracture in Children

Longitudinal growth acceleration of the femoral shaft after fracture in children has been recognized since the late nineteenth century, Orthopaedic surgeons have attempted to compensate for the leg length discrepancy by allowing union to occur with some degree of shortening, but logitudinal overgrowth following fractures of the tibial shaft in children has been rarely studied. Thirty-one children, treated for fracture of the shaft of tibia, were studied prospectively to assess the consequent increase in longitudinal growth of the tibia. The material presented in this paper is the result of the study of thirty-one children with fractured tibia who were treated in the Department of Orthopaedic Surgery of Kangnam Sacred Heart Hospital of Hallym University, since 1986 October, the age of the child varied from 3 years to 13 years, these cases have been followed from 30 months to 40 months. We have made a radiologic study of fracture of the tibial shaft with a view to analysing the results according to a number of factors, including the sex and the type, site and degree of overriding of fracture, and method of treatment, and then the degree of overgrowth was compared with unaffected side. 1. The average tibial overgrowth following tibial shaft fracture is 7mm. 2. Growth of the femur was not affected by the tibial fracture.

10.Effect Of the Polyethylene Wear On Osteolysis in Cementless Primary Total Hip Arthroplasty: Minimum 5 Years' Follow - Up Study.

We measured poiyethylene wear radiographically in 85 cementless primary total hip arthroplasties by the modified technique of Livermore et al. All the patients were followed up for at least five years (mean, 6.31 years; range, 5 years-8 years 2 months). The mean extent of linear wear was 0.88mm (range, 0.0083-3.2839mm), and the mean rate of linear wear was 0.14mm/year(range, 0.00-0.47mm /year). The mean volumetric wear was 539.42mm (range, 5.11 2022.09mm), and the mean rate of volumetric wear was 87.69mm/year(range, 0.65-290.12mm/year). The polyethylene wear was correlated inversely with the patients age only, but was not influenced by weight or gender of the patients, Harris hip score, thickness of the polyethylene, the abduction angle of acetahular cup, or the duration of in situ implantation. Osteolysis developed in the femur of 34 hips(40%) and in the acetabulum of 11 hips(12.9%). Radiographic loosening was found in one case in the femur and none in the acetabul pm. The extent of polyethylene wear was significantly correlated with frequencies and sizes of the osteolytic lesion and was not directly associated with radiographic loosening of the prostheses. Calcar resorption was observed in 39 hips(45.9%) and was not associated with polyethylene wear. These results indicated that polyethylene wear of primary total hip arthroplasty should be considered one of major causes of osteolysis and has no direct relationship with prosthesis loosening. To reduce the extent of polyethylene wear, the material characteristics of polyethylene should be improved or new, durable, articulating materials should be researched.
