5.Current status and advances of radiotherapy for limited-disease small cell lung cancer
Journal of International Oncology 2014;41(10):757-760
About 30% ~ 40% of small cell lung cancer (SCLC) patients belong to limited disease (LD).In the setting of limited-disease small cell lung cancer (LD-SCLC),the combination of thoracic radiotherapy (TRT) with chemotherapy is the standard treatment for LD-SCLC with improved survival and local control. It is proven that prophylactic cranial irradiation can improve the prognosis of patients with SCLC.Radiotherapy should join in the early chemotherapy and individualized formulate appropriate target volume.Radiation schemes of conventional fractionation or hyperfractionation way are recommended.

BACKGROUND:To evaluate the psychological characteristics of the patients waiting for renal transplantation in order to analyze coping style, thus perform the corresponding personalized clinical psychological intervention which is conductive for patients to face reality and disease. OBJECTIVE:To investigate the psychological characteristics of the patients waiting for renal transplantation and to analyze the effect of different social factors on the coping style of patients and to explore the corresponding clinical mental intervention method METHODS:Fifty-eight patients waiting for renal transplantation and 60 patients without renal transplantation were randomly selected from Second Department of Urology, Organ transplantation Center, the 309th Hospital of PLA between February 2009 and August 2010, the patients were required to fil in a psychological questionnaire form using the currently general used medical response. The effect of social factors on the coping style and psychological stress status was analyzed with multi-factor Logistic regression analysis. RESULTS AND CONCLUSION:From the questionnaire survey, the evasive and yielding value scores of the patients waiting for renal transplantation were significantly higher than those of the patients without renal transplantation (P<0.05), but there was no significant difference of confrontation score between the patients (P>0.05). It indicated that the psychological pressure of the patients waiting for renal transplantation was bigger that of the patients with out renal transplantation, which may related with course of the disease, cost and the rejection after transplantation for the patients has to endured. Multi-factor Logistic regression analysis showed that sex, education, income, payment methods and age of the patients were al related with the confrontation, evasive and yielding scores, as those with technical secondary school education or better got the best confrontation score while those with annual income less than 60 000 yuan got the highest score in evasive and yielding styles. The results show that income and education level of patients are the main factors that affect the coping style and psychological stress status of the patients waiting for renal transplantation, treating the patients with reasonable renal clinical care interventions can help the patients face to the disease positively and can reduce the damage of transplantation to the psychology of the patients.

Objective To study the effect of restraint and watered stress of different durations on the gastric empting in rats. Methods 48 male SD rats (weight ±150 g) were divided randomly into 6 groups with 8 in each. The experimental rats were restrained and watered in 23℃ water box for 1 h/d, while the controls were free. The body weight, general conditions and behavioral indexes of the rats were observed and recorded. The gastric empting rate was measured in rats with semi-solid diet after 3 d,7 d and 28 d during the statistically significant differences among different groups of different stress groups, and between the 3 d test group and 3 d control group. The gastric empting rate was higer in the 7-day test than in 7 d controlled group, lower in the 28 d test group than 28 d control group. Conclusion Different chronic stress can cause changes of gastric empting rate. With the prolonging of stress time, the empting rate is rising and then decending.

The stability of cell genetic material is influenced by a variety of factors, both internal and external, which can cause various types of DNA damage, such as DNA alkylation, oxidation, mismatching, loop structure, atypical DNA structure, single-strand break, and double-strand break. These DNA damages disrupt cellular homeostasis and dynamic equilibrium, which cause gene mutations, chromosomal abnormalities, and even degradation, aging, and death at different biological levels.By searching and identifying DNA damage sites, the cell activates a series of biochemical pathways, coordinates the progress of DNA replication and transcription, and then repairs the DNA damage. In this way, the cell maintains its independence and stability. While radiotherapy plays a role in eliminating tumors by DNA damages, it also initiates DNA damage responses.Among the responses, base excision repair, nucleotide excision repair, mismatch repair, double-strand break repair, and post-translesion synthesis repair play a key role in repairing the damages. The dysfunction of these repair pathways will cause differences in tumor radiation sensitivity. This paper summarizes recent research results in DNA damage repair, and focuses on the types of DNA damage and their repair mechanisms, so as to promote the understanding of the great significance of this field and to provide a theoretical basis for exploring the application of DNA damage repair pathways in tumor therapy.

Objective: To discuss the influence of the fine care for newborn with severe pneumonia (SPIN) combining with respiratory failure (RF). Methods: 60 cases of SPIN with RF were selected and divided into the observation group and the control group. 28 cases of SPIN with RF in the observation group were received fine care observation group after treatment, in which 16 cases were male children and 12 cases were female children. The ages of these children were from 10d to 28d day, and their average age was 18.26 + 4.16 d. The weight was from 2.4kg to 4.4 kg, and their average weight was 3.11 + / - 0.44 kg. 32 cases of SPIN with RF in the control group were received routine nursing, in which 14 cases were male children and 18 cases were female children. The ages of these children were from 10d to 28d and their age was (10 ~ 28) d. Their average age was (18.33 + 4.07) d. The weight was from 2.4kg to 4.4 kg, and their average weight was 3.08 + / - 0.46 kg. Then we analyzed espectively respectively on two groups of children with pH, arterial blood gas analysis index of oxygen partial pressure (PaO2), arterial partial pressure of carbon dioxide (PaCO2), respiratory rate (RR) and heart rate (HR) level and the total time of mechanical ventilation, the incidence of adverse circumstance and nursing satisfaction. Results: The observation group with pH, PaO2, PaCO2, RR and level of HR after fine care is superior to the control group, and the differences were statistically significant difference (t= 4.36, 3.83, 2.23, 12.11, 5.45; P <0.05). The total time of mechanical ventilation in the observation group was significantly lower than the control group, and the difference was statistically significant difference (t=7.63, P<0.05). The reintubation rate of the observation group was obviously lower than the control group and the difference was statistically significant (x2=3.83, P<0.05). Family satisfaction of nursing in the observation group was obviously higher than that in the control group, and the difference was statistically significant (x2=18.49, P<0.05). Conclusion: Fine care can effectively improve the SPIN with RF with blood gas analysis index, shorten the time of mechanical ventilation therapy, alleviate the pain and reduce reintubation rate. It has higher clinical value.
