Persistent burning and derythema were present for 10 months cin he lower lip of a 55-year-old woman, who had extreme melalignment of the anterior maxillary teeth. Candida albicans was identified not only from the surface material of the lower lip but also from tl.e malaigned dental cleft, which was thought to be the source of the organisms. We feel that the impor.ance of malaligned theeth as a source of candida albicans miist be kept in mind when we face persis ent cheilitis in a patient whose teeth arrangement is bad.

Hibernoma is a rare subcutaneous neoplasm resembling the brown fat of the hibernating animal. It is usually found in the interscapular, cervical or axillary regions. We report a case of this rare tumor which developed on the occeipital scalp of a 42-year-old woman.

Annals of Dermatology 1997;9(1):51-54
Chromoblastomycosis, a chronic fungal infection of the skin and subcutaneous tissue, is known to be difficult to treat. Recently itraconazole has proved effective for treatment of this infection, but requires a prolonged treatment course. We experienced a case of chromoblastomycosis caused by Fonsecaea pedrosoi in a 68-year-old Korean man showing a complete resolution with a short course of oral itraconazole, 200 mg daily, for six weeks.

Solitary cutaneous reticulohistiocytoma is a type of reticulohistiocytosis. The cytoplasm of tumo cells characteristically shows abundant eosinophilic "ground-glass" appearance. We report a case of this rare cutaneous tumor in a 54-year-old woman. It was present as a single nodule on the right index finger of the patient for seven years.

Korean Journal of Dermatology 1989;27(6):709-713
Two cases of folliculitis decalvans are reported. One case is a 51 year-old woman showing the lesion first on the left occipital scalp and then, 4 years later, on the left mandibular area. The other case, a 57 year-old woman. showed the lesion typically encircled with pustules and crusts for the 20 years on the vertex area.

8.The Relationship between Tinea Versicolor and Earwax Type , and Incidence and Population Densities of the Two Phases of Malassezia Furfur according to the Earwax Type.

Korean Journal of Dermatology 1994;32(5):795-801
BACKGROUND: Although the wet earwax has been known to be related with the incidence of tinea versicolor, very little has been revealed on how they are correlated with each other. The incidence of Malassezia furfur, the etiological organism of tinea versicolor, and its population densities as yearst and mycelial form in the different types of earwax, the dry and the wet, have not been studied. METHODS: The type of earwax in 65 patients with tinea versicolor were classified into 2 types, dry and wet, by its appearence and the subjective complaints of the patients. The dry and wet cerumens were obtained from 142 and 49 individulas without tinea versicolor respectively. The cerumens were stained with 10% Parker ink-KOH solution. The incidence and its population densities of yeast and mycelial M. furfur in the wet cerumens were compared with those in the dry cerumens according to a new grading method using bacterial index(BI) of lepra bacilli. RESULTS: 1. Dry and wet earwax were observed in 58.5%(38 cases) and 41.5%(27 cases) of 65 patients respectively. 2. The incidence of yeast M. furfur in the dry and wet cerumens were 100% and 63.3%(31 of 49 cases) respectively, and that of mycelial M. furfur, 28.2%(40 of 142 cases) and 32.7%(16 of 49 cases) respectively. 3. The population densities(from 3+ to 6+) of yeast M. furfur in the dry and wet cerumens were ob served in 93.0%(132 of 142 cases) and 49.0%(24 of 40 cases), and those(3+) of mycelial M. furfur, 21.1%(30 of 142 cases) and 24.5%(12 of 49 cases) respectively. CONCLUSION: The incidence(41.5%) of wet earwax in tinea versicolor patients is significantly higher than that(0.8%) in The Korean control group. The ratio between the incidence of mycelial M. furfur and that of yeast M. furfur in the wet cerumens is higher than in the dry cerumens, and the ratio of population densities in the wet cerumens is also higher. These observations suggest that the skin surface conditions of the wet earwax group may be more suitable for yeast-mycelial transformation of M. furfur than those of the dry earwax group. The comparison of chemical compositions of the skin surface between dry and wet earwax groups is needed to elucidate the correlation between tinea versicolor and the types of earwax in the future.

9.Observation of Peripheral Blood Leukocytes in Patients with Acute Urticaria.

Korean Journal of Dermatology 1986;24(4):487-492
In dermatologic practice, acute urticaria showing leukocytosis and fever suggesting syatemic bacterial infection is often observed. The clinician in dealing with such patients is tempted to use antibiotics in addition to corticosteroids and antihistamines before the patient can be sure that there is no bacterial infection. Authors observed the degree of the leukocytosis with its differential counts in 106 cases of acute urticaria to evaluate what percentages of acute urticaria patients are showing similar data with the changes seen in leukocyte counts of patients of erysipelas, and evaluated the effect of aqtibiotics-administered group with non-administered group of acute urticaria patients. The results were as follows: 1. In 106 acute urticaria patients, 42% showed higher total leukocyte count ( >12, 500/cu mm) than the mean total leukocytes of 20 erysipelas patients(12, 412/cu mm) and 29% showed greater neutrophilia(>70%) than the mean neutrophil differential count of the erysipelas (69.1%). In the cases showing both leukocytosis (>12,500/cu mm) and fever (>37. 5C) there was no difference in the course of treatment between the antibiotics-administered group and non-administered group. 3. The corticosteroid hormone reduced the total WBC count toward normal level generally within a week.

10.The Effect of Buffered Versus Non - Buffered Lidocain as an Additive to Triamcinolone Acetonide Suspension on Intralesional Injection for Alopecia Areata.

Korean Journal of Dermatology 1997;35(1):41-48
BACKGROUND: Intralesional injection of corticosteroids has widely been used for the t,reatment of alopecia areata. The pair produced during the repeated multiple injection procedures is an actual therapeutic obstacle in both adult and child patients. Buffering of the local anesthetics is known to reduce the pain. OBJECTIVE: The purpos; of this study was to evaluate the degree of pain reduction and hair regrowing effect by the use of buffered lidocain-triamcinolone acetonide suspension(BLT) compared with nonbuffered lidocairi triamcinolone acetonide suspension(LT) in the intralesional injection of alopecia areata. METHODS: We evaluat d the degree of pain reduction by the use of BLT compared with LT in the intralesional injection in 40 cases of alopecia areata. The pain was ineasured by patients verbal pain score from zerc to ten and simultaneously observed if there were be any differences in hair regrowing bet, ween the two groups. RESULTS: 1. Remarkable reduction of the pain du~ring the inject, ion was noted by the buffering agent and the pain was further reduced by the vertical insertion of the needle. The pain score of BLT was 1.5 by vertically inserted injections and 2.4 by ~30 degree angle injections while the pain score of LT was 4.4 by the vertical and 5.6 by t.he 30 degree, Z. The mean time for appearance of regrowing hairs was 3.6 weeks in the BLT area and 4.0 weeks in the LT area. The use of BLT area showed a statistically faster response than the LT area. CONCLUSION: The use ot BLT is recommended not only by it,s remarkable effect on pain reduction but also by its sigriificantly accelerated hair regrowing effect in the trea~tment of alopecia areata.
