5.A Study on the Epedemiological Characteristics of Industrial Ocular Injuries.

A total of 181 cases of industrial ocular injuries hospitalized and treated at the St. Mary's Hospital, Seoul during the 5-year period was studied on their epidemiological characteristics. Of there patients 96.7% were males, and 43.6% were in twenties, 49.7% of the cases were injured on left eyes. There were no statistically significant differences in frequencies of study subjects by day of the week and month of the year. Most common injury hour were between 10- < 12 AM(13.6%) and between 2- < 4 PM(20%). Of the total 101 cases, 37.6% were working less than 6 months, and 73.7%, less than 2 years. Of the total, 42.3% were involved in machinery work and 13.9%, in construction work. According to the American National Standard Method of recording basic facts relating to the nature and occurrence of work injuries, 62% among all source of injuries was metal item and the most common accident type was struck by flying objects(76.2%). Among 181 cases, only 2 cases wearing protective equipment when the accidents occurred.

6.Outcome of Hemodialysis Treatment on 200 Cases of Chronic Renal Failure.

Regular hemodialysis has been established as a way of treatment for end stage renal failure. In adults, hemodialysis 5 hours each time, three times weekly with one square meter hemodialyzer is now widely accepted as a standard, and there are many reports on long term follow up studies. Quite a large difference are present, however, in our community mainly originated from patient's poor compliance in frequent dialysis with short interdialysis interval. The author analyzed 200 cases of chronic renal failure who have received hemodialysis treatments during the 5 year period from July 1976 to June 1981 at the hemodialysis unit of the hanyang University Hospital and the following results are obtained; 1. Actual one year survival rate was much higher in thrice(87.1%) than one(35.1%) or twice(54.1%) weekly dialysis. 2. Long term(more than 6 months) complications were also more frequent in once(85.7%) or twice(70.2%) weekly treatments than in thrice(22.2%). The predominant complications were congestive heart failure, pericarditis, and infections. 3. Those who recieved three times weekly dialysis had better rehabilitation grades than the patients groups of twice or once weekly treatment. 4. Of peridialysis distressful symptoms, nausea, vomiting dizziness were less frequent in the group three times a week dialysis. In contrast, headache, hypotension, muscle cramps, and weakness were not significantly related with frequency of hemodialysis. 5. Those who voided a large amount of urine output had better survival and less frequent requirement of blood transfusions. 6. Main causes of death were due to patient poor compliance, hyperkalemia and cerebrovascular acidents. 7. Economic problems were the major cause of dialysis interruption.

8.Clinical experience of cartilage allograft.

Bony defect is one of the most common problems in craniomaxillofacial surgery. Although aurogenous bone graft is the best choice for the treatment of bone defect, it provides many problems such as donor site morbidity, irregular absorption, and limited amount of harvest. To overcome the shortcomings of autogenous bone graft many bone substitutes have been introduced. The ideal bone substitution is to have characteristics such as cheap, easy to obtain, rapid fusion to recipient bone, hard structure, long maintenance of shape and volume, low infection rate, and low exposure rate. Among those bone substitutes which have been widely used we chose lyophilized cartilage allograft because of low antigenecity, low resorption rate, easiness of carving and ling term preservation. From August 1993 to August 1997, 66 patients had been performed craniomaxillofacial reconstruction with lyophilized cartilage allograft. Orbital wall reconstruction and correction of enophthalmos were 24, correction of cleft lip and nose deformity were 19, temporal augmentations were 7, and others 16. Complications such as infection, exposure were not common. And partially removed cartilage was proved some calcification. Radiologic follow-up presented well positioned lyophilized cartilage allograft. Two radiologic works revealed haziness of bone density at the site of cartilage allograft. This suggests the ossification of lyophilized cartilage allograft. Together with liw infection rate, low exposure rate, and good framework for osteoconduction, lyophilized cartilage allograft are regarded as one of the good bone substitutes.

10.Serum Levels of TNF-alpha, sTNFR, sIL-2R and sICAM-1 in Patients with Hepatocellular Carcinoma.

BACKGROUND: The changes of serum TNF-alpha, sTNFR, sIL-2R and sICAM-1 levels have been recently reported in some cancer patients but the clinical significance of these cytokines is not clearly established. To define the changes and clinical significance of these cytokines in patients with hepatocellular carcinoma (HCC), serum levels of TNF- alpha, sTNFR, sIL-2R and sICAM-1 were measured. METHODS: The serum levels of TNF-alpha, sTNFR, sIL-2R and sICAM-1 were measured before and after transarterial chemoembolization (TACE) by enzyme immunoassay in 20 patients with HCC and 10 adult healthy subjects as controls. These patients were divided into two groups, no response (NR, 14 cases) and partial response (PR, 6 cases) on the basis of changes of tumor size, new growth foci on the ultrasonographic or CT findings, and serum levels of alpha-fetoprotein. RESULTS: Serum levels of TNF-alpha, sTNFR, sIft-2R and sICAM-1 were significantly higher in all patients with HCC than in those of adult healthy controls (p<0.01). Serum levels of TNF-alpha after TACE tended in to be higher in comparison with those before TACE in the NR group (p<0.1), but those were not significantly different between before and after TACE in the PR group. Serum levels of sTNFR, sIL-2R and sICAM-1 were not significantly different between after and before TACE in both groups. CONCLUSIONS: These results suggest that serum levels of TNF-alpha, sTNFR, sIL-2R and sICAM:-1 in patients with HCC before therapy are significantly increased, compared with those of the control group, and the measurement of serum TNF-alpha levels may become useful in the diagnosis and monitoring of hepatocellular carcinoma in the future.
