1.Clinical analysis of coronary angiography in coronary heart disease patients with type 2 diabetes mellitus

Objective To study the angiographie characteristics of coronary heart disease (CHD) patients with type 2 diabetes mellitus.Methods 292 patients with CHD underwent a selective angiography were selected,the characteristics of coronary arteriongraphy were compared in CHD patients with type 2 diabetes mellitus and nondiabetic group.Results In type 2 diabetes mellitus group,the degree of stenosis was higher than that in nondiabetic group (78.01% vs 52.32% )(P < 0.05 ), and the rate of three-vessel disease was higher than that in nondiabetic group (59.57% vs 36.42% )(P <0.05).Conclusion The angiographic characteristics were different in CHD patients with or without type 2 diabetes mellitus.Patients with type 2 diabetes mellitus was associated with more severity of lesion and more lesion vessel.

3.Diagnosis and treatment of acute kidney injury in patients with cirrhosis

Acute kidney injury is a common complication in patients with cirrhosis.It is characterized by a sudden drop in glomerular filtra-tion rate,retention of metabolic waste products,water-electrolyte imbalance,and acid-base disturbance.It markedly increases mortality in cirrhotic patients.Therefore,early diagnosis and treatment of acute kidney injury are essential to reduce mortality and improve prognosis. The development of the diagnostic criteria for acute kidney injury,the clinical application of new biomarkers of renal function such as cystatin C,kidney injury molecule-1 ,and neutrophil gelatinase-associated lipocalin,and the management of acute kidney injury in cirrhotic pa-tients are reviewed.Although creatinine test and monitoring of urinary output have their disadvantages,they remain the main diagnostic crite-ria for acute kidney injury.Development of new biomarkers for clinical use and elucidation of the underlying mechanisms of acute kidney in-jury have become a hotspot of basic and clinical research.

6. Effects of recombinant human insulin-like growth factor-1 on the phosphorylated serine-threonine protein kinase and apoptotic cells in edematous perihematomal brain regions following experimental intracerebral hemorrhage in rats

Objective: To investigate recombinant human insulin-like growth factor-1 (rhIGF-1) on the phosphorylated serine-threonine protein kinase (pAkt) and apoptotic cells in edematous perihematomal brain regions following experimental intracerebral hemorrhage in rats. Methods: A total of 78 healthy male rats were randomly assigned to a false operation group (n =6), a cerebral hemorrhage control group (control group for short, n=36) and a rhIGF-1 intervention group (intervention group for short, n=36). According to 6 time points after giving intervention measures, the rats of the control and intervention groups were randomly redivided into 6 subgroups: 6 h, 12 h, 1 d, 2 d, 4 d, and 7 d subgroups (n =6 in each subgroup). A rat model of acute cerebral hemorrhage was induced by stereotaxic injection of collagenase type IV. RhIGF-1 (50 μg/kg) was injected via tail veins in the intervention group, once a day, until the end of the experiment. An equivalent isotonic saline was injected via tail veins each day in the sham operation and control groups. Samples were taken from each group at a specified time. An immunohistochemical method was used to observe the expression of pAkt around hematomas ; deoxynucleotidyl transferase-mediated terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL) technique was used to observe apoptotic cells. Results: Circled digit oneThe neurological deficit score of the rats in the control group and intervention group reached the highest at day 1 or day 2 after procedure, then decreased gradually. The only score in the intervention group at day 7 after procedure was lower than that in the control group. The difference was statistically significant (P <0. 05). Circled digit twoThere were only a few expressions of pAkt positive cells in rats of the false operation group. Except the 6-hour time point, the numbers of pAkt positive cells at the remaining time points in the intervention group were higher than those in the control group. The difference was statistically significant (P < 0. 05). Circled digit threeOnly a few TUNEL positive cells were observed in the false operation group. Except the 6-hour time point, the numbers of TUNEL positive cells at the remaining time points in the intervention group were significantly lower than those in the control group. The difference was statistically significant (P < 0. 05). Conclusion: RhIGF-1 may reduce apoptosis after cerebral hemorrhage and improve the neurological function. Its role may be associated with the activation of pAkt.
7.Clinical observation of cataract extraction combined with intraocular lens implantation in the treatment of multifocal intraocular lens

AIM: To study the clinical effect of implantation of multifocal intraocular lens ( lOL ) combined with cataract extraction and lOL in cataract patients.
●METHODS:A total of 86 cases ( 86 eyes ) of cataract patients admitted to our hospital from Feb. 2014 to Mar. 2015 were divided into two groups according to the order of admission, each of 43 cases. The 43 patients with cataract extraction combined with non spherical astigmatism correction type monofocal lOL implantation for the treatment as the control group, the other 43 patients with cataract extraction combined with aspheric toric multifocal lOL implantation were treated as the observation group. After 1y of follow- up, the visual acuity, astigmatism and the contrast sensitivity of the two groups were observed.
●RESULTS: There was no difference in visual acuity between two groups (P>0. 05). Postoperative observation group of uncorrected near visual acuity ( UCNVA ) was significantly better than the control group (P<0. 05), and the other indexes were not different (P>0. 05). There was no difference in astigmatism between the two groups before and after operation ( P> 0. 05 ). There was no difference in contrast sensitivity between two groups ( P>0. 05). The contrast sensitivity of the control group was better than that of the observation group (P<0. 05), and the rest had no difference (P>0. 05).
● CONCLUSION: Astigmatism correction multifocal intraocular lens on corneal astigmatism after surgery has a good effect, the naked eye near visual effect is better, the rest of the visual acuity is stable, good visual quality, worthy of clinical application and promotion.
8.Clinical Observations on Acupoint Selection at Distant Places and Local Tenderness Points for Treatment of Migraine Without Aura

Objective To investigate the efficacy of acupoint selection at distant places and local tenderness points in treating migraine without aura.Method Sixty patients were randomly allocated, using a random number table, to two groups, 30 cases each. The treatment group received acupuncture at distant points selected along meridians and local tenderness points and the control group, acupuncture at conventionally selected points. The therapeutic effects were evaluated using the visual analogue scale (VAS) and the short-form McGill Pain Questionnaire (SF-MPQ).Result After treatment, the VAS pain score was lower in the treatment group than in the control group; there was a statistically significant difference (P<0.05). The total efficacy rate was 93.3% in the treatment group and 73.3% in the control group; there was a statistically significant difference between the two groups (P<0.05).Conclusion Both acupuncture at distant points selected along meridians and local tenderness points and acupuncture at conventionally selected points have a certain therapeutic effect on migraine, but the former produces a better effect than the later and can markedly relieve the pain and improve the quality of life in the patients. The outcome of this study provides a certain reference value for clinical treatment of this disease.

10.Effect of LPS on human CIAS1 gene expression and sequence analysis of its mutation region-NACHT domain

AIM: To observe the effect of LPS on CIAS1 gene expression and to analyze the function-structure relationship of its mutation region, the NACHT domain of cryopyrin. METHODS: RT-PCR was used to detect the expression of CIAS1 gene in human leucocytes in the presence of LPS. The methods of molecular modeling and bioinformatics were used to observe the relationship of the structure-function of NACHT domain in human CIAS1 gene. RESULTS: LPS increased CIAS1 mRNA expression in a dose-dependent manner. However, the stimulation with LPS at a concentration of 100 mg/L at different time points showed the pattern of early down-regulation and later up-regulation of CIAS1 mRNA expression. The sequence analysis suggested that the motifs Walker A and Walker B, to which the ATP and Mg~ 2+ can bind, were found in the NACHT domaim of CIAS1 gene encoded product cryopyrin. The main points of the disease-associated mutation showed the close relation in sequence and structure to these motifs. The binding sites of Ca~ 2+ and polysaccharide were observed in the leucine rich repeats region of cryopyrin. CONCLUSION: Cryopyrin may act as an important regulator in inflammation. LPS induces human leukocytes to express the CIAS1 gene product.
