1.Immunohistochemical Study of Malignant Melanoma with HMB - 45 Monoclonal Antibody and Anti S - 100 Protein Antibody.

We evaluated the characteristics of HMB-45 monoclonal antibody(MoAb) and compared it with a polyclonal anti-S-100 antibody in immunohistochemical staining of 22 primary melanomas of the skin ; 13 acral lentiginous melanomas(ALM), 3 superficial spreading melanomas(SSM), 4 lentigo maligna melanomas(LMM), 3 nodular melanomas(NMM). All speimens were formalin-fixed and paraffin-embedded, and were studied with the avidin-biotin-peroxidase complex technique. The results can be summarized as follows : l. In all tissue specimens, all or some of the melanoma cells in the dermis reacted with HMB-45 MoAb and anti-S-100 protein. 2. HMB-45 MoAb is more sensitive in detecting malignant melanoma cell heterogeneity t,han anti-S-100 protein. 3. Melanoma cells within the epidermis in 6 of 12 ALMs and 3 of 4 LMMs did not react with anti-S-100 antibody, whereas most melanoma cells within epidermis of 12 ALMs and 4 LMMs reacted with HMB-45 monoclonal antibody. These results indicated that immunohistochemical staining with HMB-45 MoAb and anti-S-100 protein is very useful in the study of biologic behavior of malignant, melanoma.

3.Amyloid Deposit in Malignant Cutaneous Epithelial Tumors.

We examined the amyloid deposit in various malignant cutaneous epithelial tumors by using Dylon stain. Histochemically identifiable amyloid deposits associated with basal cell epitheliomas(BCEs), squamous cell carcinomas(SCCs) and Bowens diseases were studied with monoclonal cytokeratin antibodies and anti-amyloid P antibodies. The results were as follaws, l. Amyloid deposits were detected in 5 of 12 BCEs, 4 of 11 SCCs, 3 of 9 Bowens diseases and 1 of 8 actinic keratoses. Amyloid deposits were not detected in o keratoacanthomas, 2 verrucous carcinomas, 1. Pagets disease and 3 extramammary Pagets diseases. 2. Anti-keratin antibody 34BE12 and anti-amyloid P antibody reacted with amyloid deposits in 3 of 5 BCEs which showed abundant amyloid deposits by Dylon stain. 3. Of 4 SCCs which showed amyloid deposits by Dylon stain, anti-keratin ant.ibody 34pE12 reacted with amyloid deposits in 2 cases and anti-amyloid P antibody reacted with amyloid deposits in 3 cases. 4. Anti-amyloid P antibody reacted with arnyloid deposits in 3 of 3 Bowens diseases which showed arnyloid deposits by Dylon stain. Anti-keratin antibody 34pE12 reacted with amyloid deposits only 1 of them. These findings suggest that epidermal keratins are the percursor substance of amyloid in malignant cutaneous epithelial tumors. The difference of antikeratin staining pattern between BCE-associated amyloid and SCC or Bowens disease-associated amyloid may be the results of difference in development stage of cutaneous amyloids.

5.The Indirect Effect of Prefrontal Gray Matter Volume on Suicide Attempts among Individuals with Major Depressive Disorder

Trait impulsivity is a known risk factor for suicidality, and the prefrontal cortex plays a key role in impulsivity and its regulation. However, the relationship between trait impulsivity, neural basis, and suicidality has been inconsistent. Therefore, this study aimed to explore the relationship between impulsivity and its structural correlates (prefrontal gray matter volume), suicidal ideation, and actual suicide attempts. A total of 87 individuals with major depressive disorder participated in study, and the gray matter volume of the prefrontal regions was extracted from T1 images based on region of interest masks. The variables for the mediation models were selected based on correlation analysis and tested for their ability to predict suicide attempts, with impulsivity and suicidal ideation as the mediation variables and gray matter volume as the independent variable. A significant correlation was observed between suicidal ideation and the left dorsolateral prefrontal cortex and right dorsomedial prefrontal cortex. The dual-mediation model revealed a significant indirect relationship between gray matter volume in both regions and suicide attempts mediated by motor impulsivity and suicidal ideation. The counterintuitive positive relationship between gray matter volume and suicidality was also discussed.
6.A cross-sectional study on prevalence rate and contributing factors of fatty liver diagnosed by ultrasonography.

Fatty liver is caused by derangement of fat metabolism and can be reversed by removal of contributing factors. The contributing factors of fatty liver is known to be overweight, chronic alcoholism, diabetes mellitus, malnutrition, and drug abuse such as tetracycline. This study was carried out on 1335 persons who visited 'Soon Chun Hyang Human Dock Center' from March to June 1990. In analysis of the data, prevalence of fatty liver diagnosed by ultrasonogram by age and sex, laboratory finding between fatty liver group and normal group, and odds ratio of known contributing factors, were compared. The results obtained are as following; 1) The prevalence rate of fatty liver diagnosed by ultrasonogram is 29.6% in male and 11.5% in female. 2) Age groups with high prevalences are 40~50's in male (32.0%) and 50's in female (24.5%). 3) The fatty liver shows significant association with style (p<0.05), whereas not with hepatitis B-virus surface antigen (p>0.05). 4) All laboratory values except alkaline phosphatase and bilirubin are elevated significantly in accordance with the degree of fatty liver (p<0.01). 5) Fatty liver diagnosed by ultrasonogram showed so strong associations with body index, triglycerides and gamma-glutamyl transferase for males, and body index and fasting blood sugar for females that these factors may be used as supplementary data in establishing diagnosis of fatty liver. 6) Odds ratio of contributing factors are as follows; If the odds ratio of below 29 year of age is 1.0 then that of 30~39 is 1.74 (p=0.33), 40~49 is 2.47 (p=0.10), 50~59 is 2.86 (p=0.0570), over 60 is 1.81 (p=0.34). If the odds ratio of female is 1.0 then that of male is 5.67 (p<0.01). If the odds ratio of body index below zero is 1.0 then that of 0~9 is 5.08 (p<0.01), 10~19 is 12.37 (p<0.01), 20~29 is 29.19 (p<0.01), 30 above is 154.02 (p<0.01). If the odds ratio of below 99 mg/dl FBS is 1.0 then that of 100~120 is 106 (p=0.76), over 120 is 1.91 (p=0.02). If the odds ratio of below 29 micron/1 gamma-GT is 1.0 then that of 30~s59 is 2.11 (p<0.01), 60~90 is 1.87 (p<0.05), 90 above is 1.69 (p=0.15). If the odds ratio of below 149 mg/dl TG is 1.0 then 150~199 is 1.49 (p=0.05), 200~250 is 1.09 (p=0.77), 250 above is 2.53 (p<0.01). In summary, early diagnosis of fatty liver could be made by ultrasonogram supplemented with body index and serum triglyceride. The fatty liver could be preventive by avoiding contributing factors such as obesity, alcohol intake, high blood sugar appropriately.

7.A Study on Respiratory Effects of Diuretics ( Furosemide ) during General Anesthesia .

Diuretics have been used as one of therapeutics for acute respiratory failure. We observed the change of arterial blood gases at the same ventilatory level, after intravenous injection of 20 mg of furosemide, in ten of twenty patients without diseases of heart, lung or kidney. The obtained results were: 1) 30 minutes and 1 hour after the injection of furosemide, there were peak increments in arterial PO2. 2) There were no significant changes in pH and PaCO2.

8.Posterior Approach for Cervical Spondylosis with Radiculomyelopathy.

Of the 68 cervical spondyltic patients showing symptoms of radiculomyelopathy, assessments were made on 29 patients who underwent neural decompressive surgery with cervical laminectomy through posterior approach for the results of surgery. Analyses were also made on the anterior-posterior diameters of cervical canal on the plain film of cervical spine. The mean values of anterior-posterior diameters measured on the levels of cervical C3-7 were 16.5+/-1.83 mm in the normal adult and 7.3+/-1.08 mm in the cervical spondylotic patients with radiculomyelopathy, which is far narrower than that of the normal. In cervical spondylotic patients, the anterior-posterior diameters of directly upper and lower parts of the lesions were 11.9+/-1.20 mm and 12.1+/-1.61 mm respectively, also much narrower mean values than those of the normal control group. The results of the 29 patients who received cervical laminectomy by posterior approach were much improved for 51.7% and improved for 44.8%.

9.Surgical Treatment for the AVM Feeding Mainly from PCA.

Among 51 intractanial AVM patients, 30 patients(58.8%) with AVM supplied mainly with PCA were studied for the relationship with feeding vessels, location and the size of AVM in connection with hemorrhage and also results in 20 surgical patients were analyzed. On cerebral angiogram, 56.7% accounted for AVM supplied only by PCA, 16.7% together with MCA and 26.6% with MCA and ACA. By location, 36.7% were for subcortical AVM and 63.3% were for deeply located paraventricular. 53.3% were large AVM and of them, 6.5% bleeded 30.0% accounted for small AVM, of which 88.9% bleeded. The total bleeding rate was high at 73.3%. Complete removal by surgery was available for 90.0%, and post operative improvement was at 75.0% with a mortality rate of 10.0%. 50.0% showed disappearance of seizure following total removal and 33.3% improved. 5.0%, however, had seizure for the first time following the surgery.

10.Follow-up Management State of Lead Battery Workers in Periodic Health Examination.

To evaluate the follow-up management state and related factor of lead battery workers in periodic health examination as part of program of group occupational health service, author studied 293 workers with questionnaire on knowledge of results and follow-up management state and related factors, and compared the responses to their periodic health examination result charts. The results were as follows: 1. 252(86%) workers responsed that they had received the health examination result chart, but only 116(39.6%) workers responsed that they had been educated or explained about the result of health examination, and 11(57.9%) workers among 19 workers with non-occupational disease D, 101(44.3%) workers among 228 workers with non-occupational disease C, and 19(28.4%) workers among 67 workers with occupational disease C knew accurately their health examination results. 2. 78(24.8%) of the workers responsed that they had follow-up management, and contents of follow-up management were follow-up(36.6%), out-patient treatment(31%), change worksite(8.5%), temporary retirement(7.0%) and others(16.9%). 3. Most of the workers responsed that the health examination were necessary, but three-fourths of the workers responsed that the health examination had been superficial or that they didn't know. 4. In this study, follow-up management show significant association with only explanation or education about health examination result chart.
