1.Evaluation of the Chemotherapy on Tuberculous Lesions of the Kidney.

In spite of the great measure of recent success in tuberculosis withchemotherapy, it is still one of the most fundamental questions in what state ofrenal tuberculosis should be treated surgically. So as to set up a criterion onthe chemotherapy, forty-nine extirpated tuberculous kidneys were observed withmy own classification of tuberculous renal lesions. 1. The lesions were classified macroscopically and pyelographically according to the development of renal tuberculosis (1) Those with neither change of calyceo-pelvic system nor pyelographical abnormality were named Class O. representing parenchymal tubercles. (2) Those with only infiltration in the tip of papilla were named Class I, showing caliectasis or clubbing pyelographically. (3) Those with initial minor ulceration in calyceal system were named Class II, showing fuzzy irregular or moth-eaten outline pyelographically (4) Those with progressed distorted ulceration were named Class III, showing definite irregular deformity but still keeping some semblance to the original calyceal form pyelographically. (5) Those with cavitation, open or closed were named Class IV, showing irregularly outlined shadow without original anatomic form pyelographically. (6) Those with tuberculous pyonephrotic ectasis were named Class E, showing less irregularly out lined cavity shadow pyelographically. 2. Tuberculous kidneys might also be graded clinically and pyelographically with the classification of the lesions. (1) The tuberculous kidneys with early lesions,i.e. Class O, Class I and/or Class II were called minimal renal tuberculosis.(2)Those with intermediate lesions. i.e. Class III were called moderate. (3) Those with a progressed lesion, i.e. Class IV or Class E were called advanced. (4) Those with two or more lesions of Class IV and/or Class E were called far advanced. 3. The lesions belong to each Class were divided again into untreated control group and three treated groups with triple drug therapy of different durations and the macroscopical and histopathological appearances in each treated group were compared with in untreated one. 4. Macroscopically the lesions in treated kidney with remaining excretory function and without obstruction showed tendency toward clearing of caseous material in each class.5. The specific reactions in tuberculous tissue were improved histopathologically in proportion to duration of the therapy. (1) Caseous material was cleared up. (2) Epithelioid cells revealed sufficient degeneration and diminution, occasionally complete disappearing. (3) Giant cells also revealed degeneration, vacuolation and diminution, frequently complete disappearing. 6. The severity of non-specific tissue reactions in the lesions. i.e. connectivetissue reaction, parenchymal degeneration and defect, interstitial inflammation and lymphocytic infiltration was not influenced by duration of the therapy, but depended on Class of lesions at the time of therapy began. 7. The repairing reactions in the lesions were increased according as prolongation of the therapy, but there might be some variety between the different reactions. (1) Vascularization: according as the duration, but not remarkable. (2) Regeneration of epithelium: according as the duration, but only partial covering. (3) Vacuolation: significant, however, also noticed in untreated group. (4) Reducing of perifocal reaction: according as the duration, but not sufficient in the progressed lesions.8. In the more progressed tuberculous lesions, specific and non-specific tissuereactions predominsted over repairing, therefore histological healing was more delayed. 9. There were noticeable evidences of histological improving in the specimens from patients with triple drug therapy for more than three months. 10. As a result of these observations, I should like to recommend that: (1) for minimal renal tuberculosis, chemotherapy is continued; (2) for moderate, chemotherapy is tried firstly, and if the lesions are persistent or worsened surgery will be considered; (3) for advanced and far advanced, surgery including partial nephrectomy is preferable after at least three month chemotherapy.

2.Subperiostedal Ganglion of the Tibia: A Case Report

Subperiosteal Ganglion is a rare cystic lession, although ganglions are frequently found in close proximities to joints and tendon sheaths. It is characterized by a thin-walled cyst containing tenacious mucoid fluid and by the pedicle attached to periosteum. This report describes a middle-aged man with a subperiosteal ganglion which occured beneath the periosteam of the tibia. Presenting symptoms were pain and circumscribed swelling over the anteromedial aspect of the right proximal tibia.

3.Culture of Nail Matrix Cells.

BACKGROUND: In vitro epidermal keratinocytes(EKs) culture systems are widely used in studying skin biology and differentiation. Recently a monolayer culture method of nail matrix cells(NMCs) has been developed. OBJECTIVE: This study was designed to establish a method of culture of NMCs and compared its characteristics with those of EKs. METHODS: Human NMCs were isolated and cultured in a defined medium. Phase contrast microscopic examination, growth rate and ketatin expression were evaluated. RESULTS: In in vitro monolayer culture, NMCs had a similar growth pattern and morphological features as EKs. The synthesis of hard keratins was detected. CONCLUSION: These observations suggest that the NMCs culture model may be useful for the study of nail properties and the effects of drugs.

4.Four cases of trichotillomania.

Trichotillomania is the cllinical syndrome of alopecia resulting from the repetative pulling, plueking and breaking of ones own hair. We report four cases of trichotillomania in 10 year old children. The lesions were limited to the scalp. All of them had psychiatric problems in the school or home according to a psychiatric consultation. The histopathologic examinatiorii revealed a normal number of hair follices, empty hair follicles and no inflammatory reaction. The treatment led to hair growth in three cases but one case did not respond to the treatment.

5.A Clinical Study of Femoral Neck Fracture

We analysed forty-six cases of femoral neck fracture treated with internal fixation and followed for more than one year at Korea University Hospital from January 1979 to June 1986 and the following results were obtained. 1. Sex distribution was 23 cases(50%) male and 23 cases(50%) female, and average age was sixty years old (range from 21 to 91 years). 2. The most common cause of injury was slip down(76%). 3. The most common type of fracture was Garden's type III(46%) and Pauwels' type III (67%). 4. In cases of internal fixation, the most commonly used method was closed reduction and multiple pinning under the local aneshtesia(83%). 5. Bony union was established within 5.9 months in average. 6. Among forty six cases, 7 cases(15%) of avascular necrosis, 4 cases(9%) of malunion, and 2 cases(4%) of nonunion was occurred. 7. The clinical results were excellent in twenty cases(43%), good in sixteen cases(35%), fair in seven cases(15%), and poor in three cases(7%) according to Lunceford's assessment.

6.Clinical Analysis of Common Peroneal Nerve Repair

Common peroneal nerve injury has been reported as the most frequent lower extremity peripheral nerve injury. The peroneal nerve may be particulary prone to direct or indirect injury by penetrating trauma or external compression by it's superficial route to bony prominence of fibular neck. By the development of microsurgical technique, recently epineural repair, fascicular group repair, and interfascicular graft were tried. The authors have experienced five cases microsurgical repair of common peroneal nerve injury around the knee joint from December 1990 to November 1991, and motor and sensory result was relatively good.

7.Lead Concentrations of Pigeon's Tissue as Indicator of Lead pollution in Air and Soil.

It has been studied that a variety of fauna and flora are sensitive biological indicators which reflect the severity of regional pollution of heavy metals, but in the center of part of Taegu city the controversial issue of lead poisoning attributable to the atmosphere which contains an increased concentrations of lead has been raised recently, it is usually hard to find suitable plants or animal in the areas with heavy traffic. Pigeons are ubiquitous in and around Taegu city area, inhabiting even the most densely populated areas with heavy traffic with its small body size, high metabolic turnover, and rather limited mobility, a pigeon, as a biological indicator is expected. This study was conducted to monitor lead pollution in the Taegu and kyongju city in Korea. We measured the lead content of the various tissue of three groups of feral pigeon(Columbia livia) and soil and atmospheric lead concentration. First group was obtained io heavy traffic area in Taegu City, the second group was obtained a park in Taegu city and the third group was obtained light traffic area in kyongju city. The air and soil lead concentration of heavy traffic area in Taegu city was 0.11 microgram/m2, 4.96 microgram/g, that of park in Taegu city was 0.05 microgram/m3, 2.65 microgram/g and that of light traffic area in kyonngju city was 0.03 microgram/m3, 0.01 microgram/g. The lead content of lung, blood, kidney, femur and liver of feral pigeons in heavy traffic area in Taegu city was significantly higher than pigeons obtained in a park in Taegu city and low traffic density area in Kyonfju city(p<0.01). But stomach lead content of three group did not reflect a significant difference. In this study positive correlation was found between atmospheric lead concentrations and the concentration of lead in the pigeon's lung(r=0.5040, p<0.001), blood(r=0.3322, p<0.01), kidney(r=0.4824, p<0.001), femur(r=0.7214, p<0.001) and liverer (r=0.4836, p<0.01). we can also found positive correlation between soil lead concentrations and the concentration of lead in the pigeon's femur(r=0.4850, p<0.001), kidney(r=0.4850, p<0.001) and liver(r=0.4386, p<0.01). In the pigeon`s tissue there were significant correlations between concentration of lead in the blood and kidney(r=0.4818, p<0.001), femur(r= 0.6157, p<0.001) and liver(r=0.3889, p<0.001). In conclusion, at the heavy traffic area in Taegu city, lead concentrations found in the atmosphere and soil are reflected in the lead concentrations of different tissue of urban pigeons. It is suggested that the tissue of pigeons can be good biological indicators of environmental lead pollution.

8.A Case of Paroxysmal Atrial Fibrillation before Induction of General Anesthesia in an Elderly Patient.

Atrial fibrillation(AF) diagnosed by fibrillatory waves of atrial activity and an irregular ventricular response on ECG is a common arrhythmia associated with increased cardiovascular morbidity and mor- tality. The incidence of AF increases markedly with advancing age and the presence of congestive heart failure. Most patients with AF can be attributed to organic heart disease such as rheumatic, atherosclerotic and hypertensive heart disease. But there is an increasing awareness of so-called lone AF which has no underlying cardiac or metabolic abnormality. We report a case of paroxysmal AF developed just before the induction of anesthesia in a 73-year-old patient who had no history of cardiovascular disease.

9.Studies on the life cycle of Culex pipiens pallens in Korea.

The present studies were carried out, under natural room condition from June to July in 1969. The period required for growth of eggs and larva varied according to various temperatures and methods of rearing. The average number of eggs laid by the Culex pipiens pallens was 137 per egg raft. The period of gonotrophic cycle was from 3 to 6 days: incubation period of eggs was from 1.5 to 2 days: larva period was from 10 to 14 days: pupa period was from 2 to 3 days. Accordingly, the period from the beginning of gonotrophic cycle to the adult mosquitos in next generation required 25 days at the longest and 16.5 days at the shortest.
