1.Purpura Fulminans: Report of A Case.

Purpura fuIminans is a rare form of nonthrombocytapenic purpura characterized by sudden onset, fever, prostration, anemia and symmetrical massive ecchymoses, usually of the lower extremities, and hypotension associated with disseminated intravascular coagulation (DIC). A 18 years old female patieat with characteristic clinical manifestations of purpura fulminans was seen at the department of Dermatology, St. Pauls Hospital. With intensive systemic corticasteroid and heparin treatmemts, the ecchymoses and necrosis began to heal, leaving multiple, thick esehar formation from 8th day, Although purpura fulminana has been known to attack mainly children, this case was of adolescent age.

2.A Clinical Review on Cor Pulmonale.

A clinical review was made on 48 cases with cor pulmonale who were admitted to medical department, National Medical Center, between 1973 and 1982, and the results were as follows : 1) Teh age of subjects ranged from 26 to 88, and their mean was 51. male to female ratio was 1.6:1 and majority of them were in their 6th and 7th decade. 2) COPD was the most common cause of cor pulmonale but pulmonary tuberculosis had also a considerable portion. Less commonly it was caused by chest deformity which was mainly from spine tuberculosis and kyphoscoliosis. 3) Almost all patients were admitted with over heart failure. 4) Laboratory features showed indirect evidence of heart failure and pulmonary hypertension in chest X-ray films, RVH patterns in EKG and hypoxia and hypertcapnea with metabolic compensation in arterial gas analysis. 5) The case fatality rate was one-forth and that from kyphoscoliosis was especially high(80%).

4.Cosmetic Repair of Nevus of Ota.

PURPOSE: To report a case of cosmetically successful treatment of nevus of Ota performed by scleral painting and scleral allograft transplantation. METHODS: Scleral painting and allograft transplantation were done in a 28-year-old woman who had cosmetic problem due to nevus of Ota, and she was followed up for 5 months postoperatively. RESULTS: The patient had no specific postoperative complication and was satisfied with the surgical results. CONCLUSIONS: For patients of nevus of Ota who have not been treated successfully, scleral painting and allograft transplation can be an effective cosmetic treatment.

5.Clinical Study of Eye-Me(R) Soft Contact lens.

A clinical evaluation of Eye-Me(R) (Paik San, Korea), daily and extended wear, soft contact lenses was performed for a 3-month period. The two kinds of lenses were worn by 20 persons (40 eyes) in each group. Measurements of the central corneal thickness were taken in 10 eyes of extended wear group following 8 hours of sleep with contact lenses. The mean percent corneal thickness change after 8 hours of sleep with lenses was 1O.6 +/- 1.9%. At 3 months after lens wearing, good corrected visual acuity (over 0.8) was obtained in 33 eyes 36 eyes (91.7%) of daily wear group, and 32 eyes of 34 eyes (94.1%) of extended wear group, respectively, During the follow-up period of 3 months, mild adverse signs were noted in 8 eyes of each group. 85% of daily wear group and 90% of extended wear group were satisfied with Eye-Me(R) lenses. The above results suggest that the visual outcome and the safety of Eye-Me(R) soft contact lens could be acceptible for its clinical application.

9.Morphometric Study of the Nerve Roots Around the Lateral Mass for Posterior Foraminotomy.

OBJECTIVE: Morphometric data on dorsal cervical anatomy were examined in an effort to protect the nerve root near the lateral mass during posterior foraminotomy. METHODS: Using 25 adult formalin-fixed cadaveric cervical spines, measurements were taken at the lateral mass from C3 to C7 via a total laminectomy and a medial one-half facetectomy. The morphometric relationship between the nerve roots and structures of the lateral mass was investigated. Results from both genders were compared. RESULTS: Following the total laminectomy, from C3 to C7, the mean of the vertical distance from the medial point of the facet (MPF) of the lateral mass to the axilla of the root origin was 3.2-4.7 mm. The whole length of the exposed root had a mean of 4.2-5.8 mm. Following a medial one-half facetectomy, from C3 to C7, the mean of the vertical distance to the axilla of the root origin was 2.1-3.4 mm, based on the MPF. Mean vertical distances from the MPF to the medial point of the root that crossed the inferior margin of the intervertebral disc were 1.2-2.7 mm. The mean distance of the exposed root was 8.2-9.0 mm, and the mean angle between the dura and the nerve root was significantly different between males and females, at 53.4-68.4degrees. CONCLUSION: These data will aid in reducing root injuries during posterior cervical foraminotomy.

10.Effect of Fetus on Balanced Anesthesia of Cesarean Section .

The authors have observed that 10 cases of gestation period of 38 weeks to 42 weeks, age from 23 to 31 women. Patients were induced smoothly with Demorol 1.0~1.5mg/kg, Valium(Diazepam 0.15~0. 20mg/kg/Wt, 0.1% succinylcholine intravenous dripping and N2O-O2, 2:1 ratio inhalation anesthesia. The following results were observed. 1) Maternal systolic and diastolic blood pressure were maintained during the anesthesia in comparing prior to induction of anesthesia. However pulse rate depreased 9% after 5 minutes of anesthesia and 15% decreased at the end of anesthesia. 2) Maternal arterial blood gas was found insignificantly as prior to induction and 5minutes following anesthesia. 3) Blood gas of umbilical vein of newborn(prior to disconnect of umbilical cord), maintained within normal limits. 4) In compared with Moya's study, Apgar score of this anesthesia was lower than spinal anesthesia, whereas the authors observed higher than cyclopropane-succinylcholine, N2O-succinylcholine or thiopental-N2O-succinylcholine anesthesia. The authors have studied as above findings, however clinical studies to be needed.
