A 6-year-old male patient had been suffering from angular cheilitis and paronychia with fragmentation and dystrophic change of the finger nails. Laboratory findings showed low serum iron level and anemia. Immunologic studies revealed defects in cell mediated immunity. KOH examination and culture of specimens from the lesions showed hyphal elements and growth of Candida albicans respectively. Concomitantly itraconazole and iron sulfate were administered. Four months after treatment he was free of any clinical evidence of the disease.

Ultrasonography is most commonly utillized diagnostic tool in obstetric and gynecology for the evaluation ofpatient with a pelvic mass or pregancy. For it is characterized by no radiation hazard, noninvasive examinationand high diagnoastic accuracy. Also it affords an accurate assessment of the presence, size, location and internalconsistency of a pelvic mass. The recent availability and improved resolution of realtime scanning have afforded amore flexible and complete approach to evaluation of normal and abnormal structures in the pelvis. We analyzedultrasonographic findings in 154 pathologically proven cases of pelvic mass examined at Korea General hospitalfrom Jan. 1983 to Apr. 1984. The results were as follows; 1. The age distribution was from 12 years to 66 yearsand the majority of patients were between the ages of 21 and 50 yeasrs (91.4%). 2. The incidence of pelvic masswas 27.9% in uterine leiomyoma, 22.7% in ovarian cyst, 13.0% in addenomyosis and 8.4% in serous cystadenoma. 3. Mild to moderately echogenic nodular uterine enlargement with some cystic change(81.5%) of leiomyoma and multiplesmall vesicular pattern of intrauterine contents with uterine enlargement of Homole were the most common ultrasonographic findings. The location and type of leiomyoma were most common in the fundus and body (95.3%), and intramural myoma(53.5%). The most frequent finding of ovarian teratoma was cystic mass with echogenic focus(41.7%) but the echogenic appearance of the lesions was extremely variable. The ultrasonographic findings ofectopic pregnancy were cystic or complelx adnexal mass(80.0%), with or without fluid in cul-de-sac and deviationof uterus by adenxal mass. 4. Accuracy of ultrasonography in detemining the overall correct diagnosis of thepelvic masses compared with proven diagnosis was approximately 61.7%. The diagnostic accuracy was 90.7% inluterine leiomyoma, 100% in H-mole nad 80.0% in ectopic pregnancy.

BACKGROUND: Superficial fungal infections are common diseases acid the prevalence of superficial fungal diseases are not static but change under the influence of various forces such as climate, migration of peoples and development in prophylaxis and therapy. OBJECTIVE: To investigate the present status of superficial dermatiphytoses and some changes on the frequency of the diseases, sex distributions and varieties of the casiative organisms. MEHTODS: The clinical and rnycological aspects of superficial fungal diseases of 3,222 patients from July 1989 to June 1992 were analyzed. Results : 1) The incidence of superficial fungal diseases was 25.4% of total out-patients. 2) The superficial fungal diseases showed a higt incidence in the third(26.4%) and the fourth decade(24.3%). 3) The ratio of male to female was 2.11:1(2,188:1,034). 4) The monthly prevalence of super ficial fungal diseases was high inhuly(17.8%), August(15.2%) and June(1.7%). 5) The incidence of superficial fungal diseases in order of frequency were tinea pedis ,28.1%, tinea cruris(19.5%), onychomycosis(15.6%), tinen orporis(11.5%), tinea manus(7.8%), tinea versicolar(7.8%), candidiasis(7.4%) and tinea capitis(2.3%), 6) Coexisting fungal infections were found in 644 patients(19.9%) and the case of tinea pedis with onychomycosis was the most common. 7) The positive rate of KOH scraping was 54.0% and the positive rate of culture on ordinary Salourauds dextrose agar media was 42.7%. 8) Trichophyton rubr,im was the most common causative orantism of superficial dermatophytes(38.4%), and other in decreasing frequency were Candida albicans(36.9%), Trichophyton mentagrophytes(23.6%) Microsporum canis(0.7%), Trichophyton ferrvgiteum(0.3%) and Epidermophyton floccosum(0.1%). CONCLUSION: These results are not significantly different from those iif previous studies. But the incidence rate of the superficial pungal diseases(25.4%) were higher thar, previous studies and showed a high incidence in young male patients and in the summer. The positiv rate of KOH scraping(54.0%) was lower than previous studies. The most common causative organism was Trichophyton rubrum(38.4 %).

PURPOSE: To assess the usefulness and reliability of the IOL power calculation computer program ('POWER!'). METHODS: Calculation times and projected postoperative refractive values of 100 patients with 'POWER!' program were compared with those of the program mounted on the Humphrey R A/B scan system (model 820) by SRK-II and SRK/T formula. RESULTS: There was no significant difference between the projected postoperative refractive values of 'POWER!' program and Humphrey R A/B scan system (paired-t test, p>0.05). Calculation time of 'POWER!' was shorter than that of the program mounted on the Humphrey R A/B scan system incalculating more than 3 sets of IOL (paired-t test, P<0.05) and there was no internal calculation error (ztest, p>0.05). CONCLUSIONS: 'POWER!' is a useful and reliable program for IOL power calculation using SRK-II and SRK/T formula.

PURPOSE: The effect of superior clear corneal incision and transverse astigmatic keratotomy on the with-the-rule astigmatism in sutureless, small incision cataract surgery was evaluated. METHODS: The postoperative keratometric changes were evaluated in 18 eyes with more than 1.4 diopter (D) of preoperative with-the-rule astigmatism, having sutureless, superior clear corneal incision phacoemulsification. Five eyes over 2.8 D with-the-rule astigmatism also had transverse corneal relaxing incision. Keratometric readings were done within 1 month preoperatively, 1 day and 2 months postoperatively and the changes were statistically analyzed. Simple subtraction method and Cravy method were used for the analysis of astigmatic changes. RESULTS: The mean astigmatism showed decreasing trend, 2.08, 2.25, 1.12 D, within preoperative 1 month, at postoperative 1 day and 2 months respectively and 3.70, 2.48, 2.06 D respectively when transverse keratotomy was added. The reduction of astigmatism was statistically significant at postoperative 2 months (P<0.05; Wilcoxon rank-sum test) and it was more significant with transverse astigmatic keratotomy (P<0.05, Mann-Whitney U test). CONCLUSIONS: The with-the-rule astigmatism over 1.40 D could be reduced in cataract surgery by sutureless, superior clear corneal incision and transverse corneal relaxing keratotomy.

The primary aims of fracture treatment include the restoration of normal anatomy and a return of function to the injuried as early as possible. In pilon fracture, various methods of treatment were developed to accomplish this purpose. Fibular fixstion is a rarely accepted mehtod in the treatment of pilon fractures because it can't provide rigid internal fixation, But it might be indicated when an open wound was : present medially over the distal tibia, the ligamentous attachments of distal fibula and medial malleolus to talus and calcaneus and of the distal tibiofibular syndesmosis are largely intict. Utilizing these attachment, Fibular fixation can stabilize comminuted fractures of distal tibia affecting ankle joint. This procedure is so effective and simple The author has used this technique in three instances for two years with excellent results. This is not an original method, but worthy of being remindful of the literature.

Diagnostic ultrasound has become one of the most useful tools in the practice of obstetics. It has been ofparticular utility in the placental localization. We analyzed 34 patients of placenta previa scanned byultrasound. The reults were as follows; 1. The age of patient ranged from 22 to 39 years, showing the highestincidence in 26 to 30 years. 2. The accuracy of correct localization was 70.6%. 3. Among 13 cases diagnosed byultrasound as total placenta previa, 2 cases were partial placenta previa and 1 was low-lying placenta at the timeof delivery. 4. Among 9 cases diagnosed by ultrasound as partial placenta previa, 1 case was total palcenta previaand 1 case was low-lying placenta and 1 case was upper segment placenta. 5. Among 10 cases diagnosed by utrasoundas low-lying placenta, 2 cases were partial placenta previa. 6. Among 2 cases diagnosed by utlrasound as uppersegment placenta, 1 case was total placental previa and 1 case was partial placenta previa. 7. Among 9 cases doneserial ultrasoud, 3 cases revealed that the placenta migrates toward fundus in the course of pregnancy, Therefore,the palcental scanning should be repeated in the last month before term to decide the mode of delivery.conclusively, ultrasonography is the imaging modality of choice in the evaluation of placental localization becuseit provides speedy and repeatable way without any known risk to both mother and fetus itself. Careful performanceand accurate interpretation shold be needed for more correct palcental localization.

Total knee arthroplasty (TKA) is a successful and cost-effective treatment for end-stage degenerative arthritis. The aging of society and an increase in the prevalence of obesity has led to increases in the prevalence of arthritis and the incidence of TKA. Currently, the total number of procedures in Korea per year has reached 90,000. With the rapid growth, we need to know about the current state of TKA. The purpose of this review is to summarize the recent literature regarding TKA. The main indication for TKA is end-stage arthritis with severe pain, reduced function, and no response to conservative management. Metal on the polyethylene-bearing surface and cobalt alloy are used in most TKAs. Despite good clinical outcomes and long-term survival rates after TKA in many papers, 20% of patients are dissatisfied with the outcome of surgery. To improve the patient’s satisfaction, surgeons should understand factors affecting patient’s satisfaction, including patient’s expectations, age, and preoperative mental state. Navigationassisted surgery and robotic surgery have been introduced in knee arthroplasty to achieve more precise and accurate alignment. There is some evidence to suggest that computer-assisted surgery reduces revision rates. However, clinical efficacy is also controversial, and a long-term follow-up study is required. The common complications of TKA include infection, polyethylene wear, loosening, stiffness, periprosthetic fracture, and thromboembolism. An understanding of the potential complications and pitfalls of TKA is essential for prevention.
