1.Clinical Experience with Lederkyn in Genito-urinary Diseases.

Lerderkyn, a recently introduced sulfonamide, has been administrated to 11 cases in doses ranging from 1 to 3 gm daily. The compound is rapidly absorbed from the gastrointestinal tract and therapeutically effective plasma concentration is maintained rapidly and for many hours, following the oral administration. The diffusion of Ledrkyn into the tissue and the body fluid occurs readily and is slowly excreted through the kidney. Maintenance of effective concentration in urine for many hours results in successful treatment especially for genito-urinary tract infection. In this study, five acute gonococcal urethritis, one non-gonococcal urethritis, two acute cystitis, three chronic prostatitis have been treated with this drug in doses of 1 to 3 gm daily and very satisfactory results obtained in acute bacterial infection.

2.Chemical Analysis of Urinary Calculi.

5.The Clinical Observation on the Acute Epidural Hematoma.

Acute epidural hematoma is the one which neurosurgeons encounter commonly in severe head injuries and needs emergency operation. But the fact that the mortality of it is still very high(20-50%), is disheartening to all neurosurgeons. In my country, the acute epidural hematoma has been recognized and treated since the 1950th by neurosurgeons. Nevertheless, the report of the collected acute epidural hematoma is rarely seen. The reason is felt that the illness is quite common around us but the mortality is so high to make the report be delayed. The authors have treated 60 cases of surgically confirmed acute epidural hematoma during 24 months from April 1969 to March 1971 and have observed clinically. Following are the results: 1) Sex incidence showed male(50), female(10), and age incidence ranged 2 years of age to 60 yrs. But it was most common in 2nd decade to 4th decade. 2) Most common mode of injury was traffic accident(33 cases) and there were 15 cases of falling down accident and 12 cases of direct batting over the head by blunt materials. 3) There were lucid interval in 13 cases. Most common papillary change was ipsilateral mydriasis(23 cases). Bilateral mydriasis were in 4 cases & other showed miotic or normal pupils. Bilateral positive Babinski's sign were noted in 22 cases and there were 14 cases of unilateral Babinski's sign. 4) Site of hematoma;Most common site of the hematoma was temporoparietal region but there were 8 cases of confined frontal pole and 3 cases of posterior fossa region. In 42 cases of 60, the site of hematoma was confirmed by carotid angiography & on the rest, direct surgery was performed without carotid angiography. 5) Mortality; 47 cases of 60, have survived. The age, level of consciousness, other intracranial associated injuries, time of operation, bilateral positive Babinski's sign, volume of hematoma were felt to be the influencing factors to mortality. In 5 cases of 13 which expired, have died 4 weeks after surgery and the causes of death were pulmonary edema, pneumonia, septicemia, tracheal bleeding & renal shut down. In survivals, hemiplegia, hydrocephalus, oculomotor nerve palsy and mental confusion were observed which improved gradually except 4 cases, & 43 cases of 60 which have survived could remain to return to his usual daily activity.

6.Acute Paraplegia as the Initial Symptom of Acute Leukemia.

Metastatic involvement of the central nervous system during the course of leukemia is an increasingly frequent occurrence because of the longer survival time brought about by the efficacious cytotoxic therapy available today. The onset of acute leukemia may be abrupt with a great variety of clinical manifestations. Most commonly, such as vomiting, headache, papilledema, lethargy and vertigo, are referable to increased intracranial pressure caused by infiltration of the cortex and meninges. The acute paraplegia as the initial symptom of acute leukemia is very rare. A case in which acute paraplegia occurred as the initial symptom of acute leukemia in a 15-year-old boy is reported.

9.Two Cases of bilateral diffuse cystic lesion.

Lymphangioleiomyomatosis(LAM) is a rare disease of unknown etiology that occurs mainly in woman in her reproductive age. We recently experienced two cases of bilateral diffuse cystic lesion of the lung on chest X-ray and HRCT. The first case, a 26-year-old female, who had been diagnosed with tuberous sclerosis be the presence of clinical manifestation such as mental retardation, bilateral renal angiomyolipoma, adenoma sebaceum and generalized seizure, was admitted due to recently developed hemoptysis. Chest PA showed diffuse ground-glass opacity with radiolucent cystic lesions of various sizes on both lung fields. HRCT showed innumerable small cystic lesions with suspicious diffuse ground-glass opacity on both lung fields. The second case, a 30-year-old female was admitted due to dyspnea and spewing of blood-tinged sputum for 2 weeks, shortly after delivery. Chest PA showed diffuse reticular and ground-glass opacities on both lung field. HRCT showed multiple well-difined and relatively uniform size air cysts with a uniform wall thickness on entire both lung fields, with small amount of right pleural effusion. By thoracoscopic lung biopsy she was diagnosed with pulmonary lymphangioleiomyomatosis. We report these cases with a brief review of the literatures.

10.Diagnostic Value of Reticulated Platelet in Thrombocytopenia.

PURPOSE: We performed this study to determine the diagnostic significance and useful cut-off value of reticulated platelet, which can be used for discriminating the destructive thrombocytopenia from underproductive thrombocytopenia. METHODS: We evaluated 37 patients with thrombocytopenia who were admitted to the Wonju christian hospital from March to July, 1995. All patients were evaluated with bone marrow megakaryocyte count. We divided them into two groups, group 1 was consisted of underproductive thrombocytopenic patients and group 2 was consisted of destructive thrombocytopenic patients. We measured the peripheral blood reticulated platelet count by FACScan flowcytometry after thiazole orange staining. RESULTS: 1) Among 37 patients, underproductive group consisted of 21 patients and destructive group consisted of 16 patients. 2) There was no significant difference in platelet count between two groups (41.7 31.8x109/L vs. 41.0 29.1x109/L). 3) Destructive group showed higher reticulated platelet count than underproductive group (7.7 3.7% vs. 19.1 11.1% : p<0.01). 4) Ideal cut-off value of reticulated platelet count for the discrimination of two groups was 11%. Its sensitivity and specificity were 88% and 95%, respectively. CONCLUSIONS: We suggest that reticulated platelet count is closely correlated with thrombopoiesis of bone marrow. The measurement of reticulated platelet can be a simple and useful diagnostic tool to discriminate destructive thrombocytopenia from underproductive thrombocytopenia.
