1.A Case of Acquired Tufted Angioma.

Acquired tufted angioma is a benign, slowly progressive angioma with a typical histological pattern that was first described by Wilson-Jones in 1976. We report a case of acquired tufted angioma in a 19 year old female who had erythematous papules and plaques on the right thigh. Histopathological findings showed multiple capillary lobules in a cannonball arrangement scattered throughout dermis, which was diagnostic of acquired tufted angioma.

2.Studies on the life cycle of Culex pipiens pallens in Korea.

The present studies were carried out, under natural room condition from June to July in 1969. The period required for growth of eggs and larva varied according to various temperatures and methods of rearing. The average number of eggs laid by the Culex pipiens pallens was 137 per egg raft. The period of gonotrophic cycle was from 3 to 6 days: incubation period of eggs was from 1.5 to 2 days: larva period was from 10 to 14 days: pupa period was from 2 to 3 days. Accordingly, the period from the beginning of gonotrophic cycle to the adult mosquitos in next generation required 25 days at the longest and 16.5 days at the shortest.

4.Histologic Changes, Hsp 70 and bcl-2 Expression and Apoptosis Following Piezoelectric Extracorporeal Shock Wave Treatment on Immature Rat Testis.

PURPOSE: The aim of this study is to evaluate the sequential histologic changes and spermatogenesis of the testis after extracorporeal shock wave treatment (ESWT). MATERIALS AND METHODS: ESWT to right testis was applied on total 48 immature, 5-week-old Sprague Dawley rats. The rats were sacrficed immediately, at 1 day, 3 days, 1 week, 2 weeks and 4 weeks after ESWT, and each group consisted of 8 rats including 2 rats as control (without ESWT). Histologic changes of both testes were examined, and serial three or more parafin sections were obtained, and immunohistochemical stains for bcl-2 and Hsp 70 and TUNEL method for apoptosis were performed. RESULTS: Hemorrhage and destruction of seminiferous tubules which appeared immediately after ESWT were maximal at 1 day after ESWT and were seen absorption and atrophy at 1 week after ESWT. Even regenerating tubules were found at 2 weeks after ESWT, no significant histologic difference between both testes experimental and control groups were seen except for some regenerating tubules. The right testicular tissues, remote from the ESWT-targeted area, showed preserved spermatogenesis since the age of 5 weeks 3 days, and was presented no significant difference between experimental and control groups. Hsp 70 and bcl-2 expression and apoptosis were maximal at 1 day after ESWT and persisted to 3 day after ESWT. Weak and focal expression were noted at 1 week after ESWT, and mostly disappeared at 2 weeks after. CONCLUSIONS: Spermatogenesis of right testis except the targeted area was not diffrent compared with that of left testis and control group. Hsp-70 expressin, which limits tissue destruction after injury, was immediately appeared for defense mechanism, and hemorrhage and tissue destruction following ESWT seemed to be due to apoptosis controlled by bcl-2 expression.

5.Arthroscopic Ankle Fusion Using Two Medial Cannulated Screws.

PURPOSE: We evaluated the clinical and radiological results of arthroscopic ankle fusion using 2 medial screws which had advantages of less morbidity, early weight-bearing and high union rate. MATERIAL AND METHODS: From April, 2002 to March, 2004, 8 patients who had ankle osteoarthritis were treated by ankle fusion using 2 medial screws under arthroscopy; five patients with post-traumatic osteoarthritis, two with post-infectious arthritis and one with paralytic foot. There were 5 male and 3 female. Average age was 67 years old ranging from 57 to 71 years. We evaluated them clinically preoperative and postoperative using AOFAS score, VAS pain scale and patient's satisfaction. In regard to radiological fusion, we checked them by simple AP, lateral and mortise view. Follow up period was average 11 months (range, 6~24 months). RESULTS: All ankles were successfully fused with 2 medial screws under arthroscopy. The mean time of fusion was 10.5 weeks (range, 8~14 weeks). Patient's satisfaction checked at 6 months after operation had favorable results (excellent and good 75%). One case had pain on medial malleolar area because of screw's protrusion. CONCLUSION: Arthroscopic ankle fusion using 2 medial screws was good modality of ankle fusion with less morbidity and early weight-bearing in some cases of ankle arthritis.

6.Chronic cough, not asthma, is associated with depression in the elderly: a community-based population analysis in South Korea

BACKGROUND/AIMS:
Depression and allergic diseases, including asthma, are frequently reported as comorbid conditions. However, their associations have been rarely examined in community-based elderly populations.
METHODS:
The analyses were performed using the baseline data set of the Korean Longitudinal Study of Health and Aging, which consists of 1,000 elderly participants (aged > 65 years) randomly recruited from an urban community. Depression was assessed using the Geriatric Depression Scale, Center for Epidemiologic Studies Depression Scale, and Hamilton Rating Scale for Depression. Major and minor depressive disorders were diagnosed by psychiatrists. Allergic conditions were assessed using structured questionnaires, lung function, and skin prick test. Quality of life and comorbidities were assessed using structured questionnaires.
RESULTS:
Prevalence of asthma and major depressive disorder were 5.4% and 5.3%, respectively. The rate of depression was not significantly different between the non-asthmatic and asthmatic groups. No correlation was observed between the scores obtained using the depression scales and self-reported asthma. However, chronic, frequent, and nocturnal cough were significantly associated with depression and scores obtained using the depression scales, which remained significant in multivariate logistic regression analyses (chronic cough: odds ratio [OR], 3.23; 95% confidence interval [CI], 2.57 to 12.74; p = 0.04). Rhinitis was independently associated with high Mini-Mental State Examination scores (OR, 1.11; 95% CI, 1.05 to 1.17; p < 0.001) and low 36-item short-form (OR, 0.96; 95% CI, 0.80 to 0.98; p = 0.002).
CONCLUSIONS
Depression may not be significantly associated with asthma and allergic diseases in elderly populations, but cough is a significant factor affecting depression.
7.A Case of Tension Pneumothorax Complicating Duodenal Microperforation after ERCP.

Endoscopic retrograde cholangiopancreatography (ERCP) has become a commonly performed endoscopic procedure for the diagnosis and treatment of pancreatobiliary disease. ERCP is a relatively safe procedure. However, there are chances of potentially severe complications such as pancreatitis, hemorrhage, infection and perforation. Duodenal perforation is an uncommon, but serious complication of ERCP, and this has occurred in 0.3 to 1.1% of most of the previous series. There are various clinical course and treatments depending on the cause of perforation. However, the development of pneumothorax in patients undergoing ERCP is rare. There are no reports of tension pneumothorax complicating ERCP in Korea. We experienced a case of tension pneumothorax with complicating duodenal microperforation following ERCP, and the patient (a 77 year old female with suspicious dysfunction of the sphincter of Oddi) was treated with conservative treatment.

8.The Analysis of Preference for Three PDE-5 Inhibitors.

PURPOSE: To evaluate the preference factors for the treatment of erectile dysfunction (ED) with three different kinds of PDE-5 inhibitors. MATERIALS AND METHODS: This prospective, open-label study recruited 140 patients from 5 medical centers and urological clinics in the Daegu and Gyeongbuk area of South Korea. All patients underwent sildenafil, tadalafil, and vardenafil therapy with at least four attempts at sexual intercourse for each medication. There was a 1-week drug washout period following each medication period. Patients were asked to state their preference among the three medications. RESULTS: The mean age of the 140 patients was 50.9 years. Most patients had mild-to-moderate ED(57 patients, 40.7%) and moderate ED(47 patients, 33.6%), the remaining patients had mild(23 patients, 16.4%) or severe ED (13 patients, 9.3%). Of 140 evaluated patients, 58(41.3%) patients preferred treatment with tadalafil, compared with 39(28.1%) with sildenafil and 37(26.6%) with vardenafil. When grouped by age, tadalafil was most preferred by men in their 30s and 40s(42.9%, 57.5%), sildenafil was preferred by those in their 50s(48.3%), and vardenafil was preferred by those in their 60s(51.6%). The reasons for specific preferences were prolonged erectile function (68.0%) and good erectile function(66.0%) for tadalafil, good erectile function (88.2%) for the sildenafil, and good rigidity of the erect penis(59.4%) for vardenafil. CONCLUSIONS: Tadalafil was most preferred among the phosphodiesterase inhibitors, but the preference rates varied for men of different ages. All the medications were well tolerated.

10.A Case of Fetal Congenital Mesoblastic Nephroma with Oligohydramnios.

Although congenital renal tumors are rare, congenital mesoblastic nephroma (CMN) is the most common renal tumor in early infancy. It is non-metastatic, well differentiated, amenable to surgical removal, and carries a good prognosis. Polyhydramnios has been detected in most of the published cases of CMN. However, we experienced a rare case of fetal CMN associated with oligohydramnios. A 28-yr old woman at 34 weeks of gestation was referred to our hospital for oligohydramnios and a fetal abdominal mass. An ultrasonography revealed a huge, well-encapsulated mass arising from the right kidney. An emergency cesarean section was performed due to fetal distress. After birth, despite intensive neonatal care, the baby died because of renal failure, disseminated intravascular coagulopathy, pulmonary edema, together with other problems.
