1.Echocardiographic Studies on Mitral Valvular Heart Disease.

Echocardiography was done with Ekoline 20 model of Smith Kline Instrument in 91 patients of mitral valvular heart disease, 46 with mitral stenosis, 10 with mitral insufficiency, 38 with mitral stenoinsufficinency and 115 normal controls. The results obtained are as follows. 1. Right ventricular dimension was 13.61+/-5.61 mm/sq m. in normal controls, 16.50+/-5.06mm/sq m. in mitral stenosis, 20.17+/-3.28mm/sq m. in mitral insufficiency, 18.61+/-8.48mm/sq m. in mitral stenoinsufficiency revealing significant increment in all mitral valvular disease groups but there was no significant difference btw. each disease groups. 2. Left ventricular end-diastolic dimensions was 29.83+/-7.06mm/sq m. in normal controls, 41.57+/-7.77mm/sq m. in mitral insufficinency, 38.48+/-10.22mm/sq m. in mitral stenoinsufficiency revealing significant increment but there was no significant difference btw. mitral stenosis and normal controls. 3. Left atrial dimension was 17.57+/-4.96mm/sq m. in normal controls, 33.22+/-5.32mm/sq m. in mitral stenosis, 35.96+/-7.35mm/sq m. in mitral insufficiency, 35.00+/-8.22 in mitral stenoinsufficiency revealing significnat increment in whole mitral valvular disease. 4. E-F slope was 90.76+/-30.20mm/sec in normal controls 18.08+/-10.3mm/sec in mitral stenosis, 29.52+/-19.3mm/sec in mitral stenoinsufficiency revealing marked decrement. In mitral insufficiency, it was 65.3+/-45.4mm/sec revealing significant decrement but not so severe as previous. 5. There was no significant difference between normal controls and mitral valvular disease in valve excursion. 6. Left atrial dimension to aortic root size was 1.13+/-0.31 in normal controls, 1.77+/-0.63 in mitral stenosis, 1.56+/-0.60 in mitral regurgitation and 1.82+/-0.56 in mitral stenoinsufficinecy revealing significant increment in whole mitral valvular heart disease.

2.The Reconstruction with Extracorporeal Irradiated Autograft for Osteosarcoma of Extremities.

PURPOSE: With advances in various treatment modalities, limb salvage surgery has been commonly used in osteosarcoma of extremities. An alternative method for skeletal reconstruction is reimplantation of the tumor bearing bone following extracorporeal irradiation (ECI). We report the long-term results of ECI autograft in aspect of the oncological and functional outcomes, and complications. MATERIALS AND METHODS: We retrospectively reviewed 31 osteosarcoma patients who underwent reconstruction with ECI between July 1995 and January 2006. There were 24 males and 7 females with a mean age of 24 (7-74 years) and a mean follow-up of 117 months (17-177 months). Twenty-five cases were reconstructed with ECI autograft, 6 cases with ECI autograft-prosthesis composite. The pathologic subtypes were conventional in 29 cases, periosteal in 1 case, and parosteal in 1 case. The most common location of tumor was distal femur (15 cases) followed by humerus (3), proximal fibula (3) and proximal tibia (3). Musculoskeletal Tumor Society (MSTS) score was used for functional evaluation. The overall survival rate, local recurrence, complications were analyzed. RESULTS: The overall survival rate was 80.6% and the disease-free survival rate was 64.5%. Five patients died of distant metastasis. One patient required above-knee amputation due to local recurrence. All of them, twenty-three complications occurred, which included nonunion in 7 cases, deep infection in 5 cases, joint instability in 4 cases, metal failure in 2 cases, Limb-length discrepancy (LLD) in 2 cases, periprosthetic fracture in 1 case, epiphyseal collapse in 1, local recurrence in 1 case. The mean MSTS functional score was 62.5%. CONCLUSION: Extracorporeal irradiated autograft can be achieved relatively good result in aspect of oncological and functional aspect, but is needed to be additional research about occurring many complications. The reconstruction with ECI after intercalary or fragmentary resection is effective reconstruction in aspect of oncological and functional result, complications.

3.Interrelation of Neuropsychiatric Symptom and Neurobehavioral Test among Workers Exposed to Organic Solvents.

A cross-sectional study of 118 car painters and controls of 113 workers was performed to investigate the interrelation between self-reported neuropsychiatric symptom and neurobehavioral performance using Scandinavian Questionnaire 16, Zung anxiety, depression scale and Swedish performance evaluation system (simple reaction time, symbol digit, digit span, finger tapping speed). The typical symptoms characteristic of painter's syndrome were not found and no significant association was found between solvent exposure and !total score of neuropsychiatric symptom but neurobehavioral performance of simple reaction time, symbol digit and digit span had association with solvent exposure. No significant associations were found between total score of neurotoxic symptom, anxiety scale, depression scale and the neurobehavioral performance. In factor analysis, the factor of questionnaire 16 measuring function of memory demonstrated no significant association with the factor of neurobehavioral test items measuring short term memory.

4.Retinal Detachment Following Scleral Fixation of Posterior Chamber Intraocular Lens.

The authors reviewed retrospectively retinal detachment(RD)s following posterior chamber intraocular lens(PC-IOL) implantation by transscleral suture fixation. One hundred twenty-two transscleral fixations of PC-IOL were performed from January, 1990 to December, 1992. The follow-up periods for this series ranged from 12 to 42 months. Six retinal detachments developed in this series. and the incidence of RD was 4.9%. There was no statistical difference between the primary operation group(unplanned posterior capsule rupture during extracapsular cataract extraction, lens dislocation/subluxation) and the secondary operation group(already aphakia after previous cataract surgery). The predisposing factors to RD were myopic eye(p=0.03), postoperative vitreous hemorrhage(p=0.001). In cases with RD, the locations of transscleral fixation sutures almost coincided with the meridians of the retinal breaks. The contributing factors to increased incidence of RD were thought to be vitreous loss during operations, vitreous traction by suturing needle or haptics of IOL, proliferative change by vitreous hemorrhage.

5.Postoperative Severe Hemorrhage Due to Disseminated Intravascular Coagulation: A case report.

Disseminated intravascular coagulation (DIC) is a pathological syndrome in which activation of coagulation cascade leads to fibrin clot formation, consumption of platelets and coagulation factors, and secondary fibrinolysis. We report a case of severe postoperative hemorrhagic diathesis due to DIC. A 59-year-old man was scheduled for reduction of tibia fracture and anatrophic nephrolithotomy of staghorn calculi. On the fifth postoperative day, second operation was performed for nephrectomy due to perirenal hematoma. Two days later, third operation was performed for hemostasis because of the continuous bleeding. Coagulation tests showed positive DIC profiles of thrombocytopenia, hypofibrinogenemia, increased fibrin degradation products, and prolonged prothrombin time and thrombin time. The patient recovered uneventfully and discharged on the 59th postoperative day.

8.Lead Concentrations of Pigeon's Tissue as Indicator of Lead pollution in Air and Soil.

It has been studied that a variety of fauna and flora are sensitive biological indicators which reflect the severity of regional pollution of heavy metals, but in the center of part of Taegu city the controversial issue of lead poisoning attributable to the atmosphere which contains an increased concentrations of lead has been raised recently, it is usually hard to find suitable plants or animal in the areas with heavy traffic. Pigeons are ubiquitous in and around Taegu city area, inhabiting even the most densely populated areas with heavy traffic with its small body size, high metabolic turnover, and rather limited mobility, a pigeon, as a biological indicator is expected. This study was conducted to monitor lead pollution in the Taegu and kyongju city in Korea. We measured the lead content of the various tissue of three groups of feral pigeon(Columbia livia) and soil and atmospheric lead concentration. First group was obtained io heavy traffic area in Taegu City, the second group was obtained a park in Taegu city and the third group was obtained light traffic area in kyongju city. The air and soil lead concentration of heavy traffic area in Taegu city was 0.11 microgram/m2, 4.96 microgram/g, that of park in Taegu city was 0.05 microgram/m3, 2.65 microgram/g and that of light traffic area in kyonngju city was 0.03 microgram/m3, 0.01 microgram/g. The lead content of lung, blood, kidney, femur and liver of feral pigeons in heavy traffic area in Taegu city was significantly higher than pigeons obtained in a park in Taegu city and low traffic density area in Kyonfju city(p<0.01). But stomach lead content of three group did not reflect a significant difference. In this study positive correlation was found between atmospheric lead concentrations and the concentration of lead in the pigeon's lung(r=0.5040, p<0.001), blood(r=0.3322, p<0.01), kidney(r=0.4824, p<0.001), femur(r=0.7214, p<0.001) and liverer (r=0.4836, p<0.01). we can also found positive correlation between soil lead concentrations and the concentration of lead in the pigeon's femur(r=0.4850, p<0.001), kidney(r=0.4850, p<0.001) and liver(r=0.4386, p<0.01). In the pigeon`s tissue there were significant correlations between concentration of lead in the blood and kidney(r=0.4818, p<0.001), femur(r= 0.6157, p<0.001) and liver(r=0.3889, p<0.001). In conclusion, at the heavy traffic area in Taegu city, lead concentrations found in the atmosphere and soil are reflected in the lead concentrations of different tissue of urban pigeons. It is suggested that the tissue of pigeons can be good biological indicators of environmental lead pollution.

9.Evaluation of the Mental Health Status of the Aged by Zung's Self-rating Anxiety Scale.

The authors studied on anxiety, using Zung's self-rating anxiety scale (SAS), on the subjects of 329 men and 522 women of 60 years old and older. This study was carried out for 8 months from November 1983 to June 1984 in the area of Seoul, Taegu and Kyungsan Gun, Kungpook province. The results showed that there was no significant difference in the means of total anxiety scores between the two groups of men and women. The anxiety scores in items of sweating, apprehension, dyspnea, restlessness and insomnia were relatively high in both groups. The scores of faintness, panic, fear, tremor and facial flushing were low in both groups. Among 329 men, 48 (14.6%) showed rather serious anxiety level of score 50 or higher, while 153 of 522 women (29.4%) showed the same score. Among these psychosocial factors, supporter, living, place, religion and security system are significantly related with anxiety scores in both groups (p<0.01).

10.Trabeculectomy with Mitomycin C in Congenital Glaucoma.

We assessed the efficacy of trabeculectomy with mitomycin C in the management of congenital glaucoma of which prognosis is known to be poor. Twelve consecutive eyes of 7 primary congenital glaucoma patients who underwent trabeculectomy with mitomycin C were retrospectively reviewed. The mean age was 3.2 years(ranging from 1 to 10 years) and the mean follow-up period was 31.6 months (ranging from 15 to 42 months). The mean postoperatie intraocular pressure (IOP) at the end of follow-up period was 11.0 +/-3.0mmHg, significantly lower than the mean preoperative IOP which was 25.4 +/-6.3mmHg (P<0.01). The overall success rate was 50% after a single procedure. All the 6 failed eyes underwent the same operation again and the overall success rate was 83%. The complications included hypotony in 4 eyes (33.3%) and shallow anterior chamber in 2 eyes (16.6%), and they were all resolved spontaneously. There was no evidence of hypotonic maculopathy and endophthalmitis. Trabeculectomy with mitomycin C successfully lowered the IOP in congenital glaucoma patients without serious complication and further investigation will be required to determine ling-term effects in terms of successful filtration and possible late complications.
