1.The value of high-field-strength MRCP in diagnosing intraductal papillary mucinous neoplasms of pancreas

Objective To explore the value of high-field-strength MRCP in diagnosing of intraductal papillary mucinous neoplasms (IPMNs)of pancreas.Methods Fifteen cases with IPMNs confirmed surgically and pathologically were recruited.The magnetic res-onance imaging (MRI)and MRCP findings were retrospectively analyzed.Results All cases were benign,including main pancreatic duct type (MPD)in 1,branch pancreatic duct type(BPD)in 8,and the combined type in 6.The MPD type showed diffuse dilation of main pancreatic duct which is about 14.5 mm with no enhancement.The BPD type displayed unilocular or multilocular cystic le-sions which showed communication with main pancreatic duct .The combined type demonstrated dilation of main pancreatic duct and branch pancreatic duct.The septa and mural nodule in the BPD type and the combined type were moderately enhanced.Conclusion MRCP is superior with regard to the diagnosis and classification of IPMN type and is helpful to differentiate malignant tumors from benign ones combined with routine contrast-enhanced.
2.MRI features for stages T1N0M0 and T2N0M0 peripheral cholangiocarcinoma of the liver

Objective To study the MR imaging appearances in a series of patients with stages T1N0M0 and T2N0M0 peripheral cholangiocarcinoma of the liver and to differentiate these tumors from hepatic metastases and from other solitary necrotic nodules.Methods A retrospective analysis was carried out on MR examinations of 26 patients with histologically confirmed choangiocarcinoma.The imaging findings were compared with those of 2 non-choangiocarcinoma groups including 23 patients with metastasis and 29 patients with solitary necrotic nodules confirmed with surgery,biopsy or follow-up imagings.Statistical analysis included Chi-square test or Fisher's exact test and the use of receiver operating characteristic (ROC) curves.Results Peripheral choangiocarcinomas in stages T1N0M0 and T2N0M0 were less than or equal to 3 cm in diameter (10 patients,≤2 cm and 16 patients,2 cm).Common findings for cholangiocarcinoma were defined margins (n =22,85%),slightly high signal on T2W1 (n =23,88%),patchy enhancement (n =20,77%),“fill-in” enhancement from periphery toward the center (n =21,81%) and diffuse hypointensity at the periphery and center of tumor (n =9,35%).There were significant differences (P < 0.05) between the choangiocarcinoma group and the non-cholangiocarcinoma group in the above imaging findings.The area under the curve (Az) for differentiating peripheral cholangiocarcinoma was 0.896 ± 0.037,sensitivity was 84.6%,and specificity was 86.5%.Conclusions Our data indicated that MRI features of peripheral cholangiocarcinoma in stages T1N0M0 and T2N0M0 could be determined.MR imaging,when combined with clinical and biochemical data,provided reliable information to diagnose peripheral cholangiocarcinoma.
3.Fruit intake and gastric cancer: a Meta-analysis study

Objective To assess the relationship between fruit intake and gastric cancer.Methods Articles published during January 2001 and October 2012 that assessed the relationship between fruit intake and gastric cancer were searched on PubMed,Ovid and Willey database.Adjusted relative risk (RR) and 95% confidence intervals (CI) were calculated by using fixed-effect or random-effect model.Variants of subgroup analysis included living regions,followup duration,and adjusted factors.Results A total of 3679 gastric cancer patients and 1 173 859 subjects from 7 prospective cohorts were included in this metaanalysis,and the pooled RR was 0.89 (95% CI:0.78-1.01).In the subgroup analysis,the pooled RR in the 10-year followup group was 0.94 (95% CI:0.85-0.99) before age,cigarette smoking and alcohol drinking were adjusted (RR =0.81,95% CI:0.54-0.99).Conclusion Fruit intake may prevent the development of gastric cancer.
4.Associations of sedentary behavior and physical activity with dyslipidemia

Objective:To analyze associations of sedentary behavior and physical activity with dyslipidemia among residents in Wuhai city.Methods: Data about social demographic characteristics, life style, health status and other covariate required for analysis in this study was obtained from a cross-sectional study on a total of 11 497 18-79 years old residents in Wuhai City by questionnaire, body mea-surement and laboratory examination.In this study, sedentary behavior and physical activity were evaluated using international physical activity questionnaire long version (IPAQ).IPAQ is widely used all over the world, and its reliability and validity have been tested in Chinese population.2016 Chinese Guideline for the Management of Dyslipidemia in Adults was used to define dyslipidemia in this study.Results: According to IPAQ scoring protocol, 124 participants were excluded as a result of reporting more than 960 min of physical activity per day.50.58% of 11 373 participants included in the analysis reported more than 4 hours of sedentary behavior per day in this study, thus 49.42% participants reported no more than 4 hours of sedentary behavior per day;the proportions of these 11 373 participants who reached Low level physical activity, Moderate level physical activity and high level physical activity were 23.43%, 37.29% and 39.28% respectively;and the detection ratios of new cases and prevalent cases of dyslipidemia in Wuhai City were 20.46% and 16.13% respectively.After controlling for confounders in this study, we found out that sedentary behavior increased the risk of new cases of dyslipidemia in women (OR=1.17, 95% CI: 1.00-1.36), and increased the risk of prevalent cases of dyslipidemia in both men (OR=1.21, 95% CI: 1.02-1.44) and women (OR=1.24, 95% CI: 1.04-1.48);as for association of physical activity with dyslipidemia, association was found between high level physical activity and prevalent cases of dyslipidemia in men in this study (OR=0.78, 95% CI: 0.62-0.98), suggested that high level physical activity may help to reduce the risk of prevalent cases of dyslipidemia in men.Conclusion: Our results from this cross-sectional study in Wuhai City suggested that sedentary behavior increased the risk of dyslipidemia;by contrast, physical activity may help to reduce the risk of dyslipidemia.

METHODSWe retrospectively assessed solitary hypovascular hepatic nodules ≤ 3 cm in 135 patients, among them there were 55 malignant nodules [29 peripheral nodules of cholangiocarcinoma, PCC, and 26 hepatic metastases, HM] and 80 benign nodules [48 inflammatory myofibroblastic tumors, IMT, and 32 hepatic hemangioma, HG], proved by surgery, biopsy or follow-up imaging. Unenhanced and dynamic enhanced MRI findings of the 135 patients were analyzed retrospectively. Statistical analysis included Chi-square test or Fisher's exact test, and receiver operating characteristic (ROC) curve.
RESULTSThere was significant difference (P < 0.05) between the malignant group and benign group in terms of location, margin, T2WI signal intensity, heterogeneity or homogeneity of the nodule, and type and degree of peritumoral and intratumoral enhancement. Area under the curve at the first film reading by three radiologists was 0.678 ± 0.047, 0.920 ± 0.022 at the second time, and there was a significant difference (Z = 5.22, P < 0.05) between them.
CONCLUSIONSOur data indicated that solitary hypovascular hepatic nodules show unenhanced and dynamic enhanced MRI features. Therefore, MR imaging combined with clinical and biochemical data does provide reliable information for a proper diagnosis of such hepatic lesions and differentiation of malignant from benign nodules.

8.The application of quantitative immune fecal occult blood test for the screening of colorectal cancer in health check?up participants

Objective To evaluate the application value of quantitative immune fecal occult blood test (FOBT) in colonoscopy for the screening of colorectal cancer in health check-up participants. Methods The subjects were selected from July 2017 to June 2018 in the Health Management Center of the Second Affiliated Hospital of Suzhou University. The subjects were the healthy individuals who chose quantitative immune FOBT or chemical method plus immunogold double-method FOBT (referred to as"double-method FOBT"), excluding those who had interfering factors. Individuals with a positive result in primary screening were selected and conducted with colorectal cancer by colonoscopy. If the polyploidy lesions were observed during colonoscopy, the biopsy or excision was performed, and the pathological diagnosis was performed. The positive rate of primary screening, compliance rate of colonoscopy and pathological results of colonoscopy were compared between the two methods. Quantitative immunoassay FOBT was analyzed in different gender, age group, physical examination nature, positive rate of primary screening, compliance rate of colonoscopy and pathological results of colonoscopy. Results 18 728 people chose quantitative immunoassay FOBT and 6 212 people chose double-method FOBT at the same time. There was no significant difference in gender and age between the two groups (all P>0.05), which was comparable. The detection rate of quantitative immune FOBT was higher than double-method FOBT (74.62% vs 32.23%, P<0.001). The positive rate of quantitative immune FOBT in primary screening was lower than double-method FOBT (4.11% vs 5.34%, P=0.003). The colonoscopy screening rate in positive population by quantitative immune FOBT was higher than double-method FOBT (27.83% vs 13.08%, P=0.001). These differences were statistically significant. The detection rate of total lesions by colonoscopy was 71.88% in positive population by quantitative immune FOBT. It was 42.86% in double-method FOBT. There was no statistical difference between the two methods (P=0.05). The detection rates of quantitative immune FOBT were significantly different among different genders, ages and physical properties (all P<0.001). The detection rate was higher in males than in females (79.14% vs 68.75%). The detection rate was highest in the group between 40 and 59 years old (79.96%). The individual detection rate was higher than the group (90.08% vs 66.07%). The positive rates in primary screening were significantly different among different ages (P=0.001).It was highest in the group aged 60 or above (5.59%). The colonoscopy screening rate in positive population by quantitative immune FOBT was highest in the group aged 50 or above (36.96%). The detection rate of inflammatory lesions were significantly different among different ages (P<0.001). The detection rate of colorectal cancer in males was higher than in females (11.11% vs 0.00%, P=0.009). In addition, with the increasing of fecal occult blood value, the detection rate of cancer was increased (P=0.041). Conclusion The quantitative immune FOBT is an ideal non-invasive examination for early screening of colorectal cancer. It has important application values.

Purpose/Significance To construct a specialized database for inflammatory demyelinating disease of the central nervous system(CNS),so as to contribute to clinical research and improve the diagnostic and treatment capabilities of primary healthcare institu-tions.Method/Process Using the internet to collect medical data,after processing and analysis,the CNS inflammatory demyelinating disease database is constructed.Using statistical analysis,natural language processing(NLP),artificial intelligence(AI)image recog-nition and data visualization and other technologies,the database information is integrated and analyzed.Result/Conclusion A standard-ized big database for CNS inflammatory demyelinating diseases is constructed,which enables visualization of clinical research data,pro-vides patient education and specialist training,and facilitates multi-center teleconsultations.The establishment of a specialized database for the CNS inflammatory demyelinating disease can promote the transformation of medical research achievements,provide references for future real-world clinical research,optimize the process of diagnosis and treatment,and improve the clinical capability of primary healthcare institutions.
10.Effective Components and Antiarrhythmic Mechanisms of Wenxin Granules Based on CMC/UPLC-Q-TOF/MS

ObjectiveTo employ the effective components and antiarrhythmic mechanism of Wenxin Granules (WXKL) by cell membrane chromatography (CMC) and ultra-performance liquid chromatography-quadrupole time-of-flight mass spectrometry (UPLC-Q-TOF/MS), combined with network pharmacology. MethodIn this study, the CMC/UPLC-Q-TOF/MS technique was employed to identify the components in WXKL that could specifically bind to myocardial cell membranes. By utilizing databases such as SwissTarget Prediction and GeneCards, the targets of WXKL's effective components and arrhythmia-related targets were mined. Cytoscape software was used to construct a "component-target-disease" network. Gene ontology(GO) function and Kyoto encyclopedia of genes and genomes(KEGG) pathway enrichment analyses were carried out, and molecular docking of key components and targets was performed. Finally, further verification was conducted through in vivo experiment of rats. ResultA total of 39 effective components were identified in WXKL. These included 13 components derived from Panax notoginseng, 15 components from Codonopsis pilosula, seven components from Glycyrrhizae Radix et Rhizoma, one component from Succinum, one component from Polygonatum odoratum, one component shared by both P. odoratum and C. pilosula, and one component shared by both Panax notoginseng and C. pilosula. Network pharmacology predicted that WXKL had 16 core antiarrhythmic targets and 79 related pathways, mainly involving adrenergic signaling in cardiomyocytes, cyclic guanosine monophosphate (cGMP)/protein kinase G (PKG), calcium signal, cyclic adenosine monophosphate (cAMP), interleukin (IL)-17, mitogen-activated protein kinase (MAPK), and tumor necrosis factor (TNF) signaling pathways. The results of in vivo experiment of rats showed that WXKL significantly improved the expression of β1-adrenergic receptor (β1-AR), cAMP, TNF-α, and calcium voltage-gated channel subunit alpha 1C (CACNA1C). ConclusionWXKL can exert its antiarrhythmic effects through multiple components, multiple targets, and multiple pathways. This study provides a scientific basis for explaining the potential pharmacodynamic substance foundation and mechanism of action of traditional Chinese medicine in treating arrhythmia.
