1.Effects of family and telephone follow-up on quality of life of patients with total hip arthroplasty

Objective To explore the effects of family and telephone follow-up on quality of life of the patients with tota1 hip arthroplasty(THA).Methods One hundred and twelve patients with THA were randomly divided into experiment group and control group with 56 cases in each group.The experiment group was treated with rehabilitative instruction by family and telephone follow-up.The control group was treated with outpatient follow-up after leaving the hospital.The living quality in both groups were compared by the medical outcomes study 36-item short-form health survey(SF-36 QOL)one year after leaving the hospital.Results One-year after leaving the hospital,the total score and demension score of experiment group were better than those of the control group.There was significant difference between the control group and in experiment group(P<0.05).Conclusion Family and telephone follow-up is an effective implementation for THA patients to accomplish physiological functional recovery and promote the quality of life.
2.Application of case-based learning in ophthalmology course and its assessment

Objective To assess the effect of case-based learning (CBL) in ophthalmology teaching.Methods Lecture based learning combined with CBL was applied in ophthalmology teaching for 19 undergraduates majored in clinical medicine of 2007 grade from Shanghai Jiao Tong University School of Medicine.Questionnaire survey was conducted for students after the course ended to understand their evaluation on CBL.Proportion of selected data was analyzed comparatively.Results 100% (19/19) students believed that CBL was essential in ophthalmology teaching and rational at the end of course.84.2% (16/19) students accounted that CBL could only be assistant teaching method.94.7% (18/19) and 84.2% (16/19) students considered that CBL was beneficial for cognition of eye diseases completely and clinical thinking formation.4-6 class hours for CBL were approved by 73.7% (14/19) students.Conclusions CBL is essential in ophthalmology teaching and is accepted and supported by students,therefore it should be the main assistant teaching method.
3.The clinical value of prenatal ultrasonography combined with maternal serology screening for chromosomal abnormality in 15 to 20+6 gestational weeks

ObjectiveTo evaluate the clinical value of prenatal ultrasonography combined with maternal serology screening for chromosomal abnormality in 15 to 20+6 gestational weeks.MethodsSix hundred and twenty-eight pregnant women (628 fetuses) in 15 to 20+6 gestational weeks were selected to undergo prenatal ultrasonography, who were in critical risk of trisomy 21 or trisomy 18 by maternal serology screening. Transabdominal ultrasonography were performed and fetal nasal bone and nuchal fold were detected. Those who had nasal bone hypoplasia, thickened nuchal fold (NF>6 mm) and other abnormal fetal soft markers underwent amniocentesis for karyotyping analysis. ResultsThere were 6 cases of nasal bone hypoplasia (0.96%, 6/628), including one case of thickened nuchal fold, two cases of echogenic bowel, two cases of choroid plexus cysts and 1 case of echogenic cardiac focus. All these 6 cases underwent amniocentesis and 2 were trisomy 21 (33.3%, 2/6). The other 4 cases had no significant chromosomal abnormality.ConclusionsPrenatal ultrasonography may improve the detection rate of chromosomal abnormality for those pregnant women who are in critical risk of chromosomal abnormality prompted by serology screening. But invasive procedures are still needed to verify the chromosomal abnormality.
4.Relationship between endothelial nitric oxide synthase Glu298Asp gene polymorphisms and the chronic periodontitis with type 2 diabetes mellitus.

OBJECTIVETo study the relationship between endothelial nitric oxide synthase (eNOS) Glu298Asp gene polymorphism and chronic periodontitis (CP) with type 2 diabetes mellitus (T2DM).
METHODSDNA from patients' buccal swabs of CP, CP with T2DM, T2DM and health was isolated and extracted. The eNOS Glu298Asp gene polymorphism were assessed by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) respectively.
RESULTSThe distribution of eNOS Glu298Asp genotype in CP group, T2DM group, CP with T2DM group and health group showed statistical differences (chi2 = 18.503, P = 0.005), and the gene frequency showed statistical differences (chi2 = 8.243, P = 0.041). Compared health group to CP with T2DM group, OR value of the genetype T was 0.962, 95% confidence interval lay in between 0.737 to 1.256, which showed T maybe a protective factor. While OR value of the genetype G was 1.043, 95% confidence interval lay in between 0.781 to 1.391, which showed G maybe a risk factor. However, neither T nor G was statistically significant.
CONCLUSIONBased on these findings, there are some association between eNOS Glu298Asp polymorphism and the risk of CP group, T2DM group, CP with T2DM group.

5.Discriminator value of Han Chinese albumin creatinine ratio for detecting microalbuminuria

Objective To investigate the discriminator value of Han Chinese first morning urine albumin creatinine ratio (ACR) for determining the microalbuminuria. Methods A total of 1056 participants (494 males and 562 females) were selected from epidemiologic study of the metabolic syndrome and chronic kidney disease in Pinggu district, Beijing. Eight-hour overnight urinary albumin excretion (UAE) was regarded as the gold standard for defining the albuminuria,and the ROC curve analysis was used to determine the ACR discriminator value for microalbuminuria. Results (1)Microalbuminuria was found in 12.5% of participants,macroalbuminuria in 1.7%. (2)The ACR discriminator value for microalbuminuria by ROC curve analysis was 1.95 g/mol (sensitivity 97.6% and specitivity 88.6%) for men, 3.62 g/mol(sensitivity 83.8% and specitivity 89.1%) for women, 2.78 g/mol (sensitivity 88.7% and specitivity 85.9%)for overall. The upper boundary of microalbuminuria by ROC curve analysis was 22.59 g/mol (sensitivity 100.0% and specitivity 98.8%) . (3)The inter-rater agreement of the result in this study showed that sensitivity was 91.3% and specitivity was 88.2%, positive likelihood ratio was 7.56 and negative likelihood ratio was 0.10, positive predictive value was 56.9% and negative predictive value was 98.4%. Conclusions The ACR discriminator value for determining microalbuminuria is obviously higher in women than that in men, and is higher than recommendation of international guidelines. The result by ROC curve analysis has better sensitivity and specitivity.
6.The relationship between urinary albumin excretion and serum uric acid in general population

Objective To investigate the relationship between the urinary albumin excretion (UAE) and serum uric acid in general population. Methods The study participants were derived from the epidemiological study on the association of metabolic syndrome and chronic kidney disease (CKD) in Pinggu district, Beijing. A total of 992 participants (463 men and 529 women) aged from 30 to 75 years were enrolled in this study. For each participant, UAE, serum uric acid, serum creatinine, and serum lipids were detected and other potential risk factors for CKD were surveyed. Results ( 1 ) The frequencies of microalbuminuria, macroalbuminuria and hyperuricemia were 12.9% , 1.8% and 4.3% respectively. The persons with hyperuricemia had significantly higher frequency of albuminuria than those without hyperuricemia (37. 2% vs 13. 7% , P <0. 01). (2) The participants were divided according to the quartiles (25% , 50% , 75% ) of serum uric acid level, and the frequencies of albuminuria in males were 13. 2% , 13. 9% , 17. 2% and 25.4% , while those in females were 8. 4% , 6. 2% , 9. 6% and 24. 8%. ( 3 ) Multivariate logistic regression analysis showed, hyperuricemia was significantly associated with albuminuria in females (OR =2. 31, 95% CI 1. 15-4. 68; P=0.02), but not in males. If the persons with reduced renal function were excluded, similar result still could be gained. Conclusions The prevalence of albuminuria increases gradually with uric acid elevation. Serum uric acid is an independent risk factor of elevated UAE, especially in females.
7.The effect of the modified root canal preparation method with different liquid continuous flushing on the anti-fracture properties of mechanical nickel-titanium file

Objective: To study the effect of the modified preparation method with root canal flushing of different solutions on the anti-fracture properties of mechanical nickel-titanium filings. Methods: A transparent resin model of root canal was established. ProTaper Universal (PTU F1) instrument was used to prepare the root canals. The number of prepared root canals by each PTU F1 with the various solutions(n = 20) was recorded and compared among groups. Results: 6. 13 ± 3. 52 root canals were prepared in distilled water group, 6. 25 ± 1. 76 in 0. 9%saline group, 6. 27 ± 2. 07 of 0. 2% chlorhexidine group, 6. 88 ± 3. 21 in 1% sodium hypochlorite group, 4. 31 ± 2. 34 in 5% sodium hypochlorite group and 3. 26 ± 2. 08 in dry drilling group. The number between each 2 of distilled water, 0. 9%saline group, 0. 2%chlorhexidine group and 1% sodium hypochlorite group, and the number between 5% sodium hypochlorite group and dry drilling group was not statistically significant(P> 0. 05). The number of prepared root canals in the first 4 groups was more than that in the latter 2 groups(P< 0. 05). Conclusion: Distilled water, 0. 9% saline, 0. 2% chlorhexidine or 1% sodium hypochlorite for root canal irrigation can improve mechanical nickel-titanium instrument for fatigue resistance in root canal preparation, but 5% sodium hypochlorite and dry drilling can not.
8.The effectiveness of different training modes of six-character qigong in improving respiratory muscle functioning after a stroke

Objective:To explore the impact of different six-character qigong training modes on respiratory muscle function early after a stroke.Methods:Sixty-six stroke survivors in the early stage of recovery were randomly divided into a control group, a modified training group, and an ancient training group, each of 22. In addition to routine rehabilitation training, the control group received conventional respiratory training. The modified training and ancient training groups were trained in modified six-character qigong or ancient six-character qigong, respectively, for two weeks. Before the treatment, after the two weeks and one month later, maximum inspiratory pressure (MIP) and maximum expiratory pressure (MEP), forced expiratory volume in one second, forced vital capacity, peak expiratory flow, maximum mid-expiratory flow, and peak inspiratory flow were measured. Diaphragm mobility during quiet inspiration and maximum inspiration were also quantified.Results:After 2 weeks of treatment and at 1 and 3 months after the end of the treatment, all three groups showed significant improvement in MIP, MEP and the pulmonary ventilation indicators, but the average improvement in the modified training group was significantly greater than in the other two groups. Their average diaphragm mobility was also significantly greater.Conclusion:Modified six-character qigong respiratory training is more effective than its ancient counterpart in improving respiratory muscle function, pulmonary ventilation, and diaphragm mobility early after a stroke, with effects which persist for at least one month.
9.The value of CT based radiomics in predicting progression of early acute pancreatitis

Objection To investigate the value of CT based radiomics in predicting progression of early acute pancreatitis (AP). Methods:From November 2013 to February 2021, 109 patients diagnosed with AP according to the new revised Atlanta classification in Yijishan Hospital of Wannan Medical College were retrospectively analyzed. The patients were divided into progressive group (40 cases) and non-progressive group (69 cases) according to the follow-up results. All patients underwent plain and enhanced abdominal CT scan within a week of onset. The patients were divided into training set (77 cases, including 28 cases in progressive group and 49 cases in non-progressive group) and validation set (32 cases, including 12 cases in progressive group and 20 cases in non-progressive group) in a ratio of 7∶3 using a computer completely random method. Manual region of interest mapping was performed on all levels of pancreas on the plain scan, arterial phase, venous phase and delayed phase CT images, then performed 3D fusion. AK software was used to extract texture features. The minimum redundancy maximum relevance and minimum absolute shrinkage and selection operator regression analysis were used to select features and establish radiomics labels of the plain scan, arterial phase, venous phase, delayed phase and combining the 4 sequences. The multiple logistic regression analysis was used to establish the clinical model by combining clinical features and CT features, and the comprehensive model was established by combining clinical features, CT features and imaging radiomics label. The receiver operating characteristic (ROC) curve was used to evaluate the efficacy of each model in predicting early AP progression and the decision curve analysis (DCA) was used to evaluate the clinical application value of each model.Results:In the training set, logistic regression results showed that edge was an independent predictor (OR=0.16, P=0.033). The clinical model was established using edge and serum calcium level, and its areas under the ROC curve (AUC) in the training set and validation set were 0.69 and 0.70, respectively. Totally 14, 11, 13, 13 and 9 optimal texture features were extracted from the plain scan, arterial phase, venous phase, delayed phase and combined sequence images, respectively. The delay phase image radiomics label had relatively better predictive performance in training set and validation set, and the AUC were both 0.85. The comprehensive model was established based on the delayed phase image radiomics label (OR=2.22, P<0.001) and edge (OR=0.02, P=0.042), and the AUC in the training set and validation set was 0.90 and 0.86, respectively. DCA showed that both the comprehensive model and the delayed phase radiomics label had better benefits. Conclusions:CT radiomics model has high value for predicting progression of AP, and its clinical benefits exhibited superior performance of clinical model.
10.Effect of peritoneal dialysis on glycemic variability in patients with diabetes and its clinical implications

Objective To compare glycemic profile between diabetic patients receiving peritoneal dialysis and diabetic patients with normal kidney function, and to investigate the impact of peritoneal dialysis on glycemic control through continuous glucose monitor system ( CGMS). Methods 19 diabetic patients with end-stage renal disease receiving regular peritoneal dialysis (DMPD group) and 8 patients with non-diabetic ne-phropathy receiving regular peritoneal dialysis ( PD group) were randomly selected and matched with 20 diabetic patients with normal kidney function (DM group) based on age, gender and 72 hours mean glucose. CGMS were applied on all patients for 72 hours. Glycemic variability parameters were compared among the three groups. Results Peritoneal transport function was positively correlated with mean glucose, glucose standard deviation and mean amplitude of glycemic excursion. Compared with PD group, multiple variation parameters, such as intraday glycemic standard deviation (P<0. 001), covariant efficiency (P=0. 009) and mean of daily difference (P=0. 043), were significantly lower in DMPD group. Though both DMPD and DM group exhibited profile as trough in wee hours and post-prandial hyperglycemia, DMPD had higher glycemic level in wee hours (P<0. 001). Conclusion Diabetic patients with end-stage renal disease receiving regular peritoneal dialysis have smaller glucose variability than diabetic patients with normal renal function.
