1.Progress of superficial temporal fascia application in otoplasty

Superficial temporal fascia,which has various names,such as temporal fascia,temporoparietal fascia,epicranial aponeurosis and galeal extension,has been used to define the fascial layers of the temporal region.All these different names reflect an anatomical feature of the related fascia.This region exists superficial temporal vessels,the temporal branch of the facial nerve and the auriculotemporal nerve,so is very important in otoplasty.This paper reviews the progress of superficial temporal fascia application in otoplasty.
2.Diagnostic value of combined detection of CA125,HE-4 and D-Dimer in ovarian cancer

Objective To investigate the separate and combined detection value of serum carbohydrate antigen 125 (CA125),human epididymis protein 4 (HE-4) and D dimer (D-dimer) in the diagnosis of ovarian cancer.Methods One hundred and twenty ovarian cancer patients were selected as the observation group,one hundred and twenty patients with ovarian benign tumor as the benign group,eighty women with healthy physical examination results as the control group,then the chemiluminescence immunoassay was used to detect the expression of serum CA125 and HE-4 of the three groups,and the immune turbidimetric method was applied to examine the expression of D-dimer.At last,the diagnosis efficiency of CA125,HE-4 and D-dimer in separate and combined detection was calculated.Results CA125 in the observation group was (623.07±274.18) U/ml,HE-4 was (594.22±329.068) ng/ml and D-dimer was (418.57±276.75) ng/L,CA125 in the benign group was (45.09±32.58) U/ml,HE-4 was (97.92±57.52) ng/ml and D-dimer was (204.52±80.07) ng/L;CA125 in the control group was (40.23±28.16) U/ ml,HE-4 was (85.65±37.27) ng/ml and D-dimer was (187.57±65.74) ng/L,the differences between the three groups were statistically significant (F=122.82,89.91,54.46;P<0.05).The level of CA125 in the serum of patients with advanced stage (stage III-IV) was (586.10±278.33) U/ml,HE-4 was (437.49±238.06) ng/ml,D-dimer was (493.78±274.45) ng/L,in the early stage (stage I-II),the level of CA125 in the serum of patients with ovarian cancer was (372.12±265.31) U/ml,HE-4 was (673.64±301.68) ng/ml,D-dimer was (364.84±267.54) ng/L,and the difference was statistically significant (t=4.244,4.78,2.560,P<0.05).The detection sensitivity and specificity of CA-125 were 71.67% and 62.50% respectively.The sensitivity of HE-4 was 74.17% and the specificity was 75.00%.The sensitivity of D-dimer was 62.5% and the specificity was 60.00%.The combined detection sensitivity of the three was 80.00% and the specificity was 77.5%.Conclusion The sensitivity and specificity of the three tumor markers in combined detection were higher than those of separate detections,so the combined detection of CA125,HE-4 and D-dimer can further improve the diagnosis level of ovarian cancer.
3.Clinical analysis on characteristics of rib cartilage calcification in congenital microtia patients.

OBJECTIVETo explore the incidence, degree, and pattern of rib cartilage calcification in congenital microtia patients, in order to provide reference for harvesting the rib cartilage, sculpturing cartilage framework.
METHODSFrom Jun. 2013 to Nov. 2014, 383 patients (age range, 6-45 years) underwent CT scans of the chest. 11 patients with bony diseases or traumatic history were excluded. The remaining 372 patients were divided by age into four groups as 6-15, 16-25, 26-35, 36-45 years old. Twenty patients (10 male and 10 female) were selected by the order of patient identification number in each age group, thus selecting a total of 80 patients (40 male and 40 female). Retrospective study of CT scans of the chest in 80 patients and the incidence, degree, and pattern of cartilage calcification of the sixth to eighth ribs were noted. A chi-square test is conducted to test whether there are significant difference between the variables through the SPSS 19.0 software.
RESULTSOverall, 40.4% (194/480) cartilage was calcified; female patients (47.50%, 114/240) showed higher frequency of calcification than male patients (33.33%, 80/240, P = 0. 025). Calcification rates of all age groups are 1.7% (2/120), 46.7% (56/ 120), 49.2% (59/120), 64.2% (77/120). Calcification rate of 6-15 years group is lowest in all groups (P < 0.05) while other three groups have no statistical significance (P > 0.05). Calcification rates of the sixth and sevent rib cartilage were higher than those of the eighth rib cartilage in all age groups except 6-15 years group, who had a similar rate of all three ribs. Calcification rate of all three rib cartilage was significantly increased with age. Calcification rates of the amle's rib cartilage and the female's in all age groups are 3.3% (2/60) and 0.0% (0/60) (6-15 years): 33.3% (20/60) and 60.0% (36/60) (16-25 years): 40.0% (24/60) and 58.3% (35/60) (26-35 years), 56.7% (34/60) and 71.2% (43/60) (36-45 years). In 6-15 years group calcification rates of male and female had a similar rate, while female's rates were higher than male's rates in other three groups. Male and females mainly had the granular type of calcification [70.0% (56/80), 63.2% (72/114)].
CONCLUSIONSFemales who are over 16 years old should pay more attention to the possibility of middle-severe calcification before harvesting rib cartilage. These patients should take CT examination if necessary. In addition, the patients who had previous operation, or traumatic history, rib deformity, or spine deformity should select the CT examination.

4.Expanded post-auricular skin flap combined with autologous rib cartilage framework for correction of concha-type microtia.

OBJECTIVETo explore the clinical effect of the method by using an expanded post-auricular skin flap combined with autologous rib cartilage framework for correction of concha-type microtia.
METHODSThe operation were performed in three stages. The expander was implanted under post-auricular skin at the first stage and expanded skin flap was formed. At the second stage, the expander was taken out and the expanded skin flap was transferred with autologous rib cartilage framework and skin graft for correction of microtia. At the third stage, the reconstructed ear was revised and new concha was formed.
RESULTSFrom August 2008 to August 2011, 108 cases with 113 concha-type microtia were corrected by this method. All patients healed primarily and were followed up for 6 months to 3 years. The reconstructed ears had a good appearance and position, and were symmetric to ear on the healthy sides.
CONCLUSIONSUsing expanded post-auricular skin flap combined with autologous rib cartilage framework is a reliable method for concha-type microtia.

5.The clinic outcome of CAD/CAM onlay in the restoration of molars with extensive defect after root canal treatment

Objective:To observe the clinical efficacy of CAD/CAM all-ceramic onlay in the restoration of molars with extensive defect after root canal therapy(RCT).Methods:260 molars with extensive defect were restored by all-ceramic onlay after RCT.The patients were followed up for 18 months.The restoration effects were assessed using USPHS criteria.Results:The success rate of CAD/CAM all-ceramic onlay was 98%.Restoration exfoliation was found in 3 cases,fractured in 2 cases.Conclusion:CAD/CAM all-ceramic onlay can be applied in restoring extensive postenor tooth-defection after RCT.
6.Preliminary clinical research with thoracic deformities in microtia.

OBJECTIVETo investigate the characteristics and incidence of the thoracic deformities in patients with microtia.
METHODSIn Plastic Surgery Hospital, we conducted a retrospective study of the clinical and radiographical data of 300 patients with microtia from March 2013 to October 2014. Pearson χ2 test was used to analyze the relationship among deformities of ribs and spine, as well as microtia.
RESULTSA total of 78 (26.0%) patients were documented with rib deformities, 26 patients (8.7%) had spinal deformities, and 17 patients (5.7% )had both. The incidence of rib deformities in microtia I, II, and III was 7.1% (2/28), 26.7% (62/232) and 35.0% (14/40) respectively. The incidence of spinal deformities in microtia I, II, and III was 3.6% (1/28), 6.5% (15/232) and 25.0% (10/40 respectively. The patients with microtia III were found to have a higher incidence of ribs and spinal deformities than those with microtia II, patients with microtia II were found to have a higher incidence of ribs and spinal deformities than those with microtia I (P < 0.05).
CONCLUSIONSThe incidence of ribs and spinal deformities is high in patients with microtia. The poorer one auricle developed, the higher the incidence of thoracic deformities.

7.Comparison of different surgical techniques for treatment of concealed penis

Objective To compare the effects of different surgical techniques for treatment of concealed penis. Methods A retrospective review of 219 patients underwent surgical correction of concealed penis between 1986 and 2007 was performed. The mean age was (10.3±2.4) years(3-15 years). The penile length was (1.9±0. 5)cm(0. 5-3. 0 cm)before operation. Ninty-three patients were the severe degree of concealed penis. The others were the moderate degree. All patients under-went operation with different techniques, including Johnston's technique in 34, Devine's technique in 42, modified Devinds technique in 125 and Brisson's technique in 18. The increased length of penile af-ter operation was compared among the 4 groups with different surgical techniques by statistical meth-od. Results The postoperative increased length of penile in Johnston's technique, Devine's tech-nique, modified Devine's technique and Brisson's technique was (1.8 ± 0. 4) cm, (2. 0 ± 0. 5) cm, (2.1±0.4)cm and (2.3±0.4)cm respectively. The difference was significant by ANOVA test (F=13.1,P<0. 001). Devines technique, modified DevineSs technique and Brisson's technique were better than Johnston's technique considering the increased length of penile. The complication of severe penile lymphedema of 4 groups developed in 8, 5, 6 and 2 patients respectively. Conclusion Modified De-vine's technique has the satisfactory increasing of penile length for treatment of concealed penis and less complication rate after operation.

Objective:To explore the technique of two-flap in ear reconstruction.Method:Quantitative tissue expansions were used in the mastoid area in the first stage.After the final injection,there was 1 month of sustaining time.Expanded skin flap and unexpanded fascia flap were designed in the second stage,so thetwo-flaptechnique was used in the ear reconstruction.From January 2004 to December 2008,1 427 patients of microtia were treated using two-flap technque.Result:The expanded skin flap could show the fine structures of the reconstructed ears.The reconstructed ears had vivid cranioauricular angle after using the unexpanded fascia flap.Conclusion: Two-flap method was easily manipulated and the complications were rare.The reconstructed ears had lucid and three-dimensional contour.

OBJECTIVE:
To explore the role of vrk1 gene in two Chinese pedigrees of the first and second branchial arch syndrome.
METHOD:
Sixty members in 2 Chinese pedigrees were recruited. The exon 2 -13 were analyzed by polymerase chain reaction and direct sequencing.
RESULT:
We found a new SNP in proband of Shandong pedigree.
CONCLUSION
vrk1 gene mutation can be excluded in 2 Chinese pedigrees of the first and second branchial arch syndrome.
