3.Analysis of the correlation between the stage of hepatic fibrosis and the diameter of portal vein and middle hepatic vein in patients with chronic hepatitis B

Objective To investigate the correlation between the stage of hepatic fibrosis and the diameter of portal vein (DPV) and middle hepatic vein (DMHV) in patients with chronic hepatitis B.Methods 320 patients with chronic hepatitis B were evaluated by ultrasonography.The DPV and DMHV were measured,and their correlation with the degree of hepatic fibrosis was analyzed after liver biopsy.Results DPV was positively related with the degree of hepatic fibrosis(r =0.384,P =0.032),while DMHV was negatively related with the degree of hepatic fibrosis (r =-0.521,P =0.015).The DPV and HMDV in S4 were bigger than those of S0,S1,S2 and S3,and statistical differences existed among them (DPV∶z =0.334,P =0.032 ; DMH∶z =0.161,P =0.015).The best value of S4 diagnosing by DPV was 13mm.The best value of S4 diagnosing by DMHV was 6mm.The area under the curve of ROC for DPV and DMHV in ≥S2 was 0.753 and 0.758 respectively.It in≥S3 was 0.795 and 0.802 respectively.It in ≥S4 was 0.797 and 0.828 respectively.The area under the curve of ROC for combination of DPV and HMDV was 0.866,which was higher than that of DPV and HMDV.Conclusion There is a good correlation between DPV,DMHV and the stage of hepatic fibrosis in patients with chronic hepatitis B.Combination of DPV and HMDV has clinical significance for diagnosing early hepatic fibrosis.
4.Study on the optimal suctioning negative pressure based on sputum viscosity in brain-injured patients with tracheotomy

Objective To determine the optimal suetioning negative pressure based on various sputum viscosity in brain-in-jured patients with tracheotomy. Methods Forty-three traumatic brain-injured patients with traeheotomy were recruited in the study. The negative pressure was determined based on the grade of sputum viscosity. Grade Ⅰ sputum was suetioned with the negative pressure of 13.3kPa,16.0kPa,20.0kPa and 24.0kPa, while Grade Ⅱ sputum with 20.0kPa,24.0kPa,26.TkPa and 29.3kPa,and Grade Ⅲ sputum with 26.7kPa,29.3kPa, 33.3kPa and 37.2kPa. The oxygen saturation(SpO2)was mort-itored before and lmin,3min,5min after endotracheal suctioning. At the same time,the suctioning time and tissue trauma were recorded. The data were analyzed by repeated measures analysis of variance with SPSS 13.0 software. Results The results showed that the optimal negative pressure was 13.3-16.0kPa for grade Ⅰ sputum,24.0-26.7kPa for grade Ⅱ sputum and 33.3kPa for grade Ⅲ sputum,which could ensure the minimum decrease of SpO2 and control the suctioning time within 25s or 30s respectively. Conclusion For traumatic brain-injured patients,varions negative pressure should be selected based on the sputum viscosity by the clinical nurses when suctioning is performed,in order to avoid hypoxemia and tissue trauma caused by suctioning.
5.Research on the detection rate of couples at high risk of bearing babies with severe thalassemia among the people at reproductive age in foshan city,Guangdong province

Objective To investigate the detection rate of couples at high risk of bearing babies with severe thalassemia and the occurrence rate of birth defect caused by severe thalassemia among the people at reproductive age in Foshan city ,Guangdong prov‐ince .Methods Peripheral blood samples were collected from 17 522 preparing pregnant couples of child bearing age .Mean corpus‐cular volume (MCV) <80 fL was used as the positive indication for screening thalassemia .And 348 positive samples were detected by molecular biological technique for further analysis .Statistical analysis was conducted based on the couple′s genetic test results . Results Among the people at reproductive age ,the detection rates of couples bearing babies with Hb Bart′s Hydrops Fetalis ,He‐moglobin H disease andβ‐thalassemia were 0 .36% ,0 .47% and 0 .20% respectively ,the total was 1 .03% .The occurrence rates of birth defect caused by the above severe thalassemia were 9 .0/10 thousand ,11 .8/10 thousand and 5 .0/10 thousand ,the total was 25 .8/10 thousand .Conclusion This research illustrated the degree of damage of birth defect caused by severe thalassemia among the people at reproductive age ,which could provide valuable fundamental materials for the launch of eugenic work in this region .
6.Pharmaceutical Care for One Patient with Severe Bronchial Asthma Complicated with Diabetes

Objective:To investigate the effect of clinical pharmacists participating in the drug therapy for the patients with severe bronchial asthma complicated with diabetes. Methods:Clinical pharmacists participated in the treatment process for one patient with severe bronchial asthma complicated with diabetes. Pharmacists analyzed and intervened the therapeutic regimen, performed pharma-ceutical care and provided pharmacy education. Results:The suggestions of treatment and care proposed by clinical pharmacists were a-dopted by clinics. The pharmaceutical care performed by clinical pharmacists could contribute to the disease control and the reduction of the incidence of adverse reactions. Conclusion:Clinical pharmacists can use their pharmaceutical knowledge to provide individual-ized pharmaceutical care and play an important role in improving the safety and efficacy of medication.
7.Study on changes of ratio of serum NEFA/ALB in children with primary ne-phrotic syndrome

Objective:To study on the changes of the ratio of serum NEFA/ALB in children with primary nephrotic syndrome , and to provide new indicators for the diagnosis and severity assessment of clinical PNS .Methods:The levels of serum albumin ( ALB) , free fatty acid (NEFA),urea nitrogen(BUN),uric acid(UA),creatinine(Scr),total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol ( LDL-C) ,high density lipoprotein cholesterol ( HDL-C) were quantitatively measured in 52 patients with PNS in acute stage ( acute stage group ) and 34 patients with PNS in complete remission ( remission stage group ) and 50 healthy children ( con-trol group).The lipid/lipoprotein and renal function parameters were also analyzed .Results: Compared with controls group ,the levels of serum ALB,HDL-C in acute group and remission group decreased significantly ( P<0.05 ) ,but the levels of serum NEFA/ALB,TC, TG,LDL-C,UA,BUN,Scr in acute group increased significantly ( P<0.05 ) ,the levels of serum TG ,LDL-C in acute group increased significantly (P<0.05).Compared with remission group,the levels of serum ALB in acute group decreased significantly (P<0.05), but the levels of serum NEFA/ALB,TC,TG,LDL-C,UA,BUN,Scr increased significantly (P<0.05).In PNS children,the ratio of NEFA to ALB was positively correlated with the levels of TC ,TG,LDL-C,UA,BUN (P<0.05),while negatively correlated with ALB (P<0.05).Multiple linear regression analysis showed that the ratio of NEFA to ALB had an independent association with BUN levels (β=0.045,t=1.602,P=0.003).Conclusion:In the development process of PNS disease ,serum ratio of NEFA to ALB increased in-dicates impaired kidney function or reduced kidney function ,and it can help doctor assess and judge the clinical condition .

OBJECTIVE:To evaluate the effects of minidose aspirin on blood uric acid,creatinine and urea nitrogen in hyperuricemia model mice.METHODS:75 mice were randomly assigned to receive 0.5% CMC-Na(normal control group and hyperuricemia control group)or aspirin(12.5,25,50 mg?kg-1?d-1)q.d for 1 week.Then all the mice except the normal control group were intraperitoneally administered with uric acid to establish hyperuricemia model.1 h later,the blood sample was collected for determination of uric acid,creatinine and urea nitrogen.RESULTS:As compared with hyperuricemia control group,mini-dose aspirin had no apparent effect on creatinine and urea nitrogen,but remarkably increased the level of blood uric acid(P

Objective:To study the extraction process of AWei capsule.Methods:Taking the contents of gallic acid and the yield of extracts as assessment index to optimize water extraction process of AWei capsule with orthogonal experimental design.Taking the contents of magnolol,honokiol and the yield of extracts as assessment index to optimize ethanol extraction process of AWei capsule with orthogonal experimental design.Results:The optimal water extraction process of AWei capsule was extracting twice,every time lasted 1.5h,every time added 14 times water.The optimal ethanol extraction process of AWei capsule was using 80%ethanol as solvent,extracting twice,every time lasted 1.5h,every time added 6 times solvent.Conclusion:The optimized extraction process is stable and reasonable.

As a kind of mineral medicine containing mercury, the toxicity of Cinnabaris has always been controversial. In recent years, along with the increasing reports and studies on Cinnabaris, it has been found that although the toxicity of Cinnabaris has effects on multi-systems, the main effect is on nervous system. In order to clarify Cinnabaris neurotoxicity and reduce its damage for nervous lesion caused in clinical application, this article made a thorough analysis on symptom expression and mechanism of Cinnabaris neurotoxicity and put forward corresponding countermeasures.
