Objective To explore the clinical effect of hysteroscopic eletrosurgery for abnormal uterine bleeding of endometrial polyps.Methods 126 patients with abnormal uterine bleeding of endometrial polyps were selected,and they were selected and randomly divided into control group and research group according to the digital table,63 cases in each group.The control group received traditional curettage surgery, the research group received hysteroscopic eletrosurgery.The intraoperative blood loss,operative time,length of hospital stay,postoperative menstruation recovery time,therapeutic effect and complications were compared between the two groups.Results The intraoperative blood loss and operation time of the research group were significantly lower than those of the control group (t1=7.44,P1<0.01,t2=7.78,P2<0.01).And there were no statistical differences between the two groups in length of hospital stay and postoperative menstruation recovery time(t1=1.82,P1=0.07,t2=1.71,P2=0.09).The total cure rate of the research group was 100.00%,which of the control group was 92.14%,the difference was statistically significant(x2=8.18,P=0.00).The incidence rate of complications of the research group was 3.17%, which of the control group was 17.50%,the difference was statistically significant(x2=11.08,P=0.00).Conclusion Hysteroscopic eletrosurgery for abnormal uterine bleeding of endometrial polyps has remarkable clinical effects and low incidence of complications,and it is worthy of clinical application.

Objective To investigate the risk factors and effective nursing for patients with benign prostatic hyperplasia(BPH)within 3 days after operation.Methods The risk factors for restlessness of patients with BPH in 2006 were analyzed and the nursing points were summarized.Results Age of patients,surgical style and sleeping time before the operation were evidently related to the incidence of restlessness.Conclusion Strengthening of psychological nursing,appropriate surgical style and insurance of abundant sleeping time before operation could reduce the incidence of restlessness of patients after operation.

Objective To observe the effect ofmycophenolate mofetil combined with prednisone in treatment of old patients with refractory nephrotic syndrome.Methods From June 2009 to June 2010,67cases of old patients with refractory nephrotic syndrome were divided into two groups by random digits table,32 cases in control group were treated with cyclophosphamide combined with prednisone and 35 cases in observation group were treated with mycophenolate mofetil combined with predrisone.The clinical effect and the levels of urinary protein,serum creatinine and serum uric acid between two groups were compared.Results There was no significant difference in total effective rate between two groups [68.8％(22/32) vs.74.3％ (26/35)] (P ＞ 0.05 ).After treatment,the levels of urinary protein,serum creatinine and serum uric acid in two groups were decreased (P＜ 0.05 ),but there were no significant differences between two groups (P ＞ 0.05 ).The adverse reaction rate in observation group was significantly lower than that in control group (P ＜ 0.05).Conclusion Mycophenolate mofetil combined with prednisone in treatment of old patients with refractory nephrotic syndrome has good effect and low adverse reaction,and it can be applied in clinic.

Objective To investigate the curative effect and safety of endoscopic nose biliary drainage (ENBD) through endoscopic retrograde cholangiopancreatography (ERCP) to treat for elderly patients with acute severe cholangitis in primary hospital.Methods Forty-two elderly patients with acute severe cholangitis were collected,the patients received ENBD through ERCP.The changes of clinical symptoms and signs,imaging examinations and laboratory examinations before and after treatment were compared.Results All 42 patients received ENBD through ERCP,40 patients were cured,1 patient failed in ERCP and then was turned to surgical operation,and 1 patient whose cholangitis condition got better died of respiratory failure because of chronic obstructive pulmonary disease (COPD).Conclusion ENBD through ERCP is a safety and effective treatment which was used to treat for elderly patients with acute severe cholangitis in primary hospital,so this technique is worthy popularizing in primary hospital.

BACKGROUND:Lumbar fusion is a conventional effective measure to treat spondylolisthesis, spinal stenosis or with deformity. Bilateral pedicle screw fixation is recognized as the standard treatment for various spinal disorders, and has biomechanical and clinical advantages. OBJECTIVE:To evaluate the effects of bilateral pedicle screw fixation in the repair of lumbar disc herniation to restore disc height from the angle of imaging. METHODS: Clinical data of 42 patients with lumbar disc herniation were retrospectively analyzed. They al received bilateral pedicle screw fixation. Pain was evaluated before implantation, immediately and 1 month after implantation using Japanese Orthopaedic Association score of lower back pain and visual analog scale score. X-ray including anteriorposterior and lateral films of lumbar spine and MRI were used. CT was utilized to verify screw placement conditions and complications. RESULTS AND CONCLUSION:A total of 42 patients were folowed up for 3-6 months. Compared with pre-implantation, Japanese Orthopaedic Association score and visual analog scale score were significantly improved immediately after implantation (P < 0.01). There was no significant difference in Japanese Orthopaedic Association score between 1 month and immediately after implantation (P > 0.05). The height of intervertebral discs was significantly higher immediately and 1 month after implantation than pre-implantation (P < 0.01). The symptoms were lessened after fixation in al cases, and their qualities of life elevated. At 1 month, X-ray films and CT images revealed that no screw loosening, breakage or displacement occurred. The height of intervertebral discs was perfectly restored. No adverse events appeared in patients. These data indicate that bilateral pedicle screw fixation for lumbar intervertebral disc herniation can effectively restore the height of intervertebral discs, improve clinical symptoms and have biological and clinical superiority.

Objective To investigate the application of lymphocyte parameters in screening the lymphocyte proliferation and re-active hyperplasia disease.Methods 128 cases as the healthy control,100 cases of lymphoma,35 cases of multiple myeloma(MM), 34 cases of lymphocytes reactive hyperplasia(RL)and 5 cases of chronic lymphocytic leukemia(CLL)were selected and performed the peripheral blood cells analysis by the Sysmex XE-2100 hematology analyzer.The relevant lymphocyte parameter values were re-corded and the comparative analysis in the difference between the disease groups and the control group was performed.The role of each lymphocyte parameter in screening the lymphocyte proliferation and reactive hyperplasia diseases was evaluated by the receiver operating characteristic curve(ROC curve).Results The high fluorescence lymphocytes(HFL),in the control group was 0.008 ± 0.008,in the lymphoma group was 0.016±0.058,in the MM group was 0.019 ±0.063,in the RL group was 0.040 ±0.070,and CLL group was 0.388±0.158.Compared with control group,the difference of RL and CLL group was statistically significant(P <0.05).The percentage of lymphocytes(LY%,:the control group:34.4%±5.9%,the lymphoma group:26.6%±13.1%,the MM group:31.0%±13.1%,the RL group:29.2%±15.4% and the CLL group:44.5%±38.5%.The difference between the lympho-ma group and the control group was statistically significant(P <0.001).The lymphocytes structural parameters X(LY-X),the con-trol group:833.5 ± 22.7,the lymphoma group:867.9 ± 28.5,the MM group:867.9 ± 26.6,the RL group:859.2 ± 27.8 and the CLL group:894.0 ± 65.4.Except for the CLL group,the differences between other groups and control group were statistically significant(P <0.001).The lymphocytes structural parameters Y(LY-Y),the control group:659.6 ± 23.0,the lymphoma group:669.4 ± 43.5,the MM group:665.9 ± 37.1,the RL group:665.9 ± 40.0 and the CLL group:778.4 ± 152.1.Compared with the control group,the difference of the lymphoma group was statistically significant(P <0.05).AUC of the ROC curve in the pa-tients with lymphocyte proliferation and reactive hyperplasia disease by LY-X screening was 0.819,with the sensitivity of 80.5%and the specificity of 60.1% when cutoff value was 842.5.Conclusion The lymphocyte parameters,especially LY-X can reveal the
morphological changes of lymphocytes sensitively and contribute to screening the patients with lymphocyte proliferation and reactive hyperplasia disease.
