1.Clinical analysis with 72 cases of respiratory failure rescued by nasotracheal intubation

Objective To investigate the clinical application and significance of nasotracheal intubation (NTI)on patients with respiratory failure.Methods 72 cases of post-operative and non-operative patients with respiratory failure were performed with urgent NTI or blind nasotracheal intubation(BNTI)and then mechanical venfilation.Results In all of the cases NTI were successful.32 cases received direct vision NTI;26 cases were BNTI:9 case8 were NTI through fibrobronchoscope and 5 cases were NTI under awake surface anesthesia.Operation were performed within 20 second to 11 minutes[(5.1±3.1)min].The remaining time of tracheal catheter were 2~21d [(9.3±5.9)d].Conclusion NTI was an important measure in rescuing the patients with respiratory failures.It is an essential rescue technique under the condition without fibrobronchoseope.
2.Explore the clinical value of adrenomedullin on children with acute heart failure treatment and prognosis

Objective To e xplore the clinical value of adrenomedullin on children with acute heart failure treatment and progno-sis.Methods Selected 58 cases of AHF children,40 patients with pneumonia,40 cases of children with congenital heart disease and 50 healthy children as the object,radioimmunoassay was taken to detected plasma adrenomedullin and endothelin level,and analysed correlation between AMD,ET-1 and cardiac function.Results Pneumonia complicated with heart failure group,congenital heart disease complicated with heart failure group plasma AMD,ET-1 levels were significantly higher than those in the control group, congenital heart disease group and pneumonia group (P<0.05),congenital heart disease group and pneumonia group of plasma AMD,ET-1 levels were significantly higher than those in the control group (P<0.05).Heart failure stage plasma AMD,ET-1 lev-els were significantly higher than those of early stage and recovery period (P<0.05),heart failure early stage plasma AMD,ET-1 levels were significantly higher than those of early heart failure recovery period (P<0.05).There was a significantly negative cor-relation between with AMD,EF-1 levels and CI,LVEF in AHF children (r= -0.614,-0.793,-0.618,-0.547,P<0.05);AMD mutation rate was significantly higher than that of EF-1 (P<0.05).Conclusion Plasma levels of ADM are closely relate with heart function in AHF children,through the determination of plasma ADM level can assessment treatment and the prognosis of children with AHF,and worthy of clinical application.
3.Analysis on present situation of patent creativity for Chinese materia medica in China

According to the relative data of the patents awarded in the field of Chinese materia medica (CMM) announced by State Intellectual Property Office of P. R. China during 1998-2003, the amounts of CMM patents awarded with different types, compounds and active constituents, and service and non-service patents of CMM were compared and analysed. The amounts of inventive CMM patents have been more than that of the patents of utility model and design, the patents of CMM compound have greatly exceeded the patents of active constituent of CMM. Non-service patents of CMM has absolute predominance compared with the service patents. However, percentage of patents of active constituents and service patents of CMM are important targets to measure the creativity of CMM patents. This difference has reflected the present situation of CMM in China-lack of creativity. Based on the authentication of CMM creative patent, the cause of the above-mentioned situation has been analysized, and that our Patent Law should perfect the criterions of the CMM creative patent has been proposed so as to encourage the individuals and enterprises to advance the creativity of CMM in skills furtherly and strengthen the ability of protecting Intellectual Property Right and power of international competition for CMM.
4.Value of MRA and DSA in diagnosing vertebral arterial insufficiency of cervical spondylosis

Objective To realize the value of magnetic resonance angiography (MRA) and digital subtraction angiography (DSA) in diagnosing vertebral arterial insufficiency of cervical spondylosis. Methods From Nov. 2001 to Feb. 2004, 35 vertebral arterial insufficiency of cervical spondylosis was diagnosed clinically. There were 11 males and 24 females with a mean age of 58.3 years(range, 23 to 76 years). All the patients were examined by MRA and DSA to investigate the tracks, diameters, and stenosis locations of vertebral articles. Results DSA was positive in 32 patients with 19 ipsilateral and 13 bilateral. Meanwhile, MRA positive were in 29 patients, 14 ipsilateral and 15 bilateral. Among MRA positive patients, excepted 5 cases, all the others were also positive with DSA. Accordance of both methods in the diagnosis of thin veterbral artery was 100%. DSA was better than MRA in the diagnosis of localized stenosis and unilateral veterbral artery absence. MRA was much more useful than DSA in the diagnosis of veterbral artery twisting caused by vertebral instability and bone spurs. Conclusion 1) DSA had more accuracy in diagnosis and distinguishing the source of veterbral artery localized stenosis. Posture changing didn't disturb the examination. Its disadvantages, however, were invasive detection, side-effect and dose limitation of contrast medium. 2) As a non-invasive method, MRA was a useful tool in diagnosing diffused, long segmental artery stenosis and obstruction, and especially available for diagnosing the cervical spondylosis combinated with other type. MRA can scan vertebral disc, spinal cord and other vessels at the same time. The indication of MRA was indefinite diagnosis, aged patients with bad tolerance to DSA. The disadvantage of MRA was its low sensitivity in detecting mild stenosis, non-obstruction artery disease, and posture demand during examination.
5.Inhibition of viral myocarditis by Astragaloside through IL-23/IL-17 signaling pathway

Objective To explore the role of interleukin-23 (IL-23)/interleukin-17 (IL-17) signaling pathway in viral myocarditis (VMC) and evaluate the intervention effect of Aastragaloside. Methods Seventy-five male BALB/c mice were randomly divided into 4 groups, control group (n=15), model group (n=20), low-dose intervention group (n=20) and high-dose intervention group (n=20). Mice in control group were inoculated with 0.1 ml virus cultivation solution intraperitoneally while mice in the other 3 groups were treated with 0.1ml virus cultivation solution containing 1×102 TCID50 coxsackievirus B3 (CVB3) to establish VMC model. On the day of inoculation, mice in low- and high-dose intervention groups were intra-gastrically administered with 0.1 ml of 1% and 9%Astragaloside solution respectively, whereas those in control and model groups were treated with 0.1 ml carboxymethycellulose solution. Astragaloside or carboxymethycellulose was given once a day and continued 15 days. The number of mice death and the performance of mice were recorded in experimental period. All mice were sacrificed on day 15. The heart and blood sample were obtained. Histological cross sections of heart were stained with hematoxylin-eosin and scored for myocardial histopathologic changes under optical microscope. Th17 cells were analyzed by flow cytometry. The mRNA and protein expression levels of myocardial IL-23 and IL-17 were detected by real-time quantitative PCR and Western blotting, respectively. Results The mortality was statistically significant differ-ences among the four groups (P= 0.013), which was the lowest in the control group. The myocardial histopathologic scores, the percen-tage of Th17 cells, as well as expression levels of myocardial IL-23 and IL-17 mRNA and protein were significantly lower in high-dose intervention group than those in model group and low-dose intervention group, but higher than those in control group (P < 0.05). The myocardial histopathologic scores, the percentage of Th17 cells, as well as ex-pression levels of myocardial IL-23 and IL-17 mRNA and protein were significantly higher in model group and low-dose in-tervention group (P < 0.05). There were no significant difference in the above mentioned indicators between low-dose inter-vention group and model group (P > 0.05). Conclusions Astragaloside may dose-dependently protect against VMC by in-hibiting IL-23/IL-17 signaling pathway.
6.Effects of Chinese Medicine Compound on Bone Loss of Weightlessness Rats Simulated by Suspension

Objective To study the effects of Chinese medicine compound on bone density, biomechanics, histomorphometry of weightlessness rats simulated by tail suspension. Methods Fifty Wistar rats were randomly divided into 5 groups with 10 rats each group:control group, model group, and low dose, medium dose and high dose Chinese medicine compound treated suspension group, the experiment period was 21 days. BMD of femur and lumbar vertebrae were detected by dual energy X-ray absorptiometry. The femoral biomechanics parameters and anti-compress ability of lumbar vertebrae were measured by three-point assay and compress test respectively. 7.The study of the subthreshold depression and their quality of life for long-term survivors after breast cancer surgery

Objective To investigate the subthreshold depression for long-term survivors after breast cancer surgery and its effect on their quality of life (QOL). Methods The self-made general questionnaire, Centre for Epidemiological Studies Depression Scale (CES- D), Hamihon Rating Scale for Depression and Functional Assessment of Cancer Therapy- Breast were used to investigate the subthreshold depression and its effect on their QOL for 210 long-term survivors after breast cancer surgery. The difference in general conditions and score of QOL between the subthreshold depression and non-subthreshold depression groups was also compared. Results The subthreshold depression with incidence rate of 46.67%(98/210) was observed for the log-term survivors after breast cancer surgery in this study. The results indicated that the age (≤50) (OR=5.627, 2.662,P<0.01), marriage (unmarried, divorced or widowed) (OR=5.087, P<0.01), employment (unemployed, sick leave or retired) (OR=2.077,P<0.05) and low income (<3000 RMB/month) (OR=3.375,P<0.01) were the key factors that led to the occurrence of subthreshold depression. The score of QOL for the subthreshold depression group were lower than those of the non-subthreshold depression group (t=-6.653--2.926,P<0.01), and had a significantly negative correlation with score of depression and QOL (β=-0.362--0.132). Conclusions A high incidence rate of subthreshold depression for long-term survivors after breast cancer surgery was observed. The incidence rate of subthreshold depression increased for the patients with younger age, low income,unmarried or unemployment. Due to their low QOL, attentions should be paid to the early detection and early prevention for the long-term survivors with subthreshold depression after breast cancer surgery.

Objective To explore the expression of interleukin-17 (IL-17) in the murine myocardium with viral myocarditis (VMC) ,and the influence of astragaloside intervention on its expression .Methods 60 male 4-week-old Balb/c mice were randomly divided into four groups ,namely normal control group ,model control group ,low-dose and high-dose intervention groups ,15 mice in each group .Mice in the latter three groups were inoculated with 0 .1 mL coxsackie B3 virus intraperitoneally .Then ,mice in low-dose and high-dose intervention groups were treated with 0 .01 g/L and 0 .09 g/L astragaloside solution ,respectively .All mice were killed on 15 days .The mortality and heart weight/body weighty (HW/BW ) were calculated .Histological cross sections of heart were stained with hematoxylin-eosin and histopathologic scores of inflammatory cells infiltration and myocardial necrosis were eval-uated under optical microscope .The expression levels of myocardial IL-17 mRNA and protein were detected through real-time quan-titative PCR and Western blot .The contents of IL-6 and tumor necrosis factor-α(TNF-α) in the myocardium were examined by ELISA .Results The mortality and histopathologic scores of inflammatory cells infiltration and myocardial necrosis in high-dose in-tervention group were significantly lower than those in model controlgroup (P<0 .05) .Compared with normal control group ,the HW/BW ,the expression levels of myocardial IL-17 mRNA and protein as well as the contents of IL-6 and TNF-αin the myocardi-um were markedly increased in model control group(P<0 .01) ,whereas these parameters were significantly decreased in high-dose intervention group as compared to model group (P<0 .05) .Conclusion IL-17 may be involved in the pathogenesis of VMC .The therapeutic effect of astragaloside on VMC may be associated with inhibiting IL-17-mediated inflammatory response .
9.Oral health related life quality assessment of young children before and after dental treatment under general anesthesia

Objective:To assess the effects of dental treatment under general anesthesia on the life quality of children and their families.Methods:59 children aged 3 to 6-year-old were included.Parents were required to complete the early childhood oral health impact scales(ECOHIS) before and 1 month after dental treatment under general anesthesia.The questionnaire includes child impact section(CIS) and family impact section(FIS).Data were analyzed using SPSS version 19.0.Results:55 out of the 59 parents completed the qustionnaire.After treatment the total ECOHIS scores decreased by 51% (P < 0.001),overall child impact section scores decreased by 50% (P <0.001),and family impact section scores decreased by 53% (P <0.001),revealing a large effect size(ES) for the child(1.4) and family (4.0) sections of the ECOHIS.Conclusion:Dental treatment under general anesthesia for uncooperative young children with extensive dental problems can improve the quality of life of both children and their families.
