1.Problems and Solutions in Development of Enterprise-Web PACS

Objective To discuss the problems in the development of enterprise -Web PACS, and find the solutions. Methods The author applied the standpoint of software engineering, and used the experience of other enterprises to find a fast, low-cost, well-maintainable, well-expansible way in development of enterprise-Web PACS. Results The solution is a reasonable development method for enterprise-Web PACS, and the more big the project is, the more obvious the advantage is. Conclusion It is an effective way of developing enterprise-Web PACS to use the reasonable software structure and good development platform and advanced development framework in increasing the software reusability, maintainability, improving the capacity of the secondary development ability.
2.Construction of PACS in Muti-layer Structure

Objective To improve the efficiency,repeatability,expansibility and maintainability and reduce the construction and maintenance cost of the PACS.Methods Struts and Hibernate frame works based on MVC and Object-Oriented Analysis(OOA) were used to build PACS.Results It saved more time for developers,and promoted the development efficiency.Conclusion It is rapid and efficient to use Struts and Hibernate frame in building PACS.[Chinese Medical Equipment Journal,2008,29(2):55-57]
3.Computed tomography analysis of anatomic structure related to endoscopic axilla approach for surgery of frontal sinus.

OBJECTIVE:
To observe the CT imaging features of anatomic structure related to endoscopic axilla approach for surgery of the frontal recess and frontal sinus.
METHOD:
Thirty patients without a history of frontal sinus disease were undergone 16 line high speed spiral computed tomography. The computed tomographic images were analyzed to measure the related structures.
RESULT:
The vertical distance from the front attachment point of the middle turbinate to the skull base was 13.88 +/- 2.59 mm. The horizontal distance from the top point of the axilla of the middle turbinate to the anterior wall of the frontal sinus outflow tract was 5.77 +/- 12.32 mm, to the anterior wall of the nasal cavity was 13.67 +/- 12.54 mm, to the lamina papyracea or lacrimal sac was 5.89 +/- 1.69 mm.
CONCLUSION
Sixteen line high speed spiral computed tomography is helpful to endoscopic axilla approach for surgery of the frontal recess and frontal sinus.

4.Three-dimensional finite element analysis of artificial femoral head replacement for unstable femoral intertrochanteric fractures

BACKGROUND:Artificial femoral head replacement provides a new idea for the repair of unstable intertrochanteric fracture. Artificial prosthesis replacement may affect original femoral biomechanical stability and lead to a variety of adverse consequences. OBJECTIVE:To analyze the stress distribution of femoral head replacement in the treatment of unstable femoral intertrochanteric fractures with three-dimensional finite element analysis. METHODS:One male old volunteer was randomly selected from population who underwent health examination. The left femur was scanned with spiral CT, and the three-dimensional finite element models of the human femur and prosthesis were established. The three-dimensional finite element model was used to simulate the actual working conditions of human climbing stairs, and the stress distribution of the bone channels around the surface of the femur and the prosthesis was analyzed with three-dimensional finite element analysis. RESULTS AND CONCLUSION: Under normal condition, the stress of the human femur was in a consistent state. Stress changed gradualy from the proximal end to the distal end. The stress of the prosthesis was concentrated in the middle section. The prosthesis of inner stress distribution was analyzed to obtain stress distribution of prosthesis and femur cancelous bone interface. The analysis found that stress change trend was consistent. The results suggest that artificial femoral head replacement does not have a significant effect on the overal stress distribution of the human femur, and the overal stress distribution does not change, and the maximum stress region is located in the middle of the whole femur. After the reconstruction, the stress concentration of the femur is not observed.
5.The research of the CENP-W in regulating invasion of gliomas

Objective To explore the expression of the CENP-W in gliomas and investigate the effects of its invasion. Methods The expression level of the CENP-W in gliomas with varied pathologic grade were detected by immunohistochemical analysis,RealTime PCR,and Western Blotting. U251 cells were transfected with the specific siRNA to repress the CENP-W expression level. The invasion ability of U251 cells were examined by Transwell Chamber assay ,while RAS mRNA and protein levels were detected at the same time. Results The expression levels of the CENP-W in glioma tissues were significantly high and the CENP-W gene could enhance the invasion of U251 cells . The expression of RAS was down-regulated when the expression of CENP-W was repressed. Conclusion The CENP-W has an oncogenic role in human brain gliomas and may regulate the invasion of gliomas by adjusting the RAS signaling pathways.
6.An implementation of constructing medical data integration platform based on web service.

8.Safety and efficacy of colistin monotherapy versus combination therapy against carbapenem-resistant gram-negative bacteria infection: a systematic review and meta-analysis

Objective To study the safety and efficacy of colistin monotherapy versus combination therapy against carbapenem-resistant gram-negative bacteria infection.Methods CNKI, Wanfang database , PubMed, Embase and Cochrane library were systematically searched.Randomized controlled trials about colistin monotherapy versus combination therapy against carbapenem-resistant gram-negative bacteria infection were enrolled.The Cochrane Reviewers′Handbook 5.2 was employed to evaluate the quality of the enrolled studies. The primary outcome was all-cause mortality.The secondary outcomes included infection-related mortality, clinical response, bacterial clearance, nephrotoxicity and hepatotoxicity.Meta-analysis was conducted by RevMan 5.3 software.Results Seven articles containing 859 patients were finally included.There were no significantly statistical differences in all-cause mortality rate (relative risk [RR]=1.07, 95%CI:0.93-1.24, P>0.05), infection-related mortality rate (RR=1.35, 95%CI:0.98-1.87, P>0.05), bacterial clearance rate (RR=0.85, 95%CI:0.71-1.02, P=0.08), hepatotoxicity development rate (RR=0.68, 95%CI:0.41-1.13, P=0.14), and nephrotoxicity development rate (RR=1.01, 95%CI:0.85-1.22, P>0.05) between colistin monotherapy and combination therapy.The clinical response rate was higher in combination therapy than that in colistin monotherapy ( RR =0.81, 95%CI:0.66 -0.98, P =0.03).In the subgroup analysis, no statistical differences were found in all-cause mortality rate between colistin monotherapy and combination therapy for carbapenem-resistant Acinetobacter baumannii infection (RR=1.00, 95%CI:0.86-1.12, P>0.05). The dosage of colistin with or without loading dose was not associated with the treatment response .Conclusions Although colistin-based combination therapy has a better clinical response against carbapenem -resistant bacteria infection, especially for Acinetobacter baumannii infection, the mortality rate dose not decline compared to colistin monotherapy.Large-scale randomized controlled trials are needed to evaluate the effect in the future.
9.Efficacy and safety of intranasal dexmedetomidine premedication for children undergoing CT or magnetic resonance imaging: a systematic review and meta-analysis

Objective:To review the efficacy and safety in children receiving intranasal dexmedetomidine premedication before CT or magnetic resonance imaging (MRI).Methods:A literature search (search terms included "dexmedetomidine" "intranasal drug administration" "children" "CT" and "MRI") was conducted using Embase, PubMed, the Cochrane Library, ClinicalTrials.gov, CNKI, Wanfang, VIP database and Chinese Biomedical Literature Database (searched from inception to March 18, 2019). Randomized controlled trials of children receiving intranasal dexmedetomidine versus chloral hydrate, ketamine or midazolam premedication before CT or MRI were included. The Cochrane Reviewers′ Handbook 5.1.0 was used to evaluate the quality of the enrolled studies. The primary outcomes were sedation success rate and sedation induction time. The secondary outcomes included respiratory depression, heart rate, systolic blood pressure and blood oxygen saturation. Statistical analyses were performed using the Review Manager 5.3 software.Results:A total of 1 167 participants in 9 randomized controlled trials were included. The results of meta-analysis showed that intranasal dexmedetomidine premedication provided higher sedation success rate than oral chloral hydrate (relative risk ( RR) =1.13, 95% confidence interval ( CI) 1.02 to 1.26, P=0.020). There was no significant difference between intranasal dexmedetomidine and midazolam. In addition, the sedation induction time of intranasal dexmedetomidine group was significantly shorter than that in the oral chloral hydrate group (weighted mean difference -1.49, 95% CI -2.87 to -0.11; P=0.030), but showed no significant difference as compared with that of intranasal ketamine or midazolam. The patients treated with intranasal dexmedetomidine also showed significantly lower heart rate ( RR=4.78, 95 %CI 1.85-12.35, P=0.001) and less respiratory depression ( RR=0.28, 95 %CI 0.09-0.87, P=0.030). There were no intergroup differences in systolic blood pressure and blood oxygen saturation. Conclusions:Intranasal dexmedetomidine provided more effective sedation and higher safety in children undergoing CT or MRI. As this meta-analysis is limited by the small sample size, further high-quality randomized controlled trials are needed.
