1.Clinical result of limited arthroscopic debridement for knee osteoarthritis under local anesthesia

Objective To evaluate the effect of limited arthroscopic debridement under local anesthesia for knee osteoarthritis. Methods A retrospective analysis was made including a total of 63 patients (77 knees ) with knee osteoarthritis.There were 11 male and 52 female,with an average age of 56.5 years old(range from 38~71 years old).The limited arthroscopic debridement included removal of loose bodies hyperplastic synovum,degenerated cartilage and the injured meniscus.The average follow-up period was 2.1 years (from 6 month to 4 year).Outcomes were assessed by JOA knee function score and rate of satisfactory. Results During the follow-up,the patients showed significant improvement of knee function score from 41 point to 78 point.Clinical outcomes of 77 knees were excellent in 31 knee,good in 28 knee,fair in 12 knee,poor in 6 knee.The rate of good and excellent results was 76.6%. Conclusions Limited arthroscopic debridement under local anesthesia for knee osteoarthritis is a relative simple and effective method.It could alleviate pain and improve function of the paticents.
2.Analysis of carbapenemase resistance genes and their homology in multi-drug resistant Acinetobacter baumannii

Abstract: Objective To analyze the genotype of carbapenemase resistance genes and their genetic homology in multidrug-resistant Acinetobacter baumannii, and to provide a theoretical basis for guiding the rational use of antibiotics and controlling the prevalence of nosocomial infections. Methods A total of 83 multidrug-resistant Acinetobacter baumannii isolated from patients in the intensive care unit (ICU) and environmental specimens in the Sixth People's Hospital of Nantong from July 2020 to December 2021 were collected. The bacteria were identified and subjected to drug sensitivity tests using the BioMérieux DL96-Ⅱ automatic bacterial identification susceptibility system. The carbapenemase-related drug resistance gene types were detected using the polymerase chain reaction (PCR) method, and clones were analyzed by pulsed field gel electrophoresis (PFGE). Results The types of 83 ICU Acinetobacter baumannii specimens include sputum (43 strains), broncholavage fluid (20 strains), and surfaces of objects such as ventilators (20 strains). The resistance rates of all strains to imipenem, tetracycline, gentamicin, amikacin and ciprofloxacin were 100%, 32.5%, 38.6%, 41.0% and 77.1% respecitively, while the resistance rates to others such as ticarcillin and clavulanate were greater than 95%. All strains carried were detected to carry OXA-23 and OXA-51 genes, while OXA-24, OXA-58, IMP-1, VIM, IMP-4, SIM and NDM-1 resistance genes were all negative. PFGE homology analysis confirmed that 83 strains of Acinetobacter baumannii, with counts of 12, 18, 12, 13, 10, 6, 7, 5 respectively, mainly A, B, C, D, E clones, the rest were sporadic clones. Conclusions The carbapenem resistant Acinetobacter baumannii isolated from our ICU are widely drug-resistant to commonly used antimicrobial drugs, with B clone strain being the major prevalent strain. Carrying OXA-23 and OXA-51 genes may be an important reason for the resistance of Acinetobacter baumannii to carbapenem antibiotics in our ICU. Rational use of antimicrobial drugs, enhanced monitoring of bacterial resistance, and effective control of the generation and further spread of drug-resistant strains should be emphasized.
3.Reconstruction of anti-hepatocellular carcinoma scFv pharmacy and its killing effect on hepatocardnoma cell in vitro

objective To observe whether the killing effect on HCC SMMC-7721 cell of the antihepatocellular carcinoma scFv reconstructed by pharmacy was enhanced or not.Methods Prokarycytic expression vector containing PET32a-RC-RNase was induced to express by IPTG.The inclusion body purified and Western-blotting was used.PC.CHOL and CHS was added in chloroform.Dry membrane was formed after chloroform was removed.RC-RNase protein solution was added to dissolute the membrane.Then pass the solution over a Sephadex G-50 column after ultrasound and filtrated to detect the encapsulation efficiency of the liposome.The solution reacted in EDC.SSNHS and MES for 30 minutes.Then add hdscFv to the solution in 4 ℃ over night.MTT method was used to detect the killing effect on HCC cell of immunoliposome RC-RNase,immunotoxin RC-RNase and liposome RC-RNase in vitro.Resuits The killing effect on HCC cell of immunoliposome RC-RNase is the best.but that of Iiposome RC-RNase is the worst.The respective JC50 are:3.28μg/ml,22.44μg/ml and 98.26μg/ml.Conclusion The anti-hepatocellular carcinoma scFv relomtructed by pharmacy can promote the killing effect on HCC cell and may have potential in the treatment of hepatocarcinoma.
4.Anesthetic management of infant lung resection of congenital cystic adenomatoid malformation by video-assisted thoracicscopy

Objective To investigate the feasibility and safety of OLV anesthesia about infant lung resection of CCAM by video-assisted thoracicscopy. Methods Endo-tracheal intubation was performed after 43 CCAM infants had undergone rapid intravenous induction. One side of lungs was ventilated by injecting 4 ~ 6 mmHg CO2 for the construction of artificial pneumothorax, and the side lung was compressed forming OLV. SpO2, ECG, MAP, PETCO2, T, PaO2, PaCO2, bleeding volume and urine volume were monitored. The numerical value of SpO2, PaO2, HR, MAP, PETCO2, and PaCO2 were recorded at scheduled intervals. Results Compared with 5min after induction,the PaO2,HR and MAP of the infants significantly reduced; the PETCO2 and PaCO2 significantly increased at OLV at 10 min and 60 min. Compared with OLV at 10 min, the PaO2, PETCO2 significantly increased at OLV 60 min. Conclusion Appropriate respiratory management and drug usage are feasible and safe for infant surgery of CCAM by video-assisted thoracicscopy.

Objective To investigate the effect of thymosin ?1 (T?1) on cellular immune function in gastric cancer patients through observing its treatment on the differentiation of T-lymphocyte subsets from screened peripheral blood mononuclear cells (PBMCs). Methods PBMCs were obtained by centrifugation of blood samples from 18 healthy subjects and 32 patients with gastric cancer,and then cultured in the presence of culture medium with addition of T?1 at 50,10 and 1 ?g/ml for 2 d. T lymphocyte subsets (such as CD4+,CD8+ and CD4+ CD25+ Foxp3+ T cells) and Th1/Th2 multiplex cytokines were detected by flow cytometry (FCM). Results After PBMCs isolated from healthy people and patients were incubated with or without T?1,there was no significant change in percentage of CD4+,CD8+ peripheral lymphocyte subsets and ratio of CD4+/CD8+. There was no obvious change in the percentage of CD4+ CD25+ Foxp3+ T lymphocyte subsets in the normal control,but a significant increase was observed in the cells from patients with gastric cancer after treatment (P
6.Effects of power-frequency magnetic fields exposure on phosphorylation and enzymatic activity of stress-activated protein kinase and its upstream kinase.

OBJECTIVETo study the effects of 50 Hz power-frequency magnetic fields on signal transduction pathway of stress-activated protein kinase(SAPK), and explore the cellular signal transduction mechanism of the biological effects induced by power-frequency magnetic fields.
METHODSChinese hamster lung(CHL) cell line was exposed to power-frequency magnetic fields with two intensities for different exposure durations. The cytoplasmic protein was extracted and the phosphorylated portion of SAPK and SEK1/MKK4 was measured with Western blotting analysis. The SAPK enzymatic activity was measured by the solid-phase kinase assay in cells exposed to power-frequency magnetic fields for 15 min.
RESULTSBoth 0.4 mT and 0.8 mT power-frequency magnetic fields could enhance the phosphorylation of SAPK in a time-relative course manner, and reached the maximum extent at 15 min, with an increase of 20% and 17% respectively. The solid-phase kinase assay showed that the enzymatic activities of SAPK were also increased, which were 2.9 +/- 0.4 and 2.1 +/- 0.9 times of control respectively. However, the duration of SAPK phosphorylation induced by 0.8 mT was longer than that of 0.4 mT, while the duration and extent of SAPK dephosphorylation was remarkably shorter than that of 0.4 mT. The power-frequency magnetic fields under equal conditions could not phosphorylate(activate) the SEK1/MKK4.
CONCLUSIONPower-frequency magnetic fields could activate the SAPK, but not SEK1/MKK4. It is suggested that power-frequency magnetic fields may activate SAPK signal transduction pathway through a kinase other than SEK1/MKK4. The activation mechanism of SAPK of power-frequency magnetic fields needs to be identified in more detail.

7.Lignans from stems of Sambucus williamsii.

OBJECTIVETo study the chemical constitutents of the 60% ethanol extract of the stems of Sambucus williamsii.
METHODCompounds were isolated and purified by Diaion D101, silica gel,Sephadex LH-20, ODS column chromatography and preparative HPLC. Their structures were identified by spectroscopic methods.
RESULTSeven lignans were isolated and identified as erythro-guaiacylglycerol-beta-O-4'-sinapyl ether (1), 1-( 4'-hydroxy-3'-methoxyphenyl) -2- [4"-( 3-hydroxypropyl) -2", 6"-dimethoxyphenoxy] propane-1,3-diol (2), isolariciresinol (3), burselignan (4), lyoniresinol (5), 5-methoxy-isolariciresinol (6), cycloolivil (7).
CONCLUSIONAll these compounds were obtained from this genus for the first time.

8.Catheter-directed thrombolysis versus surgical thrombectomy for acute lower limb ischemia: comparison of clinical effect

Objective To compare the efficacy and safety of surgical thrombectomy with those of catheter-directed thrombolysis (CDT) in treating acute lower limb ischemia (ALLI) . Methods The clinical data of 107 patients with ALLI, who underwent CDT or surgical thrombectomy at authors' hospital during the period from January 2012 to December 2017, were retrospectively analyzed. The incidence of complications, mortality, re-intervention rate and amputation rate within 30 days after operation were calculated. Cooley standard of efficacy score was used to assess the clinical curative effect of patients with grade Ⅱ of American Society of Vascular Surgeons (SVS) and International Society of Cardiovascular Surgery (ISCVS) classification of acute limb ischemia. Results A total of 107 patients with ALLI were enrolled in this study. Among them, 59 patients received CDT therapy and 48 patients received surgical thrombectomy. The technical success rate of both groups was 100%. Within 30 days after operation, the incidence of complications in CDT group and surgical thrombectomy group was 11.9% and 14.6% respectively, the mortality was 5.1% and 6.3%respectively, and the re-intervention rate was 17.0% and 22.9% respectively, all the differences were not statistically significant (P>0.05) . The amputation rate of patients with grade Ⅱ of SVS/ISCVS ischemia classification in CDT group was 8.9%, which was higher than 2.3% in surgical thrombectomy group, but the difference between the two groups was not statistically significant (P>0.05) . In patients with grade Ⅱ of ischemia assessed by the ca uses and Cooley curative effect standard at the time of discharge, the cure percentage of embolism-caused ALLI patients in surgical thrombectomy group was 82.5%, which was remarkably higher than 62.8% in CDT group (P <0.05) . The cure percentage of thrombosis-caused ALLI patients in CDT group was 92.3%, which was strikingly higher than 25.0% in surgical thrombectomy group (P <0.05) . The difference in the improvement of ischemia, which was assessed by the duration of ischemia and Cooley therapeutic criteria at the time of discharge, between the two groups was not statistically significant (P>0.05) . Conclusion In treating ALLI, no significant differences in curative effect, incidence of postoperative complications and mortality exist between surgical thrombectomy and CDT. The formulation of therapeutic regimen should be based on ischemia grade of SVS/ISCVS, etiology and coexisting underlying diseases.
9.Clinical Analysis of Arthroscopic Repairing of Ellman Grade Ⅲ Bursal-side Partial-thickness Rotator Cuff Tears

Objective To investigate the clinical outcome of Ellman grade Ⅲ bursal-side partial-thickness rotator cuff tears(b-PT-RCT)treated by the arthroscopic single-row technique with articular tendon fibers reservation.Methods A retrospective study was conducted on 31 patients with Ellman grade Ⅲ (b-PT-RCT) who underwent arthroscopic rotator cuff repair (RCR) with the single-row technique in our hospital between January 2014 and October 2016.The intact articular side tissue of the rotator cuff was reserved in 17 patients of group A,and was resected to convert to a full-thickness cuff tear in 14 patients of group B.The visual analog pain scale(VAS)score,Constant-Murley score and shoulder active range of motion(ROM),and magnetic resonance imaging(MRI)results were compared between the two groups during the last follow-up.Results Among the 31 patients,25 were followed,with 14 in group A followed for an average of 16.29(12~25)months and 11 of group B followed for an average of 15.37(12~27)months.There were no significant differences between the two groups in all measurements before the intervention and at the last follow-up.After the operation,significant improvement was observed in all planes of the active ROM for both groups(P<0.05).The VAS score of group A and B decreased significantly from 6.50 ± 1.16 and 6.63 ± 1.80 before the operation to 0.86 ± 0.57 and 0.91 ± 0.63 at the last follow-up(P<0.05 for both).However,the average Constant-Murley score improved significantly from 55.21 ± 6.40 to 86.85 ± 4.85 in Group A(P<0.05),while from 54.37 ± 6.30 to 85.73 ± 5.36 in group B(P<0.05).During the last follow-up,no retear occurred in both groups according to the MRI examination.Conclusion The outcome of single-row repair for grade Ⅲ bursal-sided partial-thickness rotator cuff tears with either articular tendon preservation or converting of the lesion to a full-thickness tear is satisfying.No significant differences were found in their clinical outcomes.
10.In vitro human lymphocyte proliferation assay under different extraction conditions and doses of two types of test samples

BACKGROUND:In vitro lymphocyte proliferation test is often used to detect the potential immunogenicity of medical devices,but no detailed extraction conditions and dose are given in the relevant standards. OBJECTIVE:To investigate the effects of different extraction conditions of the test product and different doses of the extract on in vitro human lymphocyte proliferation,and to consider the factors that need to be considered when selecting test conditions for in vitro lymphocyte proliferation test. METHODS:In the experiment,the homogenous bone repair material and heparin-modified intraocular lens were divided into the following 12 groups:(1)Experimental group 1:24-hour complete medium(RPMI modified medium containing 10%fetal bovine serum)extract of 200 μL + lymphocyte suspension of 50 μL;(2)negative control group 1:24-hour complete medium 200 μL + lymphocyte suspension 50 μL;(3)experimental group 2:24-hour complete medium extract 100 μL + lymphocyte suspension 100 μL;(4)negative control group 2:24-hour complete medium 100 μL + lymphocyte suspension 100 μL;(5)experimental group 3:72-hour RPMI modified medium extract(addition of 10%fetal bovine serum before experiment)200 μL + lymphocyte suspension 50 μL;(6)negative control group 3:72-hour RPMI modified medium(addition of 10%fetal bovine serum before experiment)200 μL + lymphocyte suspension 50 μL;(7)experimental group 4:72-hour RPMI modified medium extract(addition of 10%fetal bovine serum before experiment)100 μL + lymphocyte suspension 100 μL;(8)negative control group 4:72-hour RPMI modified medium(addition of 10%fetal bovine serum before experiment)100 μL + lymphocyte suspension 100 μL;(9)positive control group 1:complete medium containing 10 μg/mL plant hemagglutinin-M 200 μL + lymphocyte suspension 50 μL;(10)positive control group 2:complete medium containing 10 μg/mL plant hemagglutinin-M 100 μL + lymphocyte suspension 100 μL;(11)blank control group 1:250 μL complete medium;(12)control group 2:200 μL complete medium.After 3 days of culture,the proliferation of lymphocytes was detected by CCK-8 assay. RESULTS AND CONCLUSION:(1)Under different test conditions,the extracts of the allogeneic bone repair material could enhance the activity of human lymphocytes.Under the condition of 72-hour leaching in RPMI modified medium and the volume ratio of leaching solution and lymphocyte suspension was 4:1,the most significant effect was observed.Heparin-modified intraocular lens extract also had obvious inhibitory effect on lymphocyte activity under this condition;its inhibitory effect on lymphocyte activity may be related to the heparin in the extract.However,the activity of lymphocytes was slightly enhanced by heparin-modified intraocular lens extract under the experimental conditions of complete medium extraction for 24 hours and the volume ratio of extract to lymphocyte suspension was 4:1.(2)Under different extraction conditions and doses,the results of in vitro lymphocyte proliferation test may be quite different.The selection of test conditions should be combined with the clinical application of the product,and the inherent characteristics of the product should also be considered.
